[Studies of the lymph drainage of the eye. 4. Drainage of lymphotropic radioactive tracers (99mTc-Microcolloid) after intravitreal injection (author's transl)].

In 7 rabbits the drainage of 99mTc-labelled Micro-colloid from the vitreous body of the eye was studied. In vivo measurements with an Anger-type camera were undertaken as well as in vitro measurements after dissection and excision of tissue-probes. From the data followed that indicator's elimination half-life from the vitreous body was on an average 7.8 hours. The drainage occurred partially along structures which are associated to the optic nerve. Furthermore drainage to the ipsilateral retrobulbar space and to the regional cervical lymph nodes, mainly to the bilateral deep cervical lymph nodes could be observed.

Pleural effusion in cancer patients. A prospective randomized study of pleural drainage with the addition of radioactive phsophorous to the pleural space vs. pleural drainage alone.

Sixty-seven patients with disseminated cancer were randomly allocated to treatment with continuous closed chest drainage removing all fluid for 72 hours (PD) or pleural drainage for 72 hours with the instillation into the pleural space of radioactive colloidal chromic phosphate (PD + 32P). Forty-nine patients had breast carcinoma, and the remaining 18 patients had other cancers. Four of 49 patients with breast cancer and 13 of 18 with other cancer were dead in 8 weeks from the onset of effusion. In the group of patients with breast cancer PD + 32P controlled the effusion in 12 of 22 (54%) and PD alone in 15 of 30 episodes (50%). In the nonbreast group of patients PD + 32P controlled the effusion in five of six evaluable episodes (83%), and PD alone was successful in two of nine (22%). In 33% of breast cancer patients and 25% of the nonbreast-cancer patients, systemic chemotherapy produced objective remissions. Pleural effusion did not recur in any of these patients.

The effect of drainage tube size on adequacy of percutaneous abscess drainage.

Fifty-one patients with documented abdominal abscess cavities were treated by percutaneous abscess and fluid drainage (PAFD). Drainage catheters made of various materials in sizes ranging from 5 through 18 French (Fr) were retrospectively studied and prospectively assigned to patients. No significant difference in the success or failure of PAFD as a function of these factors was found once an 8.3 Fr catheter with 0.045-inch diameter side-holes was reached; catheters larger than this were not associated with improved patient outcome. Failues of PAFD occurred primarily with the presence of phlegmonous collections and cavities with fistulous connection to bowel.

[Pancreas transplantation with bladder and intestinal drainage technique with systemic-venous and initial experiences with portal venous drainage. Which technique can be recommended today?].

The experience worldwide with pancreas transplantation includes more than 9000 cases. In the most popular patient group (simultaneous pancreas-kidney transplantation, SPK) the graft-function rate at 1 year now reaches more than 80%. More than 90% of all pancreas transplants were performed using the bladder-drainage technique (BD) with systemic venous anastomosis. Late problems with the BD require enteric conversion in 10-20% in the long term. In the present study we report the results of 40 SPK using the BD (n = 20) and enteric drainage (ED) techniques (n = 20). After 3 months, the patients pancreas and kidney survival rates in the BD group were 100, 95 and 100% and in the ED group 100, 100 and 100%, respectively. No anastomosis leakage occurred in either group. In two cases of the ED group the pancreas grafts were drained portal venously via the superior mesenteric vein without surgical complications. With growing experience, excellent SPK results can be reached with both the BD and ED techniques.

Placement of endoscopic naso-biliary drainage does not preclude subsequent percutaneous transhepatic biliary drainage.

We prospectively investigated whether the placement of endoscopic naso-biliary drainage (ENBD) precluded percutaneous transhepatic biliary drainage (PTBD). In 40 patients, the caliber of the intrahepatic bile duct was measured prior to ENBD by ultrasonography. When PTBD was required after ENBD, the ENBD catheter was clamped for 1 to 2 h before PTBD, and its caliber was again measured at the time of PTBD. When PTBD was performed within 7 days (mean, 1.8 days) after ENBD (n = 27), the size of the intrahepatic bile duct was 5.0 +/- 2.3 mm before and 4.6 +/- 2.3 mm after ENBD. There was no significant difference between these values (P > 0.5). When PTBD was performed 8 to 40 days (mean, 17.8 days) after ENBD (n = 13), the bile duct diameter was significantly reduced, from 4.2 +/- 1.5 mm (pre-ENBD) to 1.8 +/- 1.7 mm (post-ENBD) (P < 0.05). When PTBD was conducted within 7 days (mean, 1.8 days) after ENBD, previous ENBD did not induce collapse of the bile duct, if the ENBD catheter was clamped for 1 to 2 h before the puncture of the bile duct.

