Is stone radiodensity a useful parameter for predicting outcome of extracorporeal shockwave lithotripsy for stones < or = 2 cm?

PURPOSE: Several factors determine the success of extracorporeal shockwave lithotripsy (SWL) for kidney stones: stone size, stone location within the collecting system, stone type, and the SWL machine used. It has been suggested that stone radiodensity, as determined either by plain radiography or computed tomography attenuation values, may be an independent predictor of SWL success. We examined the outcome of SWL for solitary stones less than or equal to 2 cm located within the renal pelvis, based on their radiodensity. MATERIAL AND METHODS: 211 patients with solitary renal pelvic stones measuring less than or equal to 2 cm were treated on a Dornier Doli 50 lithotriptor under general anesthesia. The radiodensity of the stone was determined to be either less than, equal to, or greater than the radiodensity of the ipsilateral 12th rib. Stone-free rates (SFR) were determined at 3 months by kidney, ureters and bladder (KUB) plain X-rays. Patients requiring re-treatment or auxiliary procedures were considered failures of SWL. RESULTS: Follow-up SFR information was available in all 211 patients. Stone composition was available in 158 (75%) treated patients, but no correlation was found between stone radiodensity and stone composition. For stones < or = 10 mm within the renal pelvis, the SFRs were similar (71 to 74% regardless of stone radiodensity). For stones between 11 and 20 mm, the SFR was 60% if the stone had a radiodensity > 12th rib compared to a SFR of 71% if the stone radiodensity was < or = 12th rib. However, these differences in SFRs were not statistically significant. CONCLUSIONS: On the Doli machine, stone radiodensity alone does not predict lithotripsy treatment outcome for stones < or = 1 cm within the renal pelvis. This parameter is probably only useful as the stone size becomes larger than 1 cm, and should be used in conjunction with other stone parameters to select appropriate therapy.

[A quality control of interpretation of rolled radiophotogramme used for mass screening of lung cancer in general habitants--cases difficult-to-diagnose and cases difficult-to-detect].

A mass screening for lung cancer was performed on 1,906,660 general habitants of Chiba Prefecture, using rolled radiophotogrammes (RP), a traditional mass screening method used for tuberculosis. Of those examined, 270 lung cancer cases were found. In order to evaluate the quality of RP interpretation, cases difficult-to-diagnose and cases difficult-to-detect were picked out from the detected lung cancer cases. Difficult-to-diagnose cases were defined as cases in which pulmonary tumor was not suspected in the screening. Among these cases, tuberculosis was the disease most frequently suspected. Thus, we reexamined the RP which were suspected as being tuberculosis or pulmonary tumor. There was no difference between the two diseases in terms of the location of pathological shadows. Isolated small nodular shadows and infiltrating shadows were seen as an indication of tuberculosis, while large nodular shadows were thought to indicate pulmonary tumor. In addition, by retrospectively examining the RP of the cases judged as normal in the screening of the previous year, tumor shadows could be found in 54.2% of the cases. These cases were considered to be difficult-to-detect cases of lung cancer. The tumor shadows of many cases were present in the hilar region in these re-examined X-ray films. It was difficult to recognize these shadows in the rolled X-ray films because small nodular shadows or infiltrating shadows overlapped with the opacities of normal structures. To overcome these problems of difficult-to-diagnose cases and difficult-to-detect cases, the following observations are important. (1) Care should be taken in order to obtain photogrammes with excellent quality.(ABSTRACT TRUNCATED AT 250 WORDS)

Valence-Dependent Metal-Metal Bonding and Optical Spectra in Confacial Bioctahedral [Re(2)Cl(9)](z)()(-) (z = 1, 2, 3). Crystallographic and Computational Characterization of [Re(2)Cl(9)](-) and [Re(2)Cl(9)](2)(-).

