Radiographic features and outcome of root canal treatment carried out in the Łódź region of Poland.

AIM: The aim of this study was to assess the radiographic features and factors affecting the long-term results of root canal treatment in the Lódź region of Poland. METHODOLOGY: Three hundred and fifty-five periapical radiographs of previously endodontically treated teeth (282 root-filled teeth and 73 teeth with pulpotomy) from 236 patients were evaluated in the study. The patients were aged from 15 to 76 years and randomly selected amongst subjects referred to the Institute of Dentistry. Medical University of Lódź and two private dental practices. The radiographs were examined by the authors according to criteria proposed by De Cleen et al. (1993) using an illuminated viewbox in a dark room. The Fisher's exact test was used to compare the frequencies in teeth groups. RESULTS: Amongst root-filled teeth, 49% were adequately filled within 0-2 mm of the radiographic apex. Periapical radiolucencies were observed in 25% of root-filled teeth. Teeth without periapical pathosis were more often filled within 0-2 mm of the apex (59%) than teeth with pathosis (17%). Teeth were most often adequately filled (64%) in patients attending the Institute of Dentistry. In patients attending the dental outpatients' departments, 71% of teeth were inadequately filled. Amongst the teeth treated with pulpotomy, 67% had a periapical radiolucency. CONCLUSION: The radiographic technical quality of the root canal treatment was associated with the outcome that root fillings of adequate standard were associated with health more often than inadequate root fillings. Pulpotomy was associated with periapical disease in the majority of cases.

Clonal dissemination of a toxin-A-negative/toxin-B-positive Clostridium difficile strain from patients with antibiotic-associated diarrhea in Poland.

OBJECTIVE: To determine the incidence of toxin-A-negative/toxin-B-positive Clostridium difficile strains and their genetic relatedness in the feces of patients suffering from antibiotic-associated diarrhea (AAD) in Polish hospitals. METHODS: C. difficile strains were cultured from patients' stool samples. The present study characterises these strains with respect to their cytopathogenicity on McCoy cells and the absence of toxin A despite a functional toxin B as determined with commercial test kits (Culturette Brand Toxin CD-TCD toxin A test and C. difficile Tox A/B test). In addition, PCR using different primer pairs aiming at non-repeating or repeating regions of the toxin A and B genes were used to confirm the findings. All toxin A(-)B(+) strains were genetically characterised by random amplification of polymorphic DNA (RAPD) analysis, PCR ribotyping and, in part, pulsed-field gel electrophoresis (PFGE) of DNA macrorestriction fragments. RESULTS: We here present the presence of 17 toxin A(-)B(+) strains among 159 C. difficile strains (11%) isolated from fecal samples from 413 patients with antibiotic-associated diarrhea. All 17 strains possessed the toxin B gene, demonstrated a cytopathogenic effect on the McCoy cells, and were positive in the Tox A/B test. Molecular typing of these 17 C. difficile strains revealed that 7 of 17 (41%) toxin A(-)/B(+) C. difficile strains could not be discriminated. It appeared that these strains had a genotype that could not be distinguished from that of a Japanese control strain. CONCLUSION: Our observations imply that a particular genotype of toxin A(-)B(+) C. difficile has spread extensively, not only in Poland but possibly even worldwide.

Spatial genetic structure and clonal diversity of island populations of lady's slipper (Cypripedium calceolus) from the Biebrza National Park (northeast Poland).

Three populations of the rare and endangered plant species Cypripedium calceolus were included in a study of genetic diversity and spatial genetic structure in the Biebrza National Park, northeast Poland. Analysis of 11 allozyme loci indicate that the populations of this species contained high genetic variability (P = 45.5%, A= 1.73). On the other hand, the genetic differentiation (FST = 0.014, P < 0.05) among C. calceolus populations was very low when compared to other species with similar life history characteristics. The observed high rate of gene flow (Nm = 18) may suggest that the populations studied derived from each other in the recent past. Five polymorphic allozyme markers identified 109 multilocus genotypes in three populations and the majority of them (67%) were population-specific. One of the populations studied, characterized by particularly extensive vegetative reproduction, showed the lowest clonal diversity (G/N = 0.15) and heterozygosity (HO = 0.111) values and the highest FIS(0.380), when compared to other two populations (G/N = 0.26-0.27, HO= 0.166-178, FIS = 0.024-0.055). This may indicate that clonal reproduction has an important influence on the genetic structure of C. calceolus populations. The longevity of genets, the out-crossing breeding system and the presence of recruitment from seeds are factors maintaining genetic diversity in C. calceolus.

