Ultrasonographic monitoring of artificially stimulated ejaculation in three rhinoceros species (Ceratotherium simum, Diceros bicornis, Rhinoceros unicornus).

Manual massage of the penis and rectal electroejaculation methods have been minimally effective for collecting semen from the rhinoceros. These two methods for stimulating ejaculation were evaluated by rectal ultrasonography. One individual each of three rhinoceros species (Ceratotherium simum simum, Rhinoceros unicornis and Diceros bicornis) was stimulated by manual massage of the penis and two black rhinoceroses (Diceros bicornis) were electroejaculated. On ultrasonography, neither manual massage nor rectal electroejaculation affected the accessory glands. The pelvic urethra remained empty during manual massage of unconditioned animals; however, this area was filled before commencement of massage of conditioned animals. The pelvic urethra also filled during electroejaculation. Semen accumulated in the pelvic urethra during pauses between electrical stimulations and was moved distally into the penile urethra by rectal and penile massage. The volumes of seminal fluid recovered from electroejaculation in this study exceeded previously reported attempts. This study demonstrated the potential of transrectal imaging for improving the recovery of semen by these methods.

Effects of vasectomy on fructose levels in the ejaculate of rams.

Fructose concentration was studied in the ejaculates of 20 intact and 19 vasectomised rams over a two year period. In intact rams the concentration ranged from 222-740 mg/100 ml of seminal plasma during the breeding season (October to December), with lower values (0-300 mg/100) during the rest of the year. In vasectomised rams the seasonal pattern was less marked with very high values (800-1260 mg/100 ml) as well as low values (0-20 mg/100 ml) being encountered during both the breeding and non-breeding seasons, indicating changes in accessory gland function.

Disorders of ejaculation.

Following John Hunter's example set over 200 years ago, it is clear that there is much to study and understand in disorders of ejaculation. Congenital malformations and their relationship to genetic disorders are now more clearly understood: these are matters of the utmost importance in the present era of assisted reproduction, if perpetuation of serious anomalies, such as fibrocystic disease, is to be avoided. Surgically induced injuries that impact upon male reproductive function are now recognized and largely preventable by careful attention to preservation of normal structures during extirpative surgery. Much work remains to be done on functional disorders, especially on the side effects of drug therapy, where some insight may be gained into the function and disorders of the basal ganglia--the 'dark basements of the mind'.

Serum testosterone and estrogen concentrations in the Holstein-Friesian bull after successive ejaculations.

The testosterone and estrogen concentrations in venous serum of 4 mature Holstein-Friesian bulls were determined by radioimmunoassay before and after each of 3 consecutive semen collections taken at 1-hour intervals. The hormonal concentrations were highly variable for the 2 steroids, but uniform in their patterns after the bulls had ejaculated. After the 3rd semen collection, marked changes in hormonal concentrations did not occur.

Premature ejaculation. 2. Classification and diagnosis.

A classification of premature ejaculation must distinguish between hyper-orgasmic or hypo-orgasmic, between situational or global; furthermore, it must define whether it occurs during vaginal penetration only or also in masturbation, and must study its latency periods and its relationships to the erectile dysfunction, with which it is often associated. Tests with local anesthetics, biothesiometry and penile vibrotactile stimulation, integrated with a thorough study of the general and psycho-sexual history, provide a good diagnostic classification that makes the therapeutic approach appropriate.

Premature ejaculation. 3. Therapy.

Serotonergic drugs (SSRIs) are the most commonly used, but they are characterized by relapse some time after medication interruption as well as by sexual side effects. The efficacy of phosphodiesterase-5 inhibitors seems excellent, but the risk of tachyphylaxis has been reported. The former (fluoxetine, paroxetine, sertraline, clomipramine) should be used in young patients with hyper-orgasmic forms, while the latter (sildenafil, tadalafil, vardenafil) should be used in hypo-orgasmic forms, in old age or when PE is associated with erectile dysfunction. Topical anesthetics provide satisfactory results in premature ejaculation due to hypersensitivity of the glans, and physiotherapy of the pelvic floor muscles proves successful in cases associated with pelvic floor dysfunction. Therapeutic associations and psycho-sexual therapy techniques may improve results, particularly in the long term.