The geometric structure of the confacial bioctahedral [Re(2)Cl(9)](z)()(-) anion has been determined by single-crystal X-ray diffraction in two distinct oxidation states, Re(IV)(2) and Re(III)Re(IV). [Bu(4)N][Re(2)Cl(9)] crystallizes in the monoclinic space group P2(1)/m [a/Å = 10.6363(3), b/Å = 11.420(1), c/Å = 13.612(1), beta/deg = 111.18(1), Z = 2], while [Et(4)N](2)[Re(2)Cl(9)] crystallizes in the orthorhombic space group Pnma [a/Å = 15.82(1), b/Å = 8.55(2), c/Å = 22.52(3), Z = 4]. The Re-Re separation contracts from 2.704(1) Å in [Bu(4)N][Re(2)Cl(9)] to 2.473(4) Å in [Et(4)N](2)[Re(2)Cl(9)] (or, equivalently, from 2.725 to 2.481 Å after standard corrections for thermal motions), while the formal metal-metal bond order falls from 3.0 to 2.5. SCF-Xalpha-SW molecular orbital calculations show that, despite the {d(3)d(3)} configuration, the single sigma bond in [Re(2)Cl(9)](-) dominates the observed structural properties. For [Re(2)Cl(9)](2)(-), the 0.23 Å contraction in Re-Re is attributed jointly to radial expansion of the Re 5d orbitals and to diminished metal-metal electrostatic repulsion, which act in concert to make both sigma and delta(pi) bonding more important in the reduced species. Computed transition energies and oscillator strengths for the two structurally defined anions permit rational analysis of their ultraviolet spectra, which involve both sigma --> sigma and halide-to-metal change-transfer absorptions. The intense sigma --> sigma band progresses from 31 000 cm(-)(1) in [Re(2)Cl(9)](-) to 36 400 cm(-)(1) in [Re(2)Cl(9)](2)(-), according to the present assignments. For electrogenerated, highly reactive [Re(2)Cl(9)](3)(-) (where conventional X-ray structural information is unlikely to become available), the dominant absorption band advances to 40 000 cm(-)(1), suggesting further strengthening of the metal-metal sigma bond in the Re(III)(2) species.

[The possibilities of hyperthermia from an engineering standpoint].

The possibilities of hyperthermia are discussed from the engineering technology standpoint. Microwave equipment yields powerful effects against a superficial tumor. RF capacitive method and annular phased array system (APAS) are typical heating methods against a deep-seated tumor. We have proposed that in the RF capacitive method the diameter of the applicator electrode should be more than 1.5 times the distance between two electrodes. In APAS another exciting method (TE mode) has been proposed. Electric current flows through the short axis of the body by the TE mode excitation. We are developing re-entrant resonant cavity applicators which have the characteristics to heat cylindrical regions of small diameter. We can install an x-ray tube inside the re-entrant electrode. This method is suitable for deep and concentrated heating. We conclude that the recent rapid development of various kinds of hyperthermia apparatus makes it possible to heat more safely and effectively the deep-seated regions of the human body.

[A follow-up of x-ray-treated tuberous hemangiomas after 15-20 years (author.s transl)].

The treatment of hemangiomas with X-rays has been sharply criticized because of their tendency to involute spontaneously. It has been the aim of this catamnestic study to re-examine for X-ray injury those patients, whose hemangiomas were irradiated 15-20 years ago. The findings indicate that signs of roentgenoderma can appear already with 800 r and increase rapidly over 1,500 r. The observed irreversible damages, however, were mostly not grave, but admonish a certain amount of restraint. An assessment of the effectiveness of low X-ray doses still requires verification considering a faster spontaneous involution.

Measurements of nuclear data parameters of 201Tl by gamma-ray spectrometry.

201Tl is frequently used in radiopharmaceutical applications, and therefore the gamma-ray emission probabilities and half-life have been re-determined by means of gamma-ray spectroscopy. While the activity was calibrated using the sum-peak coincidence method, the half-life was obtained by the reference source method based on simultaneous counting of a reference source and the sample. Both the measurement techniques and assignment of uncertainties are presented and discussed, and the resulting data are shown to be in good agreement with previously published studies.

DNA polymerases: Hoogsteen base-pairing in DNA replication?

Human polymerase-iota belongs to the error-prone Y family of polymerases, which frequently incorporate incorrect nucleotides during DNA replication but can efficiently bypass DNA lesions. On the basis of X-ray diffraction data, Nair et al. propose that Hoogsteen base-pairing is adopted by DNA during its replication by this enzyme. Here I re-examine their X-ray data and find that the electron density is very weak for a Hoogsteen base pair formed between a template adenine deoxyribonucleotide in the syn conformation and a deoxythymidine 5'-triphosphate (dTTP), and that the fit is better for a normal Watson-Crick base pair. As a guanine-cytosine (G-C) base pair has no potential to form a Hoogsteen base pair at physiological pH, Hoogsteen base-pairing is unlikely to be used in replication by this polymerase.

