[Gastrin is not implicated in intestinal adaptation after extensive resection of the small intestine in rats].

Because gastrin is thought to be a trophic agent for the digestive tract, we studied the effects of endogenous variations of plasma gastrin on the small intestinal mucosa after a 90 p. 100 jejunoileal resection in the rat. Hypogastrinemia was induced by antrectomy while hypergastrinemia resulted from vagotomy. Pyloroplasty had no effect on gastrinemia. Small bowel resection increased mucosal weight, DNA and protein content; these changes were more obvious in the jejunum and ileum than in the duodenum. Gastric operations performed before the resection did not significantly modify these parameters except for the level of protein. Our results indicate that endogenous gastrin plays no role in the adaptive growth response of the intestinal mucosa after extensive small bowel resection.

Early morphologic changes in the intestine following massive resection of the small intestine and parenteral nutrition therapy.

Following massive resection of the small intestine, including the ileocecal valve in seven adult mongrel dogs, the remaining 20 centimeters of the proximal part of the jejunum was anastomised to the midtransverse portion of the colon. All dogs were given parenteral nutrition for four weeks. Six weeks after resection, the dogs underwent reexploration, and gross and microscopic examinations of the remaining part of the jejunum were carried out. Grossly, there was a 29.5 per cent increase in the length and a 31 per cent increase in the circumference of the intestine. Microscopically, there was a 23 per cent increase in the depth of the crypts, a 21 per cent increase in the number of villi and a 22 per cent increase in the width of the villi. No change was observed in the height of the villi.

[New method of studying membrane hydrolysis and transport as well as metabolic processes in the small intestine in vitro (everted small intestinal sac with bilateral oxygenation)].

The effect of bilateral oxygenation (from mucosal and serosal surfaces) of the small intestinal preparation on glucose accumulation has been investigated. It has been demonstrated that such a type of oxygenation significantly (two fold) increases the absorption of glucose as compared to that under oxygenation only from musocal surface. This data sugested that oxygenation from serosal site of the small intestine had an important role in the transport of free glucose.

[Perfusion study of intestinal sugar absorption after extensive intestinal resection (author's transl)].

Small bowel adaptation after extensive intestinal resection has been studied in 5 children. A segmental perfusion technique was used to evaluate the kinetics of glucose absorption and sucrose hydrolysis. The activity of 6 brush border hydrolases and the mean villus height were measured in intestinal biopsies. Glucose absorption was greater than in 3 control children of the same age and was almost complete. Sucrose hydrolysis was increased in the same proportion and the maximum capacity of the segment was 2 mmol/min/20 cm as compared to 1.2 mmol/min/20 cm in the control group. Absorption of the released monosaccharides was complete for glucose and as high as in controls for fructose. Brush border hydrolase activities were identical to those of controls whereas the mean villus height was increased (438 micrometer as compared to 342 micrometer) although the difference was not significant. These results demonstrate the functional adaptation of the remaining small bowel and confirm that it is due to compensatory hyperplasia.

[The intestinal capsule: a new way of investigating the small intestine].

The small bowel is difficult to explore because of its location far away from the natural orifices. The aim of this study is to describe a telemetric, autonomous and multifunctional capsule (39 mm in length and 11 mm in diameter) designed according to a modular system to explore the small bowel. It consists of a central cylinder containing a location detector which allows permanent data collection concerning its position in the small bowel, the length of the small bowel and the transit velocity; secondly, several interchangeable tips allow either aspiration of a sample of intestinal juice, or release of a substance previously placed in the capsule, or to perform a mucosal biopsy. After having been swallowed by the patient, the capsule passes through the whole gut and is then recovered in the stools between 24 and 48 hours later. The preliminary study consisted of comparing, in patients undergoing a surgical procedure, the small bowel length measured by telemetry (542.3 +/- 113.8 cm) to that measured intraoperatively by the arithmetical mean of the mesenteric and antimesenteric edges (515 +/- 112.7 cm), p = 0.28. These different interchangeable working tips have already been used to determine the level of absorption of various substances, the discovery of absorption sites and the performance of mucosal biopsies without any material link between patient and the measuring or recording equipment. The main advantages of this intestinal capsule are the possibility to continuously transmit its location, its total autonomy and remote control of the desired action.

Congenital intestinal malrotation causing gestational intestinal obstruction. A case report.

