Assessment of the reproductive performance of individual boars and the growth rate of their progeny in commercial units using a computerised monitoring system.

Records of the reproductive performance of boars on a 250 sow commercial pig herd were collated for four years. A new computer program was constructed and used for data on the daily liveweight gain and carcase grading of the whole herd and of individual sows and boars for selected periods. Two separate series of records, each for 11 boars, are presented and analysed statistically to determine significant differences between the boars in each series for farrowing rates to first service, total numbers born and liveweight gain of progeny. The overall farrowing rate to first service was 85.6 per cent for boars in series 1 and 82.1 per cent for those in series 2. One boar was found to have a farrowing rate to first service consistently below 75 per cent and was culled. The decision to cull was taken although its performance was only significantly different in series 1. No statistical differences between boars were found for numbers born. Statistical differences between boars were found for liveweight gain of progeny and the use of Duncan's multiple range test on ranked means in the analysis of such data is discussed. The necessity for the use of statistical analysis rather than simple ranking in the presentation of boar data is discussed and guidelines are given.

Validation studies of Bilitec 2000: an ambulatory duodenogastric reflux monitoring system.

Bilitec 2000, a fiber-optic spectrophotometer, detects duodenogastric (DG) reflux in an ambulatory setting independent of pH by measuring bilirubin in bile. Studies were performed for independent validation and better understanding of limitations of this system. Bilitec and a conventional spectrophotometer were used to assess the effect of gastric acidity on the spectral absorbance of refluxed DG contents, employing bilirubin ditaurate (BDT) and human bile. Total bile acid concentrations correlated with Bilitec absorption. BDT and bile had hypsochromic shift (400 nm) in spectrophotometric maximum wavelength (lambda max; 450 nm) in an acidic environment (pH < 3.5), corresponding to an approximately 30% decrease in absorption measured by Bilitec. Bile acid concentrations correlated strongly with Bilitec absorption for all 63 samples (R = 0.82) with pH > 3.5 (n = 31, R = 0.81), but correlation was less strong for 32 samples with pH < 3.5 (R = 0.70). As currently designed, Bilitec adequately measures DG reflux but underestimates it by at least 30% in acidic medium (pH < 3.5). Design modifications are suggested for improving the sensitivity of this equipment.

Microsensor-aided measurements of cellular signalling and metabolism on tumor cells. The cell monitoring system (cms(R)).

Microsensors provide instruments particularly suited for the rapid, noninvasive and on-line analysis of cell and tissue cultures. The microsensor system presented in this paper is a modular arrangement of various planar and nonplanar sensor elements for the measurement of physiological parameters of cell cultures. An optic access to the cultures (e.g. for light microscopy and spectrophotometric techniques) is also provided for a parallel and comparative data acquisition. The system was originally designed for biomedical research in chemotherapy (predicative chemotherapy assays) and pharmacology but it turned out to be also an effective tool for toxicological and environmental research.

CD69 to CD3 ratio of peripheral blood mononuclear cells as a marker to monitor systemic lupus erythematosus disease activity.

CD69 is an early T cell activation marker. In order to investigate whether the disease activity of systemic lupus erythematosus (SLE) is correlated with T cell hyperactivity as well as B cell hyperactivity, we measured the CD69 to CD3 ratio (CD69/CD3) of peripheral blood mononuclear cells (PBMCs) from 42 SLE patients and 18 healthy controls. To assess B cell activation, we measured the levels of anti-dsDNA, complement C3 and C4. Disease activity was assessed with the SLE disease activity index (SLEDAI) score. The mean value (+/- standard deviation) of CD69/CD3 for SLE patients (3.34 +/- 0.486) and healthy controls (0.92 +/- 0.015) were significantly different (P < 0.05). CD69/CD3, anti-dsDNA, C3 and C4 were all significantly correlated with SLEDAI (r = 0.50, 0.51, -0.68, -0.31, respectively, P < 0.05). CD69/CD3 could explain 25.4% of the variance in SLEDAI, and 6.7% of the variance that was independent of anti-dsDNA, C3 and C4. In patients with SLEDAI scores which were discordant with anti-dsDNA, C3 and C4 values, CD69/ CD3 still showed high correlation with SLEDAI. We conclude that CD69/CD3, which detects T cell activation is correlated with SLEDAI scores. Thus, T cell activation contributes part of the disease activity independent of B cell activity. This marker can complement in the estimation of disease activity when levels of anti-dsDNA, C3 and C4 fail to show good correlation.

A new monitoring system of cultured myocardial cell motion: effect of pilose antler extract and cardioactive agents on spontaneous beating of myocardial cell sheets.

Effects of various cardioactive agents and a water extract of the pilose antler of Cervus nippon var. mantchuricus on periodic beating of cultured myocardial cell sheets were examined by using an image analyzing system. Norepinephrine increased the beating rate and the beating amplitude, whereas digoxin and forskolin enlarged only the beating amplitude. Verapamil and propranolol decreased both the beating rate and the beating amplitude. The water extract of the pilose antler showed no remarkable effects in a standard medium (2.1 mM Ca2+). However, it significantly increased the beating amplitude when the beating was suppressed by replacement with a low calcium medium (0.5 mM Ca2+). A similar effect was found for 70% ethanol-soluble and -insoluble fractions of the extract.

