The paper shows it necessary to define clinical physiology of the digestive system as a nosological entity due to the natural evolution of physiology and to the increasing clinical practical requirements. It considers its purpose, tasks, and areas under study and gives the results of some original studies of the gastroduodenal and hepatobiliary systems and the small intestine. Special emphasis is laid on new guidelines to examining the functional and morphological states of the alimentary tract.
BACKGROUND: The toxicological interpretation of blood and urine concentrations of drugs affecting the digestive system and metabolism, as with other groups of pharmaceutical specialties, is complicated. METHODS: As a continuation of our published studies on the concentrations of drugs of abuse, drugs affecting cardiovascular and hematopoietic systems, and respiratory system, we have reviewed the published data relating to the concentrations of drugs affecting the digestive system and metabolism in clinical and forensic cases. We have selected them on the basis of conservative criteria and our own experience. RESULTS: A table with concentrations of 101 drugs in whole blood, serum/plasma, and urine, corresponding to therapeutic, toxic, or lethal concentrations, is presented. CONCLUSIONS: The data can help interpret analytical results of patient or postmortem samples when there is a suspicion of poisoning with this group of drugs.
Zinc (Zn)--the important atomavit (essential chemical element), which participates in 120 reactions proceeding in a living organism. There were been found the abnormal Zn-levels in blood in the cases of the various pathologies, including malignant tumors (MT) of the digestive systems. It has been determined the Zn-levels in the blood serum of 48 patients with MT of the digestive systems (nosologies: 24--MT of stomachs, 10--esophagus, 14--colorectum) and 12 practically healthy persons. The results have been analyzed using binomial criteria (alpha=0.05; p<0.05). It has been found that in patients with MT of digestive systems increase or decrease of Zn-levels in the blood serum occurs with equal probability. That seems to reflect the peculiarities of functional disorders in mucous membranes, through which the Zn-absorption occurs.
Examination of lower part of digestive system by colonoscopy is necessity in occult fecal hemorrhage occurrence, hematochesis, unresolved loss of blood iron and suspicious finding of large bowel X-ray barium study. By therapeutical colonoscopy, haemorrhage from neoplasms and angiodysplastic changes are controlled, foreign bodies are removed as well as routine polipectomics. Balloon dilation of large bowel stenosis, and palliative treatment of unoperable stenoses lead to improvement in quality of life with significant cost-benefit effects. Follow-up of patients on high-risk for colorectal neoplasms increased early diagnosis of neoplasms.

Digestive tract cancers are infrequent during pregnancy. Prognosis is usually dismal both because diagnosis is delayed as a result of symptoms being ascribed to the pregnancy and because saving both the mother and fetus is difficult. A review of the relevant literature, focused on colorectal and gastric cancers which are the most common forms, emphasizes the need for early diagnosis and for adjusting treatment to gestational age at diagnosis.
The article presents an evaluation of the state of the digestive system in astronauts during short-term and long-term space flights. After short-term (7- or 8-day) flights, there was a transitory activation of gastrointestinal enzyme systems. After prolonged space flights, there was an increase in gastric acidity and pepsin excretion, as well as pancreatic and intestinal malfunctions. In summary, there is a transition of the digestive system to a new level of functioning.
The distribution of lipase (EC 3.1.1.3) in the different parts of the digestive system of Bagarius bagarius, Mastacembelus armatus, Glossogobius giuris and Rohtee cotio has been studied. In the stomach of Mastacembelus, Bagarius and Glossogobius the lipase activity has been found in the mucosa and gastric glands. The intestinal pulb and the intestine of herbivorous Rohtee and in the intestine of the rest of the three fishes show the enzymatic activity in the mucosal epithelium. However, in Mastacembelus and Glossogobius the lipase activity is more pronounced in the intestinal mucosa than that in Bagarius and Rohtee. In the liver, the enzyme is distributed in polyhedral hepatic cells in all the fishes studied specially around the nuclei of the hepatic cells. In the pancreas of Rohtee, the enzyme is distributed in the acinar cells and the cells of islets of Langerhans. The serosa and muscularis of the different portions of digestive system of the four fishes studied are devoid of any lipolytic activity.

