[Interposition of the fascia and capsule in treatment of hip joint dysfunction in rheumatoid arthritis].

A surgical method of recovery of functional activity and painlessness of the hip deprived of the cartilage in the loaded part has been presented. The operation consisted of interposition with the fascia or a joint capsule flap. The results obtained in 12 patients after 1-12 years of follow-up have been given. Recovery of functional activity was obtained in 8 patients and improvement in 4 patients. In comparison with a group of 32 similar cases of interposition on the background of another etiology; interposition in rheumatoid arthritis gives more good and fair results.

Rectus fascia colpopexy for complete prolapse of the vaginal vault after hysterectomy.

A simple operation, anterior abdominal wall colpopexy using autogenous strips of rectus fascia, to repair posthysterectomy prolapse of the vaginal vault, is described. It is mainly indicated for patients with complete vaginal eversion, in whom preservation of a physiologically useful vagina is highly desirable. Normal vaginal caliber and depth were secured and no recurrence has arisen in a series of nine patients operated upon for complete prolapse of the vaginal vault after hysterectomy, seven of whom have been followed up for six months to 12 years.

Ossification of the plantar fascia and peroneus longus tendons in diffuse idiopathic skeletal hyperostosis (DISH).

Extraspinal skeletal changes in diffuse idiopathic skeletal hyperostosis (DISH) consist of bony proliferation at ligamentous and tendinous insertions. We describe a patient with DISH who manifested ossific changes in the plantar fascia and peroneus longus tendon bilaterally at sites not immediately adjacent to the bony insertions of these structures. This observation expands the roentgenographic spectrum of DISH.

Control of sensory activation of granule cells in the fascia dentata by extrinsic afferents: septal and entorhinal inputs.

Three groups of rats were trained to perform a differential discrimination task in a 2-tone operant conditioning paradigm. One group received electrolytic lesions of the medial septal nuclei, another received electrolytic or knife cut lesions of the entorhinal cortex. These groups were compared with a normal control group. Recordings of granule cells in the fascia dentata were obtained in all animals during criterion performance of the behavioral task. Both lesions produced disruption of behavioral discrimination in the form of increased error and intertrial responding. Granule cell discharges to the tone stimuli were disrupted by each type of lesion. Septal lesions reduced the differential discharge tendency to CS+ and CS- and changed granule cell firing on all trials to statistically resemble firing on CS- trials in normal animals. Extensive lesions in the entorhinal cortex or knife cuts that severed the perforant path caused near elimination of the tone-evoked discharges to both the CS+ and CS-tones. Septal and entorhinal lesions caused marked changes in the sequential dependence of the granule cell discharge compared with intact animals. Results are discussed in terms of the control of the granule cell discharge by the remaining afferent pathways in each type of lesion condition.

Use of the rectus abdominis muscle and fascia flap in reconstruction of epispadias/exstrophy.

Inferiorly based rectus abdominis muscle flaps and fascial flaps have been used to construct a firm abdominal wall without hernias and to provide coverage of the bladder, bladder neck, and proximal urethra in the secondary reconstruction of patients with epispadias/exstrophy complex. They have also been used to produce an elevation of the mons area, which is lacking in the typical exstrophy patient. Rectus fascial flaps have been the mainstay of abdominal closure when wide diastasis of the rectal muscles is present and when the lower abdomen lacks fascial support. We are pleased with the results of utilizing either the rectus muscle or rectus fascia in this complex condition. We have been using fascial flaps for over 10 years in our epispadias/exstrophy closures and abdominal wall strengthening procedures. We have been using bone grafts and the rectus muscle for the construction of a mons for the past 5 years. Although the rectus muscle procedure to cover the bladder and the bladder neck has been in use for only 2 years, we have seen patients with incontinence restored to a continent state; these patients have not been plagued with fistula problems when a neourethra has to be constructed to elongate the phallus. Previous attempts at urethral repair in this area have been successful in the main, but an occasional fistula at the junction between the penile skin and the abdominopubic skin has resulted. Since using the rectus muscle to cover the bladder and bladder neck area, we have not had this problem. Patients with epispadias/exstrophy remain such difficult problems that every advance in technique should be used to improve results.(ABSTRACT TRUNCATED AT 250 WORDS)

Some biological properties of preserved bovine fascia enriched with pulverized calf cartilage.

The immunogenicity of trypsinized and radiation-sterilized bovine fascia enriched with pulverized calf cartilage was tested by the regional lymph node blastic cell reaction. The influence of these grafts on the adjacent tissue and on regeneration processes was examined histologically. After transplantation into mice the grafts stimulated growth of new connective tissue and did not evoke an increase of the blastic cell reaction in the regional lymph nodes.