Comparison of ITS profiling, REP- and ERIC-PCR of Salmonella Enteritidis isolates from Poland.

Thirty-one Salmonella Enteritidis strains isolated from chickens, broilers and hens were analysed by genotypic typing including REP-PCR. ERIC-PCR and ITS profiling (PCR-ribotyping). Analysis of DNA banding patterns generated by REP-PCR revealed the presence of 22 different genotypes, which were grouped by dendrogram analysis into three distinct lineages (maximum similarity approx. 50%). Each isolate of S. Enteritidis analysed by ERIC-PCR generated an individual DNA pattern. Again, these isolates could be divided into three distinct genomic groups (maximum similarity approx. 60%) by their ERIC-PCR fingerprints. REP- and ERIC-PCR were found to be more discriminatory for typing of S. Enteritidis than ITS profiling. Amplification of the 16S-23S rDNA spacer region gave nine different profiles of DNA, subdivided into two closely related groups by dendrogram analysis. In summary, data obtained by genotyping methods for S. Enteritidis isolates from regions located in the south-west and the central parts of Poland revealed an enormous heterogeneity among analysed samples, and proved that REP- and ERIC-PCR are highly discriminatory techniques, which can be used, in addition to conventional methods, in epidemiological studies of S. Enteritidis infections.

Survey of porcine rotavirus G and P genotype in Poland and the United States using RT-PCR.

Porcine rotaviruses are a common cause of gastroenteritis. Several serotypes have been detected based on the two surface proteins VP4 (P-types) and VP7 (G-types). However, limited studies have been performed on the relative frequency of rotavirus types in diarrhetic pigs primarily because of the lack of availability of suitable methods. In this study, we describe a reverse transcriptase-polymerase chain reaction (RT-PCR) method for the typing of P and G types of rotavirus. This method allowed to detect G and P types in 96.8 and 87.1% of isolates collected in the United States, respectively and in 54.5 and 38.6% of isolates collected in Poland, respectively. Within the US specimens the G3, G4, G5, G9 and G10 types were detected in combination with P6 and P7 types while among Polish specimens only G3, G4 and G5 types in combination with P6 and P7 types were identified. In both instances the G4 and G5 were the most prevalent types. These studies show that a RT-PCR typing method is suitable for molecular epidemiological studies and that there is more diversity among porcine rotavirus than previously reported.

Epidemiology of dermatomycoses of humans in central Poland. Part I--Superficial infections caused by yeasts and moulds.

This work presents the epidemiology of dermatomycoses and their etiological agents of people living in the Lódź region, Central Poland, over the years 1987-1996. The analysis involved subjects referred by physicians to three major mycological laboratories in Lódź (covering the entire mycological diagnostics of the Lódź region). The study comprised a total of 25,737 persons aged 1-82. Positive results of mycological examinations were obtained for 14,084 (54.7%) patients. The total number of 14,295 positive mycological results included 6902 (48.3%) isolations of non-dermatophyte fungi. The non-dermatophytes included: Candida-like (69.8%), moulds (26.9%) and lipophilic yeasts (3.3%). The distribution of non-dermatophytes was characterized by dependence on sociodemographic features such as subjects' age, place of residence, education and occupation. Candida-like fungi are etiological factors of superficial infections of all clinical types whereas moulds were found to cause infections of finger and toe nails and periungual walls.

Epidemiology of dermatomycoses of humans in Central Poland. Part II--Non-dermatophyte infections of nails and periungual walls.

This study presents the epidemiology of dermatomycoses and their etiological agents of people living in the Lódź region, Central Poland, over the years 1987-1996. The study comprised a total of 25,737 patients. Positive mycological results were obtained for 14,084 patients (54.7%). The total number of 14,295 of positive mycological results included 6902 (48.3%) isolations of non-dermatophyte fungi. Among these positive results, isolations from subjects with infections of nails and periungual walls amounted to 2034 (29.5%). Candida albicans (59.9%) and Aspergillus sp. (17.3%) were major etiological factors of those infections. The decade in question was marked by a steady growth in incidence of the infections studied, especially over the years 1994-1996.