Endoscopic ultrasound drainage of pancreatic pseudocyst: a prospective comparison with conventional endoscopic drainage.

BACKGROUND AND STUDY AIMS: Pancreatic pseudocysts are a complication in up to 20% of patients with pancreatitis. Endoscopic management of pseudocysts by a conventional transenteric technique, i. e. conventional transmural drainage (CTD), or by endoscopic ultrasound-guided drainage (EUD), is well described. Our aim was to prospectively compare the short-term and long-term results of CTD and EUD in the management of pseudocysts. PATIENTS AND METHODS: A total of 99 consecutive patients underwent endoscopic management of pancreatic pseudocysts according to this predetermined treatment algorithm: patients with bulging lesions without obvious portal hypertension underwent CTD; all remaining patients underwent EUD. Patients were followed prospectively, with cross-sectional imaging during clinic visits. We compared short-term and long-term results (effectiveness and complications) at 1 and 6 months post procedure. RESULTS: 46 patients (37 men) underwent EUD and 53 patients (39 men) had CTD. The mean age of the entire group was 50 +/- 13 years. There were no significant differences between the two groups regarding short-term success (93% vs. 94%) or long-term success (84% vs. 91%); 68 of the 99 patients completed 6 months of follow-up. Complications occurred in 19% of EUD vs. 18% of CTD patients, and consisted of bleeding in three, infection of the collection in eight, stent migration into the pseudocyst in three, and pneumoperitoneum in five. All complications but one could be managed conservatively. CONCLUSIONS: No clear differences in efficacy or safety were observed between conventional and EUS-guided cystenterostomy. The choice of technique is likely best predicated by individual patient presentation and local expertise.

Percutaneous drainage of 335 consecutive abscesses: results of primary drainage with 1-year follow-up.

Retrospective review of percutaneous abscess drainage (PAD) of 335 abscesses in 323 consecutive patients was undertaken. Particular attention was directed to body location, associated organ system, communications and fistulae, and to the underlying immunologic status of the patient. One-year follow-up was available in all patients. Overall, the cure rate was 62.4% (209 of 335 abscesses), with a failure rate of 8.95% (30 of 335 abscesses). There were 14.2% (46 of 323 patients) deaths in the follow-up period, of which 4.6% (15 of 323 patients) were believed attributable to sepsis or septic complications. The overall complication rate was 9.8% (33 of 335 abscesses), most of which were minor in nature. For the patient exhibiting immunocompromise, representing 53.1% (172 of 323 patients) of the patient population, the cure rate was 53.4% (95 of 178 abscesses), which was significantly lower than the cure rate of 72.6% (114 of 157 abscesses) for the immunocompetent patient population (n = 151) (P less than .001). The recurrence rate was 2.1% (seven of 335 abscesses), with all recurrences within 3 months of initial drainage. PAD is effective and permanent treatment for both immunocompromised and immunocompetent patients.

Short-term effects of postural drainage with clapping vs autogenic drainage on oxygen saturation and sputum recovery in patients with cystic fibrosis.

To compare the short-term effects of postural drainage with clapping (PD) and autogenic drainage (AD) on oxygen saturation, pulmonary function, and sputum recovery, we studied ten patients with cystic fibrosis (CF) randomly treated with PD or AD on separate days. Pulse oximetry was monitored and sputum was collected during and for 1 h following each treatment. Pulmonary function was measured before and then 1, 15, and 60 min after each treatment. There was no significant difference in the amount of sputum recovered with AD (14.0 +/- 3.5 g) vs PD (10.4 +/- 3.0 g) and no significant differences in pulmonary function occurred. Oxygen saturation during PD fell from 93.3 +/- 0.7% to 91.2 +/- 0.8% (p < 0.01) and required 15 min following treatment to return to baseline. Oxygen saturation did not fall during AD and increased to 94.5 +/- 0.7% by 1 h following treatment (baseline, 93.3 +/- 0.8%; p < 0.01). We conclude that AD is less likely to produce oxygen desaturation and may be better tolerated by patients with CF, while producing similar benefits in sputum clearance.

[Drainage of the thoracic duct and changes in myocardial lymph drainage under conditions of electrothermocoagulation of the coronary artery].

