Clearspeech: a display reader for the visually handicapped.

Many domestic appliances and much office equipment is controlled using a keypad and a small digital display. Programming such devices is problematical for the blind and visually handicapped. In this paper, we describe a device that may be used to read the displays on these devices. The device is designed to accept a description of the display being read, which specifies the types and locations of elements of the display. Images are captured using a handheld webcam. Images are processed to remove the distortions due to the camera orientation. The elements of the screen are interpreted and a suitable audio output is generated. In suitably illuminated scenes, the display data is interpreted correctly in approximately 90% of the cases investigated.

Otoscopic, cytological, and microbiological examination of the equine external ear canal.

Otoscopic examination and cytology of the equine ear would be beneficial in diseases such as head trauma, headshaking, otitis externa secondary to otitis media, vestibular disease, aural neoplasia and aural pruritus secondary to parasites. In practice, otic examinations of horses are rarely done due to the perceived difficulty in visualizing the equine external ear canal and tympanic membrane, as well as the need for chemical restraint. In this study, the proximal external ear canal was examined in live horses using a handheld otoscope and in cadaver heads using video otoscopy. Visualization of the proximal ear canal of the sedated horse could be done with a handheld otoscope, but more sedation or general anaesthesia and a video otoscope would be required to adequately visualize the tympanic membrane in the live horse. The proximal ear canals of 18 horses were examined cytologically and cultured aerobically. In three horses, both ears were sampled. No cells or organisms were seen on cytological examination of 11/21 ears. Nine of the 21 ears were sterile when cultured. Ten of the 21 ears had mixed growth with low numbers of organisms (Corynebacterium sp. being most common). Two of the 21 ears had heavy growth of a single organism (Corynebacterium sp. and Staphylococcus intermedius, respectively). Equine cadaver heads were examined in cross-section by computed tomography (CT) imaging and histopathology in order to further understand the anatomy of the equine external ear canal. Equine practitioners should be aware that otic examination is possible and may provide important diagnostic information.

A study of GafChromic XR Type R film response with reflective-type densitometers and economical flatbed scanners.

The GafChromic XR Type R film is a relatively new product for recording high radiation dose in interventional radiological procedures. Means of measuring the film response were studied in this investigation. Two handheld reflective-type densitometers of different models were compared in the range of 0-8 Gy. They were found to be in excellent agreement. Five reflective flatbed scanners of different models were compared by a simple preliminary test. Their widely differed performances suggest the need of testing a scanner before using it for dosimetry measurement. A selected scanner was further tested for its ability to measure radiation in the range of 0-8 Gy and for the development of a scanning protocol. This experiment suggested the inclusion of a calibration pattern with known exposures and a black reference step in the scanning of a film in RGB mode. Then the red component of this image should be used for dosimetry computation. This method was compared to the use of a red acetate filter. The latter was demonstrated to be a possible alternative for measurement below 5 Gy and when there is no software ability to split an image into color components.

A handheld calculator program for determination of gene transfer between races.

Methods of statistical analysis have permeated all biomedical fields, but the advent of inexpensive handheld calculators has enabled those individuals unskilled in advanced mathematics to make competent analyses. Statistics cannot be avoided in genetics and their evaluations, really estimated, are essential to further knowledge and understanding. Older scientists had to rely on the use of paper and pencil to derive formulas and then apply them. The experience gained in this one small area of investigation has been presented in detail to help other investigators who do not possess a broad mathematical background.

Endoprobe: a system for radionuclide-guided endoscopy.