[Surgical, biological and histological evaluation of radiation-sterilized Dallop-type muscle fascia prostheses].

The behaviour of Dallop knitted polyester nets, produced by the Research and Development Centre of Knitting Industry in łódź was experimentally tested. The prostheses were implanted in dog's abdominal walls, in subcutaneous tissue and in muscles fascias. After six months observation it was stated that they may be used in surgical clinic because of the small tissue reaction and proper healing into tissues. New type of packages for radiative sterilization was also favourably evaluated.

[Several properties of the connections between the dentate fascia and field CA3 of the hippocampus in vitro].

Topography of connections between the dentate fascia (DF) and the field CA3 was investigated on transverse hippocampal slices of guinea pigs in vitro. Recording of the extracellular spike activity from different loci in the CA3, during threshold electrical stimulation of DF has shown that while each pyramidal neurone responded to 3-4 widely spaced stimulated points, any CA3 locus responded to nearly all points in DF. The dispersion of latencies for each DF-CA3 distance was about 5--7 ms. Increase in intensity of stimulating current was followed by a decrease of long latencies to the minimal theoretically counted value characteristic for a given DF-CA3 distance. The same process was observed in the course of post-tetanic potentiation. It is suggested that dispersion of response latencies depends upon different levels of synaptic modification, obtained in vivo.

Tracheostomy tube insertion in the pretracheal fascia.

A case of massive dissecting emphysema secondary to attempted emergency tracheostomy is reported. Malpositioning of the tube in the pretracheal fascia and positive pressure ventilation were the cause of the dissecting emphysema. This problem was recognized with subsequent removal of the tracheostomy tube, but unfortunately the skin was sutured which prevented air from escaping. The diagnosis was made by physical examination and chest roentgenogram. It was successfully treated by reopening of the wound and evacuation of clots. Careful technique performed under optimal conditions is of paramount importance in avoiding this complication.

Reactive synaptogenesis and neuron densities for neuropeptide Y, somatostatin, and glutamate decarboxylase immunoreactivity in the epileptogenic human fascia dentata.

This study determined differences of fascia dentata (FD) peptide and inhibitory neuroanatomy between patients with epileptogenic hippocampal sclerosis (HS), those with extrahippocampal seizure pathologies, and autopsy comparisons. Surgically treated temporal lobe epilepsy patients were clinically classified into two pathogenic categories: (1) HS with focal mesial temporal neuroimaging and histories of initial precipitating injuries to the brain (n = 18) and (2) non-HS patients with extrahippocampal mass lesions or idiopathic seizures (i.e., without lesions or HS; mass lesion/idiopathic; n = 9). The hippocampal sections were studied for (1) granule cell, hilar, CA4, and CA3 neuron densities; (2) hilar densities and the percentage of neurons immunoreactive (IR) for neuropeptide Y (NPY), somatostatin (SS), and glutamate decarboxylase (GAD); (3) densities of GAD neurons in the lower granule cell and infragranular zone (basket-like cells); (4) the semiquantitative pattern of IR peptides/GAD FD molecular layer axon sprouting; (5) IR gray values (GV) of the FD molecular layers; and (6) the thickness of the supragranular molecular layer. Results showed the following. (1) Compared to autopsies, both HS and mass lesion/idiopathic patients showed less granule cell and CA3 neuron densities, but there were no statistical differences between the latter two pathogenic categories. (2) By contrast, compared to autopsies and mass lesion/idiopathic cases, HS patients showed less hilar and CA4 neuron densities, and there were no differences between autopsies and mass lesion/idiopathic. (3) Compared to autopsies, the NPY and SS hilar neuron densities in HS patients, but not mass lesion/idiopathic cases, were less. (4) Compared to autopsies, the hilar GAD neuron densities for HS and mass lesion/idiopathic patients were not less. (5) In HS patients the averaged percentages of hilar SS neurons were less than autopsies, and no other differences of IR hilar percentages were found. (6) The densities of GAD basket-like neurons and the thickness of the supragranular molecular layer were not different between any combination of pathogenic categories and autopsies. (7) By semiquantitative visual assessments, peptides/GAD axon sprouting into the FD was greater in HS compared to mass lesion/idiopathic or autopsies. (8) Compared to mass lesion/idiopathic cases, in HS NPY outer molecular layer GVs were lower, SS GVs were not different, and GAD inner molecular layer GVs were higher. (9) Analyses comparing the two pathogenic categories and neuron densities with peptides/GAD axon sprouting found six comparisons that correlated sprouting with hilar and CA4 neuron losses, and four comparisons showing greater sprouting in HS compared to mass lesion/idiopathic.(ABSTRACT TRUNCATED AT 400 WORDS)

[Fascia superficialis temporalis in facial reconstructive surgery. Apropos of 35 cases].

