[Professional status and development of morbidity of health insurance patients in the period 1990 to 2003. A longitudinal Analysis of Routine Data from the Gmünder Ersatzkasse].

In Germany there are only insufficient data both with regard to general morbidity events as well as with regard to the effects of specific professions on the morbidity process. For this reason the data of the Gmuender Ersatzkasse (GEK) have been analysed with a longitudinal design for the period 1990 to 2003. Our sample includes all employed members of the GEK who had been insured as of January 1, 1990 and were between 30 and 59 years old at this baseline. The total sample comprises 129,173 men and 13,567 women. The statistical analysis was performed with the statistical package "Transitional Data Analysis" (TDA) which was developed especially for longitudinal data. The analysis includes both cumulated morbidity rates as well as transition rates (Cox regression). The analysis shows that the cumulated morbidity rates of the seven diseases are higher for women than for men in the age category 30-39 years. In the age categories 40-49 and 50-59 years the cumulated morbidity rates are higher for men than for women. With regard to the occupations, the highest morbidity rates are found for manual occupations and services without special skills. The lowest rates are found for professions with high skills, engineers and managers. The results of the longitudinal analysis show distinctive social gradients. For occupations with lower skills the morbidity rates are about 100 percent higher than those of occupations with higher skills. Longitudinal analyses on the basis of health insurance fund data can make an important contribution to the monitoring of health and morbidity and should therefore be conducted also by other health insurance funds.

Methylmercury and neurodevelopment: longitudinal analysis of the Seychelles child development cohort.

BACKGROUND: The Seychelles Child Development Study (SCDS) has been longitudinally following a cohort of over 700 children enrolled in 1989. Their mothers consumed a diet high in fish during pregnancy. Repeated examination of the SCDS cohort at six different ages through age 11 years has shown no pattern of adverse effects. Some early appearing beneficial associations between both prenatal and postnatal hair MeHg and several child development endpoints were noted. We hypothesized these might be related to micronutrients in the fish, but they were not found when the children reached middle school age. These findings suggest that the associations observed between MeHg and developmental outcomes may vary with developmental stage. METHOD: We examined the main cohort of the SCDS to determine if this might be true using a longitudinal multiple regression analysis design that focused on those endpoints that were repeatedly measured at different ages. The primary endpoint analyzed was global cognition, involving a measure of developmental quotient or IQ. Secondary analyses included other domains such as Reading and Mathematics scholastic achievement, social behavior, and memory. Analyses involved two different approaches, one including incorporation of a passage of time variable, the other including a difference of scores across time points. RESULTS: No significant associations were found between prenatal MeHg exposure and any of the repeatedly measured endpoints. CONCLUSIONS: These results suggest that even when individual subject variance is controlled there was no consistent pattern of associations between child development outcomes and prenatal exposures to MeHg from maternal consumption of a diet high in fish. The Seychellois diet contains about 10 times more ocean fish than is typically consumed by US citizens. Our primary focus on IQ should further inform growing scientific interest in the analysis of the risks and benefits of fish consumption on overall cognitive ability.

Effect of smoking habit on age-related changes in serum lipids: a cross-sectional and longitudinal analysis in a large Japanese cohort.

To observe the effect of smoking habit on age-related serum lipid levels, we examined a large cohort of Japanese cross-sectionally and longitudinally. The participants included 103,648 Japanese men and women 17-94 years of age, who had received annual health examinations from 1989 to 2003. In cross-sectional analysis, total and LDL cholesterol levels of smokers were lower than those of nonsmokers up to an elderly age in men and up to middle age in women. Smoking was associated with decreased HDL cholesterol levels up to the 65-74 years age group in men and 55-64 years in women. The triglyceride levels were higher in smokers in both genders than those of nonsmokers below 55-64 years. In the longitudinal analysis, although smoking was associated with lower total and LDL cholesterol up to 60 years of age in women, beyond the sixties an inverted association was observed. The associations of smoking with lower LDL cholesterol levels in men and lower HDL cholesterol in both genders were fairly consistent at any given age. The increase of triglyceride levels in female smokers remained rather constant between 25 and 75 years, whereas the increase in triglyceride levels in male smokers was greater with older ages up to middle age. These results suggest that the effect of smoking on the serum lipid levels is dependent on age and gender.

