Acceptability of the Checkpoints Parent-Teen Driving Agreement: pilot test.

BACKGROUND: Parent-teen driving agreements are potentially important tools to facilitate parental management of teen driving and reduce adolescent driving risk. The Checkpoints Parent-Teen Driving Agreement (Checkpoints P-TDA) was designed so that parents could initially impose strict limitations on teen driving in high-risk driving conditions (e.g., at night and with teen passengers) and gradually increase driving privileges over time as teens demonstrate responsible driving behavior. METHODS: To assess the acceptability of the format and content of the Checkpoints P-TDA, it was pilot tested with a convenience sample of 47 families recruited as their teens tested for a driver's license at five private driving schools in Connecticut. Family members were interviewed at the driving schools about potential limits on teen driving, asked to use the driving agreement, and re-interviewed within 3 months about acceptability of the driving agreement and initial driving limits placed on teens. RESULTS: Most families (38 of 47) used and liked the agreement. In addition, most parents placed the recommended strict initial limits on teen driving related to driving unsupervised at night, with teen passengers, and on high-speed roads. Moreover, parents reported placing more strict limits on their teens' driving than they originally intended. CONCLUSIONS: The results showed promise for the acceptability of the Checkpoints P-TDA, which will be tested statewide.

[Efficacy and acceptability of the fixed fluticasone + salmeterol combination in the treatment of acute asthma attacks. Results of a one-year comparative study].

BACKGROUND: The objective of an asthma treatment is to control asthma, particularly to prevent exacerbations. OBJECTIVE: To study the efficacy, acceptability and safety of the fluticasone/salmeterol (FP/S) combination in preventing asthma exacerbations in comparison with the continuation of previous treatment (TA). METHODS: This was a multicentre, randomised, parallel-group study to compare the fixed combination FP/S to TA (treatment with a free combination of an inhaled corticosteroid and a long-acting beta2-agonist) over one year in patients whose asthma was well controlled with their current treatment. RESULTS: Five hundred and twenty patients were randomized and their data analyzed on an intent-to-treat basis. Seventy-four percent of the patients in the FP/S group and 71% in the TA group had no exacerbation. The 3.12% difference in favor of the FP/S group (90% CI: -3.32% to 9.56%) demonstrated that FP/S was at least as effective TA in preventing asthma exacerbations. Treatment acceptability, evaluated by the patient on visual analog scales (inclusion as part of the daily habits, constraint, simplicity) was better with FP/S (p<0.001) than with TA. Clinical safety was good and comparable in the two groups. CONCLUSION: The fixed fluticasone/salmeterol combination provided a protection as good as that obtained with free combinations over a one-year treatment period, with a significant improvement in treatment acceptability and a good clinical safety.

Cephalometric pattern of Jewish East European adolescents with clinically acceptable occlusion.

Knowledge of the normal dentofacial patterns of various ethnic groups is important for clinical and research purposes. The objective of this study was to describe the dentofacial pattern of one Jewish ethnic group and to compare it with accepted standards for other white populations. Lateral cephalometric radiographs of 18 children of Jewish East European origin with clinically acceptable permanent occlusion, ages 11 to 13 years, were used. A computer-aided cephalometric analysis was performed, and the mean values and standard deviations for the various cephalometric variables were obtained. When compared with some classical standards, it was found that this ethnic group tended toward a mild skeletal Class II, a convex profile, and a more vertical direction of growth. These features appeared in persons selected on the basis of a clinically acceptable occlusion, suggesting the existence of dental compensations. Understanding these natural accommodations of the dentition to skeletal variations can be clinically useful when establishing treatment goals.

Characteristics of meta-analyses related to acceptance for publication in a medical journal.

Editors of medical journals select manuscripts for publication based, in part, on the perceived quality of the manuscript submitted. The objective of this study was to describe associations between acceptance for publication and quality-related methodologic characteristics of meta-analyses. This was a prospective observational study. The setting was editorial offices of JAMA and offices of external reviewers. The manuscripts reviewed were 112 consecutive meta-analyses submitted to JAMA during 1996 and 1997 whose authors agreed to participate. The main outcome measures were ratings of 16 methodologic characteristics reflecting quality of the meta-analysis and acceptance for publication. A "high" rating for one methodologic characteristic, whether the report of the meta-analysis provided sufficient detail to enable replication, was related significantly to publication (RR = 2.79, 95% CI = 1.13-6.89). This relationship persisted when other variables were controlled for in the model. Generally, rejected manuscripts had fewer factors rated "high," but differences were not significant. We found that inclusion of sufficient detail to allow a reader to replicate meta-analytic methods was the only characteristic related to acceptance for publication.