The fascia superficialis temporalis (FST), one of the planes of cranial cover, can be raised to form an axial pedicle flap or a vascular island flap on superficial temporal vessels. Its fibrovascular structure, the area able to be raised and the possible addition of other superficial tissues (scalp, forehead skin) and deep tissues (calvarial bone) mean that this flap can be used in surgery of the external and intracavitary facial contours and shapes. The authors report their experience of 35 facial reconstructions: 7 ear reconstructions, 5 suspensions of a paralysed labial commissure, 10 conjuctivo-palpebral revisions, repair of 5 subcutaneous defects, 3 haired resurfacings, 1 inferior labial, 1 cheela, 1 orbitomalar reconstruction, 1 malo-commissuro-labial repair, 1 prefabricated free flap. The donor site sequelae were considered to be negligible: 1 case of temporal anaesthesia with dysaesthesiae, 4 cases of band alopecia. The analysis of these 35 reconstructions reveals the quality of all cases of defect repair, especially palpebral; the ease and persistence of suspensions of the labial commissure; the value of FST as a conjunctival and buccal mucosal substitute; the density of the hair contribution. However, total or partial ear reconstruction using this technique constitutes a more limited indication.

[Functional follow-up of combined musculocutaneous tensor fasciae latae and rectus femoris flap repair. Supplementary information on the Handchir. Mikrochir. Plast. Chir. 21 (1989) 315-317 contribution].

As a supplement to our publication in the Journal Handchirurgie, Mikrochirurgie, Plastische Chirurgie 21 (1989) 315-317, we would like to report a five-year follow up of a patient with recurrent sigmoid carcinoma, infiltrating all layers of the abdominal wall. Treatment was by radical tumor excision and reconstruction with a combined tensor fasciae latae and rectus femoris flap. Abdominal wall, hip, and knee functions were evaluated by gait-analysis (Motion Analysis Corporation AMTI) and dynamometry (Cybex 6000). The dynamometry of knee motion showed a slight deficit of the operated side regarding the parameters of strength-endurance as well as work-recovery. All other parameters (peak-torque) were equal for both sides corresponding to the activities of daily living of the patient. Gait-analysis showed a reduced rotation of the pelvis as well as a functional deficit of extension of the knee during stance and swing phase. We can document a good functional result in all motions without major impairment of every-day activities.

[Quality control after introduction of a new hernia technique. Barwell transversal fascia-plasty].

From 1987 to 1991 we operated on 320 patients with 360 inguinal hernias with the recently introduced transversalis fascial repair after Barwell. The transversalis fascia is duplicated with an atraumatic looped nylon 0.43 trainees performed 70% of the operations. The follow-up was 30 months (6-62) for 317 (88%) hernias (two examiners). Over all we observed eleven (3.5%) recurrences, in 8 cases after primary operation. Two of this recurrences were initially missed femoral hernias. In the learning phase we found five early recurrences in the first 60 patients and in the following 257 patients with the same follow-up only 6 recurrences. We conclude that with the new technique of transversalis fascial repair after Barwell very good results can be achieved even with a big number of young surgeons in training.

Prevertebral fascia flap in hypopharyngeal reconstruction: a preliminary report.

Ablative surgery for pyriform sinus cancer often leaves a defect which cannot be closed primarily. Many techniques for hypopharyngeal reconstruction have been described but no single technique can be recommended for use in all situations. The prevertebral fascia flap described in this article provides an alternative to hypopharyngeal repair when a more complex technique is contra-indicated. It is a one stage procedure, fashioned with locally available tissue, and technically simple. Its use in two anecdotal cases is described herewith.

[Use of a fascia-cutaneous island flap based on the posterior interosseus artery of the forearm--a case report].

The use of fascia-cutaneous island flap based posterior interosseous of the forearm to cover cutaneous defect of the thumb after resection of the scar is presented. Loss of the posterior interosseous artery does not influence adversely circulation of the hand. Preparation of is technically demanding; unsightly scar on the dorsal forearm restricts its use in women. The flap is being used in secondary repairs.