Transurethral microwave thermotherapy: first experiences in Poland.

OBJECTIVE: To present early experiences in Poland with high-energy transurethral microwave thermotherapy (HeTUMT), considered as a serious alternative to surgery in managing benign prostatic hypertrophy (BPH). PATIENTS AND METHODS: Sixty-one patients were treated in two centres using a commercial TUMT system. The patients were assessed at 5-6 weeks, 3 and 6 months after treatment, evaluating subjective complaints, the International Prostate Symptom Score (IPSS), urinary flow rate and postvoid residual volume. RESULTS: A complete follow-up was available in 44 patients; most had symptomatic improvement 3 months after HeTUMT but it was slightly less pronounced after 6 months. The improvement in objective variables increased up to 6 months after HeTUMT. There was > or = 50% improvement in subjective complaints in 43% of the patients, in urinary flow rate in 25% and in residual urine in half. Two patients required re-treatment for a urethral stricture and ineffective HeTUMT. CONCLUSIONS: HeTUMT may be a useful treatment for BPH but the outcome for an individual patient seems to be unpredictable. The indications for TUMT should be reviewed individually for each patient.

Hospital infection control in Poland.

During the last 10 years, as in other areas of the public sector in Poland, the healthcare system has undergone important changes presenting a challenge for medical staff and healthcare managers alike. Since 1997, the Polish Government has undertaken important healthcare reforms. New regulations have introduced decentralization of management and a financing system for hospitals. These changes have provided favourable conditions in which to implement an infection control system for hospitals. The most important recent initiatives are training courses and workshops for hospital staff organized in co-operation with microbiologists and epidemiologists as well as preparation of recommendations and guidelines for hospitals. Several non-governmental organizations have been set up to improve hospital hygiene, infection control and monitoring of antimicrobial resistance.

Microneurovascular transfer of contralateral latissimus dorsi in Poland's syndrome.

A 2-year-old girl with Poland's syndrome presented with syndactyly of the first, second and third webs of the left hand and ipsilateral absence of the sternocostal portion of pectoralis major. Latissimus dorsi on the same side was also absent. A microvascular free contralateral latissimus dorsi muscle transfer was undertaken to reconstruct the chest wall and anterior axillary fold. The transfer was innervated by intercostal nerve transfer and produced a neuromuscular unit of good aesthetic appearance and function.

Iodine prophylaxis in the aftermath of the Chernobyl accident in the area of Sejny in north-eastern Poland.

The aim of our study was the estimation of the effects, possible side-effects and immunological reactions after the mass iodine prophylaxis following the Chernobyl nuclear disaster. 1457 subjects, aged 6-55 yrs, filled in our questionnaires and in 1191 of them the titres of antithyroid antibodies (TA) including ATMA--Anti-Human Thyroid Membrane Antibodies and TGAb--Anti-Thyreoglobulin Antibodies were estimated. Our study revealed that the prophylaxis, recommended in Poland only for children and teenagers was widely used (more than 90%) and relatively safe. No serious side effects, especially in pregnant women consuming KI were reported. The frequency and mean titres of TA were similar in adults who took and did not take stable iodine. The incidence of TA could be connected with many different factors including iodine deficiency, endemy of goitre as well as iodine prophylaxis. The reason of this phenomenon may be explained in long-term population studies.

Poland's environment--past, present and future state of the environment in the Vistula and Odra River basins.

In the middle of the last century in the case of many river basins, it became obvious that the targets of the Helsinki-Commission (HELCOM) concerning a 50% reduction of nitrogen and phosphorus inputs to the Baltic Sea had not been reached. Such conclusions could be derived from the observed loads. But the measurements regarding the analysis were not sufficient to determine what the reasons for this report were, what has to be done to fulfil this agreement and to evaluate the time at which the targets can be reached. This contribution presents the current state of Polish surface water quality as well as results of modelling nutrient and heavy metal emissions into the two biggest Polish river basins: Vistula and Odra, which both cover approximately 90% of Poland's territory.

