Extending polymerization time of polymethylmethacrylate cement in percutaneous vertebroplasty with ice bath cooling.

Currently available preparations of polymethylmethacrylate (PMMA) cement for percutaneous vertebroplasty have injectability times of 4-15 minutes. The potential for early polymerization requires procedures to be performed as fast as possible, sometimes with suboptimal results and waste of the cement. By cooling the PMMA mixture in an iced bath of sodium chloride solution, we can extend its injectability to over 2 hours and use one kit for the controlled treatment of multiple levels with successive needle placements. We have been using this technique since mid-1998, treating more than 600 vertebral bodies.

[Update of DIN 19.643--treatment and disinfection of swimming pool and bathing tub water].

German Standards Specification DIN 19,643 is at present under revision for health reasons and because of both negative and positive experiences gathered in practice. To enable adaptation of the standards specification to future developments, a Part I of the specification is being created comprising the demands to be made on the quality of the water and general demands on the construction and operation of swimming pools and tubs and basins in bath houses, e.g. in spas or municipal swimming pools. The subsequent parts of the new specification (Part 2 to Part n) concern the demands to be made on individual combinations of processes; these can be supplemented at any time in accordance with technical progress without requiring revision of the entire standards specification. Essential innovations are the reformulation of the required efficiency of disinfection, the introduction of the parameters Legionella pneumophila, trihalogen methane (THM) and the reduction of the limit value for chloramines. Technically speaking, the new features concern the automatic measurement of the auxiliary parameters of hygiene such as redox potential, pH value and free chlorine, automatic control of disinfectant additions, automatic filter rinsing with fluidization of the filter-bed to a prescribed minimum bed expansion, and the sight-glas at the filter container. The demands made on Jacuzzi and warm water spouted bed besins are integrated into the specification, thus obviating the need for German Standards Specification DIN 19,644.

Influence of calcium removal from bath solution on isolated rat duodenum contractions.

In this study whether extracellular Ca++ is essential to produce an increase of tension of isolated rat duodenum by ACh, 5-HT, AVP and KCl-, was examined. KCl- and AVP-evoked contractions were almost totally prevented by Ca++ removal from bath solution. The increase of tension of isolated duodenum caused by ACh or 5-HT was totally prevented after adding nifedipine, a Ca++ channel blocker, into Ca free solution. Our results suggest that ACh or 5-HT utilizes intracellular as well as extracellular sources of Ca++ to produce contraction in rat duodenum, whereas AVP-or KCl evoked contraction was mainly due to influx of Ca from extracellular sources.

Photodegradation of hydrophobic disperse dyes and dye-bath effluents by silicadodecatungstate (SiW12O40(4-/5-)) nanoparticles.

Polyoxometalate (POM) silicadodecatungstic acid has been applied as a photochemical catalyst for the degradation of SETAPERS Black WNSP, a disperse dyestuff preparation widely used to dye polyester and polyamide fabrics. It could be demonstrated that the disperse dyestuff was photo-reduced by SiW12O40(5-), the one-electron reduced form of POM, as evidenced by Heteropolyblue (HPB) formation. For completion of the photochemical redox cycle, isopropanol (IsOH) was required. Acetone (Ac) served solely as an effective solute and photosensitizer; however this effect was suppressed in the presence of POM. Threshold (0.087 mM) and optimum (0.375 mM) POM concentrations existed and decolorization kinetics were inhibited upon the addition of dye auxiliary chemicals. Increasing the dyestuff concentration from 50 mg/L to 150 mg/L did not affect initial decolorization kinetics revealing that not the formation of the excited [POM-Substrate]* complex, but its reduction to HPB was the rate limiting step. POM-mediated, IsOH-assisted UV-photodegradation of disperse dyes and dye-baths is by far more effective than applying other, more well known chemical oxidation methods (O3, H202/UV, Power Ultrasound). Key to the action of POM redox catalysts is the feature that particularly heteropoly tungstates undergo facile re-oxidation to their original state, thus allowing regeneration of the photocatalyst, a feature that may become critical for real-scale application.

Impact of agricultural practices and river catchment characteristics on river and bathing water quality.