Comparison of calcium/phosphorus analysis, longitudinal microradiography and profilometry for the quantitative assessment of erosive demineralisation.

The study is a comparison of methods for the quantitative measurement of erosive mineral loss including longitudinal microradiography (LMR), profilometry (PM), and analysis of calcium (CA) and phosphorus (PA) in the erosion solution. Polished human enamel samples were taped, covered with nail varnish and the edges of the resulting enamel window marked with drilled holes as a reference. All samples were subjected to baseline LMR. Sixty samples each were eroded with citric acid (0.05 M; pH 2.3; 10 ml per sample) for 30, 60, 90, or 120 min. Erosive loss of each sample was estimated by the four methods. All methods revealed a linear erosive loss over time and showed good linear correlation. Values calculated from PM and LMR were both approximately 20% lower than those from CA and PA. After 30 min erosion, LMR showed no significant correlation with the other three methods. With LMR, erosive loss below 20 microm should be interpreted with care.

A longitudinal analysis of the effect of prenatal alcohol exposure on growth.

Cross-sectional analyses have demonstrated that prenatal exposure to alcohol can result in growth deficits at birth and in early childhood. However, there have been few longitudinal analyses of this relationship. This study presents an analysis of the longitudinal effect of prenatal alcohol exposure on growth. The model used for analysis was a general unbalanced repeated-measures model with a fully parameterized covariance matrix. Application of the model demonstrated that for length and head circumference, the effect of prenatal alcohol exposure was constant over time. The relationship between prenatal alcohol exposure and growth in weight was more complex. Prenatal alcohol exposure affected the rate of growth between birth and the eight month. Subsequently, the relationship between weight and prenatal alcohol exposure was constant. Therefore, for length and head circumference, prenatal alcohol exposure suppresses the rate of growth in the fetus but not at subsequent time points. For weight, exposure affects the rate of growth through the eighth month but not at subsequent time points. For each of the growth parameters, there is no catch-up growth, and the smaller size observed in the offspring is maintained.

Professional socialization in dentistry: a longitudinal analysis of changes in students' expected professional rewards.

In this continuing longitudinal study on professional socialization of dental students, further analysis of the changes occurring in their attitudes towards the chosen profession is carried out. Although students' basic norms and values remained generally unchanged over time, disillusionment set in with regard to most of the expected professional rewards, in particular the anticipated 'intellectual challenge'. Unlikely, medical students who served as a comparison showed greatest disparity between expectation at admission and at graduation with regard to the economic side of their profession ('high income' and 'job security'). In spite of the growing cynicism of dental students throughout the years of education, apparent in the steady increase in expectations of extrinsic rewards and decreases in expectations of intrinsic rewards, these changes may be only situational in nature, for the same group of graduates showed a return to a more idealistic outlook of their profession 8 years post-graduation.

A longitudinal analysis of psychological adaptation among survivors of cancer.

This longitudinal study examined the psychological adaptation of newly diagnosed cancer patients. Findings showed a decline in patient's mental health status over time. Initial psychological status and other psychosocial factors, and not illness-related factors, were predictive of subsequent adaptation. Two psychologically vulnerable patient groups were identified: those who remained distressed over time and those whose mental health status declined over time.

Longitudinal analysis of clock drawing in Alzheimer's disease patients.

