A case of an intussuscepted neuroendocrine carcinoma of the appendix.

We have described a previously unreported entity of an intussuscepted neuroendocrine carcinoma of the appendix. Our patient was a 70-year-old man whose only complaint was insipient weight loss. Colonoscopy showed a malignant cecal "polyp", and an extended right hemicolectomy was performed. We have reviewed the literature on the causes of appendiceal intussusception and their appropriate treatment options, and clarified the classification of neuroendocrine tumors of the gastrointestinal tract.

[Abdominal metastases from signet cell carcinoma of the appendix vermiformis].

Signet cell carcinomas are often aggressive tumours. A patient with this tumour, originally located in the appendix, was diagnosed with peritoneal metastases. When she later got abdominal symptoms, computer axial tomography indicated large infiltrating tumour masses in the pelvis. However, when a laparotomy was performed, the only macroscopic tumour masses was localised to the ovaries (Krukenberg tumour). The surgical intervention gave the patient an improved quality of life, and most likely prolonged survival. We stress the importance of thinking of this possibility.

Primary malignant lymphoma of the appendix (a case report and review of the literature).

Authors report a case of a 29-year-old male with primary malignant lymphoma of the appendix with involvement of lymph nodes in the mesenteries. Surgical intervention was performed, adjuvant chemotherapy was applied. No recurrences, metastases or complaints occurred after one year of follow-up. The case is of interest due to the clinical and histopathological diagnostic problems as well as the dimension of the metastatic lymph node in the ileal mesentery.

Further studies of the glandular tissue of the Sauromatum guttatum (Araceae) appendix.

Electron microscopic studies showed that the trans-Golgi network (trans indicates the polarity of cisternae within the Golgi apparatus; it is opposite to the cis-face that is adjacent to the rough endoplasmic reticulum) was involved in the processing of the osmiophilic material present in the appendix of the inflorescence of Sauromatum guttatum. This material accumulated in the rough endoplasmic reticulum and in special pockets of the plasma membrane prior to heat production. Associations between the endoplasmic reticulum and trans-Golgi network were observed. The Golgi apparatus was composed of 5-6 dictyosomes on one side and one or two somewhat detached cisternae on the other side. Various nonosmiophilic Golgi-derived vesicles were observed: small ones covered with spike-like material, large ones with a smooth surface, and irregularly shaped ones. These electron-translucent vesicles seemed to accumulate in specific localities at the plasma membrane surface in the vicinity of the osmiophilic material; they were not found when the aroma was released. During heat production, the Golgi structures shrank and the activity of the trans-Golgi network seemed to be reduced. At the same time, coated pits were seen at the plasma membrane surface. In some cells, hypertrophic Golgi apparatuses were seen with only 2-3 dictyosomes that contained granulated material in their lumens. Finally, the osmiophilic material was also found in the plasmodesmata.

[Classic carcinoids of the appendix].

Nine classic carcinoids smaller than 2 cm, situated elsewhere than at the base of the appendix have been diagnosed in the Department of Pathology of the University Hospital in Olomouc during the period from 1994 to 1999. Appendectomy including removal of the mesenteriolum was performed in all cases. The literature mentions favourable prognosis of patients with small appendiceal carcinoids--100 per cent 5-year survival rate. Tumours greater than 2 cm recur and metastasise. These tumours require oncological therapy in cases with unresectable metastases. This worsens the 5-year survival rate to 21-42%.

[Ultrasound first in acute appendix? Unnecessary laparotomies can often be avoided].

Acute appendicitis is one of the most common causes of acute abdomen. Prospective studies have shown the overall accuracy of the clinical diagnosis to be not more than 70-78% in suspected appendicitis, with a correspondingly high rate of 20-25% unnecessary laparotomies. High-resolution ultrasonography (US) is a readily available, radiation-free noninvasive diagnostic modality that enables visualization of the inflamed appendix and a variety of relevant diseases in the differential diagnostic work-up. Prospective studies have shown that the overall accuracy of US in acute appendicitis ranges between 86 and 87% (sensitivity 75-92%, specificity 92-100%), with positive and negative predictive values of 89-96%, respectively. In experienced hands, US has significantly improved the diagnostic accuracy in suspected appendicitis, while halving the negative laparotomy rate to 8-15%. Since, however, a low percentage of false-negative US explorations due, for example, to accumulation of bowel gas or obesity, final therapeutic decisions be based on a careful consideration of US findings together with history, symptoms and clinical findings.