Nurses' acceptance of behavioral treatments and pharmacotherapy for behavioral disturbances in older adults.

Behavioral disturbances are common in elderly individuals, particularly in those suffering from dementia. Nurses are called on to manage these problem behaviors both in the nursing home and through home health care. There has been growing interest in nonpharmacological interventions for these problems, most notably behavioral therapies. However, until recently, no information was available regarding the acceptance by health care professionals on the use of these treatments with elderly individuals. The purpose of this study was to assess geriatric nurses' acceptance of two behavioral treatments (differential reinforcement of incompatible behavior and time-out from positive reinforcement) and the most commonly used treatment at this time, pharmacotherapy. Nurses were asked to read a clinical case vignette and three treatment descriptions, rating the treatments with Kazdin's Treatment Evaluation Inventory. Results show that both behavioral treatments, including a mildly aversive treatment (time-out) were rated more positively than pharmacotherapy. Nurses modified their ratings in response to described differences in the patient's cognitive functioning and place of residence. Most interesting, acceptability ratings were influenced by characteristics of nurse raters, including the nurses' educational background and the amount of contact with geriatric patients.

Testing homology with Contact Accepted mutatiOn (CAO): a contact-based Markov model of protein evolution.

Point Accepted Mutation (PAM) is the Markov model of amino acid replacements in proteins introduced by Dayhoff and her co-workers (Dayhoff et al., 1978). The PAM matrices and other matrices based on the PAM model have been widely accepted as the standard scoring system of protein sequence similarity in protein sequence alignment tools. Here, we present Contact Accepted mutatiOn (CAO), a Markov model of protein residue contact mutations. The CAO model simulates the interchanging of structurally defined side-chain contacts, and introduces additional structural information into protein sequence alignments. Therefore, similarities between structurally conserved sequences can be detected even without apparent sequence similarity. CAO has been benchmarked on the HOMSTRAD database and a subset of the CATH database, by comparing sequence alignments with reference alignments derived from structural superposition. CAO yields scores that reflect coherently the structural quality of sequence alignments, which has implications particularly for homology modelling and threading techniques.

The acceptance of hearing aids for children with otitis media with effusion.

Conservative treatment for otitis media with effusion (OME) led us to consider the use of hearing aids as a way of managing the associated hearing loss. This study aimed to assess the compliance of patients and acceptance of hearing aids for the management of children with OME. Thirty-nine children who had been given binaural hearing aids to manage OME were assessed at routine follow-up after six months. A clinician who did not prescribe the aid administered a questionnaire to assess compliance, change in symptoms and acceptance of the aids. Thirty-eight parents thought the aids were easy to use and 25 (66 per cent) were completely satisfied with the management. Aided hearing improved by a mean of 17 dB (Range 10-30) over three frequencies, 0.5, 1.0, 2.0 Khz and all parents reported subjective hearing improvement in their children. The stigma of an aid was reported as minimal under the age of seven. Hearing aids provide a non-invasive way of managing the problems associated with OME which is acceptable to certain parents and children. Long-term effects of using aids need to be evaluated before they can be recommended.

[Compliance with and acceptability of a new electronic peak flow meter, the PiKo-1].

BACKGROUND: Monitoring airflow obstruction is an essential component of asthma management. We examined home recording of PEFR using a new electronic peak flow meter in terms of compliance and acceptability in a group of children with asthma. METHODS: Twenty three children (3 with intermittent asthma and 20 with persistent asthma) (average age 10.9 +/- 3.8 [5-18] yrs) were asked to assess their PEFR every day during a period of 5.8 +/- 1.2 [4-8] weeks and record it in a diary card. Patients were not aware that their data was also being stored on the PiKo-1. At the end of the study, the written data were compared to the stored data. A multiple choice questionnaire was given to each subjectto check the acceptability of the PiKo-1. RESULTS: 2 patients were lost to follow up. The compliance (expressed as a percent of the number of recordings that should have been made) was more that 80% for 14/21(67%) patients and less than 45% for 3/21 (14%). Compliance decreased during the study (96% in the first week, 68% during the fifth). 12% of the values were falsified. The PiKo-1 was considered to be small, attractive and useful. Some children found the mouthpiece too small. CONCLUSION: The good results for compliance that we observed might have been due to the short duration of the study. PiKo-1 was well accepted by the subjects. It will be possible to monitor PEFR and forced expiratory volume in the first second of expiration at home using this new device.