Coal workers' pneumoconiosis: a study of prevalence in coal mines of eastern Madhya Pradesh and Orissa states of India.

With objective to find out prevalence of Coal Worker's Pneumoconiosis and variation among readers in reading x-ray plates for pneumoconiosis, a retrospective epidemiological survey of Coal Worker's Pneumoconiosis was undertaken in 72 collieries of Madhya Pradesh and Orissa by re-reading of x-ray plates taken during the Periodical Medical Examination at the Occupational Health Units over a period of 5 years. Six readers, trained abroad in reading pneumoconiosis x-ray plates, were involved for the study. Each reader reported approximately one sixth of the available x-ray plates of all the collieries and classified on the 12 point scale of I.L.O. (International Labour Organisation) 1980 in special format. Total 43,504 chest x-rays were reviewed. The overall prevalence was found to be 3.03%, ranging from 1.52% to 4.76% between 10 areas (group of mines). Major category of profusion was category-I (81.09%), followed by category-II (17.84%). Only 3 cases of Progressive Massive Fibrosis (PMF) were detected. Round shaped opacities are predominant (89.59%) in Coal Worker's Pneumoconiosis. Among the opacities, 'p' type is more prevalent (48.29%) followed by 'q' type (40.62%). There was variation amongst the different readers and ranged from 1.14% to 6.76% for reporting the prevalence of Coal Worker's Pneumoconiosis. However, when analysis of six readers for inter reader variation was conducted, that shows no abnormal deviation in the reading of any of the readers.

Cambial reactivation in locally heated stems of the evergreen conifer Abies sachalinensis (Schmidt) masters.

A study was made of cambial activity, the localization of storage starch around the cambium, and the localization and occurrence of microtubules in cambial cells from dormancy to reactivation in locally heated (22-26 degrees C) stems of the evergreen conifer Abies sachalinensis. Heating induced localized reactivation of the cambium in the heated portions of the stem. Erect ray cambial cells resumed cell division 1 d prior to the reactivation of fusiform cambial cells and procumbent ray cambial cells. The re-initiation of the division of fusiform cambial cells occurred first on the phloem side. During the heat treatment, the amount of storage starch decreased in procumbent ray cambial cells and in the phloem parenchyma adjacent to the cambium but increased in fusiform cambial cells. Preprophase bands of microtubules, spindle microtubules and phragmoplast microtubules were observed both in erect ray cambial cells and in procumbent ray cambial cells. By contrast, no evidence of the presence of such preprophase bands of microtubules was detected in fusiform cambial cells. The results suggest that the localized heating of stems of evergreen conifers might provide a useful experimental model system for studies of the dynamics of cambial reactivation in intact trees.

Fixation of the electrode array with bone paté in cochlear implant surgery.

The aims of this study are to report on our experiences using a bone paté (a mixture of bone dust and fibrin glue) fixation technique of the Combi 40/Combi 40+ electrode array during cochlear implant surgery. A retrospective study involving 57 patients implanted with Combi 40/Combi 40+ cochlear implants over 31 months fixated with bone paté was performed, utilizing our electrical fitting data as well as re-operation and X-ray findings. We found no signs of electrode retraction in any of the 57 patients. Electrical fitting data showed no reduction in the number of active channels between the first and last times patients were evaluated (a period of 4-31 months). Furthermore, a comparison of X-rays performed within the first week following cochlear implant surgery and again 7-27 months later showed no signs of electrode retraction in 10 patients. Three patients who required re-operations after implantation allowed us the opportunity to examine the effectiveness of the bone paté fixation and revealed new bone growth with the electrode fixed in place. Histological examinations of the newly built bone showed laminar and compact bone structures. The use of bone paté was thus found to be a suitable method of electrode fixation in cochlear implant surgery. Compared with other fixation methods, it is inexpensive and uses no foreign materials.

Coordination Chemistry of Low-Valent Rhenium Polypyridyl Complexes: Synthesis, Reactivity, and Electrochemistry.