Longitudinal changes in the role of conceptual, spatial, and graphic processes in the drawing deficits of patients with Alzheimer's disease (AD) were assessed over a 2-year period. Both the drawing-to-command and copy conditions of the Clock Drawing Test were administered to 33 AD patients as part of three consecutive annual examinations. The drawings from each administration were evaluated for overall accuracy and the types of errors manifested. Although the patients showed a gradual decline in overall accuracy on both command and copy conditions over the three test sessions, they performed more poorly and evidenced a steeper decline on the command condition. Conceptual errors in the command condition were noted in the early stages of AD and increased over the three administrations. Stimulus-bound errors were usually associated with conceptual errors in the same or in a contiguous year. When the patients' performances on the first test session were correlated with their yearly scores on Mattis' Dementia Rating Scale, the presence of conceptual errors was associated with a steep decline on this measure of mental status. These results suggest that drawing deficiencies in AD often reflect impairments in conceptualization and semantic knowledge and that such losses may prove useful in predicting the disease's rate of progression.

Prevalence and stability of physical aggression between spouses: a longitudinal analysis.

Community couples (N = 272) were assessed in a longitudinal study of early marriage. More women than men reported physically aggressing against their partners at premarriage (44% vs. 31%) and 18 months (36% vs. 27%). At 30 months, men and women did not report significantly different rates of aggression (32% vs. 25%). However, using either the self-report or the partner's report, the prevalence of aggression was higher for women than men at each assessment period. Modal forms of physical aggression for both men and women were pushing, shoving, and slapping. Conditional probability analyses indicated that the likelihood of physically aggressing at 30 months given that one had engaged in such aggression before marriage and at 18 months after marriage was .72 for women and .59 for men. Furthermore, 25-30% of the recipients of physical aggression at all three assessment periods were seriously maritally discordant at 30 months.

The Willowbrook futures project: a longitudinal analysis of person-centered planning.

We conducted a longitudinal comparative evaluation of person-centered planning processes and outcomes for 20 individuals with intellectual disabilities and problem behavior (former residents of Willowbrook) and a matched contrast group, who received traditional interdisciplinary service planning (ISP). At the inception of the study, all participants were living in one of four other developmental centers (institutions) in New York City. Process and outcome data obtained from questionnaires completed by team members approximately every 8 months at four time periods showed that the rate of improvement in both person-centered planning process and outcomes for the intervention group was significantly greater than that of the comparison group. Eighteen of 19 person-centered planning participants moved to community living arrangements, as did 5 of 18 in the contrast group.

A gender-based dynamic multidimensional longitudinal analysis of resilience and mortality in the old-old in Israel: the cross-sectional and longitudinal aging study (CALAS).

The objective was to examine gender differences and similarities in health, function, familial and non-familial social networks; longitudinal resilience in those factors; and their association with risk of mortality in Israeli men and women aged 75-94. We used the Cross-Sectional and Longitudinal Aging Study (CALAS), a stratified random sample of 960 Israeli Jews aged 75-94, drawn on January 1, 1989 from National Population Registry, stratified by gender, age (75-79, 80-84, 85-89, 90-94), and place of birth (Europe/America, Middle East/North Africa, Israel), interviewed twice (Wave 1, 1989-1992; Wave 2, 1993-1995); Wave 1 values and longitudinal resilience predicted the 1999 mortality risk for those alive at both waves. Gender differences and similarities were found at Wave 1 in longitudinal resilience and in risk factors for mortality, partially supporting a gender paradox. Men were more physically active, had better cognition, gave more help to children, relied less on paid caretakers, and attended synagogue more than women, factors associated with better health and functioning. Women had poorer health and functional status and more help from children. More physical activity, synagogue attendance, and resilience in activities of daily living (ADL) were associated with lower risk of mortality for both genders. Women's risk of mortality was reduced by smoking reduction and higher cognitive vitality, and men's by emotional support and solitary leisure activity. Both men and women were resilient, yet there were differences. Gender-neutral mortality reduction programs would include physical activity, religious services, maintenance and improvement of ADL, and engaging in solitary leisure activities; for women, smoking cessation and cognitively challenging activities; and for men, maintaining or increasing emotional ties.

