[Inhibition of 3', 5'-cyclic AMP phosphodiesterase in the guinea pig lung by a new anti-allergic: 10-(3-quinuclidinylmethyl) phenothiazine (LM 209)].

The action of a new antiallergic agent, 10-(3-quinuclidinylmethyl) phenothiazine or LM 209, on cAMP phosphodiesterase (PDE) was studied on a guinea-pig lung preparation and compared with that of other compounds such as cromoglycate (I), dexamethasone (II), dexchlorpheniramine (III), promethazine (IV) and theophylline (V). Compounds I, IV and V are competitive inhibitors whereas LM 209 and compound III are non competitive inhibitors of PDE. Compound II is practically inactive on the enzyme. Compounds III and V produce an inhibition of equal intensity, independently of the substrate concentration. Compounds I and IV are more active on PDE with low affinity than on PDE with strong affinity, whereas it is the contrary with LM 209. The mechanism of action of LM 209 at the pulmonary level is discussed in the light of these findings.

[Neuro-monitoring using visual evoked potentials in comatose neurologic intensive care patients].

The application of visual evoked potentials (VEP) as a monitoring tool was studied in 40 comatose neurological intensive care patients. With increasing depth of coma, the number of detectable VEP components decreased. In deeply comatose patients, VEP were regularly displayed as a negative wave with a latency of app. 100 ms and an amplitude of app. 8 microV (N100). The administration of promethazine or flunitrazepam used as sedatives produced a decrease of amplitude and an increase of latency of N100 that was reversed within 30 or 60 min respectively. In 10 patients, in which intraventricular pressure (IVP) was measured continuously by means of an intraventricular catheter, it was found that increases of IVP produced a considerable decrease of the amplitude and area of N100; after reduction of the increased IVP, both parameters quickly recovered. These observations indicate that VEP may be a useful tool for neuro-monitoring in neurological intensive care patients.

[Plasma levels of adrenaline and noradrenaline, plasma renin activity and arterial blood pressure in patients after kidney transplantation].

In 64 kidney transplant patients (KTP) (35) treated by cyclosporine A (CyA)+ prednisone and 29 treated by azathioprine (Aza)+ prednisone and promethazine and in 15 healthy subjects plasma adrenalin (A), noradrenaline (NA), plasma renin activity (PRA) and mean arterial pressure (MAP) in basal conditions and after water immersion test (WI) were measured. In basal conditions the A and PRA were higher, but NA was normal. After WI statistically significant decrease of MAP, PRA, A and NA were observed in all examined groups. In KTP mean decrease MAP, PRA and A was higher, but NA lower than in normals. No significant correlation between A or NA and PRA, and MAP and A or NA was observed. In KTP the change in sympathetic+ activity was observed. Participation of catecholamines and renin in the pathogenesis of hypertension in KTP is not dominant. Kind of immunosuppressive therapy (CyA or Aza) haven't significant influence on A, NA nad PRA levels.

Binding of antagonists of H1 and H2 histamine receptors to peripheral blood lymphocytes of atopic and healthy subjects.

The binding of the antagonists of histamine H1 and H2 receptors by peripheral blood lymphocytes from atopic and healthy subjects was investigated. We found that lymphocytes from atopic subjects showed statistically significant decrease in the binding of H2 receptor antagonist - ranitidine. In addition, lymphocytes from atopic and control subjects had similar capacity of binding of H1 receptor antagonist - promethazine. The ratio of the amount of H1 and H2 antagonists, bound to lymphocytes from atopic and healthy subjects, was calculated. The difference between the values in the group of atopic (2.55) and control subjects (1.55) was statistically significant.

Effects of antihistamines on wound healing.

Role of antihistamines (H1 and H2 blockers) in wound healing by utilizing incision and dead space wound models in albino rats was investigated. H1 blockers (mepyramine and promethazine) were found to decrease breaking strength of 10 day old dermal incision wounds and collagen content (as hydroxyproline) and breaking strength of granulation tissue harvested over tubular implant. On the other hand H2 blockers (Cimetidine and ranitidine) did not alter the above parameters. The findings that H1 blockers suppress healing implicate H1 receptors in alleged prohealing effect of histamine, and suggest clinical evaluation of these agents for suppression of overhealing states like keloid, adhesions and strictures.

