Secular changes of PCB concentration accumulated in organs and tissues of Japanese from 1974 to 1989.

The concentration and component types of PCB in adult human liver and fat tissue (64 samples) from forensic autopsy cases of Osaka Prefecture in 1989 were measured for comparison with the data for the same organ and tissue during the period from 1974 to 1984. The mean values of PCB levels in 1989 were 0.06 microgram/g (whole base) in the liver and 2.20 micrograms/g (fat base) in the fat tissue. These concentrations both were the same as those observed for the 15-year period. The percentages of the 5,6-chlorinated compound of PCB in the liver and fat tissue were lower but those of the 7-chlorinated compound were 15-20% higher.

Stroke incidence, prevalence, and survival: secular trends in Rochester, Minnesota, through 1989.

BACKGROUND AND PURPOSE: Trends in stroke incidence and mortality are available from few sites worldwide. The unique nature of the medical care in patients from Rochester, Minn, has allowed the ascertainment of virtually all cases of stroke for community residents for the period dating from 1955. This study updates incidence rates, stroke mortality, radiological imaging availability, and prevalence for stroke and subtypes of stroke though 1989 and evaluates trends since 1955. METHODS: Medical records of all residents of Rochester with potential diagnosis of stroke during the 5-year period through December 31, 1989 were screened to determine whether the case met the criteria for stroke. The type of stroke was determined with the use of all available data. Average annual age- and sex-adjusted incidence rates for stroke and various types of stroke were calculated for 5-year periods from 1955 to 1989. Prevalence of stroke was also determined. Poisson regression was used to better clarify the dependence of stroke incidence on multiple variables. Cox proportional hazards modeling evaluated trends in short-term and long-term stroke survival. RESULTS: We found that 496 incidence cases of stroke occurred among Rochester residents during 1985 to 1989. The annual age- and sex-adjusted stroke incidence rates was 145 per 100 000 population, which was virtually unchanged from 1980 to 1984 and 13% higher than the rate determined in 1975 to 1979. Increasing incidence rates compared with the 1970s were noted in all groups aged older than 54 years and in both sexes. Survival after cerebral infarction was dependent on age and calendar year when trends dating from 1955 were evaluated. The annual incidence rate of intraparenchymal hemorrhage was twice that of subarachnoid hemorrhage. CONCLUSIONS: Incidence rates for stroke have remained at a high rate than those determined in the 1970s. Although the occurrence of untreated hypertension has been reported to be stable or decreasing, the incidence rate of stroke is higher than those noted during the decline in stroke incidence during the 1960s and 1970s. In addition to the impact of radiological imaging, increased contribution of patients with ischemic heart disease, and the possible detection of milder cases of stroke, other factors, which are undefined, may be contributing to the increased stroke incidence rates detected over the last 10 years.

[Comparative studies on activities of antimicrobial agents against causative organisms isolated from patients with urinary tract infections (1994). III. Secular changes in susceptibility].

Susceptibilities of Enterococcus faecalis, Staphylococcus aureus, Citrobacter spp., Enterobacter spp., Escherichia coli, Klebsiella spp., Proteus mirabilis, Pseudomonas aeruginosa and Serratia spp. isolated from patients with urinary tract infections (UTIs) in 10 hospitals during June 1994 to May 1995 to various antimicrobial agents were compared with those in the same period of previous years according to a classification, uncomplicated UTIs, complicated UTIs without indwelling catheter, and complicated UTIs with indwelling catheter. No remarkable changes were found in susceptibilities of Citrobacter spp., Enterobacter spp. and Serratia spp. The susceptibilities of E. faecalis to amikacin and quinolones were better than those in 1993. As for S. aureus, susceptible strains to all drugs increased in uncomplicated UTIs. Against E. coli in 1993, the antimicrobial activities of piperacillin, cefotiam and aminoglycosides have decreased, however, in 1994, these activities have turned to the better state. As for Klebsiella spp. susceptible strains to ABPC decreased. The susceptibilities of P. mirabilis to all drugs except minocycline were good. Against P. aeruginosa in 1993, the activities of aminoglycosides have decreased, but, in 1994, these activities have turned to the better state. These data should be considered in clinical treatment of various urinary tract infections.

The secularization of U.S. Catholic birth control practices.

Except for sterilization, Catholic and non-Catholic contraceptive practices are now quite similar. Within several years, even sterilization will probably be adopted by the same proportions of Catholics as non-Catholics, and the rhythm method is destined to be of historical interest only. The wide gulf between official Catholic doctrine and the birth control behavior of Catholics can only deepen in the next few years.

Secular trends in ischemic heart disease mortality in California versus the United States, 1980 to 1991.

