Rare tumors of the thyroid gland.

Although the vast majority of thyroid cancers are well-differentiated and carry an excellent prognosis, several rare tumors occasionally involve the thyroid gland and require unique treatment paradigms. Anaplastic carcinoma of the thyroid is an extremely aggressive neoplasm that is generally best managed with surgery followed by a multimodality regimen of radiotherapy and chemotherapy. Squamous cell carcinoma of the thyroid often mimics the clinical course of anaplastic carcinoma and is best treated with aggressive surgery followed by postoperative radiotherapy. Treatment and prognosis of lymphoma of the thyroid varies depending upon the specific pathological subtype, with combination chemotherapy and radiation therapy used primarily. Sarcomas of the thyroid are primarily managed with surgery, although radiation and chemotherapy are indicated in some cases.

[Differential diagnosis of the low-echo thyroid gland].

Hypothyrosis as a result of inflammatory disease of the thyroid gland, often is a diagnostic problem because of the insidious onset and its lack of specific symptoms. In the rather large amount of literature about sonography of the thyroid no papers with a systematic approach to this entity are found. In a prospective study we evaluated 36 proven cases and found a characteristic sonographic pattern of the thyroid with regard to its size, contour and structure.

[Two cases of papillary adenocarcinoma of the thyroid gland associated with chylothorax].

Case 1. An 85-year-old woman had a papillary adenocarcinoma of the thyroid gland and a pleural effusion. The pleural effusion appeared to be a chylous exudate and it did not re-accumulate after thoracenthesis. Thoracic imaging indicated that the chylothorax was caused by direct invasion of the thoracic duct by the thyroid carcinoma. Case 2. A 53-year-old woman had a 20-year history of recurrent chylothorax. She died due to sepsis one year after the third admission for dyspnea and chylothorax. The autopsy findings included papillary adenocarcinoma of the thyroid gland with metastasis to the left supraclavicular lymph nodes. The thoracic duct was inflamed, fibrotic, and completely obstructed. Invasion by the carcinoma may have compressed and destroyed the thoracic duct, and caused chylothorax. Recurrent inflammatory granulation caused total obstruction of the thoracic duct. Reports of chylothorax associated with carcinoma of the thyroid gland are rare.

Discontinuity of thyroid gland response to hormonal stimulation: effect of TSH and cAMP on iodide organification.

The action of TSH on the process of iodide organification was studied in rat thyroid glands under different experimental in vitro incubation conditions. The effects on glands of both short and prolonged exposure to TSH were evaluated using two different procedures: continuous and pulse labelling of thyroids with radioactive iodide. It was demonstrated that during prolonged contact with thyroid cells, TSH stimulates iodide organification periodically. This periodic effect is cyclic and is composed of a stimulatory and an inhibitory phase. Each cycle lasts 30-45 min, and several cycles follow one another in a regular manner. Furthermore, it has been shown that the periodic effect of THS is due to an intrinsic property of the thyroid cell to respond in a cyclic manner to hormonal stimulation. TSH stimulated the accumulation of organic iodide only when introduced at a precise phase of the cycle. The same type of discontinuous thyroid cell response was obtained when TSH was replaced by its intracellular mediator, cAMP. This indicates that the initiation of the cyclic response to hormonal stimulation is localized at the steps after that of cAMP formation. It seems, therefore, that this cyclic response of thyroid glands is not related to the recently observed phenomenon of desensitization. This phenomenon, characterized by the development of resistance in many target organs and cells toward their respective hormonal stimulators, is due to modifications in steps preceding those of cAMP formation. The discontinuity of thyroid gland response to both TSH and cAMP described in this work seems to be a new phenomenon, whose physiological significance and possible molecular mechanisms are discussed.

[Thyroid gland surgery in elderly patients (author's transl)].

In our hospital during the last 8 years, 583 operations on the thyroid gland were performed. 95 patients were aged between 61 and 90, and there was an increased risk in more than half of these cases. Our observations show that thyroid gland surgery is relatively harmless with insignificant complications, except for cancer of the thyroid, which has a high lethality.

[Effect of lithium chloride on the thyroid gland of white rats].