Tissue level, distribution, and total body selenium content in healthy and diseased humans in Poland.

The authors obtained tissue samples taken at autopsy from 46 healthy individuals killed in accidents and from 75 corpses of victims of various diseases to analyze selenium levels. The per-weight-unit basis of selenium levels (all expressed as ng/gm wet tissue) in tissues decreased in the following order: kidney (469) > liver > spleen > pancreas > heart > brain > lung > bone > skeletal muscle (51). The highest proportion of body selenium was found in skeletal muscles (27.5%); much less selenium was found in bones (16%) and blood (10%). In the tissues of cancer corpses, the selenium levels were much lower than levels in controls. The lowest selenium levels were found in the livers of alcoholics. Tissue selenium levels found in the study were significantly lower than levels reported in Japan, United States, Canada, and other countries. The low selenium levels in the tissues of Polish residents result from inadequate selenium levels in the soil. The authors used selenium levels in tissues to calculate the amount of selenium in humans in Poland (i.e., approximately 5.2 mg). This level was similar to levels found in New Zealand (i.e., 3.0-6.1 mg), but it was lower than the mean level found in Germany (i.e., 6.6 mg) and in the United States (i.e., 13.0-20.3 mg).

Vertical transmission of HIV-1 in Poland.

The aim of our study was to evaluate changes in vertical transmission of HIV infection in Poland after introducing zidovudine prophylactic strategies. Data from the Department of Children's Infectious Diseases (a paediatric HIV referral centre) at the Medical University, Warsaw was studied. Since 1989 vertical transmission of HIV-1 has been studied in 100 children born to 91 HIV-positive mothers (2 sets of twins). Zidovudine therapy, mode and timing of delivery and their relationship to perinatal HIV-1 infection were analysed. From 1989 to 1994 the transmission rate was 31.5%. Since 1995, when recommendations based on ACTG 076 were issued, a decline in a transmission rate to 19.6% was reported. 62% (32 out of 52) mother-infant pairs received zidovudine therapy. None of those children have become HIV infected. Zidovudine chemoprophylaxis regimen reduces the risk for mother to child transmission. It should be recommended for all HIV-infected pregnant women or women in labour and their infants.

Prevalence of non-O157 Escherichia coli strains among shiga-like toxin-producing (SLTEC) isolates in the region of Lower Silesia, Poland.

During 5 y (1997-2002) in the region of Lower Silesia, Poland, 55 shiga-like toxin-producing strains were isolated from children with diarrhoea, none of whom developed haemolytic uremic syndrome. Shiga-like toxin production was detected on Vero cells and confirmed by the detection of slt1 and slt2 gene sequences. Most of isolates examined possessed slt2 gene alone (41.8%), or in combination with slt1 gene (38.2%). None of the shiga-like toxin-producing strains was of serotype O157:H7. It appears that non-O157 shiga-like toxin-producing E. coil strains are prevalent as the causal agent of severe diarrhoea in the region.

First isolation of Borrelia lusitaniae DNA from Ixodes ricinus ticks in Poland.

European genospecies of B. burgdorferi sensu lato were identified by examining unfed I. ricinus ticks collected from 10 locations in northwest Poland. Research was conducted using 3 methods: PCR amplification of the fla gene with the FLA1 and FLA2 primer set conserved in all European species of B. burgdorferi sensu lato, PCR-RFLP, and sequencing. There were 5 restriction patterns obtained in this study: 4 characteristic for genospecies B. burgdorferi sensu stricto, B. garinii, B. valaisiana, and B. afzelii and 1 untypical restriction pattern type. PCR products for all restriction patterns were sequenced. Polish sequences of the fla gene for B. burgdorferi s.s., B. garinii, B. valaisiana, and B. afzelii were identical with sequences from GeneBank at 99.79%, 99.58%, 100% and 100% respectively. The fifth sequence demonstrated 99.79% identity with sequences of a B. lusitaniae PotiB2 isolate from Portugal, and also clusters with this strain. B. lusitaniae DNA in I. ricinus was detected in 3 out of 10 localities and constituted 5.9% of infected individuals with I. ricinus.