Methods to guide the surgical treatment of cancer utilizing handheld beta-sensitive probes in conjunction with tumor-avid radiopharmaceuticals [such as 18F-fluorodeoxyglucose (FDG)] have previously been developed. These technologies could also potentially be used to assist in minimally invasive techniques for the diagnosis of cancer. The goal of this project is to develop and test a system for performing radionuclide-guided endoscopies. This system (called Endoprobe) has four major subsystems: beta detector, position tracker, endoscope, and user interface. The beta detection unit utilizes two miniaturized solid state detectors to preferentially detect beta particles. The position tracking system allows real-time monitoring of the unit's location. The beta detector and position tracking system's receiver are mounted on the tip of an endoscope. Information from the beta detector and tracking system, in addition to the video signal from the endoscope, are combined and presented to the user via a computer interface. The system was tested in a simulated search for radiotracer-avid areas of esophageal cancer. The search for esophageal cancer was chosen because this type of cancer is often diagnosed with endoscopic procedures and has been reported to have good affinity for FDG. Accumulations of FDG in the normal organs of the abdomen were simulated by an anthropomorphic torso phantom filled with the appropriate amounts of radioactivity. A 1.5- mm-thick gelatin film containing FDG was used to simulate radiotracer uptake in the lining of normal esophagus. Esophageal lesions (both benign and malignant) were simulated by thin disks of gelatin (diameters=3.5-12 mm) containing appropriate concentrations of FDG embedded in the gelatin film simulating normal esophagus. Endoprobe facilitated visual identification and examination of the simulated lesions. The position tracking system permitted the location of the Endoprobe tip to be monitored and plotted in real time on a previously acquired positron emission tomography-computed tomography (PET-CT) image of the phantom. The detection system successfully acquired estimates of the beta flux emitted from areas chosen by the user. Indeed, Endoprobe was able to assist in distinguishing simulated FDG-avid areas as small as 3.5 mm in diameter from normal esophagus (p value <0.025). In addition to FDG, Endoprobe can be used with other positron or electron-emitting radionuclides such as IC or 131I. The next phase of this project will focus on modification of the prototype to make it more suitable for clinical use.

Ranging accuracy test of the sonic microstereometric system.

We have recently introduced a new type of ultrasonic-based, three-dimensional ranging system for the localization of small handheld surgical tools to be used during operation in open stereotactic brain surgery. This targeting system has now been extensively laboratory tested during thousands of measuring cycles with a CNC-based measuring table. The typical accuracy found in a single direction was 0.4 mm and better. The accuracy vector in space was 0.897 mm (n = 2312). The test-retest accuracy was typically 0.4 mm. With the given accuracy, the system is suitable for all ranging tasks in open stereotactically guided microsurgery.

Lymphatic mapping and gamma probe guided laparoscopic biopsy of sentinel lymph node in patients with clinical stage I testicular tumor.

PURPOSE: We attempted to detect lymphatic drainage and sentinel lymph node with radioactive tracer in patients with testicular tumor. We then tried to determine if sentinel lymph node biopsy with gamma probe guided laparoscopic procedure was feasible as a staging tool for patients with clinical stage I testicular tumor. MATERIALS AND METHODS: Technetium-labeled phytate was injected around the tumor in 15 consecutive patients with clinical stage I testicular tumor. Lymphatic drainage and sentinel lymph nodes were imaged by a gamma camera. Localization of the sentinel lymph node was confirmed with a handheld gamma probe. After we confirmed that sentinel lymph nodes were detected in the initial 10 patients, gamma probe guided laparoscopic sentinel lymph node biopsy was performed after routine orchiectomy in the next 4. To confirm whether the radioactive node was really a sentinel lymph node the final patient in this series underwent laparoscopic retroperitoneal lymph node dissection with a unilateral template. RESULTS: Sentinel lymph nodes were detected in all patients by lymphoscintigraphy and handheld gamma probe, and each node varied. Right tumors in sentinel lymph node were detected at the inter-aortocaval, paracaval or common iliac region. Para-aortic lymph nodes were detected as sentinel lymph node in cases of left tumor. In the 4 patients who underwent gamma probe navigated laparoscopic procedure sentinel lymph nodes were easily detected and safely removed for pathological examination. In the last patient who underwent laparoscopic lymph node dissection micrometastasis was found only at the sentinel lymph node. CONCLUSIONS: Sentinel lymph node can be detected by lymphoscintigraphy and handheld gamma probe. Gamma probe guided laparoscopic biopsy of sentinel lymph node is technically possible. These techniques may have a role in the management of clinical stage I testicular tumor but further trials are required for establishment of the concept of sentinel lymph node in testicular tumor.