The morphological and functional changes in the thyroid tissue of white rats injected lithium chloride at doses 0.5 mekv/kg and 1.0 mekv/kg (groups 1 and 2, respectively) during three weeks were studied by the radiometric, histological and biochemical methods. The radiometric, histological and biochemical methods. The inhibitory action of lithium chloride on hormonogenesis in the thyroid gland and secretion of thyroid hormones into the blood was proved to be directly related to the lithium dose and concentration in the blood. The data of intravital radiometric and morphological analysis of the organs of the 1st group animals suggest some activization of the gland function, with the secretion of the hormones into the blood being suppressed. Increased concentration of the drug inhibits hormonogenesis and secretion of thyroid hormones into the blood.

Presence of adrenocorticotropin-potentiating activity in a calcitonin preparation derived from porcine thyroid glands.

A calcitonin preparation derived from porcine thyroid glands exhibited potentiating activity on the steroidogenic action not only on ACTH-(1-24) but also of ACTH-(1-18) in isolated rat adrenal cells in vitro. This activity was demonstrated in various molecular weight fractions resulting from gel filtration of the preparation. This preparation also contained minute amount of immunoreactive and biologically active ACTH.

Expression of intermediate filament proteins in normal and diseased thyroid glands.

A total of 67 samples from normal and pathological thyroid glands were stained (as formalin fixed paraffin sections) with a panel of monoclonal antibodies directed against intermediate filament proteins. The study confirmed previous reports of cytokeratin and vimentin coexpression in primary thyroid carcinomas, but coexpression was also detected in normal thyroid and in a range of benign conditions including follicular adenomas, Hashimoto's thyroiditis, and diffuse hyperplasia (thyrotoxicosis). Prekeratin expression was found (using antibodies recognising higher molecular weight cytokeratins) predominantly in areas of squamous change, independent of the underlying thyroid pathology. This study does not therefore support previous findings that prekeratin expression provides a reliable means of distinguishing follicular pattern papillary carcinoma from follicular carcinoma with its poorer prognosis or that it helps distinguish benign from malignant papillary lesions. No evidence of desmin or neurofilament expression was seen, and in particular, neurofilaments could not be detected in any of the cases of medullary carcinoma studied.

[Results of a survey on the use of 123 iodine for scintigraphic exploration of the thyroid gland (author's transl)].

A survey of thyroid gland scintigraphy with 123 Iodine was carried out on 1131 patients, and data concerning the time of examination as well as indications, scintigrams and uptake measurements are presented. Minimal dosage provides a sufficient counting rate and good quality images and delivers a radioactive dose 15 times lower than with conventional 131 Iodine scintigraphy.

Solubilization, purification, and partial characterization of thyrotropin receptor from bovine and human thyroid glands.

In an attempt to purify the thyroid receptor for TSH and to study interaction with the thyroid-stimulating antibody (TSAb) of graves' disease, we used both bovine and human thyroid glands. With either tissue, the 10,000 X g pellet of homogenized material was solubilized with 0.5% Triton N-101; excess Triton was removed by Amberlite XAD-2, and purification was effected by TSH affinity chromatography, followed by gel filtration on Sepharose 6B. Greatest purification was achieved with bovine tissue; this receptor preparation was 183-fold concentrated over the starting material, but contained only 6% of the original TSH-binding activity, due in part to spontaneous loss over the 4 days required for processing. On polyacrylamide gel electrophoresis, there were at least three protein bands, one of which was probably a subunit of thyroglobulin. Purified immunoglobulin G with thyroid-stimulating antibody activity inhibited the binding of TSH at all stages of purification of the receptor.

Prenatal growth and asymmetry of thyroid gland in the cattle (Bos primigenius f. taurus).

Growth and bilateral asymmetry of thyroid gland were studied in 291 fetuses (from 219 to 31,300 g) of the black-white cattle divided into 4 groups according to body weight. Relation between body weight (X) and thyroid weight (Y) was described using allometric equation Y = aXb. Allometric coefficient b was different in each of the four groups (1.198; 1.179; 1.314; 0.941) and amounted to 1.076 for the entire fetal period. The coefficient of fluctuating asymmetry of the thyroid gland decreased with increased fetal weight in the following way: 0.16; 0.10; 0.09; 0.07; this coefficient amounted to 0.02 for all the fetuses examined.