BACKGROUND: Intestinal obstruction in pregnancy is rare. The mortality rate is higher during pregnancy than in the general population and applies to fetal as well as maternal survival. Major causes of intestinal obstruction in the pregnant women include adhesions, volvulus and intussusception. CASE: A 27-year-old woman, gravida 2, para 1, estimated gestational age approximately 26 weeks, with a high-level, complete small bowel obstruction. The patient failed a trial of conservative management and required laparotomy, at which time a congenital malrotation was found. She was treated successfully with a modified Ladd's procedure. There was no maternal or fetal morbidity. CONCLUSION: This case represents an extremely rare cause of bowel obstruction in pregnancy; a paucity of such cases have been reported.

Intestinal absorption of vitamin B12 in patients with chronic pancreatic insufficiency and the effect of human duodenal juice on the intestinal uptake of vitamin B12.

The mean absorption of vitamin B12 (Schilling test) was 13.1 +/- 1.0 (% +/- S.E.M.) in 21 patients with chronic pancreatic insufficiency and 17.6 +/- 1.4 in 13 control patients (p less than 0.01). There was no correlation between pancreatic bicarbonate production after secretion stimulation and vitamin B12 absorption in the patient group (r = 0.117). Human duodenal juice reduced the uptake of 57CoB12-rat intrinsic factor (IF) by perfused rat small intestinal segments in vivo (p less than 0.01) as well as the uptake of 57CoB12-human IF by purified guinea-pig intestinal brush borders in vitro (p less than 0.01). The results confirm reduced absorption of vitamin B12 in chronic pancreatic insufficiency, but the mechanism remains uncertain.

Expression of cytotoxic molecules in intestinal T-cell lymphomas. The German Study Group on Intestinal Non-Hodgkin Lymphoma.

Intestinal T-cell lymphoma (ITCL) represents a subgroup of peripheral T-cell lymphomas which is thought to arise from alpha beta intraepithelial T-lymphocytes. Since these lymphocytes may contain cytotoxic molecules, the question of whether this also holds true for ITCL arises. Twenty ITCL cases were examined for the presence of granzyme B, perforin, and T-cell-restricted intracellular antigen (TIA-1)/granule membrane protein of 17 kD (GMP-17). Two molecules with restricted expression in cytotoxic cells, granzyme B and perforin, were detected by immunocytochemistry and by in situ hybridization with an isotopically labelled RNA probe, respectively. Immunocytochemistry was also performed with the antibody 2G9, which recognizes two molecules, one expressed by cytotoxic cells (TIA-1) and the other found in granulocytes and cytotoxic cells (GMP-17). Granzyme B, TIA-1/GMP-17, and perforin were found in the neoplastic cells of 16/19 cases, 19/20 cases, and 16/17 cases, respectively, of ITCL, but not in the tumour cells of the control group, which consisted of intestinal B-cell lymphomas (five cases) and CD8-negative peripheral nodal T-cell lymphomas (six cases). At least one of these molecules was expressed in the tumour cells of all ITCL cases. 2G9 proved to be the most sensitive immunohistological marker, since reactivity with this antibody was not only observed in the highest number of cases, but also found in high numbers of neoplastic cells in positive cases. In conclusion, ITCL appears to show cytotoxic differentiation in all cases. In conjunction with immunophenotypic and genotypic data, our results support a uniform derivation of this tumour from intraepithelial alpha beta cytotoxic T-lymphocytes.

Neonatal intestinal perforation caused by intestinal muscularis defect associated with vascular ectasia.

We describe a rare case of a viable preterm infant who developed a spontaneous localized perforation of the small bowel caused by a segmental absence of the intestinal musculature and associated with vascular ectasia. The findings may support a causal relationship between absence of the intestinal muscle coat and a vascular network disorder which may be responsible for the gastroenteric defect.

Gastric carcinoma and intestinal metaplasia. Significance of types of intestinal metaplasia upon development of gastric carcinoma.

For clarifying significance of subtypes of intestinal metaplasia upon the development of stomach carcinoma, 34 stomachs surgically removed for mucosal carcinoma were morphologically studied. Intestinal metaplasia was divided into incomplete and complete types by alcian blue-PAS staining. The incomplete type has been understood as an immature stage of the development of the metaplasia as previously reported by us. In this, study, it was revealed that the incomplete type of metaplasia showed more dysplastic features than the complete type. Analysis of the metaplasia of the gastric mucosa surrounding cancer tissue revealed that well differentiated adenocarcinoma frequently came into contact with the incomplete type of metaplasia in small cancer, less than 1 cm. This relationship became less evident as carcinoma became larger. In poorly differentiated carcinoma, however, any correlations between types of metaplasia and size of cancer were not demonstrated. From these results, it was suggested that well differentiated adenocarcinoma arises from the incomplete type of metaplasia.