The objective was to investigate the potential risk of faecal indicator organism (FIO) bacteriological contamination of river catchments and coastal bathing waters from farm management practices and to develop practices to reduce the risk. A risk assessment on 117 farms was carried out in two river catchments in south-west Scotland. Manure storage facilities, farming practices, field conditions and catchment characteristics were assessed. River samples at 33 locations were regularly taken and analysed for FIOs. Available manure storage capacity and farm management practices are inadequate on a high proportion of farms and FIO contamination of watercourses was likely the result of effluent transported into watercourses due to non-collection or poor containment. In addition, surface run-off or leaching following land application of manure or intensive stocking in adverse conditions was a high risk on up to 50% of farms. The concentrations of FIOs in the streams of two sub-catchments with high livestock intensity was 4 to 8 times higher compared to the two sub-catchments which had a low livestock intensity. The majority of potential risks of agricultural pollution to watercourses may be eliminated through improved manure and dirty water management, forward planning of manure spreading activities and improved operational procedures.

Inductively coupled plasma compared with direct current arc spectrometry for analysis of minor elements in aluminium baths.

A validation study was carried out in order to evaluate the efficiency of inductively coupled plasma-optical emission spectrometry (ICP-OES) for the analysis of minor elements (manganese, chromium, copper, iron, and titanium) in aluminium alloys. Aluminium casting samples were obtained by adding compressed powder compacts of each alloying element and aluminium (minitablets) to aluminum baths in a laboratory crucible furnace. Digestion of solid samples was performed using concentrated HCI and H202 35% (v/v) previous to analysis by ICP-OES without any matrix separation. This solution-based method was validated considering direct current arc spectrometry as the reference method based on direct analysis without any pretreatment of the solid samples considered. Univariate statistical procedures were carried out, for which precision <3% and trueness of the analytical results were taken into account.

Evaluation of a Romanian version of the Bath Ankylosing Spondylitis Functional Index (BASFI) in patients with spondylarthropathies.

OBJECTIVE: To develop a Romanian version of the Bath Ankylosing Spondylitis Functional Index (BASFI) and to determine its metric properties in patients with spondylarthropathies (SpA). METHODS: A Romanian version of BASFI was obtained after translation, back-translation and pretesting. Altogether 41 SpA patients fulfilling the ESSG criteria were included, 34 of them satisfying the modified New York criteria for ankylosing spondylitis. BASFI was completed by the patients along with assessment of clinical and disease activity variables by both the patient and physician. Scalability, internal consistency, reliability and construct validity of the Romanian BASFI were assessed. RESULTS: Mean time to complete the questionnaire was 2 minutes. Scalability (0.2-9.7, 95% of the scale), internal consistency (Cronbach's alpha = 0.93), reliability (intraclass correlation coefficient = 0.82) and construct validity (correlations (absolute value(r) = 0.37-0.59) with spinal pain, nocturnal pain, patient's and physician's global assessment, fingers-to-floor distance, occiput-to-wall distance, modified Schober test, intermalleolar distance, and Dougados articular index) were all very satisfactory. CONCLUSIONS: Translation and cultural differences did not alter the well-known value of BASFI. Therefore, the Romanian version of BASFI is reproducible and valid for the assessment of patients with spondylarthropathies.

Knowledge of the effects of sun exposure of Turkish high school students and their sun bathing habits.

Cancer, long a serious problem in developed countries, is now becoming a serious health concern throughout the world. There has been an alarming increase in the number of new cases of melanoma each year, this cancer increasing at a faster rate than any other neoplasm in some regions. This may be a result of depletion of the ozone layer. An association between non-melanocytic skin cancer and exposure to the sun appears to have first been suggested in 1894; it was not until about 1952 that it was argued that exposure to the sun also causes melanoma. It is commonly believed that skin cancers develop only after long-term exposure to UVR. At the individual level, people who live in sunnier climates comparatively have a higher risk for skin cancer than do people who live in colder climates. This is particularly the case for migrant populations with a fair skin. The present study was conducted to evaluate the knowledge of the young generation in Turkey related to the side effects of sun exposure, and their sun bathing habits. Sun sensitivity, use of sunscreens, sunbathing habits and protective behaviours were determined for a total of 1244 high school students from the answers to a questionnaire completed by them. Data from 627 (50.4 %) females and 617 (49.6 %) males were evaluated. All were aged between 15 and 18 years. Sunscreen use was found to be higher in female students (59 %) than in their male counterparts (45.8 %). The wearing of sunglasses was reported by 36.7 %, whereas the incidence of hat wear was found to be 55.1% overall. The results of this study indicate that, although most high school students are aware of the side effects of sun exposure, they do not pay sufficient attention to protective behaviour. A further study should now be conducted to evaluate the use of sunscreens in a large group.