[A case of primary adenocarcinoma of the appendix].

The authors report a case of primary adenocarcinoma of the appendix. They found that achieving correct diagnose during the operation is very difficult. Once the histology result is available they propose right hemicolectomy to try to achieve possible tumour clearance.

Spastic hypertonia. Appendix.

This appendix reviews the most commonly used clinical methods for assessing the clinical treatment of spastic hypertonia. The definitions and rating scales shown often are accepted by the Food and Drug Administration for pharmaceutical and investigational trials to obtain a clinical indication for use in spasticity clinic.

[Laparoscopic appendectomy as first choice intervention in the treatment of inflammatory diseases of the appendix. Analysis of 397 laparoscopic interventions].

Despite the increasingly widespread use of laparoscopy, especially for gallbladder diseases, laparoscopic appendectomy has not yet met with universal acceptance. The aim of the present retrospective study was to illustrate the technical aspects and the results of 397 laparoscopic appendectomies carried out at the Surgical Department of the San Giovanni Battista Hospital in Zagarolo, Rome, from January 1993 to December 2000. The patients included 260 females with a mean age of 35.5 years and 137 males with a mean age of 38.5 years. All the appendectomies were carried out laparoscopically, with a conversion index of 0%, by two surgeons, utilising only three trocars and the Veress needle technique. Mean operating time was 22.5 min (range: 15-30 min). The mean postoperative hospital stay was 1.5 days. Morbidity was 1.76% (7 cases) and mortality nil. The authors emphasise the numerous advantages of laparoscopic techniques in their experience, including the excellent cosmetic results, reduced postoperative pain, rapid functional recovery, and lower incidences of adhesions, wound infections and laparocele, and believe that laparoscopic appendectomy is a reliable operation for treating all inflammatory diseases of the appendix.

[Clinical picture and treatment of carcinoid of the appendix].

The clinical syndrome of a carcinoid tumor of the appendix is presented in 7 such cases. In spite of its slow growth and late metastatic dissemination iliocoecal resection must be performed dependent on the extent and location of the tumor.

[The relationship of the left atrial spontaneous echo contrast integrated backscatter with left atrial appendix function and tissue doppler characteristics].

OBJECTIVE: Integrated backscatter (IBS) is an objective method to measure spontaneous echo contrast (SEC) quantitatively. The purpose of this study was to investigate the relationship between the quantitative values of left atrial SEC measured by using IBS with its qualitative gradation as well as the left atrial appendix (LAA) functions and LAA tissue Doppler characteristics (TD). METHODS: Thirty-two patients (23 female, 9 male) with various diseases undergoing transesophageal echocardiography (TEE) were included. Mean age was 52 + 13 years. Left atrial SEC was graded as no SEC (n = 12), mild (n = 12) or severe SEC (n = 8) in patients underwent TEE. LAA peak emptying and LAA apical and basal TD velocities were measured. LAA ejection fraction was assessed by standard means. RESULTS: Between no SEC and mild SEC groups, left atrium IBS and LAA apical TD values were found statistically significant. But there were no differences between two groups regarding LAA emptying velocities, LAA ejection fraction values and LAA basal TD values. All of these parameters were found statistically different between severe and mild SEC groups. The left atrial IBS values were found to be correlated positively with the qualitative SEC grade. The left atrial IBS values correlated negatively with LAA emptying, apical TD velocities and LAA ejection fraction values. CONCLUSION: Integrated backscatter provides an objective quantitative measure of SEC that correlates well with LAA apical TD velocities and the other parameters of LAA.

Purification and Characterization of a Novel 21 kD Calcium-binding Protein from Rabbit Appendix Lymphocytes.