Cytokeratin expression patterns in noncardia, intestinal metaplasia-associated gastric adenocarcinoma: implication for the evaluation of intestinal metaplasia and tumors at the esophagogastric junction.

BACKGROUND: Barrett esophagus (BE)/Barrett adenocarcinoma and distal gastric intestinal metaplasia (IM)/adenocarcinoma are similar histologically, but they differ in their clinical presentation, epidemiology, and pathogenesis. Differentiating BE from gastric IM and Barrett adenocarcinoma from gastric adenocarcinoma is difficult, especially when IM is short or tumors are large and involve both sides of the esophagogastric junction. Previously, the authors identified unique cytokeratin (CK) immunoreactivity patterns that were associated strongly with BE and Barrett adenocarcinoma. The specificity of CK7 and CK20 (CK7/20) expression patterns in patients with IM-associated gastric adenocarcinoma, which is distinct epidemiologically from BE/Barrett adenocarcinoma, has not been evaluated. The objective of the current study was to evaluate the CK7/20 expression patterns in noncardia, IM-associated gastric adenocarcinoma in a Chinese population with a low risk for BE and esophageal adenocarcinoma and a high risk for Helicobacter pylori infection and gastric carcinoma. METHODS: Endoscopic biopsy specimens of gastric IM and adjacent tumor from 50 consecutive patients with advanced noncardia gastric carcinoma were immunostained for CK7 and CK20. Clinical and endoscopic features and H. pylori status were documented. Two gastrointestinal pathologists, blinded to clinical and endoscopic data, independently assessed CK7/20 immunohistochemistry. RESULTS: H. pylori infection was present in 43 of 50 patients (86%). In the area of IM, patchy CK7 staining was seen in 9 patients (18%), and diffuse CK20 staining was seen in all 50 patients (100%). The BE CK7/20 pattern characterized by CK7 staining in superficial and deep glands and the CK20 staining in surface epithelium was not seen in any of the 50 patients. Only one patient (2%) demonstrated a CK7 positive/CK20 negative immunophenotype characteristic of Barrett adenocarcinoma. The remaining 49 patients (98%) showed non-Barrett adenocarcinoma patterns of CK7/20 staining, i.e., a CK7 positive/CK20 positive pattern was seen in 33 patients (66%), a CK7 negative/CK20 positive pattern was seen in 12 patients (24%), and a CK7 negative/CK20 negative pattern was seen in 4 patients (8%). CONCLUSIONS: In a patient population without risk factors for the development of BE/esophageal adenocarcinoma, the CK7/20 pattern characteristic of BE was not present in gastric IM adjacent to adenocarcinoma, and the CK7/20 pattern characteristic of Barrett adenocarcinoma also was extremely rare. These results support the hypothesis that, despite the presence of intestinalized mucosa in both disorders, BE/Barrett adenocarcinoma and gastric IM/adenocarcinoma are two distinct clinical entities with unique demographic, clinical, and CK immunophenotypic findings. These results may have application to the evaluation of patients with IM and adenocarcinoma arising at the esophagogastric junction.

The expression of an intestinal form of glutathione peroxidase (GSHPx-GI) in rat intestinal epithelium.

We have previously identified and characterized GSHPx-GI, which is a cellular selenium-dependent glutathione peroxidase (GSHPx) distinct from the classic GSHPx-1 and phospholipid hydroperoxide glutathione peroxidase (PHGPX). We have determined the level of GSHPx-GI mRNA expression in the rat gastrointestinal tract from esophagus to colon. Although GSHPx-GI mRNA is readily detectable throughout the GI tract, the highest level is detected in the ileum and cecum. We have also determined the levels of GSHPx-GI mRNA expression and several antioxidant enzyme activities along the villus-to-crypt axis in the rat small intestine by cell fractionation. GSHPx-GI mRNA is present at a similar level in all of the epithelial fractions, whereas GSHPx-1 mRNA is detectable only in the remnant. This suggests that GSHPx-GI is the major cellular tetrameric GSHPx expressed in intestinal epithelium, and the expression of GSHPx-GI in the GI tract is not likely regulated differentially through maturation of epithelial cells. In terms of enzymatic activity, although we detected lower glutathione S-transferase activity in the crypt epithelium, there was a marginal increase of PHGPX activity, a twofold increase of GSHPx activity, and a three- to fivefold increase of catalase activity in the crypt relative to the distal villus. Thus, the crypt epithelial cells may be better protected from peroxidative damage.

