Preoperative angiographic uterine artery embolization in the management of cervical pregnancy.

The diagnosis of a cervical ectopic pregnancy is often made intraoperatively in the presence of extensive hemorrhage during attempted evacuation. In some cases, hysterectomy has been the necessary treatment to control bleeding. We report two cases of cervical pregnancy detected by ultrasound. Before pregnancy termination, the uterine arteries were successfully embolized using angiographic techniques. As a result, surgical evacuation was performed with minimal hemorrhage; hysterectomy was not required and the patients' potential fertility was retained.

Histologic investigation of vascular malformations of the face after transarterial embolization with ethibloc and other agents.

Twenty-one vascular malformations located in the facial area, 11 high-flow arteriovenous malformations and 10 slow-flow malformations, underwent combined treatment by embolization and later surgery. Embolization was performed simultaneously with superselective angiography of the branches of the external carotid artery. The new biodegradable fibrosing agent Ethibloc was used in 16 cases. Histologic examination of the surgical specimens confirmed the good target orientation by the transarterial injection of Ethibloc. Limitations of this technique are discussed. The agent proved to have thrombogenic and fibrogenic properties. Some of the vascular walls degenerated and ruptured following the embolization, but there were no instances of necrosis of interstitial tissue or skin. Embolization treatment of vascular malformations of the face was not curative, but it facilitated subsequent surgery in all examined cases.

Inflammatory intracranial mass lesion: an unusual complication resulting from the use of Gelfoam.

There are few reported complications associated with the use of Gelfoam. We report the case of a 29-year-old woman who developed signs and symptoms of meningeal inflammation and an expanding intracranial mass related to the intracranial use of Gelfoam. Findings were confirmed by surgical exploration. Signs and symptoms resolved promptly after therapy with intravenous dexamethasone, but complete resolution of radiological findings required 5 months. As the use of Gelfoam is essential to neurosurgical practice, it is important to be aware of significant complications related to its use.

[Embolization therapy of kidney tumors--experience with a group of 60 patients].

A series of 60 patients is reported, in whom embolization was carried out because of a kidney tumor. In 9 cases, this treatment was given for purely palliative reasons, but in the remaining 51 cases, this treatment was a preoperative measure in order to achieve coagulation or "dry the tumor out" intra vitam. Although we formerly used materials such as gel foam and muscle homogenates, in the last three years, Ethibloc or pure alcohol has been used exclusively. The risks involved in this therapy are discussed. In patients with large arteriovenous shunts or fistulas, alcohol should not be used and we prefer polyacrylic agents like Ethibloc.

Clinical evaluation of hepatic artery embolization: comparison between Gelfoam and Lipiodol with anticancer agent.

Transcatheter embolization of the hepatic artery for hepatomas was performed with the use of Gelfoam and Lipiodol-ADM in 98 patients (Gelfoam 63 patients; Lipiodol-ADM 35 patients). The cumulative one-year survival rate was 52.9% for the Gelfoam group and 28% for the Lipiodol-ADM group. Response to hepatic embolization was most remarkable in the nodular type; CR was obtained in five patients and PR in 31 patients (response rate: CR + PR = 57.1%) in the Gelfoam group, with no CR and six PRs (response rate, 17.6%) in the Lipiodol-ADM group. Alfa-feto protein (AFP) decrease of more than 50% was observed in 78.1% of the Gelfoam group and 40% of the Lipiodol-ADM group. The side effects were transient and controlled with conservative treatment. Gelfoam seemed to be more effective in the treatment of hepatoma, but the damage to normal tissue was more severe with this embolic material.

[The treatment of maxillofacial hemangioma with transcatheter embolization].

Preliminary experience obtained from 7 cases with maxillofacial hemangioma treated with the external carotid arterial transcatheter embolization is presented. Of the 7 cases 2 were diagnosed as racemose hemangioma, 1 case as cavernous hemangioma and 4 cases as central mandibular and zygomatic arch cavernous hemangioma. External carotid arterial transcatheter embolization was adopted as radical treatment of hemangioma in 3 cases. Of them, excellent result was achieved in 1 case, and good in 2. Embolization was used as preoperative preparation for the treatment of hemangioma in 2 cases. Two operations were successfully performed and the results of hemostasis were excellent during the operation. The arterial transcatheter was all performed with Seldinger technique and through transfemoral approach. The embolization agents used included gel form, absolute ethanol, medical TH glue, sodium morrhuate, etc. The advantages of this method, and the relationships between the properties of the embolization agents and the effect of embolization, the physiological function of the target organ and the complication are discussed and evaluated.