Bathing trunk naevus and neurofibromatosis type 1: a unique association of Friedrich's ataxia.

A 15-year-old male presented with history of progressive instability of gait for last 6 months. General examination revealed multiple subcutaneous nodules all over the body. There was a large pigmented naevus over the lower back area along with a subcutaneous bosselated swelling over right arm. Neurological examination revealed features of gait ataxia. Plantar reflexes were extensor on both sides. Skin biopsy from the pigmented naevus showed features of melanocytic naevus. Electrophysiological studies of both common peroneal and sural nerves revealed features of axonal neuropathy. With the above clinical and laboratory findings a diagnosis of neurofibromatosis type 1 accociated with bathing trunk naevus and Friedrich's ataxia was entertained.

[Effectiveness of balneotherapy in patients with neurasthenia using baths of mineral water under conditions of low-mountain health-resort Nunisi].

Total of 118 patients with neurasthenia were investigated. Among them in 62 persons a hypersthenic form and in 56 -- a hyposthenic form of the pathology were noted. It has been established that balneotherapy using baths of mineral water, which is low-mineralised weakly sulfide, chloride-hydrocarbonate mineral sodic water under conditions of low-mountain health-resort Nunisi induces a decrease right up to disappearance of complaints and pathological changes in neurological status in patients with mentioned pathology. At the same time it improves indices of perception, attention and memory, state of vegetative nervous system and functional state of the brain, has a normalising action on the indices of cardiohemodynamics and lipid metabolism as well as on the excretion with urine of adrenaline and noradrenaline, neutral 17-ketosteroids, free and total 17-oxicorticosteroids. Mentioned positive shifts were more pronounced at hypersthenic form of neurasthenia. The treatment carried on had a positive influence in 62 (100%) patients with hypersthenic form of neurasthenia and in 51 (91,07%) -- with hyposthenic form.

Bathing-suit ichthyosis. A peculiar phenotype of lamellar ichthyosis in South African blacks.

Autosomal recessive congenital ichthyosis is a heterogeneous group of disorders of cornification showing a wide spectrum of clinical phenotypes. Here, thirteen cases of a so far unreported distinct type of autosomal recessive congenital ichthyosis are described. All the skin lesions were present at birth and ten were then compatible with collodion baby. Shedding of the collodion led to an ichthyotic condition with a very characteristic distribution of the lesions which involved the trunk, the most proximal parts of the upper limbs, scalp and neck but spared the central face and extremities. The term "bathing-suit" ichthyosis is suggested for this unusual phenotype of lamellar ichthyosis.

Estimation of the Bath Ankylosing Spondylitis Disease Activity Index cutoff for perceived symptom relief in patients with spondyloarthropathies.