Changing cost of English HIV service provision 1996-1997. NPMS Steering Group. National Prospective Monitoring System.

The objectives of this study were to provide individual and population-based unit cost estimates of HIV treatment and care by stage of HIV infection for adults in England and estimate the financial impact of the use of combination antiretroviral therapy. Individual unit cost estimates were calculated, based on 1997 activity data, and linked to the number of diagnosed HIV-infected individuals using statutory medical services by clinical stage of HIV infection in England during 1997 to obtain population-based cost estimates; these were compared with 1996 estimates. Most clinical guidelines now recommend the use of 3 antiretroviral agents, but cost estimates for mono and dual therapy were included as baseline estimates. Baseline costs for treating AIDS patients with zidovudine (AZT) monotherapy were estimated at pound sterling 16,830 (95% CI 14,633-18,985) per patient-year which was substantially lower than the 1996 estimate; costs for asymptomatic individuals and people with symptomatic non-AIDS were pound sterling 4450 (95% CI 3521-5612) and pound sterling 7289 (95% CI 6169-8386) per respective patient-year which did not differ substantially from 1996. The total annual population cost estimate for HIV service provision amounted to pound sterling 128 million (95% CI pound sterling 109m to pound sterling 147m), if all patients with HIV disease were treated with AZT monotherapy only. For all eligible patients to be treated with 2 nucleoside reverse transcriptase inhibitors (NRTI) (AZT and didanosine (ddI) or zalcitabine (ddC)), cost estimates amounted to pound sterling 161m (95% CI pound sterling 141m to pound sterling 181m), while for triple therapy, annual estimated expenditure amounted to pound sterling 185m (95% CI pound sterling 165m to pound sterling 206m) when a non-nucleoside reverse transcriptase inhibitor (NNRTI) (nevirapine) was included or pound sterling 205m (95% CI pound sterling 186m to pound sterling 235m) when a protease inhibitor was included. Compared with 1996 population-based cost estimates, the estimates for monotherapy decreased by 14%, by 11% for dual therapy, by 10% for triple therapy which included a NNRTI and by 9% if a protease inhibitor was used as part of a triple therapy regimen. Similarly, compared with 1996 estimates, the proportion of total costs attributable to treating asymptomatic individuals increased by 5% and 2-3% for people with symptomatic non-AIDS, while the proportion attributable for treating people with AIDS decreased by 8-9%.

Development of economic thresholds and monitoring systems for Helicoverpa armigera (Lepidoptera: Noctuidae) in tomatoes.

Field sampling methods and economic thresholds were developed to provide management recommendations for Helicoverpa armigera (Hübner) on processing tomatoes, based on a commercially acceptable damage level of 5% fruit damage. Population estimates from destructive sampling and a rapid 1-min plant scouting method were related to fruit damage, and a nominal economic threshold of one larva per plant was derived. The economic threshold was confirmed in a designed trial where it resulted in acceptable levels of fruit damage. The 1-min scouting method and economic threshold was validated in 17 commercial crops in the Gisborne, Poverty Bay region of New Zealand. Scouting in these fields was based on 10 plants in each of four quadrants proportionally representing the topography of each field. In unsprayed areas, egg and larval populations were usually below the economic threshold in early-planted crops but often exceeded thresholds in late-planted crops. In commercial demonstration trials where standard calendar spraying practice was compared with no spraying, calendar-based applications maintained fruit damage below 3.4%, but insecticide was applied unnecessarily to more than half the crops. Larval populations were a significant predictor of damage in these commercial crops. In 12 implementation trials, where spraying recommendations were based on the 1-min scouting threshold of one larva per plant, the worst fruit damage observed was 2.3%. The definition of an economic threshold, scouting methods, and establishment of parasitoids have reduced spray applications and contributed to the implementation of an integrated pest management program for processing tomatoes in New Zealand.

A computer-based record and clinical monitoring system for ambulatory care.

The Regenstrief Medical Record system is a computer-based medical record system for ambulatory care which has been in operation since 1973. For each patient served, it contains a core medical record including most objective data (treatment records, lab, x-ray and EKG, etc.) and selected items of subjective data. Its supports the care of patients by presenting the clinical data in clear and compact reports and by reminding the physician about clinical conditions that need corrective action. The generation of these computer reminders is totally controlled by physician authored protocols. Each protocol defines an important clinical condition and the content of the reminder to be triggered when that condition occurs. Physician compliance to the standards implicit in these protocols is improved by this feedback. Patient data are captured by computerized ancillary services, (laboratory and outpatient pharmacy) and by manual techniques. Much of these data are recorded by hand (as marks or numbers) on computer-tailored forms which can be optically read by machine. This system represents a different approach to the difficult problems of efficiently using and maintaining patient records.
