[Clinical experiences of lividomycin on respiratory tract infections (author's transl)].

Clinical effects of a newly developed aminoglycoside antibiotic lividomycin, were investigated in 13 patients suffering from respiratory infections. 1. A total of 13 patients with the following infectious diseases was treated with 1 g/day of lividomycin for 3 to 11 days (average 6.7 days): Acute pneumonia 3 cases, bronchiectasis 3 cases, acute bronchitis 5 cases, suppurative diseases of the lung 1 case, and pyothorax 1 case. As the results, the antibiotic was effective in 5 patients, moderately effective in 4 patients and ineffective in 4 patients, and thus the global effective rate was 62.9% (9/13). 2. As in the case of kanamycin, lividomycin indicated fairly good sensitivity against a total of 17 strains comprising Staphylococcus aureus (7 strains), alpha-Streptococcus (2 strains), beta-Streptococcus (3 strains), gamma-Streptococcus (1 strain), Neisseria (2 strains) and Haemophilus (2 strains) which were isolated from sputum. 3. No side effects attributable to lividomycin were observed.

[Considerations on the antibiotic activity (M.I.C.) revealed in 247 serotypes of Salmonella].

247 strains of Salmonella, isolated in Brescia province duriing 1973-1974 have been typed by serological methods. The MIC has been determined, using the following antibiotics: chloramphenicol, tetracycline HCL, ampicillin, doxycycline, rifampicin, cephazolin, carbenicillin, nifuratel, gentamicin, aminosidine, trimetho-prim-sulphamethoazole, nalidixic acid. The pattern of resistence of the various serotypes is quite constant. The relationship between diffusion and epidermiological factors are discussed.

[ANT(4')I: a new aminoglycoside nucleotidyltransferase found in "staphylococcus aureus" (author's transl)].

A new nucleotidyltransferase has been obtained from clinical isolates of kanamycin, amikacin, tobramycin, neomycin and lividomycin resistant strains of Staphylococcus aureus. The enzyme has been purified by affinity chromatography then characterized in a kinetic and physicochemical point of view. The results obtained support that the target of this enzyme is the 4'-hydroxyl group of a number of aminoglycosides. We propose to call it ANT(4')I.

A sensitive assay of translational fidelity (readthrough and termination) in eukaryotic cells.

The process of translation termination in eukaryotes has been monitored by different types of assays, each with its own merits. We have developed an in vivo system where the reporter protein is secreted from the cells in culture thus enabling continuous monitoring of translation termination activity by simple sampling of the cell culture media. Using this system, cell cultures can be challenged with various stimuli during growth and the cellular responses on the translational level can be investigated in vivo as well as in vitro. Sampling is rapid, easy, and non-destructive to the cells, which enables measurement of translational fidelity in real time during time-course experiments. In particular with this system it is possible to assess very low levels of stop codon suppression. The reporter enzyme, secreted alkaline phosphatase (SEAP), becomes tagged with the S-peptide when there is readthrough of a stop codon placed between the C-terminus of the SEAP and the S-peptide. The tagged SEAP is bound to a matrix and the bound SEAP activity is measured versus total SEAP activity in the medium as a reference. With this assay we have confirmed that eRF1 acts as an antisuppressor in cells transfected with a cognate suppressor tRNA as well as in control cells, where a small but significant level of readthrough (suppression) could be detected. We have also characterized suppression of the three stop codons individually, and especially UGA is prone for wobbling.

[Antistaphylococcal activity of gentamicin].

A review of the literature suggests the following conclusions: 1) since its first practical use in 1965, namely over the past 10 years, there has been no drop in activity of Gentamicin on Staphylococcus aureus and on numerous other Staphylococcus and/or Micrococcus species. 2) In comparison with the other aminoglycosidic antibiotics employed up to the present, i.e. Streptomycin, Neomycin, Kanamycin, Amminosidine, and Framycetin, Gentamicin has demonstrated a much superior antistaphylococcic activity, and this has also been documented on numerous strains of staphylococci recalcitrant to Kanamycin, Streptomycin, Framycetin and Neomycin. This goes to prove the absence of any cross resistance between Kanamycin, Streptomycin, Framycetin and Neomycin on the one hand and Gentamycin on the other. 3) Along with Rifampicin, certain Cephalosporins (Cephalotine, Cephaloridine) and Pristinamycin-Virgimycin, Gentamicin must undoubtedly be considered a "greater" antibiotic as far as antistaphylococcic activity is concerned. It also has the advantage over other antistaphylococcic-acting antibiotics that only in exceptional cases does it give rise to resistant strains.

The kinetics of aminosidine in renal patients with different degrees of renal failure.

The plasma concentration and urinary excretion after a single 500 mg dose of Aminosidine have been studied in 12 patients with different degrees of renal failure and 4 normal subjects. In normal subjects the plasma half-life is 2.47 hr; in patients with creatinine clearance (Ccr) of 30-60 ml/min, its 6.7 hrs.; in patients with Ccr of 10-30 ml/min, it is 16.7hrs.; in patients with Ccr less than 10 ml/min, it is 36.6 hrs. A dose of 0.5 g of Aminosidine should be given to normal subjects every 12 hr. When renal function is reduced, the interval (in hr) between doses should be the following: Ccr 60-40 ml/min: 19-28;Ccr 40-30 ml/min; 28-35; Ccr 30-20 ml/min: 35-47; Ccr 20-10 ml/min: 47; Ccr less than 10 ml/min: 76.

[Committee on anthelmintics. Residual group].

A number of the older and a few recent anthelmintics are enumerated or briefly discussed. Moreover some attention is paid to anthelmintics, used for nematodes and trematodes which have a limited effect on cestodes.

The anti-mycoplasmal activity of aminosidine.

Some in vivo experiments of therapeutic activity in severe experimental infection (intracranial) in the mouse due to different strains of Mycoplasma proved the high efficacy of the antibiotic aminosidine when administered subcutaneously at the dose of 30 mg/kg and have also demonstrated that oral treatment is moderately effective against Mycoplasma hominis 2. Sensitivity test carried out in vitro on the same Mycoplasma strains confirmed the bactericidal activity of aminosidine. These experiments have therefore shown that the therapeutic spectrum of aminosidine, which includes Gram-positive, Gram-negative, acid-fast bacteria and some protozoa, may be enlarged to include the genus Mycoplasma. For this reason the antibiotic can find a useful application in the treatment of primary atypical neumonia. In addition, from a clinical research point of view, aminosidine may constitute a promising tool for the treatment of different illnesses in which Mycoplasma have been described as important co-factors of pathogenicity in man.

[Recurrent episodes of acidosis with encephalopathy in a hemodialysis program patient with short bowel syndrome].

We present a case of a patient with short bowel syndrome in a hemodialysis program, with recurrent episodes of serious acidosis. The presence of a D-lactic acidosis peak secondary to bacterial overgrowth in the intestine was discovered during an acute episode of acidosis, with neurological affection. The detection of acidosis in predialysis measurements and the acute episodes of acidosis, made it necessary to administer bicarbonate to the patient and give him additional hemodialysis sessions.