Dimensions and subtypes of recent-onset schizophrenia. A longitudinal analysis.

A sample of young patients with schizophrenia was studied in a longitudinal design to investigate the stability of an earlier reported four-dimensional symptom model (positive symptoms, negative symptoms, disorganization, and depression). Symptoms were measured in an acute phase soon after hospitalization, in a remitted stage 3 months later, and in a "mixed" phase 1 year after the second assessment. A different factor structure was found at each measurement, but a stable four-dimensional structure could be defined that closely resembled the factor structure of the first assessment. The symptom dimensions were uncorrelated, except disorganization and depression. Whether meaningful subtypes could be defined using statistical criteria also was investigated. With cluster analysis, four subtypes were defined that could be characterized as follows: rapid treatment responders, slower responders, and two relapse groups. Although some indication of the validity of these subtypes was found, it is concluded that there is a high risk of creating artificial boundaries with this approach.

Description and prediction of physical functional disability in psoriatic arthritis: a longitudinal analysis using a Markov model approach.

OBJECTIVE: To describe the longitudinal course of physical functioning in patients with psoriatic arthritis. METHODS: Between June 1993 and June 2003, 341 patients attending the University of Toronto Psoriatic Arthritis Clinic completed 2 or more Health Assessment Questionnaires (HAQs). At the time of administration of each HAQ, patients were assigned to 1 of 3 physical functional disability states, based on their HAQ score. A Markov model that allowed for transitions to and from these 3 disability states was used to characterize the longitudinal course of physical functioning, as well as to identify factors for both progression and regression of disability. RESULTS: Despite patient variability in the course of physical functioning, the following 3 longitudinal patterns were observed: 1) a stable state of disability throughout the entire study period, with 28%, 12%, and 6% of patients experiencing no, moderate, or severe disability, respectively; 2) a steady improvement or deterioration in disability over time (this pattern was observed in 27% of patients); and 3) a fluctuating state of disability, occurring in 27% of the patients. Sex, age, disease duration, number of actively inflamed joints, and number of deformed joints predicted transitions between disability states. CONCLUSION: Although 28% of patients appeared resistant to becoming disabled over the duration of this study, the remaining patients were observed either to experience enduring disability or to move between disability states.

Stability and accuracy of metamemory in adulthood and aging: a longitudinal analysis.

The stability and accuracy of memory perceptions in 2 longitudinal samples was examined. Sample 1 consisted of 231 adults (22-78 years) tested twice over 2 years. Sample 2 consisted of 234 adults (55-86 years) tested 3 times over 6 years. Measures of perceived and actual memory change were obtained. A primary focus was whether perceptions of memory change stem from application of an implicit theory about aging and memory or from accurate monitoring of actual changes in performance. Individual differences in metamemory were highly stable over time. Results suggested at least some accurate monitoring of memory in Sample 2, in which actual change was greatest. However the overall pattern of results is largely consistent with predictions derived from an implicit theory hypothesis.

Personality, work characteristics, and employee well-being: a longitudinal analysis of additive and moderating effects.

This study tested the longitudinal influence of personality (measured by the characteristics growth need strength, negative affectivity [NA], and upward striving) on 3 psychological outcomes (intrinsic work motivation, emotional exhaustion, and turnover intention), using a pattern of specific relationships between work characteristics and these outcomes as a framework. The study hypotheses were tested in a multioccupational sample consisting of bank employees and teachers, using a 2-wave panel design with a 1-year time interval and structural equation modeling. NA had a cross-lagged direct and additive relationship with emotional exhaustion and also moderated the relationship between Time 1 workload and Time 2 emotional exhaustion. The authors concluded that NA may have multiple effects on emotional exhaustion that persist over time.

Anxiety and depression after spinal cord injury: a longitudinal analysis.

