Seasonal variations in weight of children attending an under-fives clinic in Lesotho.

The incidence of weight loss between successive visits, prevalence of under centile 3 weight-for-age and over centile 97, showed a marked bimodal annual variation for 1243 children attending an under-fives clinic. 9949 weighings on 661 boys and 682 girls over a five year period, contributed to the database. Season of birth also significantly influenced the centile distributions of weight-attained-for-age. Children over 1 1/2 years old showed considerably higher rates of weight loss (about 1 in 5 children attending in their fifth year of life), than children younger than the mean age of weaning. The rates of weight loss, together with seasonal variation, was considerably higher for a group of 'Regular Attenders' to the clinic, than for a group of 'Low Attenders'. Seasonal influences on growth need to be taken into accounts in evaluating the quality of care and outcome produced by under-fives clinics.

Rheumatoid arthritis in Lesotho.

Thirty-nine black African patients were seen with probable, definite or classical rheumatoid arthritis (RA); 32 of these were seen in a prospective study out of a total of 15 834 new patients 15 years and older at presentation seen in a 12-month period. The diagnosis was based on the American Rheumatism Association criteria for RA or the Rome criteria for inactive RA. The high incidence of severe disease as well as the occurrence of advanced disease in young patients is notable. 80% of the patients came from rural areas. Radiological lesions were found in 74% and serological abnormalities in 92%. Rheumatoid factor (RF) was found to be positive in 12% of an age and sex matched control group and in 19% of a group of patients suffering from tuberculosis.

Highland and lowland populations of Lesotho.

It has not been possible to demonstrate significant sero-genetic differences between lowland and highland Sotho populations; the differences which do exist may well be attributable to random genetic drift. The study shows that the Sotho have received an appreciable genetic contribution from the San they have absorbed but their sero-genetic profile remains eminently Negro. A low frequency of the PTC non-taster allele was found (t = 0.142 +/- 0.029) as was the overall frequency for colour blindness (cb = 0.013 +/- 0.009).

Screening for cervical cancer in Butha Buthe, Lesotho. A study of Papanicolaou (Pap) smears in a previously unscreened community over a one-year period.

The results of 2896 cervical smears taken from 2713 women at the outpatient department of Butha Buthe Government Hospital between January and December 1981 are reported. A high prevalence of mild and moderate dysplasia (18 per 1000) and of frank invasive carcinoma (5.2 per 1000) was found in this previously unscreened population. The rate of carcinoma-in-situ was 2.8 per 1000. The positive results are compared with other studies and analysed according to age and parity. The problems of treating cervical cancer in a Third World country with limited facilities are revealed and a cost/benefit analysis is attempted. Criteria for selecting a risk group are examined.