It was demonstrated in dog experiments that the drainage function of the heart lymphatic system and coagulation of the central lymph are disturbed in acute myocardial infarction. Drainage of the thoracic duct with diversion of the flow of the toxic lymph fails to correct the diminished rate of lymph flow in the myocardium and does not prevent the increase of the hypercoagulation shift in the lymph. It is concluded that lymph stimulation and correction of the disorder of the lymph coagulation potential are necessary for effective pathogenetic treatment of myocardial infarction.

[Acute hemorrhagic pancreatitis--extended abdominal drainage and perirenal drainage links].

Acute haemorrhagic pancreatitis, when mainly situated in the body and tail of the pancreas, may be associated with abscess formation in the left perirenal region through the radix mesosoli transversi. Next to the extensive drainage of the abdomen and pancreas, we advise supplementary primary left perirenal-retroperitoneal drainage, considering how difficult it can be to loalize a secondary postoperative pancreatic abscess, especially when a computer-assisted tomography (CAT) is unavailable. In those cases in which the transverse colon had suffered circulatory damage from the acute pancreatitis, good results have been obtained with temporary coecostomy.

Proximal gastric vagotomy without drainage and selective gastric vagotomy with drainage for surgical therapy of duodenal ulcer: a retrospective study.

During a 3-year period proximal gastric vagotomy without drainage and selective gastric vagotomy with drainage were performed in 61 patients with duodenal ulcer. Of these, 57 patients were followed for 3-6 years. 77% were symptom-free (Visick I); 8,3% were improved but still have periods of dyspepsia (Visick II) and 14% were failures because of recurrent ulcer (Visick III). There were seven duodenal recurrences in the bulb, and one prepyloric recurrence. There were no operative deaths or major complications. The side effects, like diarrhoea and dumping, after proximal gastric vagotomy and selective vagotomy were mild and rare. The majority of our patients gained their ideal body weight within the first six months from surgery. Blood chemistry did not show any deficiency in haemoglobin secondary to vagotomy, but plasma basal level of gastrin was constantly higher after surgery. It is concluded that 3-6 years after proximal gastric vagotomy and selective gastric vagotomy for duodenal ulcer there was a 14% recurrence rate, but the absence of mortality, severe complications or significant side effects seems to be at least as important as the high recurrence rate.

[Gravity suction drainage--an alternative to suction drainage in trauma surgery? A prospective randomized comparison in knee operations and hip joint alloarthroplasty?].

In two prospective randomized studies the common drainage system according to Redon was compared with the gravity system according to Robinson in 80 patients undergoing elective knee joint surgery and in 60 patients undergoing total hip replacement. By use of clinical criteria such as output volume and rate of hematomas in both groups no statistically significant difference could be established. However, there is a tendency to a lower hematoma rate in patients with knee joint operations and treated with the Robinson system. On the other hand after total hip replacement a tendency to lower hematoma rate was observed in the Redon group. These differences can be explained by the different output volumes after hip and knee joint operations. Probably the Robinson system cannot drain as much volume as the Redon system. A further explanation could be that after hip replacement an external compression cannot be sufficiently performed compared to the knee joint operations. Furthermore the remaining subfascial cavity after hip replacement should be drained with suction in order to reduce the extent of the cavity. This point of view is supported by the observation that the gravity system seems to be only an overflow one in clinical practice. The main advantage of the Robinson system is the significant lower pain score when removing the drainages and even when the drains are lying in situ. This advantage, however, is possibly a relative one, when the Redon drains are removed without suction. So the postulation that the Robinson system should be generally introduced in orthopedic surgery cannot be accepted. At the moment there are indications for both systems.(ABSTRACT TRUNCATED AT 250 WORDS)

Impact of industrial and mine drainage wastes on the heavy metal distribution in the drainage basin and estuary of the Sado River (Portugal).

This paper presents results from a survey of the heavy metal distribution in sediments in the drainage basin and estuary of the Sado River (Portugal). In the drainage basin, heavy metals originate mostly from pyrite outcrop erosion and mining activities (Cd, Zn, Cu and locally Hg, Pg), and also from crust erosion (Sn, Ni, Ti, Zr). These sources are not correlated with the particulate organic carbon (POC) and so the metals are thought to be in inorganic forms in this area. Anthropogenic heavy metal sources (urban and industrial) are found in the lower estuary (Sn, Cd, Hg, Zn, Pb and Cu) along with high POC concentrations. In this zone, these metals are thought to be strongly adsorbed onto organic particles. Furthermore, organo-metallic species are likely to be present, as demonstrated in the case of Sn, since methyl- and butyl-tin species were detected in sediments from this area. This suggests the need for the detection of organo-metallic species to understand the heavy metal geochemical cycles. No long-term changes in metal concentrations are found in sediment cores, except in the middle estuary (Zn, Cu) due to the development of mining activities on an industrial scale in the 1860s.