Cardiac histamine receptors: differences between left and right atria and right ventricle.

Histamine can stiumulate the heart by directly interacting with cardiac histamine receptors. In the present study we have investigated the cardiac effects of histamine, 4-methylhistamine (a specific H2-receptor agonist) and 2-pyridylethylamine PEA, a specific H1-receptor agonist] on spontaneously beating right atria and electrically driven left atria and right ventricle strips of the guinea pig. Left atria were driven at 1 Hz and right ventricle strips at 2.5 Hz and at twice the threshold voltage. Histamine and PEA produced a dose-dependent positive inotropic effect on the left atria. The dose-response curves were shifted to the right in a parallel fashion by promethazine (3 x 10(-6)M)+... Burimamide did not affect either dose-response curve. Histamine and 4-methylhistamine had a positive chromnotropic effect on right atria and a positive inotropic effect on right ventricle strips.

[The effect of cyanides and antidotes on the heart rate in rat heart preparations in situ].

Effects of sodium-cyanide and some of its antidotes and antagonists such as sodium-nitrite, 4-dimethylaminophenol (DMAP), sodium-thiosulphate and alpha-adrenergic blocking agents on the heart rate of rats in situ under conditions of the artificial breathing have been studied. Also, effects of opioid receptor agonist, naloxane, on the heart of the poisoned animals have been studies. Sodium-cyanide causes strong and irreversible effects on the heart rate which are completely abolished by effects of sodium-nitrite, 4-DMAP and sodium-thiosulphate, and partly and timely limited by effects of alpha adrenergic blockers, phenotholamine and promethazine, and to a significantly slower degree by naloxane.

[Comparative clinical studies of oral premedication in childhood with reference to various pharmaceuticals].

In a randomized prospective study of 206 children aged between one and 14, the influence of oral praemedication on the psychic condition and with regard to anaesthesiologic measures was investigated. Heart rate, inhibition of salivation and the kind and frequency of side-effects were also examined. The praemedication was carried out using diazepam (0.3 mg/kg), promazin (1.0 mg/kg), promethazin (1.0 mg/kg) and chlorphenaethazin (1.0 mg/kg). It was not possible to determine statistically relevant differences between the various drugs concerning the fixed criteria. In general, however, diazepam seems to achieve the best results. In the younger children (1 to 6 years) a frequent and insufficient effect of praemedication was noted immediately. Oral praemedication proved to be a valuable and practicable method which was well tolerated by the great majority of the children.

Involvement of cell calcium and transmembrane potential in control of hepatocyte volume.

This study examined the role of hepatocyte calcium and cytoskeleton in activation of hyposmotic stress-induced increases in hepatocyte transmembrane potential and control of cell volume. Hepatocyte transmembrane potential was measured by glass microelectrodes in mouse liver slices before and after exposure to hyposmotic medium. Hepatocytes were loaded with tetramethylammonium by briefly exposing liver slices to nystatin, a cation poreforming antibiotic. Changes in hepatocyte steady-state water volume were determined by changes in intracellular tetramethylammonium activity measured with tetramethylammonium-sensitive, double-barrel micro-electrodes 4 min after exposure to hyposmotic medium. Hyposmotic stress of 74% of the control osmolality (approximately 280 mOsm) hyperpolarized hepatocyte transmembrane potential by 1.83 times the control hepatocyte transmembrane potential, and cell water volume increased by a factor of 1.19. The Ca2+ channel blocker verapamil (100 mumol/L) completely inhibited hyposmotic stress-induced hyperpolarization of hepatocyte transmembrane potential. This inhibitory effect diminished at doses of 37.5 or 50 mumol/L, but even these hyperpolarizations were decreased significantly compared with control. Hyposmotic stress during added verapamil dosage (50 mumol/L) also resulted in 23% greater cell swelling compared with control. Ca(2+)-free medium plus ethylene glycol-bis (beta-aminoethylether)-N,N'-tetraacetic acid (5 mmol/L) inhibited hyposmotic stress-induced increases in hepatocyte transmembrane potential and resulted in 16% greater cell swelling compared with control. Calmodulin inhibitors trifluoperazine (100 mumol/L) and promethazine (100 mumol/L) inhibited the hyperpolarization of hepatocyte transmembrane potential caused by hyposmolality, as did 3,4,5-trimethoxybenzoate 8-(N,N-diethylamino)octyl ester) (50 mumol/L), which inhibits mobilization of Ca2+ from intracellular stores. Cytochalasin B (50 mumol/L), which disrupts microfilaments, also inhibited hyperpolarization of hepatocyte transmembrane potential with osmotic stress.(ABSTRACT TRUNCATED AT 250 WORDS)