A novel 21 kD calcium-binding protein from rabbit appendix B lymphocytes has been purified and characterized. Through heat-denaturation, using Phenyl-Sepharose and DEAE-Sepharose chromatography, we obtained 5.3 mg SDS-PAGE homogeneous CaBP(21) from 1 kg lymphocyte cells. Amino acid analysis showed the acidic amino acid content (Asp + Glu) to be 26% after HCL hydrolysis. The blocking of the N-terminus of CaBP(21) prevents the de novo Edman degradation, like most of the other calcium-binding proteins. CaBP(21) has 46% of hydrophobic amino acid (with Gly, without Trp) content, 10% of basic amino acid content and 44% of acidic and polar amino acids. Peptide mapping and SDS-PAGE combined Sephadex G-25 gel filtration proves that CaBP(21) consists of two identical or similar subunits. Ca(2+)-binding assays using Arsenazo III indicated one protein to bind 4 Ca(2+) with dissociation constant (K(d)) for Ca(2+) about 10(-5) mol/L.

[Case of carcinoid of the appendix in a patient with endometrial cancer and uterine myoma].

Reports on coexistence of the three cancers: a carcinoid of the appendix, myoma of the uterus and adenoma of the uterine cavity occurring in a female patient are exceptionally rare. Such a case may occur with a frequency of 2 every 10,000 hysterectomies. The patient was subjected to hysterectomy, appendectomy and had her obturator and iliac lymph nodes removed. Postoperative treatment comprised radiotherapy. It is estimated that on overall chance of 5 year survival is close to 50%. The overall good state of the patient, 4 years after operation, speaks for recommending this kind of treatment. The authors suggest that the carcinoid does not necessarily must be an inductor of a cancer of the endometrium and they indicate that a cancer of the endometrium is not an inductor of the carcinoid.

A case of linitis plastica involving the entire colon, ileum, and appendix.

Linitis plastica of the colon is an uncommon presentation of primary colorectal cancer. This entity of colorectal cancer is characterized by a diffuse infiltrating tumor with desmoplastic reaction and poor prognosis. Although widespread infiltration is the main feature of linitis plastica, the tumor extending to more than 2 segments of the colon is uncommon. We report a case of primary linitis plastica involving the entire colon, ileum and appendix. The clinical characteristics are discussed with a review of literatures.

Secondary intestinal obstruction due to low-grade mucinous cystadenocarcinoma of the appendix.

Appendiceal mucoceles are a group of lesions in which the appendiceal lumen becomes distended with mucus. They may be associated with an unusual form of gelatinous ascites termed pseudomyxoma peritonei. We report herein a 77-year-old male patient with appendiceal mucinous cystadenocarcinoma and pseudomyxoma peritonei, whose initial symptoms were abdominal pain and distension and intestinal obstruction. He had a history of explorative laparotomy for an abdominal mass and ascites 14 months ago at another center. He was suffering from abdominal pain and intestinal obstruction on admission to the emergency service. We performed right colectomy and total resection of the mass which originated from the appendix. Pathologic examination of the specimen showed low-grade mucinous cystadenocarcinoma and pseudomyxoma peritonei. The postoperative period was uneventful.

[Appendix to the "Darmstadt Johannes-Alexandrinus Fragment"].

This appendix to "Das Darmstädter Johannes-Alexandrinus-Fragment" answers to the critical note by Klaus-Dietrich Fischer, and shows that these new-founded fragments are a testimony of an until now unknown abbreviated version of the Alexandrinian commentary on Galen's book 'De sectis'.

Mucinous cystadenocarcinoma of the appendix with pseudomyxoma peritonei presenting as total uterine prolapse. A case report.

Mucinous cystadenocarcinoma of the appendix occurred with symptoms limited only to a total uterovaginal prolapse. Preoperative intravenous pyelogram and pelvic ultrasonography demonstrated the presence of a large pelvic mass. Exploratory laparotomy revealed the mass to be appendiceal adenocarcinoma, which was treated with extirpation of all the visible tumor and repair of the anatomic defect.