Modulation of IL-1, tumor necrosis factor, and C5a-mediated murine neutrophil migration by alpha-melanocyte-stimulating hormone.

This study was designed to examine the effects of i.p.-injected alpha-melanocyte stimulating hormone (MSH) on murine neutrophil migration into subcutaneously implanted sponges in response to IL-1-alpha, TNF-alpha, and C5a. The results show that as little as 0.1 ml of 5 x 10(-7) M MSH injected i.p. significantly blocked the accumulation of neutrophils in sponges in response to IL-1. This action of MSH was dose dependent, reversible, and was maximally effective if MSH was given at the same time as the injection of IL-1. This effect of MSH was not restricted to IL-1-induced neutrophil emigration, because MSH also antagonized the accumulation of neutrophils in response to both TNF and C5a. The proopiomelanocortin-derived peptide ACTH which contains the MSH sequence also significantly reduced neutrophil accumulation in response to IL-1, although less effectively than MSH. Similar studies with beta-endorphin showed that it had no effect on neutrophil accumulation in this system. The direct injection of MSH, beta-endorphin and ACTH into sponges or i.p. did not stimulate a neutrophil emigration and eliminated the possibility that MSH or ACTH suppressed the neutrophil influx in response to IL-1, TNF, or C5a by competing for circulating neutrophils. The action of MSH on IL-1, TNF, and C5a-induced neutrophil emigration suggests that this peptide may be an important regulator of the inflammatory response.

[Therapy of hepatic carcinoma by the intra-arterial injection of lipiodol, antineoplastic agents and gelfoam].

Chemoembolization by selective intra-arterial injection of lipiodol--chemotherapeutic agents (mitomycin and/or adriamycin)--followed by terminal embolization with gelfoam was performed on 20 inoperable patients with hepatic tumors: 13 hepatocellular carcinomas (HCC), 6 metastases from colorectal cancer, and one adenoma. A total of 29 embolizations were performed, 17 of them followed by gelfoam embolization. Significant decrease in tumor size or no tumor increase was shown in 8 patients--6 capsulated and well-limited HCC's, and 2 multinodular HCC's. Apart from occasional episodes of post-embolization syndrome (fever, vomit, abdominal pain), no significant complications were observed. Nine patients died at intervals ranging from 2 weeks to 37 months after the first embolization; in 6 patients the cause of death was not related to the advancing of the cancer, but to the complications of the underlying cirrhosis. In cirrhotic patients, damage to the rest of liver parenchyma can be limited by using superselective catheterization. Of the 6 patients with greater than 12 months follow-up, 4 are alive and 2 have died; 2 more patients are still alive 18 months after the first embolization. Finally, lipiodol was useful as radiopaque marker to detect lesion changes during the follow-up.

Transcatheter arterial chemotherapy using doxorubicin, iodized oil and Gelfoam embolization in hepatocellular carcinoma.

Computed tomography (CT) was performed in 54 patients with hepatocellular carcinoma three weeks after transcatheter arterial chemotherapy using iodized oil and doxorubicin with or without gelfoam embolization. Patients with iodized oil retention in the tumor greater than 50 per cent of tumor size survived longer than patients with retention of less than 50 per cent. Differences were also found within Okuda stages I and II, but they were significant only in Okuda stage I (p less than 0.0001). These results suggest a possible relationship between iodized oil retention and survival. In addition to Okuda stage, several factors affected iodized oil retention: tumor vascularity, tumor size, portal thrombosis and Gelfoam embolization. These factors may thus influence the prognosis after transcatheter arterial chemotherapy.

[Superselective transcatheter-arterial-chemo-embolization in uterine cervical cancer].

Transcatheter-arterial-chemo-embolization (TACE) was performed on 7 cases of uterine cervical cancer from T1b to T3b. The purpose of this approach is to stop the uterine bleeding caused by malignancy and disrupt the cancer cells under the feeding arteries. TACE by Seldinger's method was undertaken selectively on both uterine arteries with suspension of Lipiodol, Adriamycin and Gelfoam. In each case, the bleeding stopped soon after the operation, and the lesion began to necrose within a few days. Though the primary effects were confirmed, complete remission could not be expected, so radiation therapy was performed. The longest survival among these patients was over 6 years. It should be noted that the other major advantage claimed for TACE is to decrease the dose of radiation.