The accuracy of a handheld portable spirometer.

BACKGROUND: Objective measurement of lung function is considered essential in the management of patients with asthma and COPD. Many primary care practitioners lack the means necessary to obtain these measurements conveniently. To meet this need, electronic spirometers, offering portability, ease of operation, and timesaving readout options have been introduced. We compared the accuracy of a typical pneumotachograph-based device with a conventional volume displacement spirometer. METHODS: We compared indexes of pulmonary function (FVC, FEV1, mean forced expiratory flow during the middle half of FVC, [FEF25-75%], and peak expiratory flow rate [PEFR]) measured by the handheld device with those measured by a conventional spirometer in 75 white subjects (33 men, 42 women) with a median age of 43 years (22 to 77 years) who were either healthy or were referred to the pulmonary function laboratory of a large tertiary care teaching hospital. The order of the instrument tested first was randomized and the patients were blinded to which instrument was being studied. RESULTS: There was a linear relationship between instruments for all indexes measured (r = 0.97, 0.98, 0.94, 0.94 for FVC, FEV1, FEF25-75%, and PEFR, respectively, for all p < 0.001). The random error (precision) was within 5% only for FEV1. The mean of the differences between the values obtained using both instruments (the bias) +/- limits of agreement (+/- 2 SD) were 0.06 +/- 0.56 L for FVC (p = NS), 0.2 +/- 0.44 L for FEV1 (p < 0.05), 0.61 +/- 1.26 L/s for FEF25-75% (p < 0.05), and 0.44 +/- 1.9 L/s for PEFR (p < 0.05). CONCLUSION: Our data suggest that measurements obtained using the pneumotachograph device are closely related to those obtained by volume displacement spirometry and that the handheld device may be useful in clinical practice. However, because the limits of agreement are wide and the difference between the two instrument measurements are significant for FEV1, FEF25-75%, and PEFR, the bias between them is not consistent nor is it insignificant. Therefore, the measurements made with the two types of machine cannot be used interchangeably.

Wide-field handheld high frequency ultrasonography using a variable high frequency ultrasound system.

Wide-field handheld high frequency ultrasonography using a variable high frequency ultrasound system is an imaging modality in the frequency range of 35 to 50 MHz. Using a water bath immersion technique, high-resolution digital movies obtained from a variety of anterior segment ocular conditions were particularly helpful in establishing the diagnosis in each case. Images taken with the wide-field limbus-to-limbus mode provided a comprehensive picture of large and extensive lesions, whereas the focal mode was for small to medium lesions or specific areas of interest.

In the palm of your hand.

The explosive growth of handheld personal digital assistants (PDAs) in health care has been nothing short of amazing. What applications--business and clinical--do these devices have in medicine, and what is their potential? PDAs are simple and intuitive; their applications require minimal interaction time, so they have minimal impact on work flow: the investment is small; and the lightweight form is relatively nonintrusive during a patient encounter. The devices are being used to capture charges for medical services at the point of care. Encounter capture, online prescription writing and other applications will soon come on the scene. This article discusses current and possible future uses for PDAs in health care, interfaces with other technologies and security concerns.

Scanning laser ophthalmoscopy and angiography with a wide-field contact lens system.

