Mercury content and its bioconcentration factors in wild mushrooms at Łukta and Morag, northeastern Poland.

Total concentrations of mercury were determined using cold-vapor atomic absorption spectroscopy (CV-AAS) in the fruiting bodies of 16 species of wild mushrooms and underlying soil (0-10 cm) substrates collected in the areas of the Communes of Morag and Łukta in the county of Ostróda in northeastern Poland in 1997-1998. A total of 174 composite samples of caps, 174 stalks, 80 whole fruiting bodies (collectively 1254 specimens), and 252 soils were examined. Among several species of mushrooms analyzed, the greatest concentrations were between 1300 and 71000 ng.g(-1) of dry matter. These levels were found in the caps of Sweating mushroom (Clitocybe rivulosa), King Bolete (Boletus edulis), and Common Puffball (Lycoperdon perlatum) and also were characterized by the highest bioconcentration factors (BCF) for Hg, which ranged between 160 +/- 82 and 110 +/- 34. The cap to stalk quotient for mercury concentrations was approximately 2 for most of the species except Poison Pax (Paxilus involutus), which had a greater concentration in caps than in stalks and a quotient of 4.4 +/- 7.2. Hg concentrations in the underlying soil substrates (0-10 cm layer) ranged between 21 +/- 21 and 390 +/- 130 ng.g(-1) of dry matter. The results showed that the consumption of mushrooms, considered to be the sole dietary source of mercury at the highest or mean element concentrations found, is not hazardous at daily ingestion rates of less than 70 and 210 g of fresh product, which would result in a hazard index value of less than unity.

An ecological analysis of leukemia incidence around the highest 137Cs concentration in Poland.

Cancer has long been known to be a hazard of exposure to ionizing radiation. However, the assessment of health effects from exposure to radiation is a matter of considerable controversy. This paper presents results of a retrospective study of leukemia incidence (203-207, ICD-9) around the highest 137Cs pollution in Poland (as an effect of the Czarnobyl disaster and/or military bomb tests). The data relating to all the registered leukemias in males and females originated from the Regional Cancer Registry in Opole. The information on 137Cs concentration rates in Opole province was derived from the state monitoring provided by the Polish Geological Institute in Warsaw. The spatial analysis--based on the random-effects Poisson regression model--was carried out via the Markov Chain Monte Carlo (MCMC) technique (Gibbs sampling) using BUGS software. The model incorporated epidemiological data and an ecological covariate--isotope concentrations--and provided a framework for estimating the strength of a dose-response relationship. The differences in incidence levels were quantified by traditional standardized morbidity ratios (SMRs) and presented in thematic maps as well as in combined charts of distance-disease-dose relations. Additionally, to assess spatial disease clustering, a Tango test was adopted. The results of this ecological study suggest that the 137Cs concentrations did not have any negative influence on the exposed population.

Metal concentrations in deciduous tree leaves from urban areas in Poland.

Accumulation of metals in deciduous tree foliage from urban areas in western-south Poland was monitored during the vegetation season of 2000 year. Concentrations of Al, Ba, Ca, Cd, Cr, Cu, Fe, Mg, Mn, Ni, Pb, Sr, Ti and Zn were measured in birch, willow, linden and maple leaves using the ICP-AES method. Seasonal variations of metal concentrations and their relations with sampling site were investigated. The most dynamic accumulation of Al, Cd, Cr, Ni and Pb was observed for examined species. The highest differences in element concentrations for investigated sites were found for Ba, Cd, Mn and Ni. Interelement correlations were investigated. In all foliar samples synergistic relationships between Al-Cr and Ca-Sr were found. Statistically significant negative correlations were observed only for Cd and Ti in birch leaves.

Search for hepatitis delta virus (HDV) infection in hepatitis C patients in north-eastern Poland. Comparison with anti-HDV prevalence in chronic hepatitis B.

Hepatitis C virus and hepatitis D virus have been shown to suppress HBsAg synthesis. Thus it is possible that HDV infection occurs despite the lack of detectable HBsAg. The aim of our study was to (a) determine the prevalence of HDV infection in patients with chronic hepatitis C (b) compare it with the prevalence of HDV infection in HBsAg positive patients with hepatitis B. The study group consisted of 51 chronic hepatitis C patients, 30 HIV infected drug addicts (27 of them were also positive for anti-HCV) and 102 hepatitis B patients. The participants were tested for anti-HDV, anti-HCV and HBsAg. All anti-HCV positive patients were negative for anti-HDV. Four individuals with anti-HDV belonged to hepatitis B group and constituted 3.9% of all HBsAg positive subjects. We conclude that (a) there is currently no evidence of HDV infection among HCV infected patients in our region (b) hepatitis delta infection is rare in north-eastern Poland.

