Family physicians' growing attention to the nature of their patients' live should include the social and cultural factors that influence patient health and illness behavior. Patient visits to a family practice residency program were found to be influenced by the patients' beliefs about symptoms and the beliefs of their significant others, and symptom interference with valued activities. Data from physician-patient encounters suggest that physician attention to such sociocultural information as occupation and family structure may have positively influenced rapport. Results from this pilot study confirm the feasibility of observational research by physician-behavioral scientist teams in a primary care setting.
Interested family physicians with proper training and minimal extra equipment can now employ transvenous pacemaking. Some cardiac centers have developed training programs for general internists and family physicians. We believe that this valuable procedure can, with proper physician training, be offered to patients in rural areas as well as in cardiac centers. While the number of patients in this rural family practice who require this procedure has not been great (ten patients in four years), the need is often urgent when it does occur. Use of this treatment modality with consultation in a 44-bed hospital was found to be safe and practical, although sometimes technically difficult. This paper describes the training, techniques used, results, and complications in a rural community in southeastern Nebraska. It is suggested that every hospital caring for seriously ill cardiac patients should have at least one physician trained to perform this emergency procedure.
Family physicians may lack discriminatory ability to differentiate normal aging from disease states. To assess such ability, 53 aging-related indicators or symptoms were presented to 65 physicians in three family practice residency programs. Respondents classified each symptom as normal aging or disease. On average, residents classified 73.4% of symptoms correctly. They were more likely to classify disease states correctly (80.0%) than to classify signs of normal aging correctly (66.8%). Misattribution of normal aging signs as disease may prompt physicians to overmedicate and overtreat patients, resulting in adverse clinical outcomes.

Family practice is a very new medical specialty in Turkey. Family practice residency programmes have been attended since 1985 only in state hospitals and in 1995 departments of family practice will be funded in universities, too. In this article, we review the problems of family practice in Turkey and we mention our own opinions and comments about various aspects of it.

Three years ago, our residency program began a new approach to teaching practice management to our second- and third-year residents. The underlying principles for the new curriculum involved a realization that our residents lacked basic business understanding and that they would likely learn more effectively through a hands-on approach. The new curriculum, which we describe in this article, is in large part built around the establishment of a mock practice during the second year of residency. Although the curriculum is still evolving, initial response and evaluation have been encouraging.