Correlation of denitrification-accepted fraction of electrons with NAD(P)H fluorescence for Pseudomonas aeruginosa performing simultaneous denitrification and respiration at extremely low dissolved oxygen conditions.

In cystic fibrosis airway infection, Pseudomonas aeruginosa forms a microaerobic biofilm and undergoes significant physiological changes. It is important to understand the bacterium's metabolism at microaerobic conditions. In this work, the culture properties and two indicators (the denitrification-accepted e- fraction and an NAD(P)H fluorescence fraction) for the culture's "fractional approach" to a fully anaerobic denitrifying state were examined in continuous cultures with practically zero DO but different aeration rates. With decreasing aeration, specific OUR decreased while specific NAR and NIR increased and kept Y(ATP/S) relatively constant. P. aeruginosa thus appeared to effectively compensate for energy generation at microaerobic conditions with denitrification. At the studied dilution rate of 0.06 h(-1), the maximum specific OUR was 2.8 mmol O2/g cells-h and the Monod constant for DO, in the presence of nitrate, was extremely low (<0.001 mg/L). The cell yield Y(X/S) increased significantly (from 0.24 to 0.34) with increasing aeration, attributed to a roughly opposite trend of Y(ATP/X) (ATP generation required for cell growth). As for the denitrification-accepted e- fraction and the fluorescence fraction, both decreased with increasing aeration as expected. The two fractions, however, were not directly proportional. The fluorescence fraction changed more rapidly than the e- fraction at very low aeration rates, whereas the opposite was true at higher aeration. The results demonstrated the feasibility of using online NAD(P)H fluorescence to monitor sensitive changes of cellular physiology and provided insights to the shift of e- -accepting mechanisms of P. aeruginosa under microaerobic conditions.

Ladder-type pentaphenylenes and their polymers: efficient blue-light emitters and electron-accepting materials via a common intermediate.

A new route to ladder-type pentaphenylenes has been developed in which both good hole-accepting p-type and electron-accepting n-type materials can be prepared from a common intermediate. This key intermediate is a pentaphenylene diester 5 obtained in high yield by Suzuki coupling of 2 equiv of fluorene boronates with 2,5-dibromoterephthalate. Addition of aryllithium followed by ring closure with boron trifluoride produced a blue-emitting ladder-type pentaphenylene. Bromination followed by reductive polymerization with nickel(0) gave new high molecular mass polymers, which show efficient blue emission with a very small Stokes shift. These polymers bridge the gap in emission between polyfluorenes and fully ladder-type polyphenylenes. An alternative ring closure of the dibromopentaphenylene diester 14 with acid made a diketone that is a good electron-accepting material, as it displays a reversible two-electron reduction. The reduction onset potential of -0.875 V against Ag/Ag(+) corresponds to a lowest unoccupied molecular orbital (LUMO) energy level of 3.53 eV, comparable to the work function of magnesium, suggesting that this unit could be used to greatly increase the injection of electrons into polymers containing it in a light-emitting diode (LED) or solar cell. A red-emitting material was prepared by Suzuki coupling of the dibromopentaphenylene 10b with a perylene dye, thus offering the prospect of tuning the emission from pentaphenylene materials over the whole visible range by attachment of suitable dyes. Unoptimized single-layer organic LEDs that used 11b showed stable pure-blue emission with brightnesses of over 200 cd/m(2) at 7 V, with moderate efficiencies.

Theoretical studies of the modulation of polymer electronic and optical properties through the introduction of the electron-donating 3,4-ethylenedioxythiophene or electron-accepting pyridine and 1,3,4-oxadiazole moieties.