The complex Re(III)(benzil)(PPh(3))Cl(3) (2) is used to synthesize a variety of Re(III) and Re(II) polypyridyl complexes of the type cis-[Re(III)(L(2))(2)Cl(2)](+), [Re(II)(L(2))(3)](2+), Re(III)(L(3))Cl(3), [Re(III)(L(3))(2)Cl](2+), and [Re(III)(L(4))Cl(2)](+), where L(2) = bpy (3and 6), tbpy (4 and 7), phen (5 and 8); L(3) = terpy (9and 10); L(4) = TMPA (11). The complex cis-[Re(III)(bpy)(2)Cl(2)](+) (3) is a useful synthon in the formation of complexes of the type [Re(bpy)(2)L(x)()](n)()(+) that are six- or seven-coordinate Re(III) complexes (13, 16, and 18) or octahedral Re(II) or Re(I) complexes (12 and 17). The [Re(III)(terpy)(2)Cl](2+) (10) complex can be reduced to form the Re(I) complex, [Re(I)(terpy)(2)](+) (21) and then electrochemically reoxidized to form new complexes of the type [Re(III)(terpy)(2)L](n)()(+). Similar behavior is observed for the [Re(II)(bpy)(3)](2+) (6) complex where [Re(III)(bpy)(3)((t)BuNC)](3+) (20) and [Re(I)(bpy)(3)](+) (19) may be formed. The electrochemistry of these complexes is discussed in relation to their reactivity and the observed pi-acidity of the polypyridyl ligands. In addition, X-ray crystal structures for cis-[Re(III)(bpy)(2)Cl(2)]PF(6) (3) and [Re(I)(bpy)(3)]PF(6) (19) are reported. cis-[Re(III)(bpy)(2)Cl(2)]PF(6) (3, ReC(20)H(16)N(4)Cl(2)F(6)P) crystallizes in the monoclinic space group C2/c with Z = 4 and lattice parameters a = 15.043(5) Å, b = 13.261(4) Å, c = 12.440(4) Å, and beta = 108.86(2) degrees at -100 degrees C. [Re(I)(bpy)(3)]PF(6) (19, ReC(30)H(24)N(6)F(6)P) crystallizes in the rhombohedral space group R&thremacr;c(h) (No. 167) with Z = 12 and lattice parameters a = 13.793(3) Å and c = 51.44(3) Å at -100 degrees C.

Biophysical mechanism of radiation damage to mammalian cells by X- and gamma-rays.

Published information on the reproductive death in mammalian cells irradiated by a wide range of X- and gamma-ray energies has been re-analysed to extract intrinsic efficiencies of damage for the secondary electrons in transient equilibrium. On a log-log plot, a linear dependence on the track average l.e.t. and the average specific primary ionization is found, indicating that either serves as a good quality parameter. The soft X-ray data are consistent with this conclusion. Upon comparison with data for fast heavy ion irradiations, the average specific primary ionization is shown to be applicable independently of radiation type whereas track average l.e.t. is not. Furthermore it is revealed that electrons are most damaging near the end of their range but their efficiency is only about 10-20 per cent of that of fast ions at the same quality, possibly due to the influence of multiple scatter on the electron penetration depth. It is deduced that, for the dose rates involved, the damage by electrons is predominantly by intra-track action and not inter-track action. The results are consistent with the suggestion that optimum damage occurs when the mean free path between ionizations is equivalent to the strand separation in the double-stranded DNA.

Significance of plasmalemma disruption in bovine and equine spermatozoa.

We have investigated fresh and cryopreserved bovine and equine spermatozoa using light and transmission soft X-ray microscopy. Spermatozoa were examined, in the presence or absence of semen, after using Percoll gradient centrifugation and re-suspending in medium. X-ray microscopy provided high resolution (30 nm) transmission images of whole cells in solution with high contrast, while retaining the simple preparation techniques used in light microscopy. We demonstrated translucent, membrane-bound vesicles in the acrosomal and midpiece regions that were similar in size and we noted their incidence in both fresh and frozen-thawed material from both animals. The vesicles were formed by the separation and expansion of the plasmalemma away from the underlying structure but were not caused by the freeze-thaw process. We suggest that these structures form part of the normal ultrastructure of spermatozoa and are damaged during preparation of the samples for transmission electron microscopy, resulting in a structure previously and incorrectly identified as damaged by the freezing and thawing process.

Follow-up results 20 years after surgical treatment of habitual ventral shoulder luxation using the Eden-Lange technique.

