Free tensor fasciae latae flap reconstruction of defects of the chest and abdominal wall: selection of recipient vessels.

Reconstruction of a full thickness defect of the abdominal or chest wall requires a combination of a rigid or semi-rigid layer and skin cover. The tensor fasciae latae (TFL) flap is unique in that it provides both of these in substantial quantities, but the most difficult aspect of using this flap in the anterior chest and abdomen is finding suitable recipient vessels. We describe a series of nine cases in which full thickness defects of the chest and abdominal wall were repaired using free vascularised TFL flaps. The recipient vessels included the gastroepiploic vessels (n = 2), the deep inferior epigastric artery (n = 3), the internal mammary artery (n = 2), and the superior thyroid and acromiothoracic arteries (n = 1 each). The gastroepiploic and internal mammary vessels are preferred because of their reliability and because the flap pedicle enters the centre of the deep surface of the flap so that if these vessels are used, circumferential tight closure of the fascial layer can be achieved, with consequent decreased risk of vessel kinking and future herniation.

The hemithigh microvascular transfer (combined anterolateral thigh flap and tensor fasciae latae flap) for a full thickness abdominal wall reconstruction: a case report.

In large, full thickness upper abdominal wall defects a free microvascular tissue transfer is the only option which will enable reconstruction of the structural integrity of the abdominal wall as well as give a good aesthetic appearance. We present a case in which such a defect was reconstructed by a 29 x 19 cm hemithigh free flap, combining the adjacent vascular territories of the anterolateral thigh flap and the tensor fasciae latae flap based on the lateral circumflex femoral artery.

Initial release of the lateral pelvic fascia.

Initial incision of the lateral pelvic fascia before division of the dorsal vein complex allows posterior displacement of the neurovascular bundles and development of the proper prostatorectal plane. This technique may decrease positive surgical margins while preserving the neurovascular bundles. This technique also completely preserves the posterior attachments to the urethra, allowing preservation of maximal urethral length.

[One-stage repair of hypospadias using a scrotal septal skin flap based on peri-meatus dartos fascia].

OBJECTIVE: To explore the feasibility of one-stage repair of hypospadias using scrotal septal skin flap based on peri-meatus dartos fascia. METHODS: Nine patients were repaired by this method. RESULTS: All patients achieved good results without urethral fistula or stricture. The angle of the penis to the scrotum is normal. CONCLUSION: The flap has reliable blood supply and the flap creation is easy. It is a simple, convenient and effective method for one-stage repair of hypospadias.

[Sensory characteristics of the cortical input of the hippocampus, the dentate fascia].

Investigation of sensory reactions of the neurones of the dentate fascia was performed in unanaesthetized rabbits. Spontaneous activity usually consisted of long aperiodic high-frequency bursts with silent periods between them. Multimodal convergence was observed in 76% of records. Responses to sensory stimuli constituted three nearly equal groups: 1 -- tonic inhibitory reactions; 2--phasic (equal to stimulus duration) activatory responses; 3--simple "specific" responses of on-type, sometimes with secondary phasic components. In one microelectrode track, perpendicular to the double lamina of granular cells, all neurones usually responded in the same fashion.

[Laparoscopic correction of uterus retroflexion by sewing the round ligament of the uterus to the abdomen fascia in treatment and prophylaxis of pain syndromes].

The use of laparoscopy in diagnosis, investigation, and therapy is very important. The paper describes the laparoscopic technique of correction of a retroflexed uterus. This technique is based on sewing the round ligaments of the uterus to the abdomen fascia. Both simplicity and usefulness of the method are demonstrated.

[Musculocutaneoplasty by combined flap with tensor of the fascia lata- gluteus medius in trochanteric pressure sores. Apropos of a clinical case].

The authors describe a musculocutaneous flap which can be used in trochanteric pressure sores in paraplegics. Described by Little and Lyons (1983), this reconstructive unit combines the proximal muscular segment of the tensor fasciae latae (TFL) and gluteus medius muscles in a round shaped flap pedicled on the lateral circumflex femoral artery. The anatomical basis of this muscle combination is found in the anastomotic network between the superior gluteal artery which supplies gluteus medius muscle and lateral circumflex femoral artery, the principal vascular pedicle of the TFL muscle. Cutaneous defects of the trochanter and associated osteitis are frequent complications in paraplegic patients which often require ostectomy and coverage of the bony prominence by well vascularised, padded skin. Musculocutaneous flaps are well suited for this purpose and we review various reconstructive units that may be brought into trochanteric defects. Among these, the gluteus medius-TFL flap appears to be an interesting option for, in case of recurrent ulcers, other donor areas are left intact, and the flap itself could give rise to a subsequent "traditional" TFL flap.