One serious problem associated with polyfluorene and derivatives (PFs) as blue luminescent polymers is the significant energy barrier for hole or electron injections; thus they usually face charge injection and transport difficulties with the currently available cathode and anode materials. The incorporation of an electron-donating or -accepting unit is expected to improve the recombination of the charge carriers. In this paper, we apply quantum-chemical techniques to investigate three fluorene-based copolymers, copoly(2,5-ethylenedioxythiophene-alt-9,9'-dimethylfluorene) (PEF), copoly(2,5-pyridine-alt-9,9'-dimethylfluorene) (PPyF), and poly[(fluorene-2,7-diyl)-alt-(1,3,4-oxadiazole-2,5-diyl)] (PFO), in which Delta(H)(-)(L) [the energy difference between the highest occupied molecular orbital (HOMO) and the lowest unoccupied molecular orbital (LUMO), when n = infinity], the lowest excitation energies (E(g)), ionization potentials (IP), electron affinities (EA), and lambda(abs) and lambda(em) are fine-tuned by the regular insertion of electron-donating unit 3,4-ethylenedioxythiophene (EDOT) or electron-withdrawing units pyridine and 1,3,4-oxadiazole. The results show that the alternate incorporation of electron-donating moiety EDOT increases the HOMO energy and thus reduces the IPs, and consequently the hole injection was greatly improved. On the other hand, even though both kinds of charge carriers will improve the electron-accepting ability, the results show that electron-withdrawing moieties greatly facilitate the electron-transporting. Especially in PFO, the highly planar structural character resulted from the strong push-pull effect between the fluorene ring and the 1,3,4-oxadiazole ring and a weak interaction between the nitrogen and oxygen atoms in 1,3,4-oxadiazole ring and the hydrogen atom of the fluorene ring, significantly lowering the LUMO energy levels and thus improve the electron-accepting and transporting properties by the low LUMO energy levels.

Acceptability of green soybeans as a vegetable entity.

A profile of the sensory characteristics of green soybean and consumer intent of purchasing the product were determined. Thirty-one green soybean genotypes from maturity groups III to VI harvested between R6 and R7 were frozen, boiled, shelled, and evaluated for color, texture, sweetness, nuttiness, beaniness, oiliness, aftertaste and overall eating quality by three sensory panels. Results showed highly significant (p <0.01) variabilities among the genotypes in all sensory parameters and highly significant correlations among several parameters. The green soybeans, when boiled, ranged from lightly green to green, were slightly resistant to chewing, slightly nutty, slightly beany, not oily, imparted a pleasant aftertaste and had a fairly good overall eating quality. The green soybeans would be potentially acceptable as a vegetable in the frozen state and for use in recipes. When selecting genotypes for production, consideration should be given to the sensory attributes of the genotypes, because there was significant variability among the characteristics of the green soybeans, and several characteristics together enhanced the overall acceptability of the vegetable soybeans. To further increase the acceptability of green soybeans as a vegetable, geneticists need to consider breeding a vegetable soybean that has as many desirable sensory attributes as possible.

Pregnancy discovery and acceptance among low-income primiparous women: a multicultural exploration.

OBJECTIVES: As part of a larger study exploring psychosocial factors that influence self-care and use of health care services during pregnancy, we investigated the process of pregnancy discovery and acceptance among a culturally diverse group of women who had given birth to their first child in the year preceding data collection. METHODS: Eighty-seven low-income women from four cultural groups (African American, Mexican, Puerto Rican, and white) participated in eight focus groups held in their communities. The focus groups were ethnically homogenous and stratified by early and late entry into prenatal care. A social influence model guided the development of focus group questions, and the study followed a participatory action research model, with community members involved in all phases of the research. RESULTS: Issues that emerged from the focus groups as possible influences on timing of pregnancy recognition include the role of pregnancy signs and symptoms and pregnancy risk perception in the discovery process, the role of social network members in labeling and affirming the pregnancy, concerns about disclosure, "planning" status of the pregnancy, and perceived availability of choices for resolving an unintended pregnancy. CONCLUSIONS: The pregnancy discovery process is complex, and when protracted, can potentially result in delayed initiation of both prenatal care and healthful pregnancy behaviors. Enhancing our understanding of pregnancy discovery and acceptance has clear implications for primary and secondary prevention. Future research is needed to further explain the trajectory of pregnancy discovery and acceptance and its influence on health behaviors and pregnancy outcome.

Family caregiving of the elderly in Botswana: boundaries of culturally acceptable options and resources.