Rosai-Dorfman disease (RDD), originally described as sinus histiocytosis with massive lymphadenopathy, is a rare histiocytic proliferative disorder with a distinctive microscopic appearance. Formerly thought to be a process limited to lymph nodes, involvement by RDD has now been documented in many organ systems, notably bone, skin and soft tissue, central nervous system, eye and orbit, and upper respiratory tract. The digestive system, however, is affected only exceptionally, as reflected by the existence of only a handful of individual case reports. In this article, we report 11 patients in which the disease involved intestinal tract, liver, or pancreas, and describe the most salient clinicopathologic features. The specific site of involvement within the digestive system was gastrointestinal tract in 5, liver in 5, and pancreas in 1. Most patients also had evidence of disease in other extranodal sites, as well as in 1 or more lymph node groups.
Fifteen digestive duplications, corresponding to fourteen patients, have been surgically treated in "La Paz" Pediatric Hospital between 1966 and 1984. The most common localization (40%) was the large bowel. The first choice treatment was the complete excision of lesions, with led to cure of all patients. Embryologic theories and clinical forms are also reviewed.
AIM: To characterize p62 expression and define the relationship between p62 expression and cell proliferation in primary carcinomas of the digestive system. METHODS: p62 expression was characterized in surgically resected tumor specimens from 60 patients with primary carcinomas of the digestive tract (including 22 esophageal carcinomas, 17 gastric carcinomas, and 21 colorectal carcinomas) and 40 patients with hepatocellular carcinoma (HCC) by immunohistochemistry (IHC). The cell proliferation was determined by IHC of Ki-67 in 40 patients with HCC. RESULTS: Twenty-two cases of esophageal carcinoma were histopathologically diagnosed as squamous cell carcinoma. We combined the gastric and colorectal carcinomas based on the equivalent histology. The 38 tumors in the combined groups, consisted of 17 well-differentiated, eight moderately differentiated, nine poorly differentiated carcinomas, and four mucinous adenocarcinomas. According to the criteria of Edmondson and Steiner, 40 patients with HCC were graded (2 grade I, 17 grade II and 21 grade III). p62 expression in primary carcinomas of the gastrointestinal tract (60/60,100%) was higher than that (27/40, 67.5%) of HCC (P<0.01, chi(2) = 19.63). High expression levels of p62 were positively correlated with histological grades in gastric and colorectal carcinomas (P<0.0001) and inversely associated with those in HCC (P = 0.0322). No significant correlations were observed for esophageal carcinomas (P = 0.8246). p62 expression was also detected in the cytoplasm of morphologically normal columnar epithelial cells adjacent to the cancer foci of gastric and colorectal carcinomas. In 40 HCC specimens, the mean Ki-67 labeling index (LI) was (19.6+/-16.0)%. It was (28.3+/-18.73)% in 12 cases with high p62 expression (+++), (7.53+/-14.83)% in 13 cases without p62 expression(-). Patients with a high p62 expression showed a significantly higher level of Ki-67 staining than those without p62 expression (P<0.05, t = 2.069). CONCLUSION: p62 expression is common in carcinomas of the digestive system and higher in carcinomas of the gastrointestinal tract than in primary HCC. p62 is a cellular differentiation-related protein. Cancer cells with a high p62 expression exhibited highgrowth fractions in HCC.
The focus of the presentation will review the distribution of nitric oxide (NO)-producing sites in the digestive system in mammals and nonmammalian vertebrates and will center on the roles that NO plays in modulating physiological and pathophysiological functions in digestive system.
The distribution of endo-beta-1,4-glucanase (EG) components in the digestive system of the wood-feeding termite, Coptotermes formosanus Shiraki, was investigated by zymogram analysis using polyacrylamide gel electrophoresis, followed by N-terminal protein sequencing. EG components similar to glycoside hydrolase family (GHF) 9 members were restricted to the salivary glands, the foregut, and the midgut, whereas components similar to GHF7 members were confined to the hindgut where numerous cellulolytic flagellates were harbored. RT-PCR experiments revealed that five GHF9 EG mRNAs (1348 bp) homologous to other termite EGs were expressed in the salivary glands and the midgut. The crude extract prepared from the midgut as well as that from the hindgut produced glucose from crystalline cellulose. These data suggest that C. formosanus has two independent cellulose-digesting systems: one in the midgut where cellulose digestion is accomplished by endogenous cellulases and the other in the hindgut which makes use of other cellulases possibly from symbiotic flagellates.