OBJECTIVE: To estimate the best Bath Ankylosing Spondylitis Disease Activity Index (BASDAI) cutoff based on patients' perceptions of symptom relief collected in a large population of patients with spondyloarthropathies (SpA), in comparison to the BASDAI cutoff determined by experts. METHODS: A survey of patient perceptions about current disease control was conducted among the members of Spondylis, one of the main not-for-profit SpA patient organizations in France. BASDAI was among data collected by the questionnaire. To estimate the best BASDAI cutoff for discriminating between poor and well perceived controlled groups, we plotted the receiver operating characteristic (ROC) curve. We also determined the cutoff separately in male and female patients. RESULTS: Of the 1000 mailed questionnaires, 485 were returned without any missing data regarding perceived disease control and the BASDAI. Of these patients, 55.3% perceived inadequate control of their disease. The mean BASDAI in the overall population was 43.5 +/- 22.9, 30.4 +/- 19.9 in the well controlled group and 54 +/- 19.4 in the poorly controlled group (p < 0.001). The best BASDAI cutoff for discriminating between patients in the 2 groups was 39 (sensitivity 74.6% and specificity 72.4%). According to gender, the best cutoff was 44 for women and 36 for men. CONCLUSION: The best BASDAI cutoff of 39 based on patients' perceptions was very similar to that selected by international experts, i.e., 40. Gender affected the cutoff for perceived symptom relief in our study. These results need to be confirmed by further studies collecting the opinions of both patients and physicians.

Redox properties of the hydroxylase component of methane monooxygenase from Methylococcus capsulatus (Bath). Effects of protein B, reductase, and substrate.

The reduction potentials of the hydroxylase component of the soluble methane monooxygenase from Methylococcus capsulatus (Bath) have been investigated through potentiometric titrations. The potentials were determined by EPR spectroscopic quantitation of the mixed valent hydroxylase as a function of added sodium dithionite in the presence of appropriate mediators. The reduction of the oxidized Fe(III).Fe(III) form to the mixed valent Fe(II).Fe(III) form occurs at 48 mV versus NHE while the potential for the formation of the fully reduced Fe(II).Fe(II) species from the mixed valent form was determined to be -135 mV. Addition of the substrate propylene to the hydroxylase did not have a major effect on the reduction potentials. Introduction of the protein B and the reductase components, however, completely inhibited reduction of the hydroxylase at potentials as far negative as -200 mV. Addition of propylene to all three methane monooxygenase components greatly facilitated hydroxylase reduction. Under these conditions, the fully reduced form of the protein was obtained at potentials of greater than 150 mV. This high redox potential indicates that the oxidized form of the protein is highly reactive, as required for methane oxidation. The present results reveal aspects of how both protein B and substrate can regulate electron transfer into and out of the hydroxylase component of methane monooxygenase.

[MUG-lauryl sulfate bouillon--an optimal medium for the detection of total coliforms and fecal coliform bacteria in relation to hygienic evaluation of bathing waters according to the European Community Guideline 76/160 EWG].

In the monitoring of bathing waters according to the EC-guideline, total coliform and faecal coliform bacteria were obtained from 97 samples of river waters by the comparison of two fluorogenic assays using MUG-Lauryl Sulfate Broth and MUG-BRILA Broth with an usual method with MacConkey Broth, Tryptophane Broth and BRILA Broth. MUG-BRILA Broth had the lowest average counts and proved to be not an appropriate medium. Compared with the usual method, however, MUG-Lauryl Sulfate Broth yielded similar high counts, reduced the investigation period from four to two days and diminished the expenditure of work by one third. In addition, the indole test for the confirmation of presumptive faecal coliforms might be abolished. Therefore, the examination with MUG-Lauryl Sulfate Broth can be recommended.

[Medical Ischl. Brine bath, imperial summer residence, military city].

In 1823 Josef Goetz and Franz Wirer v. Rettenbach established the first brine bath at Ischl. Having finished a cure there, Archduchess Sophie, who had been childless, gave birth to a son, who subsequently became Emperor Franz Joseph. For 65 years (as from 1849) Ischl was his summer residence. Wirer, Josef Brenner-Felsach and Max Mayer founded the first hospitals for the inhabitants of Ischl. Another medical institution was the Emperor Franz Joseph Children's Hospital of the City of Vienna at Ischl/Sulzbach. In 1943 the military hospital of the German Air Force for treatment of airmen with injuries of the brain, spinal cord and nerves was transferred to Ischl from Berlin. Under Dietrich Wolfgang Krüger it became the cradle of Austrian neurosurgery after the war (till 1968). The neurologist Karl Theodor Dussik, head of the neurological ward of the Military Hospital at Ischl was the first to use ultrasound for the diagnosis of diseases of the central nervous system. He is internationally regarded to be the father of ultrasonic diagnosis.