Trace element contamination in industrial regions of Poland studied by moss monitoring.

The use of terrestrial mosses as biomonitors in large-scale multi-element studies of heavy metal deposition from the atmosphere is a well established technique in Europe. In such studies it is advantageous to determine as many elements as possible in order to distinguish between different source categories. A combination of INAA and AAS has been found very useful in this respect, in particular when epithermal activation is used for instrumental neutron activation analysis. A total of 33 elements (Al, Cl, Sc, V, Cr, Mn, Fe, Co, Ni, Cu, Zn, As, Se, Br, Rb, Sr, Ag, Cd, Sb, I, Cs, Ba, La, Ce, Tb, Yb, Hf, Ta, W, Au, Pb, Th, and U) in Pleurozium schreberi (Brid.) Mitt. moss samples from the Silesia-Kraków Industrial Region and Legnica-Głogów Copper Basin and from the control (background) area in Northeast Poland were identified. The highest concentrations of the majority of trace elements were found in mosses growing in the Silesia-Kraków Industrial Region, only Cu and As concentrations were higher in mosses in the Legnica-Głogów Copper Basin. These results are in conformity with the load of trace elements emissions in these areas.

Epidemiology of school accidents during a six school-year period in one region in Poland.

BACKGROUND: The aim of the study was to analyse the incidence of school accidents in relation to school size, urban/rural environment and conditions of physical education classes. METHODS: 202 primary schools with nearly 50,000 students aged 7-15 years were studied during a 6-year period in the Włocławek region in Poland. RESULTS: There were in total 3274 school accidents per 293,000 student-years. Accidents during breaks (36.6%) and physical education (33.2%) were most common. Most frequently accidents took place at schoolyard (29.7%), gymnasium (20.2%), and in the corridor and stairs (25.2%). After adjustment for students' age and sex, student-staff ratio and duration of school hours, urban environment increased the probability of accident (OR: 1.25; 95% CI: 1.14-1.38). Middle-size schools (8-23 classes) had similar accident rate as small schools (OR: 0.93; 95% CI: 0.83-1.04), while schools with 24-32 classes (OR: 1.26; 95% CI: 1.10-1.43) and with > or = 33 classes (OR: 1.36; 95% CI: 1.17-1.58) had increased accident rate. Presence of a gymnasium was also associated with increased probability of accident (OR: 1.49; 95% CI: 1.38-1.61). CONCLUSIONS: Urban environment, larger school-size and equipment with full-size gymnasium are important and independent risk factors for school accidents. These findings provide some new insights into the epidemiology of school-related accidents and may be useful information for the planning of strategies to reduce accident incidence in schools.

Lower extremity counterpart of the Poland syndrome.

Below-the-knee right leg hypoplasia and ipsilateral toe brachysyndactyly were observed in a 4-year-old female with an otherwise normal phenotype. Electromyographic and nerve conduction studies were normal. The Doppler evaluation was consistent with a 50% reduction in the blood supply from the femoral artery, suggesting vascular disruption as the pathogenic mechanism. Our observations support the hypothesis that a lower extremity counterpart of the Poland syndrome does exist and that the extent of limb involvement is dependent on the level of vascular disruption.

Skin reactivity to histamine and to allergens in unselected 9-year-old children living in Poland and Italy.

Several studies have shown a higher prevalence of positive skin-prick tests to airborne allergens in Western than in Eastern European countries. We have recently reported that skin histamine reactivity significantly increased in Italy over the past 15 years. Population differences in skin histamine reactivity could, at least in part, explain the reported differences in positive allergen skin tests. To test this hypothesis we compared histamine skin reactivity and the prevalence of allergen positive skin-prick tests in a sample of Italian and Polish schoolchildren. A total of 336 unselected 9-year-old-schoolchildren (198 in Italy and 138 in Poland) underwent skin-prick tests with three different histamine concentrations (10, 1 and 0.2 mg/ml) and with a panel of common airborne allergens according to the ISAAC protocol, phase two. Mean wheals elicited by skin-prick tests with the three serial concentrations of histamine were significantly larger (p < 0.001) and shifted more toward higher values (p < 0.001) in Italian than in Polish children. The differences were greater for the intermediate histamine concentration tested (1 mg/ml) than for the highest concentration (10 mg/ml). Skin-prick tests for airborne allergens were more frequently positive in Italian children: wheals >or= 3 mm induced by any allergen [odds ratio (OR) 1.69; confidence interval (CI) 0.98-2.92] by Dermatophagoides pteronyssinus (OR 1.92; CI 0.97-3.80) and by D. farinae (OR 3.15; CI 1.16-8.63). Labeling as positive allergen wheal reactions half the size of the 10 mg/ml histamine wheal or larger reduced but did not abolish the Italian-Polish differences. The significantly higher skin histamine reactivity observed in Italian children could help to explain why allergen skin-test reactions differ in the East and West European populations. Moreover, differences in nonallergen-specific factors among populations should be considered in the interpretation of skin test results (e.g. cut-off points). To obtain meaningful results, epidemiological studies of allergies should include serial histamine dilutions.

