[Organization of antituberculosis care under the new economic conditions].

The organizational forms of antituberculosis care now applied fail to promote the achievement of better final results at minimal expenses of material and personnel resources. New principles of financing of the antituberculosis service are required as well as the assessment criteria of its activity and changes in the antituberculosis aid tactics. The principle of separate financing of long-term tuberculosis-related programs opens perspectives for the improvement of the organizational forms of antituberculosis aid. It is required that the preventive antituberculosis measures exercised among the whole population be conducted in groups of population subjected to a high risk of tuberculosis infection.

[Organization of the training of research personnel and specialists for public health service in the new economic conditions based on the Moscow Research Institute of Tuberculosis, RSFSR Ministry of Health].

The tasks of high priority set before the main establishment consist in training of research personnel and improving the professional skill of doctors. The Moscow Research Institute of Tuberculosis, which is in charge of the antituberculosis service in the RSFSR, has developed an effective system of training of research personnel and various specialists for the RSFSR health care. The system is based on the study of demands for highly qualified staff, analysis of the personnel potential, clearly defined planning of training, choice of candidates, control and assessment of the performed research and objective personnel certification. The contract system of self-accounting relations between the establishments engaged in doctors' training has been introduced, which regulates a number of ungrounded claims for training and increases the responsibility of researchers concerned with training.

The effect of socio-economic conditions on the time of diagnosis and compliance during treatment in growth hormone deficiency.

In 78 patients with idiopathic growth hormone (GH) deficiency the effect of the fathers' educational level on the age and the extent of growth retardation at diagnosis was studied. There was a tendency for an increase of the age and the degree of growth retardation with the decrease of the fathers' completed grades. The occurrence of height SD scores less than -4.5 was conversely related to the number of grades completed by the father (chi 2 = 19.2 p less than 0.001). Eighteen of the 70 patients treated with growth hormone discontinued treatment after 0.3 to 6 years. Compliance was closely related to the grades completed by the father (chi 2 = 24.7 p less than 0.001). Six out of 7 patients with a height SDS less than -4.5 at diagnosis and with a father of low level of education (less than 8 grades) became non compliant. It is concluded that the degree of growth retardation at diagnosis and compliance at treatment in GH deficiency is related to the educational level of the father.