Small intestinal metastasis from esophageal carcinoma associated with small intestinal obstruction: report of a case.

The small intestine is rarely involved with metastatic tumors from outside the abdomen, and few case reports have been documented in the literature. We describe herein what to our knowledge is the third case of a solitary metastasis from squamous cell carcinoma (SCC) of the esophagus being found in the jejunum, causing small intestinal obstruction.

The application of the "Lojda" method for intestinal lactase in intestinal biopsies from jaundiced newborn infants.

A new use of the histochemical method for intestinal lactase activity is described. Peroral intestinal biopsies from newborn light-treated infants with diarrhoea were investigated for brush-border lactase. The lactase activity found in these infants by the histochemical method correlated well with the infants ability to hydrolyze lactose judged by lactose tolerance test.

Studies on translocation of immunoglobulins across intestinal epithelium. III. Failure of cholera enterotoxin to stimulate secretion of IgA by rabbit intestine.

We tested the hypothesis that the secretion of IgA into intestinal fluids is modulated by increases in epithelial cell cyclic AMP. Cholera enterotoxin instilled into intestinal loops of rabbits did not induce IgA secretion, even though an appropriate fluid response was obtained. The findings, therefore, do not support the hypothesis.

The value of double-contrast study of the small intestine in immunoproliferative small intestinal disease.

Radiological findings of 2 patients with immunoproliferative small intestinal disease were studied. One of the cases was associated with alpha-chain disease. Both patients complained of severe diarrhea, and diagnosis was made by endoscopic biopsy of the duodenum and jejunum. Double-contrast studies of the small intestine and hypotonic duodenographies in these patients revealed thickened folds and innumerable fine granular elevations without ulcerations or luminal narrowings. Double-contrast studies are one of the most important examinations in the diagnosis of this disorder.

Effects of diclofenac sodium on intestinal anastomotic healing. Experimental study on the small intestine of rabbits.

To study the possible effects of diclofenac sodium on intestinal anastomoses, 48 rabbits were submitted to surgery consisting of two single-layer ileal anastomoses performed with separate propylene 5-0 sutures. The animals were divided at random into two groups (test and control). The animals in the test group were given intramuscular injections of diclofenac sodium at the dose of 3 mg/kg body weight at 24-hour intervals, and the control animals were given injections of an identical amount of 0.9 percent saline. The animals were sacrificed on the 3rd, 7th, and 14th postoperative days for macroscopic evaluation of the peritoneal cavity and of the anastomoses, tensile strength measurement, hydroxyproline determination, and histopathologic examination. The following results were observed: anastomotic dehiscence followed by peritonitis and death in five test animals (20.83 percent) and no control animals; decreased anastomotic tensile strength on the 7th day in test animals (P less than 0.05); delayed acute inflammatory response and onset of fibroblast proliferation in the test group; and similar hydroxyproline levels in both groups. On the basis of the results obtained, we conclude that diclofenac sodium had a negative effect on intestinal anastomotic healing.

Small-intestinal involvement in familial polyposis diagnosed by operative intestinal fiberscopy: report of four cases.

Operative intestinal fiberscopy, in which the duodenal fiberscope was introduced during laparotomy for colectomy in familial polyposis via the enterotomy opening, permitted the demonstration of small intestinal polyps in six of seven consecutive cases. Four of the six patients had adenomatous polyps in the proximal jejunum, including one patient with the concomitant presence of ileal adenomas. Polyposis due to lymphoid hyperplasia in the terminal ileum was found in three patients. Preoperative upper gastrointestinal surveys revealed adenomas in the duodenums of all seven patients, adenomas in the gastric antrum in three, and multiple hamartomas in the gastric corpus in two. Thus, in familial polyposis or Gardner's syndrome, more or less the entire gastrointestinal tract seems to be involved and the term "gastrointestinal polyposis" seems to describe these conditions.