[Trial droperidol premedication in infants and children].

Droperidol premedication for children (0.3 to 0.5 mg/kg) was compared with pentobarbital and Dolosal--promethazine). Results are analyzed using criteria proposed by Burken and Nisbet. Droperidol compared very favourably with the order drugs. However a decrease in efficacy was noted with infants and neuw-born children, even with higher doses. The possible incidence it hypertonic seizures renders this drug unsuitable for ambulatory surgery.

Intrapartum fetal vibratory acoustic stimulation during spontaneous and induced states of low activity and low heart rate variability.

Fetal response to vibratory acoustic stimulation (VAS) was studied in 12 healthy parturients in the active phase of labor. All fetuses were evaluated in both spontaneous and analgesia-induced states of low activity and low cardiac beat-to-beat variability. Analgesia was induced with intravenous maternal administration of meperidine, 0.8 mg/kg, plus a fixed dose of promethazine (25 mg). The vibratory acoustic stimulus was applied ten minutes after the start of a fetal quiet state, and ten minutes of fetal behavior was observed after application of the stimulus. The number and mean duration of fetal body movements in the unmedicated fetuses were statistically significantly higher than in the medicated group. Similarly, the number of fetal heart rate accelerations was significantly higher in the unmedicated fetuses than in the medicated ones. Ten minutes after VAS no fetus in the unmedicated group was in a quiet behavioral state, while 10 of 12 medicated fetuses (83%) returned to a quiet state.

Calcium-binding proteins and insulin release. Differential effects of phenothiazines on first- and second-phase secretion and on islet cAMP response to glucose.

Calcium and cAMP are interdependent regulators of glucose-induced insulin release. In the present study we investigated the importance of cAMP and calcium-binding proteins for biphasic insulin secretion by assessing the effects of two phenothiazines known to block such proteins, trifluoroperazine (TFP) and promethazine (PMZ). In isolated rat islets, during 60-min incubations with 16.7 mmol/l glucose both agents inhibited the insulin response with ID50 values of 15 mumol/l for TFP and 5 mumol/l for PMZ. Both agents decreased the maximal insulin response without gross changes in the islet sensitivity to glucose. TFP (15 mumol/l), whereas inducing 50% inhibition of second-phase insulin release, totally suppressed the cAMP response to glucose and the accompanying first-phase insulin secretion (5-min incubations); these effects of TFP could be partially reversed by isobutyl methylxanthine (IBMX). In contrast, 5 mumol/l PMZ, which produced 60% inhibition of second-phase insulin release, had no effect on first-phase insulin and cAMP responses to glucose. Furthermore, IBMX did not modify the inhibitory effect of PMZ on second-phase insulin secretion. The following is concluded: 1. TFP acts preferentially on first-phase insulin release and inhibits cAMP formation; this suggests that calmodulin plays a major role in mediating the initial glucose effect on secretion via stimulation of cAMP. 2. The islet probably contains calcium-sensitive proteins other than calmodulin, since the low concentrations of PMZ shown to inhibit second-phase insulin release lack effects on calmodulin. Synexin could be such a protein.(ABSTRACT TRUNCATED AT 250 WORDS)

Increased growth inhibition and DNA lesions in human colon adenocarcinoma cells treated with methotrexate or 5-fluorodeoxyuridine followed by calmodulin inhibitors.