Partial or total anomalous pulmonary venous drainage caused by malposition of septum primum: echocardiographic description of a rare variant of anomalous pulmonary venous drainage.

We describe the echocardiographic findings in 9 patients, a rare variant of partial and total anomalous pulmonary venous drainage directly to the morphologic right atrium, caused by malposition of septum primum. Displacement of septum primum to the morphologic left atrium was present in all 9 cases leading to partial or complete anomalous drainage of pulmonary veins to the morphologic right atrium. However, all pulmonary veins were connecting normally to the morphologic left atrium. The views that best demonstrated the deviation of septum primum were subxiphoid coronal, apical 4-chamber, and parasternal long-axis.

Closed catheter drainage and urinary infection--a comparison of two methods of catheter drainage.

The incidence of urinary infection following urological operations is compared using 2 methods of closed catheter drainage in 214 patients. No significant difference was observed. The overall infection rates for the whole series were 32%. The most significant influence on the rate of infection appears to be the duration of catheter drainage. Failure of chemotherapy to eradicate infection while the catheter was still in situ was mostly due to the use of inappropriate antimicrobials. Only 2 of the 35 cases with infected urine had succeeded in clearing their infection at the 4-week follow-up without chemotherapy.

[Studies on the lymph drainage of the eye. 2. Drainage of lymph-dependent radioactive substances from the anterior chamber. (author's transl)].

In order to study lymph drainage from the eye colloid solutions (198Au-colloid and 99mTc-sulfur-colloid) were injected into the right anterior chamber of the eye in 12 rabbits. The distribution of the radioactivity was measured in vivo with an Anger-type camera and in vitro with a sodium iodine crystal well-counter after dissection. The drainage of radioactivity from the anterior chamber had an average half-life of 36 min. The drained activity could be found mainly in blood, liver, kidney, and bone tissue. Moreover, a concentration of radioactivity significantly surpassing the concentration in blood was measured in the right retrobulbar space in 75% of the cases and in the cervical lymph nodes in 50% of the cases. These findings substantiate a communication between the anterior chamber of the eye and the retrobulbar space in the form of a lymphatic or a prelymphatic-lymphatic flow.

Drainage of the exocrine pancreas in clinical transplantation: comparison of bladder versus enteric drainage in a consecutive series.

The purpose of this study was to define the incidence of urologic and metabolic complications after simultaneous kidney/pancreas transplantation (SKPT) with bladder drainage (BD). Review of 55 SKPT with BD performed between 1989 and 1995 demonstrated patient, kidney, and pancreas survival rates of 95%, 89%, and 78%, respectively, with a mean follow-up of 41 months (range 12-78 months). Over this follow-up period 78% of these patients experienced a urinary tract infection, 27% had hematuria, and 38% had at least one hospital admission for dehydration. Recent experience with primary enteric drainage of the exocrine secretions of the transplanted pancreas (n = 11) has demonstrated the total absence of these complications (follow-up range 2-12 months). These results suggest the value of continuous re-evaluation of surgical techniques as the care of transplant patients evolve.

The results of surgical techniques in hepatic hydatidosis: treatment with drainage versus treatment without drainage--a 6-year experience.

Fifty-nine consecutive patients who underwent surgery for hepatic hydatid cysts between 1 March 1988 and 31 July 1994, were included in this study. The aim was to compare the results of surgical techniques with respect to post-operative complications, morbidity and recurrence of the disease. Patients were divided into two groups. The first group (1) of patients (n = 30) were treated surgically without drainage and the second group (2) of patients (n = 29) were treated surgically with tube drainage of the cystic cavity. In the study, there were 12 (20.33%) male and 47 (79.66%) female patients, with an age range of between 16 and 85 years. In five of the patients a communication between the cyst and the biliary system was documented. 11.8% of the cysts were localized extrahepatically. Post-operative morbidity was 10% in group 1 and 44.7% in group 2 (P < 0.05). Average post-operative hospital stay was 8.5 days in group 1 and 18.6 days in group 2 (P < 0.05). During the follow-up period, which lasted from 1 to 6 years, recurrence rates were not significantly differentiated in the two groups (P > 0.05).