OBJECTIVE: To examine the prevalence of anxiety and depression longitudinally in a sample of patients with a spinal cord injury (SCI). DESIGN: A prospective, longitudinal, multiple wave panel design with measures taken on 14 observational periods ranging from initial contact in the acute stages of hospitalization to 2 years' postdischarge to the community. SETTING: The National Spinal Injuries Centre, Stoke Mandeville Hospital, Aylesbury, UK, and the general community. PARTICIPANTS: The cohort consisted of 104 patients with traumatic SCI (19 women, 85 men), although the numbers assessed at each interval ranged from 5 to 85. MAIN OUTCOME MEASURES: Measures included the Beck Depression Inventory, the Beck Hopelessness scale, the State Anxiety Inventory, the functional independence measure, and the Social Support Questionnaire. RESULTS: When examined longitudinally, the data illustrate a consistent pattern of results across measures, with scores highest in the acute phase of the injury and during the months leading up to discharge. CONCLUSION: The numbers of persons scoring above clinical cut-off scores for anxiety and depression highlight the need to continue to ensure that appropriate psychological care is available within SCI rehabilitation settings. Moreover, the nature of the longitudinal results provides an indicator of subtle changes in anxiety and depression over time.

Depressive mood and adolescent illicit drug use: a longitudinal analysis.

Through the technique of Goodman's log linear method, longitudinal data are analyzed to clarify the relationship between depressive mood and illicit drug use among youths. A representative sample of adolescents (N = 8206) from New York State secondary schools was followed over one academic year. At one point in time, users of illicit drugs other than marihuana (multiple drug users) were significantly more depressed than either nonusers or users of marihuana only. Differing interactive relationships were found over time depending upon the drugs involved. Depressive mood was related to the onset of marihuana use among nonusers and to the termination of marihuana use among users. Depressive mood also predicted the use of other illicit drugs by marihuana users. While the beginning use of illicit drugs other than marihuana was positively associated with increased depressive mood, continued multiple drug use was related to reduced depressive mood.

Longitudinal analysis of serial measurements of free and total PSA among men with and without prostatic cancer.

OBJECTIVES: Evaluation of free and total serum prostate specific antigen (PSA) levels before diagnosis of prostate cancer. METHODS: Free and total PSA levels were measured on frozen sera samples of 26 men with no history of prostate disease (controls), 29 men with a histologic diagnosis of benign prostatic hyperplasia (BPH) made at simple prostatectomy (BPH cases), and 23 men with a histologic diagnosis of prostatic cancer (cancer cases). Longitudinal regression analysis was used to evaluate PSA levels as a function of years before diagnosis of prostate disease. RESULTS: On average, mean total serum PSA was statistically significantly greater for subjects with cancer (5.0 ng/mL +/- 0.9) versus BPH (2.8 ng/mL +/- 0.3) and control subjects (0.8 ng/mL +/- 0.1) by 4 years before diagnosis, whereas free PSA levels were similar among groups at 4 years before diagnosis. The ratio of free to total serum PSA continuously decreased among cancer cases over the decade before cancer diagnosis. At a time when mean total and free PSA levels were similar among groups (8 years before diagnosis), the ratio of free to total PSA was statistically significantly lower for cancer cases (0.13 +/- 0.01) compared with BPH (0.17 +/- 0.01) and control cases (0.21 +/- 0.02). Use of a free to total PSA ratio of < or = 0.12 when total PSA was between 4.0 and 10.0 ng/mL resulted in the highest sensitivity (76%) and specificity (94%) for diagnosis among subjects with and without cancer. Lowering the reflex range to 2.5 ng/mL increased false positive tests more than it increased sensitivity. CONCLUSIONS: The ratio of free to total PSA is the earliest serum marker predicting a subsequent diagnosis of prostate cancer. Measurement of the free to total serum PSA ratio would appear to reduce false positive results among men without prostate cancer.