Evolution of the mental health care system in Poland.

OBJECTIVE: The aim of this paper is to trace evolution of mental health system in Poland. METHOD: Available Polish literature and fundamental policy documents including mental health legislation are reviewed and major milestones in this evolution identified and then discussed against the background of political and social developments. RESULTS: The mental health system evolved since the beginning of the 1970s from large hospitals towards community-based care. It was found that the changes were rather slow due to financial shortages and lack of clear demand from users. Recent transitions offer opportunities to increase impact of users and their families. On the other hand, however, introduction of health insurance system reduces prospects for well co-ordinated mental health policy and may lead to inequalities in access to services. CONCLUSION: It can be concluded that evolution of mental health services has been influenced by prevailing ideologies as much as by technical and professional considerations.

HLA class II genotypes associated with chronic hepatitis C virus infection and response to alpha-interferon treatment in Poland.

AIMS/BACKGROUND: Recent evidence suggests that spontaneous clearance of hepatitis C virus (HCV) may be associated with the HLA DQB1*0301 allele but there is still some debate over the role of other alleles and HLA haplotypes in HCV infection. As this may best be resolved by studying genetically different populations, we have investigated HLA class II-encoded susceptibility and resistance to HCV infection in a relatively sedentary population of patients from northwestern Poland. METHODS: The distributions of HLA class II DRB1, DQA1, DQB1 and DPB1 alleles were determined by standard PCR-protocol in 129 unrelated patients with chronic hepatitis C (anti-HCV and HCV-RNA positive) and 103 healthy unrelated racially-matched control subjects. Fifty-five patients were treated with alpha-interferon (5 MIU thrice weekly for 6 months) out of whom 29 showed a complete response and 26 were non-responders. RESULTS: A significantly reduced frequency of the DQB1*0301 allele in the patients was observed (24.0% vs. 38.8%; p=0.015). Additionally, two different DR-DQ haplotypes were found to be associated with chronic HCV infection: DRB1*1501-DQA1*01-DQB1*0602 (24.0% vs. 12.6%; p= 0.027) and DRB1*0701-DQA1*0201-DQB1*02 (31.8 vs. 12.6%; p=0.0006), the latter difference being most pronounced in those patients who responded to alpha-interferon treatment (41.4% vs. 12.6%; p=0.00048). CONCLUSIONS: The results confirm the negative association between chronic HCV and DQB1*0301 and identify two novel genetic associations. In particular, the DRB1*0701-DQA1*0201-DQB1*02 haplotype is associated with both chronic infection and response to alpha-interferon. Interestingly, the same haplotype is reportedly associated with non-response to hepatitis B vaccination.

Empowerment and continuous improvement in the United States, Mexico, Poland, and India: predicting fit on the basis of the dimensions of power distance and individualism.

Although variations in national cultures predominate as explanation for the belief that universal approaches to management do not exist, there have been few reports of systematic studies. Data from employees of a single firm with operations in the United States, Mexico, Poland, and India were used to test the fit of empowerment and continuous improvement practices with national culture. Using the theoretical constructs of individualism-collectivism and power distance, the authors predicted that the practices would be more congruent in some cultures than in others and that value congruence would result in job satisfaction. Using structural equations modeling, the authors found that empowerment was negatively associated with satisfaction in India but positively associated in the other 3 samples. Continuous improvement was positively associated with satisfaction in all samples. Substantive, theoretical, and methodological implications are discussed.

Familial defective apolipoprotein B-100 in a group of hypercholesterolaemic patients in Poland. Identification of a new mutation Thr3492Ile in the apolipoprotein B gene.

The prevalence of the familial defective apolipoprotein B-100 (FDB) Arg3500Gln mutation in 525 unrelated hypercholesterolaemic Polish subjects was evaluated. DNA samples were screened for FDB mutation using SSCP method. Presence of mutation was confirmed using a mismatch MspI PCR strategy. Plasma lipid levels and clinical characteristics of 13 patients identified as carriers of the mutation and of their 23 affected relatives were analysed and compared with non-affected ones. In the affected individuals a variable expression of lipid concentrations and of atherosclerosis symptoms were observed. The prevalence of FDB Arg3500Gln mutation in hypercholesterolaemic Polish subjects (3.7%) seems to be similar to the frequency reported in other Caucasian hypercholesterolaemic populations. The estimated prevalence of the mutation in general Polish population is relatively high being 1/250. The same haplotype at the apoB locus in the carriers of this mutation in Poland as in other populations from Western Europe suggests its common origin. In one hypercholesterolaemic subject a non-hitherto described mutation was identified. It consisted in C-->T transition in apoB codon 3492 leading to threonine to isoleucine substitution in 3492 position of apoB gene (Thr3492Ile).