OBJECTIVE: To perform fluorescein and indocyanine green angiography for large or peripheral chorioretinal structures using a contact lens system that provides a 5-fold increase in the field of view of a confocal scanning laser ophthalmoscope (SLO). METHODS: Separate handheld contact and noncontact ophthalmoscopic lenses were manually aligned with the optical axis of a confocal SLO to demonstrate the feasibility of wide-field SLO angiography. An integrated, wide-field contact lens system was then designed and constructed to increase the SLO's 10 degrees , 20 degrees , and 30 degrees imaging fields to 50 degrees , 100 degrees , and 150 degrees , respectively. RESULTS: Simultaneous fluorescein and indocyanine green angiography was performed with the integrated, wide-field contact lens system for more than 50 patients with disorders that affect their peripheral retina and choroid. Retinal and choroidal abnormalities, including neovascularization and capillary nonperfusion, are easily detected and documented well beyond the range of conventional fundus cameras and SLOs. Peripheral retinal and choroidal hemodynamics can be readily observed and recorded. CONCLUSIONS: A confocal SLO has adequate resolution for clinically useful reflectance and angiographic imaging even when its field size is increased 5-fold by a wide-field contact lens system. Dynamic and static wide-field angiography can be performed without the limitations of manual or computer-automated photomontages. Peripheral retinal conditions can be studied and recorded to confirm observations from indirect ophthalmoscopy and to facilitate retinal photocoagulation and vitreoretinal surgery.

Iterative least-squares fitting programs in pharmacokinetics for a programmable handheld calculator.

Programs that perform a nonlinear least-squares fit to data conforming to one-compartment oral or two-compartment intravenous pharmacokinetic models are described. The programs are designed for use on a Hewlett-Packard HP-41 CV programmable calculator equipped with an extended-functions module and one or two extended-memory modules. Initial estimates of variables in the model are calculated by the method of residuals and then iteratively improved by the use of the Gauss-Newton algorithm as modified by Hartley. This modification minimizes convergence problems. The iterative-fitting procedure includes a routine for estimation of lag time for the one-compartment oral model. Clinical applications of the programs are illustrated using previously published data. Programming steps and user instructions are listed. The programs provide an efficient and inexpensive method of estimating pharmacokinetic variables.

A comparison of the cell phone driver and the drunk driver.

OBJECTIVE: The objective of this research was to determine the relative impairment associated with conversing on a cellular telephone while driving. BACKGROUND: Epidemiological evidence suggests that the relative risk of being in a traffic accident while using a cell phone is similar to the hazard associated with driving with a blood alcohol level at the legal limit. The purpose of this research was to provide a direct comparison of the driving performance of a cell phone driver and a drunk driver in a controlled laboratory setting. METHOD: We used a high-fidelity driving simulator to compare the performance of cell phone drivers with drivers who were intoxicated from ethanol (i.e., blood alcohol concentration at 0.08% weight/volume). RESULTS: When drivers were conversing on either a handheld or hands-free cell phone, their braking reactions were delayed and they were involved in more traffic accidents than when they were not conversing on a cell phone. By contrast, when drivers were intoxicated from ethanol they exhibited a more aggressive driving style, following closer to the vehicle immediately in front of them and applying more force while braking. CONCLUSION: When driving conditions and time on task were controlled for, the impairments associated with using a cell phone while driving can be as profound as those associated with driving while drunk. APPLICATION: This research may help to provide guidance for regulation addressing driver distraction caused by cell phone conversations.

A 3D reconstruction algorithm for EIT using a handheld probe for breast cancer detection.

A 3D reconstruction algorithm for electrical impedance tomography is presented for determining the distribution of electrical properties inside the body, given electrical measurements made on the surface. A linearized reconstruction algorithm using planar electrode arrays in a handheld probe geometry developed by Mueller et al (1999 IEEE Trans. Biomed. Eng. 46 1379-86) has been refined and extended in this paper. This algorithm is based on linearizing the conductivity about a constant value. We have extended the distance below the electrodes at which a target can be imaged by using a combination of two regularization schemes and a weighted mesh. An appropriate combination of Tikhonov and NOSER regularization produces satisfactory static images of a 2 cm cube placed 2 cm below the array, and difference images of a 1 cm cube 4 cm away from the array. The weighted mesh allows use of fixed regularization parameters for all depths of the target.

Design, testing, and clinical studies of a handheld polarized light camera.

