Hip joint range of motion reduction in sports-related chronic groin injury diagnosed as pubic bone stress injury.

Traumatic osteitis pubis is a non-specific entity that relates to chronic groin injury and has recently been described as being akin to a pubic bone stress injury. It is uncertain whether or not reduction of hip joint range of motion occurs in traumatic osteitis pubis. The purpose of this study was to establish whether there is a reduction of hip range of motion in athletes who have chronic groin injury diagnosed as pubic bone stress injury. A case-control study was performed whereby 89 Australian Rules footballers underwent, with clinical history unknown, clinical and MRI examination of the groin region. Clinical criteria (pain with tenderness) and MR-criteria (pubic bone marrow oedema) were used for diagnosis of pubic bone stress injury. End-range internal and external rotation hip motion was measured using a goniometer. Athletes with and without symptoms were compared, as were athletes with current symptoms with athletes who had recovered from their groin pain episode. Chronic groin injury was diagnosed in 47 athletes with 37 having pubic bone stress injury. Thirteen athletes had previous groin injury. A reduction of internal and external hip range of motion was demonstrated in athletes with pubic bone stress injury (p < 0.05) and in athletes who had current symptoms compared to those who had recovered from their groin pain episode (p < 0.05). A reduction in hip range of motion was evident in athletes with chronic groin injury diagnosed as pubic bone stress injury. There may be a role for increasing hip range of motion in rehabilitation.

[Polyethylene wear and late loosening of a total prosthesis of the hip joint. New perspectives for metal/metal pairing of the capsule and head].

Twenty-nine years' experience with total hip replacement surgery, including continuing quality control and follow-up studies, indicate that late loosening after 10-15 years is related to wear of the polyethylene cup. Metal-to-metal prostheses implanted 20 years ago and longer, on the other hand, show no signs of wear or loosening. In conclusion, polyethylene does not last long enough when compared to biocompatible precision-made artificial metal-metal hip joints.

[Spectral data of the elements of bone tissue in patients with hip joint implants].

The results of study of the elemental composition of bone tissue in patients with hip joint implants are described. The study was based on spectral analysis and correlation-regression analysis. It was shown that the elemental composition of bone tissue is not affected by long-term contact with the implant. This result should be taken into account in prophylaxis of implant instability.

[Septic infection of hip joint prosthesis with Listeria monocytogenes].

A case of Listeria monocytogenes septicaemia and infection of a prosthetic hip joint is presented in a patient with non-insulin-dependent diabetes mellitus. The patient developed the infection five and a half years after primary alloplastic operation. The prosthesis was removed and a temporary prosthesis with gentamicin was placed ("spacer"). The antibiotic treatment included ampicillin, pivampicillin and sulfamethoxazole-trimetroprim. Six weeks after removal of the prosthesis an un-cemented prosthesis was placed, and the patient had a full functional recovery.

[Peripheral nerve injuries following implantation of total endoprosthesis in the hip joint and their treatment].

The complication rate after 844 implantations of total endoprosthesis in the hip-joint, in the form of postoperative nerve injuries was found to be about 1 per cent. A number of causes may be responsible for nerve lesions, and some of these may be eliminated with a sparing operative technique. Although there are good prospects for the disappearance of paralytic symptoms, meaningful pysiotherapeutic after-care is very important. In the paralytic stage consistent exponential electrotherapy is recommended, and for recurring voluntary mobility active exercises against resistance. This will greatly improve the functional result of an operation with regard to nerve lesions.

[Seasonal fluctuations of intraoperative blood loss in hip joint replacement].

Under analysis was the level of intraoperative blood loss in total endopropsthesis of the hip joint in 513 patients operated upon for degenerative-dystrophic processes of consequences of trauma in the period from 1996 to 1999. The assessment of seasonal variations of its value was made. It was shown that the volume of blood loss is dependent on two groups of factors: medical and associated with the influence of the environment. The least "chronorisk" of excessive blood loss was noted in the period from July to November, the greatest--from January to June (with the maximum in March and reduction in May).

[The choice of the method of anesthesia as a risk factor in the femoral fracture near the hip joint in elderly patients].

Between January 1983 and December 1988, 317 patients with fractures near the hip joint were operated upon. In a retrospective study we compared 180 patients with general anesthesia (AN) and 137 patients with spinal anesthesia (RA). The average age of patients with RA was 80.5 and with AN 79.8 years. The preoperative management was done by an internist. The operation followed at an average of 2.8 days (Tables 1, 2). All patients were placed into 3 risk groups, based on after preoperative cardiac and pulmonary risk scores, as modified by Goldman (Table 3). Spinal anesthesia was performed with 2 ml 4% hyperbaric mepivacaine (80 mg mepivacaine hydrochloride, 190 mg glucose monohydrate). The AN was induced with thiopental (3-5 mg/kg) and maintained with N2O and enflurane. Fentanyl (0.05-0.15 mg) was given for analgesia. The statistical examination was done using the chi-square test (Fisher-exact or Pearson). The groups were compared for age, time of operation, and risk score (Table 4). The mortality in operations with protection of the femoral head (FP) (Ender pinnings, Böhler pinnings and dynamic hip screws) was significantly higher after RA (13.8%), than with AN (6.5%). In operations with hip prostheses (HP), the mortality for RA (19.0%) was also significantly higher then for AN (4.2%) (Fig. 1). The mortality in group I (4-20 points) was 1.7% in patients with AN (FP 1.4%, HP 2.1%) and 3.7% for RA (FP 4.1%, HP 0%).(ABSTRACT TRUNCATED AT 250 WORDS)

Glycosaminoglycan metabolism of the medial meniscus, the medial collateral ligament and the hip joint capsule in experimental osteoarthritis caused by immobilization of the rabbit knee.