Association between SLC11A1 (formerly NRAMP1) and the risk of sarcoidosis in Poland.

Sarcoidosis (SA) is a systemic granulomatous disorder of unknown etiology characterized by T helper 1-type inflammatory responses at sites of disease with signs of B cell hyperactivity. Like rheumatoid arthritis and diabetes, an infectious etiology has frequently been postulated but no single infectious trigger definitively identified. Polymorphic alleles at SLC11A1 have previously been associated with susceptibility to both the putative infectious agents and to these autoimmune disorders. We therefore investigated its candidacy as a genetic determinant of SA in Poland in an association-based study comparing 86 SA patients with 85 tuberculosis (TB) patients and 93 control subjects. The functional promoter (GT)(n) polymorphism and four of 10 other single nucleotide or insertion/deletion polymorphisms genotyped across SLC11A1 were informative in our sample. Consistent with previous autoimmune disease studies, allele 3 at the functional (GT)(n) promoter region repeat polymorphism was significantly associated with SA when compared with healthy controls (odds ratio 1.68; 95% CI: 1.01-2.81; P=0.04) or with TB patients (odds ratio 1.69; 95% CI: 1.042-0.78; P=0.03).

Blood pressure in centenarians in Poland.

The objective of the study is assessment of the prevalence and type of hypertension in centenarians in Poland. The investigations included 92 people who had turned 100 years of age, who, within the protocol of the Project of Investigation Polish Centenarians, underwent genetic, anthropometric, psychological and sociological examinations, and whose cardiovascular system was assessed. In the present analysis, we are analysing data concerning their blood pressure (BP) assessed by several measurements (3-6) with the mercury sphygmomanometer on both arms in sitting (if possible) or lying position performed during one visit. Hypertension was diagnosed when average BP value exceeded > or = 160/95 or > or = 140/90 mmHg. The average of age was 101.2 years (range 100-111 years), the respective values for BP were: systolic 146.7 mmHg (99-213 mmHg), diastolic BP--80.3 mmHg (55-114 mmHg) and pulse pressure (PP) 66.4 mmHg (31-129 mmHg). Hypertension diagnosed based on the criterion > or = 160/95 mmHg was found in 29% of subjects, and according to the recent WHO criterion (> or = 140/90 mmHg) in 65% of subjects. PP exceeded 65 mmHg in 44.6%, and was above 50 mmHg in 91% subjects. In conclusion, hypertension occurs less frequently in centenarians, than in the entire population of old people, but it nevertheless cannot be considered a rare condition.

Mercury in wild mushrooms and underlying soil substrate from the great lakes land in Poland.

Fifteen species of wild mushrooms and underlying soil samples collected in a virgin landscape of Augustowska Forest in northeastern Poland in 1997-98 were analyzed for total mercury to evaluate the status of contamination and usefulness of higher mushrooms as possible bioindicators of mercury pollution. Among the 15 species analyzed, Pinewood King Bolete, Scaly Tooth and King Bolete showed relatively high bioconcentration factors (BCF: dry-weight normalized concentrations of mercury in mushrooms relative to concentrations in soil) for mercury, which varied between 69 and 110. These three species were also characterized by great concentrations of total mercury in caps (between 2,000 +/- 800 and 2,300 +/- 1,100 ng g-1 dry wt) and stalks (between 850 +/- 390 and 1,000 +/- 500 ng g-1 dry wt.). Species such as Red-hot Milk Cap, Poison Pax and Common Chantherelle had mercury BCFs of less than 1, while Gipsy Bolete, Orange Birch Bolete, Brown Scaber Stalk, Variegated Bolete, Sandy Knight-cap and Yellow-cracking Bolete were weak or moderate mercury accumulators with BCFs between 1 and 40. Concentrations of mercury in mushrooms were greater than the tolerance limits suggested for mercury in plant foods.

210Po bioaccumulation by mushrooms from Poland.

The paper presents results on 210Po activity concentration measured in mushroom samples collected in northern Poland (Białogard and Elblag areas). Among 20 species of wild mushrooms, King Bolete (Boletus edulis), accumulated 210Po in the highest degree. Therefore this species of higher mushrooms is an excellent bioindicator for 210Po radioactivity in the land environment. Finally, the effective dose of polonium emission was calculated for a consumer of King Bolete. Wild mushroom consumption contributed up to 37 microSv to the effective dose in an individual consuming about 5 kg (fresh weight) of heavily contaminated Boletus edulis species per year.