Anatomical laparoscopic hernia repair of direct or indirect inguinal hernias using the transversalis fascia and iliopubic tract.

Fourteen patients with symptomatic inguinal hernias underwent anatomic repair, which approximates the transversalis muscle and fascia to the iliopubic tract. Polyproprolene mesh was used to obliterate the canal space formerly occupied by the hernia sac. Nine patients were operated on for a direct inguinal hernia, four for an indirect hernia, and one recurrent direct hernia. Two patients required the laparoscopic approach to abandoned in favor of an open method. The remaining 12 patients underwent laparoscopic repair and were discharged within 23 h of the procedure.

Inguinal prosthetic hernioplasty by anterior prosthetic enforcement of the fascia transversalis--a novel surgical technique.

A new technique of prosthetic inguinal hernioplasty is presented. The originality of the technique is the position of the mesh: between the transversus muscle and fascia transversalis. The surgical technique is presented in details. The procedures of choice for inguinal hernia repair, from the literature of the past years, are discussed.

[Recurrent inguinal hernias: surgery with transversalis fascia repair].

Some authors recommend different prosthetic repairs for treatment of recurrent groin hernia. In our institution groin hernias are treated by a modified Shouldice-repair, which considers the transversalis fascia layer only. In this prospective study 240 consecutive patients with 298 groin hernias were operated on. 46 patients were treated for 53 recurrent hernias. It was possible to carry out the operative procedure of transversalis plasty in all recurrent cases. 90% of these patients were followed-up by personal examination after a median of 26 months after operation (range 12-36 months). 89% of the patients with primary operation have been checked 15 months postoperatively. 2 patients (4.5%) of the recurrent hernias had a recurrence. This compares well with the 3.2% recurrence rate (7 patients) in the patients with primary hernias. The difference is not significant (p greater than 0.5). The only observed difference between recurrent and primary hernias was in the rate of testicular atrophy (6.8% for recurrent and 0% for primary hernias). According to our experience large prosthetic repairs are not necessary for the common case of recurrent groin hernia, but may be useful in specially selected situations.

Intracortical dentate fascia grafts in the adult rat hosts. Ultrastructure of the graft/host interface.

The ultrastructure of cellular composition of the graft/host interface and its permeability for the connecting nervous fibers was investigated in the heterotopic intracortical grafts of the dentate fascia. The areas of interface, where definite border between the graft and host tissues was detected at the semithin sections, were analyzed. Three types of the graft/host interface were distinguished: i) astrocytic; ii) ependymal; and iii) perivascular. The astrocytic interface was crossed by bundles of thin unmyelinated axons (similar to the dentate mossy fibers), by fascicles of thick myelinated axons (resembling those of the neocortex), and by dendrites. Ependymal sheath, which was overdeveloped in the grafted dentate tissue, was perforated by axonal and axo-dendritic bundles, but only if they were accompanied by fibrous astrocytic processes. Wide perivascular spaces of thick-walled interface blood capillaries, where processes of perivascular glia were intermingled with numerous basal laminae, were impermeable for the nervous fibers. The data show, that extensive exchange by the nervous fibers exits between dentate and surrounding neocortex, and that various types of glial cells have different roles in supporting, or preventing development of the graft/host nervous integration.

[Repair of the defect after laryngeal surgery using the sternohyoid muscle and fascia].

In order to achieve a radical excision of laryngeal cancer with preservation of its functions, the authors devised an operative procedure in which the defects of the larynx, oral pharynx, laryngeal pharynx and/or cervical esophagus were repaired with sternohyoid muscle and fascia. Since May 1981, this operation has been done on 52 patients with satisfactory results. The operative procedure and clinical materials are reported in this paper. This operation is simple, postoperative complications are rare, and it warrants further clinical trial.