[Treatment of infertility, caused by retrograde ejaculation, using intrauterine insemination with sperm enhanced by the "swim up" method].

The causes and the pathomechanism of retrograde ejaculation are reported by the authors. In their case following the failure of the conservative treatment a successful intrauterine insemination was carried out in the superovulatory monitored cycle by semen are covered from the postcoital urine and washed in Ham-F-10. After an undisturbed twin pregnancy a mature male and a female infant was born using Caesarean section. There is a greater chance for pregnancy if the sperms recovered from the postcoital urine are numerous and if their motility is good.

[Premature ejaculation--results of treatment of men of 2 different cultural backgrounds].

The object of this investigation was to compare Danish and Muslim men treated for premature ejaculation in the Department of Sexology, Rigshospitalet, Copenhagen. The investigation is a retrospective review from the period 1986-1989. The following variables were included in the investigation: Mode of referral, personal data, duration of sexual dysfunction, previous sexological/psychiatric treatment, methods of treatment and results of treatment. The material consists of 60 Danish and 32 Muslim men. The investigation reveals that the Muslim men predominantly receive individual therapy of brief duration. Fewer Muslims were instructed in sensuality training and fewer carry out stop-start-treatment. The Muslim men responded more poorly to treatment than the Danish men. The differences in the therapeutic methods and therapeutic results cannot be explained on the basis of the personal parameters and possible cultural obstacles to treatment are suggested. For example, the therapist must become familiar with taboos and beliefs in the Muslim culture. The referring instance should inform the patients about the therapeutic principles at the Department of Sexology and attempts must be made to motivate possible partners to participate in the treatment.

[Premature ejaculation: diagnosis, classification, algorithm of the patients' examination and results of their application].

We have analysed literature data and own experience with management of premature ejaculation (PE) in 3200 patients. This gave grounds for formulation of this phenomenon, creation of the classification and the examination algorithm. We applied the proposed methods in the treatment of 258 PE patients and made the conclusion about their adequacy and efficacy. PE is a polyetiological phenomenon and a multidisciplinary problem. Urological examination must be used as the first stage of the diagnostic process.

[Characteristics of associated microflora of ejaculate from males with gonococcal infections].

Species composition and biological properties of aerobic and anaerobic microflora from an ejaculate from males with acute and chronic gonococcal infection and from healthy men were studied. Patients with gonococcal infection were shown to present quantitative and qualitative changes in microbiocenosis of the reproductive tract. Patients with acute gonorrhea displayed decreased species diversity in contrast to healthy persons, while different species of associated with gonococci microbes with high persistent potential, were cultured in patients suffering from a chronic form of infection. The role of revealed pathological biocenosis patterns in developing chronic infection as well as postgonorrheal gonococci-free urethritis is discussed.

[Interventional effect of behaviour psychotherapy on patients with premature ejaculation].

OBJECTIVE: To investigate the interventional effect of comprehensive behaviour psychotherapy on the ejaculatory latency of premature ejaculation (PE) patients, sexual satisfaction of sexual partners, as well as its influence on the results of clinical treatment. METHODS: Ninety PE patients were randomly divided into a psychological intervention group (n = 45) and a control group (n = 45). Both groups were given medicine therapy, and the former also received comprehensive behaviour psychotherapy for 6 weeks. All the patients were assessed with the Chinese index of sexual function for PE and ejaculatory latency in the vagina, and the clinical efficacy was compared between the two groups. RESULTS: Before treatment, the ejaculatory latency in the vagina was (0.69 +/- 0.25) min and (0.71 +/- 0.19) min respectively in the intervention and the control groups, as compared with (5.87 +/- 0.59) min and (4.76 +/- 0.54) min before treatment, with significant difference (P < 0.01). In the intervention group, the scores in the control of ejaculatory reflex, the sexual satisfaction of the patients and their sexual partners and anxiety or depress in sexual activity in CIPE were higher than in the control group, with significant difference between the two groups (t = 2.12, 2.31, 2.01, 2.24, P < 0.05). The difference in the SAS score after therapy was of significance (P < 0.01). A month after treatment, the effectivity rates of the two groups were 82.9% and 30% respectively, and the difference was significant (P < .01). CONCLUSION: Comprehensive behaviour psychotherapy obviously adds to the clinical efficacy of drugs in the treatment of PE.