We compare the recent trends in ischemic heart disease mortality in California and the United States. Because California was among the first states to have declines in ischemic heart disease mortality, an examination of these recent trends may provide important clues for upcoming national trends. Age-adjusted and -specific ischemic heart disease mortality rates were calculated by sex for persons aged 35 and older during the years 1980 to 1991. Log-linear regression modeling was used to estimate the average annual percentage change in mortality. Between 1980 and 1991, the annual age-adjusted ischemic heart disease mortality declined less in California than in the United States for both women (1.9% versus 3.1%) and men (3.1% versus 3.5%). In California, it increased slightly between 1986 and 1990 for the oldest women and men. The slower rates of decline in mortality of this disease in California compared with the United States and the rising rates among the most elderly Californians suggest that careful attention should be paid to these trends in death rates of and risk factors for this disease in California.

[New investigations on the question of secular acceleration of permanent dentition].

A sample of 1,310 girls, 5 to 13 years of age, drawn from Mainz, Wiesbaden and vicinity was studied to establish the eruption time of the permanent dentition. The data of the median date of eruption of different teeth was determined employing a nonparametric test for the estimation of a survivorship function with doubly censored data (TURNBULL 1974). The method presented shows a marked superiority over other techniques used to verify the median date of eruption. The diachronic comparison of several samples--mainly from the Rhine-Main-Area-encompassing the years 1909 till today, showed no uniform trend towards an earlier eruption date. In some cases, temporal differences were ascertained which can be readily attributed to varied definitions of dental status. Furthermore, a recalculation--using this innovative mathematical approach--of a sample from the Frankfurt area, originally studied by FRANZ (1938) did not show any unambiguous acceleration tendencies-at least not within the past 50 years. These results suggest that the human dentition is predominantly genetically determined and therefore environmental factors play a by far lesser role than in general physical development.

Secular changes in radical prostatectomy utilization rates in Olmsted County, Minnesota 1980 to 1995.

PURPOSE: We estimated the changes in utilization of radical prostatectomy for treatment of prostate cancer and describe the clinical characteristics of men undergoing radical prostatectomy in a population based setting. MATERIALS AND METHODS: The Rochester Epidemiology Project was used to identify all Olmsted County residents who underwent radical prostatectomy from 1980 to 1995. The community medical records of these men were reviewed to determine the clinical and pathological stage and grade at biopsy and following surgery. RESULTS: From 1980 to 1995, 311 radical prostatectomies were performed on Olmsted County men. From 1980 to 1987 prostatectomy rates ranged from 6.3 to 31.0/100,000 men but rates increased dramatically to 53.6/100,000 in 1988 and 106.2/100,000 in 1992. The rate after 1992 decreased to 53.0/100,000 and then increased slightly to 80.4/100,000. There was a shift to younger age in more recent times (mean patient age 65.4 years in 1980 to 1986 and 62.4 in 1993 to 1995, p = 0.02), a nonsignificant (p = 0.10) trend toward lower pathological stage in recent years (42% stage pT2 in 1980 to 1986 versus 55% in 1993 to 1995) and a significant decrease in the proportion of cases of disease up staged following surgery (53% in 1980 to 1986 versus 37% in 1993 to 1995, p = 0.03). There was no significant trend in pathological grade with time (63% Mayo grade I or II in 1980 to 1986 versus 52% in 1993 to 1995, p = 0.30). CONCLUSIONS: These findings demonstrate an increase in radical prostatectomy rates that coincided with increases in prostate cancer incidence. There was a decrease in population prostatectomy rates in 1993 which was followed by modest increases to levels lower than the peak in 1992. However, the clinical characteristics of patients during this period did not change dramatically, suggesting that in a population based setting the selection factors for patients undergoing surgical treatment may not have changed.

Secular change in the torsional angle of Japanese femora.

The torsion angle of the Japanese femora of the early modern Edo (early and late series) and modern periods were measured. In the modern period, this angle is in males and females significantly larger than in the early series of the early modern Edo period and shows a gradual increase from the early modern Edo to the modern period. It is was always larger in females than in males. This change may be in part due to sedentary habits or living posture of the latter half in the early modern Edo period.

[Comparative studies on activities of antimicrobial agents against causative organisms isolated from patients with urinary tract infections (1996), III. Secular changes in susceptibility].

Susceptibilities to various antimicrobial agents were examined for Enterococcus faecalis, Staphylococcus aureus, Escherichia coli, Klebsiella spp., and Pseudomonas aeruginosa that were isolated from patients with urinary tract infections (UTIs) in 10 hospitals during June 1996 to May 1997, and the results were compared with those obtained during the same period in earlier years. 1. E. faecalis Among E. faecalis strains, those with high susceptibilities to ampicillin and minocycline appeared to have decreased in the latest study period. 2. S. aureus To almost antimicrobial agents, S. aureus isolated from uncomplicated UTIs showed low susceptibilities. But the MIC50s of those agents for S. aureus from complicated UTIs have changed better state. Particularly, the MIC50s of imipenem and clindamycin were 0.125 microgram/ml or below in the latest period for the first time in our history. 3. E. coli The susceptibilities to piperacillin and quinolones of E. coli isolated from uncomplicated UTIs were better than those isolated from complicated UTIs. 4. Klebsiella spp. The susceptibilities to almost antimicrobial agents of Klebsiella spp. have been better during the latest period, compared to those during period of 1995-1996, but to ofloxacin and ciprofloxacin have appeared to have been lower. 5. P. aeruginosa The susceptibilities to quinolones of P. aeruginosa have been better during the latest period compared those during periods of 1995-1996. But, the susceptibilities to cefozopran, carbapenems and monobactams of P. aeruginosa isolated from complicated UTIs appeared to have been lower. These susceptibility changes should be utilized in determining clinical treatments.