A study was made of glycosaminoglycan metabolism in experimental osteoarthritis caused by immobilization of the rabbit knee in extension. Samples from the medial meniscus, the medial collateral ligament of the knee and the hip joint capsule were obtained and analysed after 2, 6, 10, 17, 30 and 87 days of immobilization, samples from the mobile limb serving as controls. The tissue concentrations of glycosaminoglycans were determined from measurements of hexosamine and uronic acid after prior papain proteolysis and subsequent purification. The uptake of 35S-sulphate (DPM/microgram hexosamine) was used as an indicator of the synthesis rate of sulphated glycosaminoglycans. In both early and advanced immobilization osteoarthritis, the synthesis rate and the content of glycosaminoglycans were increased in all tissues.

Changes in proximal femoral bone mineral density around a hydroxyapatite-coated hip joint arthroplasty.

OBJECTIVE: To present the results of a prospective analysis of proximal femoral bone mineral density changes around a hydroxyapatite-coated total hip joint replacement. METHODS: 14 patients with osteoarthritis of the hip were enrolled in the study and treated with an uncemented ABG prosthesis. Dual energy X-ray absorptiometry scanning was performed in 9 patients preoperatively, and at 3, 6, 12, and 24 months postoperatively. An orthopaedic software program was used to determine the bone mineral density in the proximal femur, expressed as a percentage of the preoperative value. RESULTS: The values of Gruen zones 1 to 6 averaged between 96.0% and 113.8% of the preoperative value by 24 months (overall average, 104.1%). In zone 7, however, there was a gradual decline in bone mineral density to an average of 72.1% of the preoperative value by 24 months. This represented ongoing loss of bone from the calcar; although this may not pose a problem to the prosthetic's short-term stability, it may render potential revision surgery more difficult. CONCLUSION: The initial outcome of uncemented total hip replacement appears to be promising. There was excellent maintenance of bone around the femoral component in all regions other than the calcar and lesser trochanter. Further scans are required to see if these trends continue in the long term.

[New instruments for the removal of an artificial hip joint from the shaft].

Two new instruments for the removal of intact of broken stems of artificial hip joints are described. The instruments and their application are illustrated in the figures 1-4. They produce a force along the longitudinal direction of the femur which breaks the metal to bone-cement bond. The use of these instruments reduces problems which may arise from fracture of the cortical bone of the femoral shaft or from an impairment of sterility.

Assessment of hamstring muscle length in school-aged children using the sit-and-reach test and the inclinometer measure of hip joint angle.

BACKGROUND AND PURPOSE: The sit-and-reach test (SRT) is commonly used to assess flexibility of the spine and length of the hamstring muscles. The purposes of this study were (1) to describe hamstring muscle length as reflected by use of the SRT and the hip joint angle (HJA) in children, (2) to examine the correlation between SRT and HJA measurements, and (3) to examine gender differences for both measures. SUBJECTS: The participants were 410 school-aged children (211 girls, 199 boys). METHODS: Each child performed the SRT. In the final position, the SRT score was obtained and the HJA was measured using an inclinometer placed over the sacrum. RESULTS: A mean SRT value of 24 cm and a mean HJA value of 81 degrees were obtained for all subjects. There was a strong correlation between the SRT and HJA measurements (r = .76). There was a difference between boys and girls for both measures. CONCLUSION AND DISCUSSION: The results suggest differences in expectations for hamstring muscle length in boys and girls. Although scores for the SRT and HJA were correlated, we prefer to assess hamstring muscle length using HJA scores because these scores are not influenced by anthropometric factors or spinal mobility. The results of this study suggest that HJA measurements guide treatment more effectively than do SRT measurements.

Subsurface microstructure of metal-on-metal hip joints and its relationship to wear particle generation.

To control and minimize wear of metal-on-metal hip joints it is essential to understand the mechanisms of debris generation. In vivo, mainly nanosize globular and needle-shaped particles are found. These can neither stem from the action of abrasion nor from tribochemical reactions. In this study the acting wear mechanisms have been first identified on the surface by means of scanning electron microscopy (SEM). Afterwards, the microstructures of the subsurface regions of explants have been investigated using a transmission electron microscope (TEM). Observation of the subsurface gave additional insight about the microstructural changes of cobalt-base alloys subjected to wear. At some distance from the surface, a network of stacking faults and hexagonal epsilon-martensite was found strengthening the bulk material. This microstructure changed into a nanocrystalline type moving closer towards the surface. A comparison of in vivo debris size and grain size of the surface suggests that the globular wear particles result from torn off nanocrystals, while the needle shaped particles are generated by fractured epsilon-martensite. Identified cracks, propagating through the nanocrystalline layer, further support these findings. Thus, it is suggested that the dominating mechanism of particle generation for metal-on-metal joints is surface fatigue within a nanocrystalline surface layer.

[Differentiated blood replacement therapy in endoprosthesis surgery of the hip joint].

In a prospective clinical study with 37 patients (41-84 years) undergoing a first implantation of hip joint endoprothesis, a total blood loss of two liters in average was registrated including the first 24 h postoperative wound drainage blood. Sufficient volume replacement was achieved without any homologous blood components by consequently using a comprehensive concept of autologous transfusion including acute preoperative hemodilution, intra and postoperative blood salvage and preoperative plasma predeposit (ATU = Autologous Transfusion-Concept of Ulm). Plasma and blood substitution was managed mainly by polygelin and autologous fresh frozen plasma completed by autologous warm blood and processed autologous washed packed red cells. Through this any organization troubles or disadvantages of whole blood predeposit may be avoided. To increase the net gain of washed packed red cells time limited use of blood salvage disposables should be extended.