[Profile of the membrane potential of mammalian sperm during maturation, ejaculation and capacitation].

The accumulation of lipophilic cation radiolabeled tri-phenylmethyl phosphonium (TPMP+) was utilized to determine the resting membrane potential across the plasma membrane psi. Washed sperm of mammalian was suspended and incubated in low-K+ and high-K+ medium and allowed to take up the cation to a steady-state (20 min at 37 degrees C). By using this differential, the value obtained was transformed at value concentration using the intracellular volume and with these values inserted in the Nernst equation, we obtained the psi. During the maturation process the psi obtained in sperm cells of caput of the epididymis of New Zealand white rabbits was -75(+/-)-3 mV, in sperm cells of corpus and cauda, the psi obtained were -60(+/-)-5 and -52 +/- 3.5 mV, respectively. In the ejaculated spermatozoa the psi obtained was -69 +/- 2 mV. Finally, during the capacitation of human spermatozoa induced by the addition of calcium ions and the ionophore A23187 the psi obtained was -35 +/- 4 mV. These changes in the psi are significant because when the mammalian sperm cells are under different physiological conditions the membrane polarity is modified for the hyperpolarization and depolarization observed.

Effect of repeated semen ejaculation on sperm quality.

The semen quality of 21 Medical students who produced sperm by masturbation has been assessed. The mean density of 21 samples examined on the first day was 64,4 millions per millilitre, while that on the third and fifth day of the study period were 52.2 and 50.7 millilitre respectively. This showed a statistically significant difference between the first and the third samples. Another statistically significant difference (P less than 0.5) was demonstrated in the semen volume of the first and third, and the third and fifth samples respectively. Significant differences were, however, not observed in the motility and morphology changes in the group samples. We therefore conclude that even though repeated semen ejaculations on alternate days do affect the density and volume of the semen, the most important overall effect is not such as to impair the potential fertility of the man who has normal initial sperm characteristics.

Delay of appearance of spermatozoa in the ejaculate after vaso-epididymostomy e vaso-vasostomy.

After ultimately successful microsurgery for male infertility the delay for appearance of spermatozoa in the ejaculates of the patients is not uniformly immediate. For the older latero-lateral epididymo- vasostomy it could take several months. It is much shorter when the actual microsurgical techniques are used. Occasionally however, even in the latter group, very long delays have been observed. There is no definite satisfying answer for this. The most likely explanation is a combination of reinforced peristalsis in the previously pathologically dilated epididymal tubules and the secondary breakthrough of a plug of epithelial sloughs and fibrin at the site of microsurgical anastomosis.

Erectile dysfunction and premature ejaculation.

In parallel with the increased recognition of organic causes of erectile dysfunction, several new physical methods of treatment have been developed for this problem. These include intracavernosal injections of vasodilators, penile prostheses, vascular surgery, vacuum condoms, and medication. However, psychological treatment approaches are still of considerable importance in the treatment of both erectile dysfunction and premature ejaculation.

Cortical evoked potentials in subjects with true premature ejaculation.

Cortical somato-sensory evoked potentials (SEPs) from pudendal stimulation were determined in 23 patients with true premature ejaculation (TPE) and in 25 subjects as a control group. SEPs give a cortical representation of the sensory stimuli arising from the genital area. The amplitudes of the evoked response in the cortical area (P1-N1-P2 wave) were 4.96 +/- 1.89 microV in subjects with TPE and 3.79 - 1.38 microV in the controls (means +/- SD). This difference is statistically significative (P less than 0.005). Our results show a greater cortical representation of the sensory stimuli from the genital area and suggest an organic basis for TPE.