After undergoing an operation according to the Eden-Lange technique, 56 patients with recurrent anterior shoulder luxation were re-examined clinically at an average follow-up period of 20 years. The mean age of the patients at the time of the operation was 24.9 years; the ratio of male to female patients was 2.3:1. The Ellman score (maximum of 27 points), stability tests, reluxation rate and X-ray examinations served as re-examination criteria. The mean score for the Ellmann test was 18.3 points; 29 patients had ventral instability. In the meantime 11 shoulders had redislocated. Fifty-one patients had arthrotic alterations in the operated shoulder. Because of the unsatisfactory functional and radiographical results, the Eden-Lange technique can no longer be accepted as the only treatment of choice for anterior shoulder dislocation.

Re(CO)(3) complexes synthesized via an improved preparation of aqueous fac-[Re(CO)(3)(H(2)O)(3)](+) as an aid in assessing (99m)Tc imaging agents. Structural characterization and solution behavior of complexes with thioether-bearing amino acids as tridentate ligands.

Parallel studies of the preparation of Re and (99m)Tc agents aid in interpreting the nature of tracer (99m)Tc radiopharmaceuticals. Aqueous solutions of the fac-[(99m)Tc(CO)(3)(H(2)O)(3)](+) cation are gaining wide use and are readily prepared, but such solutions of the fac-[Re(CO)(3)(H(2)O)(3)](+) cation (1) are not so easily accessible. Herein we describe a new, reliable, and straightforward preparation of aqueous solutions of 1, characterized by HPLC and ESI-MS. Treatment of solutions of 1 with thioether-bearing amino acids, AAH = S-methyl-l-cysteine (MECYSH), S-propyl-l-cysteine (PRCYSH), and methionine (METH), gave high yields of fac-Re(CO)(3)AA complexes. X-ray crystallographic and NMR analyses indicated that MECYS(-), PRCYS(-), and MET(-) were bound in fac-Re(CO)(3)AA complexes as tridentate monoanionic ligands through amino, thioether, and alpha-carboxyl groups. In CD(3)OD, (1)H NMR spectra have broad signals but have two sets of signals at -10 degrees C, consistent with two isomers with different configurations at the pyramidal sulfur; these interconvert slowly on the NMR time scale at low temperatures. Indeed, the crystal structure of the fac-Re(CO)(3)(PRCYS) reveals a mixture of the two possible diastereoisomers. S-(Carboxymethyl)-l-cysteine (CCMH(2)) and 1 gave two products, 5A (kinetically favored) and 5B (thermodynamically favored). X-ray crystallographic analyses of a crystal of 5B and of a 1:1 cocrystal of 5A and 5B showed that 5A and 5B are diastereoisomers with the CCMH(-) alpha-carboxyl group dangling. In addition to the amino and thioether groups, the S-(carboxymethyl) carboxyl group is coordinated, a feature that slows interconversion of diastereoisomers relative to the other fac-Re(CO)(3)AA complexes because interconversion can now occur only after the rupture of Re-ligand bonds. These N, O, and S tridentate adducts are quite stable, and the grouping has promise in (99m)Tc(CO)(3) tracer development.

[Longevity factors in total trapezometacarpal prostheses].

We report the long term outcome of the trapeziometacarpal prosthesis in a personal series of 13 cases with a follow-up ranging from 12 to 17 years. This ball and socket prosthesis with a metal on polyethylene pairing is cemented. Several modifications were performed along the years: such as increased range of motion, better fit between the metacarpal medullary canal and the proximal part of the stem and addition of a versatile, intermediate component in order to re-establish length of the first ray. The status of the metacarpo-phalangeal joint is critical. Natural hyperextension of this joint has been seen in about two-third of th normal individuals. If the metacarpal component become loose, the thumb column become shorter and hyperextension of the metacarpophalangeal joint is followed by an adduction contracture of the metacarpal which produces a shear force component on the cup and finally the loosening of the cup (sequential loosening). Keeping or re-establishing the length of the first ray is the major principle in the trapeziometacarpal prosthesis. On the other hand, metacarpophalangeal arthrodesis is a contraindication for a trapeziometacarpal prosthesis. Trapeziometacarpal prosthesis should only be used as a last resort when there is severe pain. If the dominant hand is involved, intensive use is a contraindication. The trapezium must be large enough to accommodate the cup. The author recommends a posterior approach which preserve the anterior ligaments and allows a proper bone resection and a good positioning of the cup.