The antineoplastic agents methotrexate and 5-fluorodeoxyuridine induce DNA lesions, although the drugs are not incorporated into DNA. The lesions arise as a result of reduced repair of damage occurring in DNA independently of the treatment with antineoplastic agents. When cells containing DNA lesions are treated with calmodulin inhibitors (W-7, phenothiazine, promethazine), the amount of lesions is increased, in all probability due to reduced DNA repair. This is paralleled by increased growth inhibition. Hence by inhibiting calmodulin, one can modulate the levels of DNA lesions and change the growth-inhibitory effect induced by methotrexate or 5-fluorodeoxyuridine. This strongly supports the importance of this type of DNA lesion in the toxic effects induced by the drugs.

[The effect of histamine and histamine H1 and H2 receptor blockers on the tone of the longitudinal muscles in the human appendix].

The aims of this study were to determine whether the effect of histamine on a smooth musculature of the human appendix is mediated by histamine H1-receptors only or whether histamine H2-receptors are involved, as well. The obtained results indicate histamine increases tonus in musculature; however, histamine H1-receptor antagonist (promethazine) decreases tonus. Histamine H2-receptor antagonist (cimetidine) has caused a mild increase in tonus of musculature what indicates that it has a certain intrinsic activity. By blocking histamine receptors with H1 and H2 antagonists which were used either separately or together before cumulative histamine concentrations, it has been concluded that histamine actions are exerted not exclusively by H1-receptors but also histamine H2-receptor antagonists can significantly decrease the tonicizing effect of histamine.

Inhibition of acid secretion in guinea pigs by tricyclic antidepressants: comparison with ranitidine and omeprazole.

The antisecretory properties of imipramine on gastric secretion in guinea pig in comparison with other antisecretory agents was determined. In awake guinea pigs s.c. infusion of histamine (30 micrograms/kg/hr) increased acid and fluid secretion by 3- to 4-fold. When acid output peaked, a bolus administration of the tricyclic anti-depressant imipramine inhibited acid and fluid secretion. Imipramine and other agents, such as ranitidine and omeprazole, inhibited gastric secretion in a dose-dependent fashion. The most potent was the H2-antagonist ranitidine (IC50, 0.2-0.3 mumol/kg), followed by the gastric H-K-adenosine triphosphatase inhibitor, omeprazole (IC50, 0.5-0.6 mumol/kg). Imipramine (IC50 1-2 mumol/kg) was the least potent of the inhibitors. Both ranitidine and omeprazole could abolish acid secretion, but maximal inhibition with imipramine was 60% of initial. Promethazine (25 mumol/kg), an H1 antagonist, and atropine (12 mumol/kg), a muscarinic antagonist, inhibited gastric secretion by 40 to 50%. Imipramine and atropine also inhibited basal acid secretion. In dispersed gastric cells comparison between imipramine and omeprazole showed that imipramine was about 5-fold more potent than omeprazole in blocking histamine or dibutyryl cyclic AMP stimulation of aminopyrine accumulation. Imipramine probably acts as a protonophore by increasing the rate of proton-gradient dissipation rather than by interfering with the hydrogen-pump system because, in gastric membranes, imipramine was 20-fold less potent than omeprazole in inhibiting the gastric H-K-adenosine triphosphatase activity. These results suggest that imipramine administered s.c. in guinea pigs is a potent antisecretory drug. Its action may be due to a combination of anticholinergic and antihistamine H2 activities.

Unusual effects of histamine antagonists on canine nasal blood vessels.

This paper documents some of the lesser-known effects of antihistamines on nasal blood vessel smooth muscle. An in vitro muscle tension detecting technique was utilized. Three different H1 receptor antagonists (pyrilamine maleate, chlorpheniramine maleate, promethazine hydrochloride), and the H2 antagonist metiamide were used. Besides their basic antihistaminic effect, H1 antagonists (classical antihistamines) induced a contraction, enhanced norepinephrine- and epinephrine-induced contractions, and relaxed the sustained contraction induced by methoxamine hydrochloride or high concentrations of potassium. Although these effects were quite similar to those induced by histamine, the mechanism of each effect was apparently different. Metiamide did not produce these effects except for the relaxation of the sustained contraction. The actions of histamine antagonists seem to be much more complicated than expected.