Polarized light imaging has been used to detect the borders of skin cancer and facilitate assessment of cancer boundaries. A design for an inexpensive handheld polarized camera is presented and clinical images acquired with this prototype are shown. The camera is built with two universal serial bus (USB) color video cameras, a polarizing beamsplitter cube, and a 4x objective lens. Illumination is provided by three white LEDs and a sheet polarizer. Horizontal and vertical linearly polarized reflected images are processed at 7 frames/s and a resulting polarized image is displayed on screen. We compare the performances of cheap USB camera and a 16-bit electronically cooled camera. Dark noise and image repeatability are compared. In both cases, the 16-bit camera outperforms the USB cameras. Despite these limitations, the results obtained with this USB prototype are very satisfactory. Examples of polarized images of lesions taken prior to surgery are presented.

Online handwritten script recognition.

Automatic identification of handwritten script facilitates many important applications such as automatic transcription of multilingual documents and search for documents on the Web containing a particular script. The increase in usage of handheld devices which accept handwritten input has created a growing demand for algorithms that can efficiently analyze and retrieve handwritten data. This paper proposes a method to classify words and lines in an online handwritten document into one of the six major scripts: Arabic, Cyrillic, Devnagari, Han, Hebrew, or Roman. The classification is based on 11 different spatial and temporal features extracted from the strokes of the words. The proposed system attains an overall classification accuracy of 87.1 percent at the word level with 5-fold cross validation on a data set containing 13,379 words. The classification accuracy improves to 95 percent as the number of words in the test sample is increased to five, and to 95.5 percent for complete text lines consisting of an average of seven words.

Correlation of cartilage stiffness to thickness and level of degeneration using a handheld indentation probe.

The in vivo assessment of the quality of cartilage is a critical tool to determine how to clinically treat damaged cartilage. Utilizing nondestructive indentation, a hand-held instrument (ACTAEON Probe) was used to rapidly measure the stiffness of articular cartilage, which can then be correlated to cartilage condition. An integrated series of tests were performed to determine whether the Probe is sensitive enough to detect early degenerative changes in articular cartilage and to examine if it is dependent on the thickness of the cartilage being tested. Results demonstrated that there is no effect of thickness on the Probe measurements for clinically relevant human cartilage samples, that the Probe is sensitive enough to detect different levels of cartilage degeneration, and that Probe data correlate well with traditional creep indentation testing. This study demonstrates the utility of the Probe for detecting degenerative changes in animal and human joints.

Dense photometric stereo: a Markov random field approach.

We address the problem of robust normal reconstruction by dense photometric stereo, in the presence of complex geometry, shadows, highlight, transparencies, variable attenuation in light intensities, and inaccurate estimation in light directions. The input is a dense set of noisy photometric images, conveniently captured by using a very simple set-up consisting of a digital video camera, a reflective mirror sphere, and a handheld spotlight. We formulate the dense photometric stereo problem as a Markov network and investigate two important inference algorithms for Markov Random Fields (MRFs)--graph cuts and belief propagation--to optimize for the most likely setting for each node in the network. In the graph cut algorithm, the MRF formulation is translated into one of energy minimization. A discontinuity-preserving metric is introduced as the compatibility function, which allows alpha-expansion to efficiently perform the maximum a posteriori (MAP) estimation. Using the identical dense input and the same MRF formulation, our tensor belief propagation algorithm recovers faithful normal directions, preserves underlying discontinuities, improves the normal estimation from one of discrete to continuous, and drastically reduces the storage requirement and running time. Both algorithms produce comparable and very faithful normals for complex scenes. Although the discontinuity-preserving metric in graph cuts permits efficient inference of optimal discrete labels with a theoretical guarantee, our estimation algorithm using tensor belief propagation converges to comparable results, but runs faster because very compact messages are passed and combined. We present very encouraging results on normal reconstruction. A simple algorithm is proposed to reconstruct a surface from a normal map recovered by our method. With the reconstructed surface, an inverse process, known as relighting in computer graphics, is proposed to synthesize novel images of the given scene under user-specified light source and direction. The synthesis is made to run in real time by exploiting the state-of-the-art graphics processing unit (GPU). Our method offers many unique advantages over previous relighting methods and can handle a wide range of novel light sources and directions.
