[Vascular compression of the duodenum related to a plaster cast (the cast syndrome)].

Case report of a 15-year-old girl presenting compression of the duodenum by the superior mesenteric artery, occurring after the application of a body plaster cast for correction of a idiopathic thoracic scoliosis, which required a gastrojejunostomy. The clinical and radiological pictures of this case fall into the rare entity called the "cast syndrome". The salient features of this syndrome are commented.

The biocompatibility of solution cast and acetone-extracted cast Biomer.

The cellular response to films of cast Biomer and acetone-extracted Biomer were investigated over a 21-day implantation period, using an in vivo cage implant system. Film samples were characterized by scanning electron microscopy (SEM), attenuated total reflectance infrared (ATR-IR), electron spectroscopy for chemical analysis (ESCA) and by contact angle measurements before implantation, and by SEM and ESCA after implantation and cleaning. Cellular and protein components of the inflammatory response were analyzed at periodic observation points after implantation. In addition, film samples were retrieved at 4, 7, and 21 days after implantation and analyzed for leukocyte adhesion by light microscopy and SEM. The results demonstrated that cast Biomer contains an extractable fraction, which when removed significantly improves the biocompatibility of the material.

Smoking cessation and arrhythmic death: the CAST experience. The Cardiac Arrhythmia Suppression Trial (CAST) Investigators.

OBJECTIVES: This study was performed to assess the effect of cigarette smoking cessation on overall mortality and the incidence of arrhythmic death in the population of the Cardiac Arrhythmia Suppression Trial (CAST). BACKGROUND: Cigarette smoking is a known risk factor for sudden cardiac death. Some of the adverse effects of smoking have been shown to dissipate with smoking cessation, but the time frame over which these changes occur and the population that stands to benefit have not been well delineated. CAST was a multicenter double-blind placebo-controlled study to determine whether suppression of ventricular ectopic activity by means of antiarrhythmic drugs in patients with left ventricular dysfunction after acute myocardial infarction would reduce the incidence of arrhythmic death. METHODS: Of 2,752 patients randomized to blinded therapy, 1,026 were smoking at the time of their baseline examination and completed a 4-month follow-up visit. Of these, 517 stopped smoking by the time of this visit ("quitters") and 509 continued to smoke ("smokers"). RESULTS: Over a mean follow-up period of slightly < 16 months, there were 17 arrhythmic deaths and 32 total deaths among the quitters versus 30 and 45, respectively, among the smokers; these differences were of marginal statistical significance. Most of the fatal events occurred in a group at high risk of ongoing ischemia: the 558 patients who did not have thrombolysis or undergo revascularization after their qualifying myocardial infarction. In this high risk cohort, smoking cessation greatly reduced the incidence of arrhythmic death and was associated with a statistically significant benefit in survival. CONCLUSIONS: Smoking cessation was accompanied by a marked reduction in arrhythmic death and overall mortality that achieved statistical significance in a high risk cohort. These data imply that smoking cessation is important in risk factor reduction in patients with advanced ischemic heart disease.

CAST: randomised placebo-controlled trial of early aspirin use in 20,000 patients with acute ischaemic stroke. CAST (Chinese Acute Stroke Trial) Collaborative Group.

BACKGROUND: Aspirin is effective in the treatment of acute myocardial infarction and in the long-term prevention of serious vascular events in survivors of stroke and myocardial infarction. There is, however, no reliable evidence on the effectiveness of early aspirin use in acute ischaemic stroke. METHODS: The Chinese Acute Stroke Trial (CAST) was a large randomised, placebo-controlled trial of the effects in hospital of aspirin treatment (160 mg/day) started within 48 h of the onset of suspected acute ischaemic stroke and continued in hospital for up to 4 weeks. The primary endpoints were death from any cause during the 4-week treatment period and death or dependence at discharge, and the analyses were by intention to treat. 21,106 patients with acute ischaemic stroke were enrolled in 413 Chinese hospitals at a mean of 25 h after the onset of symptoms (10,554 aspirin, 10,552 placebo). 87% had a CT scan before randomisation. It was prospectively planned that the results would be analysed in parallel with those of the concurrent. International Stroke Trial (IST) of 20,000 patients with acute stroke from other countries. FINDINGS: There was a significant 14% (SD 7) proportional reduction in mortality during the scheduled treatment period (343 [3.3%] deaths among aspirin-allocated patients vs 398 [3.9%] deaths among placebo-allocated patients; 2p = 0.04). There were significantly fewer recurrent ischaemic strokes in the aspirin-allocated than in the placebo-allocated group (167 [1.6%] vs 215 [2.1%]; 2p = 0.01) but slightly more haemorrhagic strokes (115 [1.1%] vs 93 [0.9%]; 2p > 0.1). For the combined in-hospital endpoint of death or non-fatal stroke at 4 weeks, there was a 12% (6) proportional risk reduction with aspirin (545 [5.3%] vs 614 [5.9%]; 2p = 0.03), an absolute difference of 6.8 (3.2) fewer cases per 1000. At discharge, 3153 (30.5%) aspirin-allocated patients and 3266 (31.6%) placebo-allocated patients were dead or dependent, corresponding to 11.4 (6.4) fewer per 1000 in favour of aspirin (2p = 0.08). INTERPRETATION: There are two major trials of aspirin in acute ischaemic stroke. Taken together, CAST and the similarly large IST show reliably that aspirin started early in hospital produces a small but definite net benefit, with about 9 (SD 3) fewer deaths or non-fatal strokes per 1000 in the first few weeks (2p = 0.001), and with 13 (5) fewer dead or dependent per 1000 after some weeks or months of follow-up (2p < 0.01).