Undifferentiated carcinoma of the thyroid gland: sonographic findings.

We report high resolution sonographic (7.5 MHz) findings in four cases of undifferentiated carcinoma of the thyroid gland. Sonographic findings in these four cases included poorly marginated, hypoechoic masses associated with calcifications, and invasion of adjacent cervical structures. A knowledge of the sonographic features of undifferentiated carcinoma of the thyroid gland is of clinical importance, since the tumour has a grave prognosis, quite different from the relatively favourable prognosis of well differentiated thyroid carcinoma.

[Histogenesis of the thyroid gland in cultivation in diffusion chambers].

The histogenesis regularities and sources of the thyroid gland regeneration in white rats and steppe tortoises under conditions of auto- and allotransplantation in diffusion chambers are described. Dynamics of the reactive changes indicates the heterogenous character of morphofunctional differentiation of the thyroid structures. Differences in the adaptive plastic rearrangement of the thyroid gland of the vertebrates studied are related to the hystogenesis rate and depend on evolutionally fixed stability of the tissue to hypoxic state.

The influence of temperature on the thyroid gland of Chthonerpeton indistinctum (Gymnophiona, Amphibia).

The thyroid gland of the tropical amphibian Chthonerpeton is markedly stimulated when the animal is exposed for two days to relatively high temperatures (36 degrees C). Control animals were kept at 20 degrees C. The stimulated gland exhibits collapsed follicles and a very tall follicular epithelium. At the ultrastructural level the activated epithelial cells were characterized by an increased number of apical microvilli, increased amounts of rough endoplasmic reticulum, an extended Golgi apparatus and a high number of various granular and vesicular inclusions. These differences in histology and fine structure were not paralleled by quantitative results, which were approximately the same in both groups of animals. The values of these determinations were relatively high.

[Malignant lymphomas of the thyroid gland].

The authors evaluate their experience with malignant lymphomas of the thyroid gland, recorded in 22 patients from a total of 4501 operated on account of thyroid diseases in the course of 22 years. Although thyroid lymphomas are rare, their number is increasing. In localized findings curative procedures can be used, possibly supplemented by operations of the lymph nodes. The method of choice is still radiotherapy and surgery, in particular in lesions restricted to the thyroid gland. The role of chemotherapy is still controversial.

[C-cells of the thyroid gland in experimental osteoporosis].

The population of thyroid C-cells of female rabbits with ovariectomy-induced osteoporosis was studied using immunohistochemical method with antibodies against calcitonin (CT) and morphometrically. The development of osteoporosis was confirmed using radiology and densitometry. 8 weeks after the operation, after the detection of hypercalcemia and hypercalciuria, some of the animals were given a correcting treatment with calcium-containing drug and vitamin D3 or with a combination of calcium drug with hormonal estrogen-containing drug. 4 weeks after the treatment was started, thyroid C-cells in animals that received the drugs, were significantly larger than in the cells of control and ovariectomized rabbits that received no treatment. Following 12 and 22 weeks, in the animals treated with hormonal drug, both calcium concentrations in blood and urine and C-cell dimensions were normalized. Large C-cells with an intensive reaction to CT were observed at weeks 12 and 22 (weeks 20 and 30 after the operation) in the thyroid gland of the animals that received no treatment and of the animals that received calcium with food. In the last case, the morphological changes of C-cells were more pronounced, despite the insignificant differences in blood calcium concentrations in both groups. These results permit to suggest that the intensity of CT synthesis by C-cells may be influenced by calcium concentrations not only in the blood, but also in the intestinal lumen.

Surgical management of the substernal thyroid gland.

From 1992 to 1996, 19 patients had surgical resection of a substernal thyroid gland at the University of Rochester Medical Center, Rochester, New York. Based on a retrospective review of the 21 procedures performed, an algorithm for evaluation and surgical intervention is proposed. A limited medical workup is indicated and consists of a chest x-ray, thyroid function tests, computed tomography scan, and, possibly, fine-needle aspiration. Given a modestly high malignancy rate (16%) and a 37% reoperative rate, total thyroidectomy should be performed. When extensive mediastinal disease is encountered, a median sternotomy affords excellent access to the substernal thyroid gland.