Secular trends in prevalence and concentration of pulmonary asbestos bodies--1940 to 1972. A necropsy study.

This study was designed to inquire into changes in the prevalence and concentration of asbestos bodies in the lungs of the necropsy population in Sinai Hospital of Baltimore during a 33-year period, 1940 to 1972. A tissue digestion technique was used to extract and quantitate asbestos bodies not only in fresh samples of pulmonary tissue but also in older fixed and blocked samples. A significant increase in the proportion of lungs containing asbestos bodies was found in the later years, as well as an increase in the concentration of asbestos bodies in lungs of the older age groups. Currently, 91.1%of this necropsy population in Baltimore harbor asbestos bodies in their lungs. The biologic importance of this increase is uncertain and speculative.

Rapid rise in the incidence of type 2 diabetes from 1987 to 1996: results from the San Antonio Heart Study.

BACKGROUND: The prevalence of type 2 diabetes has increased in the early part of the 20th century, particularly in developing countries. There is now evidence that the prevalence also continues to increase in developed countries, including the United States. However, it is unknown whether this increase is due to a rise in the incidence of diabetes or to decreasing diabetic mortality or both. METHODS: Participants in the San Antonio Heart Study, who were nondiabetic at baseline and who returned for a 7- to 8-year follow-up examination, were examined for secular trends in the incidence of type 2 diabetes. Risk factors for diabetes, such as obesity, were also examined. Patients were enrolled in the San Antonio Heart Study from 1979 to 1988 and 7- to 8-year incidence of diabetes was determined from 1987 to 1996. RESULTS: A significant secular trend in the 7- to 8-year incidence of type 2 diabetes was observed in Mexican Americans (5.7% for participants enrolled in 1979 to 15.7% for participants enrolled in 1988). In non-Hispanic whites, the incidence increased from 2.6% for participants enrolled in 1980 to 9.4% for participants enrolled in 1988 (P = .07) . After adjusting for age and sex, the secular trend remained significant in Mexican Americans and borderline significant in non-Hispanic whites. This indicates that between 1987 and 1996 the 7- to 8-year incidence of type 2 diabetes approximately tripled in both ethnic groups. The overall secular trend also remained significant after adjusting for additional risk factors for diabetes, such as obesity. A rising secular trend in obesity was also observed. CONCLUSIONS: There has been a significant increasing secular trend in the incidence of type 2 diabetes in Mexican Americans and a borderline significant trend in non-Hispanic whites participating in the San Antonio Heart Study. Unlike other cardiovascular risk factors such as lipid levels, cigarette smoking, and blood pressure, which are either declining or under progressively better medical management and control, and unlike cardiovascular mortality, which is also declining, obesity and type 2 diabetes are exhibiting increasing trends. Thus, obesity and diabetes could easily become the preeminent US public health problem.

Increasing body size among adult cook islanders between 1966 and 1996.

There is limited evidence of a secular trend toward increased body size among populations in the Pacific Region, although some populations have shown a clear rise in overweight and obesity across the past 30 years or so. Mean height, weight and body mass index (BMI) of adults surveyed in 1996 on Rarotonga, the Cook Islands, are reported by age group and compared with data collected in 1966 by Evans and Prior, to determine the extent to which a secular trend in increasing body size has taken place across this 30-year period. In addition, a comparison of these anthropometric variables between subjects born on the most modernized island of Rarotonga with those born on other less-modernized islands is made, to determine the extent to which adult body size differs according to place of origin. Comparing the sample of those born on Rarotonga, the most modernized island, with those born on other, less modernized islands shows those born on Rarotonga to be taller, but not relatively heavier than those born elsewhere in the Cook Islands. Both males and females of the 1996 sample of adults are significantly taller, heavier, with higher BMI compared with the 1966 sample, indicating a secular trend toward increased body size across this 30-year period. The mean BMIs of the younger age groups in 1996 are greater than those of the same age groups in 1966, indicating a secular trend toward greater body fatness. There has been a significant increase in obesity among both males and females. In 1966, 14% of males had a BMI greater than 30, while in 1996, the proportion was 52%. Among females, 44% of those measured in 1966 had a BMI greater than 30, while in 1996 the proportion was 57%. The higher mean stature of younger adult males relative to older ones suggests that the secular trend toward increasing height may have been underway prior to 1966. The mean statures and weights of adults aged 60 years and older in the 1996 sample are more similar to values given for most age groups in the 1966 sample, indicating that the secular trend toward increased weight and height may have begun 50-60 years ago.