Gypsum-bonded investment and dental precision casting (III) Composition of investment for the quick casting technique.

A simultaneous differential thermal analysis and thermogravimetry method previously established was used to estimate the composition of gypsum-bonded investment marketed for the quick casting technique. Three commercial investments of this type were heated to 700 degrees C at 10 degrees C/min and the hemihydrate content was estimated by the mass decrease reached at 300 degrees C after subtracting the mass decrease at 100 degrees C as moisture content. The hemihydrate contents were between 25% and 30%, which appears to be the range also chosen for the conventional gypsum-bonded investment of cristobalite type over 70 years by the industry. However, the new type of investment contained both cristobalite and quartz. The small sample size is a disadvantage of the present method but this can be overcome by more frequent use of the method by investigators.

Segmental framework matrix: master cast verification, corrected cast guide, and analog transfer template for implant-supported prostheses.

A procedure is presented whereby a sectioned acrylic resin framework pattern for an implant-supported prosthesis is used for multiple purposes. It captures the three-dimensional spacial relationship of a dental implant. It acts as an analog transfer template, corrected cast guide, and a rigid scaffolding for the actual sculpting of the framework pattern.

Gypsum-bonded investment and dental precision casting (II) investment for the quick casting technique.

Part I of this study presented a hypothesis that the thermal expansion of gypsum-bonded investment should become higher under heating conditions favorable for the formation of ahemihydrate. The present study aimed at proving this hypothesis by simulating the rapid heating condition employed in the quick casting technique. The expansion values obtained were 1.72-1.93% after 30 min in three investments, two rapid heating and one conventional types. Under a comparable measuring pressure a common laboratory measurement (5 degrees C/min) gave significantly lower expansions of 1.32-1.36% at 700 degrees C for the same investments (p<0.01). The shrinkage observed at 350-400 degrees C in this measurement was not observed in the rapid heating. Realistically large molds prepared from the conventional gypsum-bonded investment need to be investigated for their potentially high thermal expansion.

Effect of age on postmyocardial infarction ventricular arrhythmias (Holter Registry data from CAST I and CAST II). Cardiovascular Arrhythmia Suppression Trials.

Holter data from the 17,609 postmyocardial infarction patients in the CAST Registry were analyzed to study the effect of age on VPC and VT incidence and frequency. Multivariable analysis demonstrated that age is an important independent predictor of increased ventricular ectopy, with the median rate of VPCs/hour increasing from 0.4 (patients aged < 50 years) to 4.0 (patients aged 75 to 80 years), and the prevalence of VT increasing from 7.3% (patients aged < 50 years) to 15.3% (patients aged 75 to 80 years).

The development of a new method of cephalometric and study cast mensuration with a computer controlled, video image capture system. Part II: Study cast mensuration.

A computerized system for mensuration of dental casts, developed at the Royal Berkshire Hospital in Reading was tested against a conventional manual method and a computer controlled reflex microscope. Systematic errors, calibration, and handling aspects are discussed. Results showed that the computerized system provided data which while statistically different, were clinically comparable with the same measurements carried out using conventional manual or digitizing techniques. The results indicated that future research is required with respect to object illumination for all indirect techniques.

["Circular bands" on the casts of the microcirculatory bed of the human heart in ontogenesis (data from scanning electron microscopy of corrosive casts)].

By means of scanning electron microscopy of corrosive casts, applied to the human cardiac hemomicrocirculatory bed vessels, beginning from 6-9-month-old fetuses up to persons of elderly and old age, peculiarities in form, dimensions and distribution have been revealed in the wall of the arterial part of the vessels of the hemomicrocirculatory bed of so called circular strips. They are inevitable structural components in the wall of the vessels mentioned and correspond to smooth myocytes, shifted at methylmetacrilate injection.