Care of the elderly in Botswana is solely a family responsibility. In the absence of formal programs for the elderly it is important to understand the cultural beliefs that guide decision making regarding acceptance of elderly care. The purpose of this study was to explore the experiences of family caregivers to older persons in Botswana. Using grounded theory methodology, 24 caregivers who were caring for an older person were interviewed. The findings indicate that the perceptions that guided the caregivers' decision making process of acceptance or non-acceptance of assistance from both family members and the government fell into three inter-related but distinct categories: Stigma, Appropriate-inappropriate forms of care; and Sense of Place. It is recommended that cultural sensitivity should be considered in designing models of support for families who are looking after their older relatives. Finally, a clear understanding of the perceptions of family members' needs must be emphasized as these differ from health workers' perceptions of family needs.

A pilot study of immigration status, homosexual self-acceptance, social support, and HIV reduction in high risk Asian and Pacific Islander men.

This article reports the results of a cross-sectional study that was conducted to describe the sexual behavior and HIV risk reduction behaviors of homosexual and bisexual Asian and Pacific Islander men and to relate immigration status, self-acceptance as a homosexual, and levels of social support to the adoption of safe sexual behaviors in this population. Thirty-one gay and bisexual Asian and Pacific Islander men in San Diego County, California, participated. Generally high levels of knowledge about HIV and transmission risks as well as self-acceptance and social support were found. While most (84%) reported some attempts to increase condom use in the previous 6 months, 42% reported engaging in unprotected intercourse during that same time period. An inverse relationship between self-acceptance and utilization of risk reduction strategies was found. No association was found between immigration status or self-reported HIV status and level of HIV knowledge, level of HIV risk behavior, or level of HIV risk reduction efforts. The findings are discussed within the context of other social network studies and HIV prevention programs for gay and bisexual Asian and Pacific Islander men.

Chlorhexidine-containing mucoadhesive polymeric compacts designed for use in the oral cavity: an examination of their physical properties, in vitro/in vivo drug release properties and clinical acceptability.

This study describes the formulation, physicochemical and mucoadhesive properties and in in vitro/in vivo release of chlorhexidine from mucoadhesive, polymeric compacts, designed for application within the oral cavity. Compacts were prepared by direct compression of mixtures containing 100 mg sodium carboxymethylcellulose (NaCMC), 25 mg hydroxyethylcellulose (HEC)/75 mg polyacrylic acid (PAA) and 75 mg HEC/25 mg PAA. The mechanical and mucoadhesive properties of the drug-loaded compacts were examined using a texture analyzer in compression and tension modes, respectively. Evaluation of mucoadhesion was performed using a mucin-coated gauze substrate. In vitro release of chlorhexidine was performed under sink conditions (phosphate buffered saline, pH 7.0, 37 degrees C) using a Caleva 7ST dissolution apparatus. Salivary chlorhexidine levels were determined following intra-oral placement of drug-containing formulations. Quantification of the mass of chlorhexidine released both in vitro and in vivo was performed using HPLC with ultraviolet detection. Furthermore, the in vivo acceptability of the various polymeric compacts was assessed in volunteers using standard questionnaires. Compacts composed of HEC/PAA exhibited greater in vivo retention than those composed of NaCMC. Compacts composed of 25 mg PAA and 75 mg HEC displayed greatest patient acceptability. Introduction of chlorhexidine into these compacts did not significantly compromise either the work required for compact fracture or the in vitro mucoadhesion. Controlled release of chlorhexidine from these compacts was observed both in vitro and in vivo, the concentration of chlorhexidine in saliva exceeding the minimum inhibitory concentration of the common oral pathogens over the study period. In light of the patient acceptability and in vivo performance, it is suggested that the compact composed of 25 mg PAA/75 mg HEC containing 10 mg chlorhexidine offers considerable promise for use as an antimicrobial agent in the oral cavity.

Public acceptance of restrictions on mentally ill people.

OBJECTIVE: To assess the influence of demographic, psychological, sociological and cultural variables on the public acceptance of restrictions on mentally ill people. METHOD: Multiple logistic regression analysis of the results of an opinion survey conducted on a representative sample in the German, French and Italian-speaking part of Switzerland. RESULTS: Public acceptance of restrictions depends on age, education and gender. The influence of demographic variables is reduced when including rigidity, anomie, social distance and contact with mentally ill people in the analysis. The cultural influence of the language region is significant. All these factors predict acceptance. Stereotypes and emotional reactions have no influence. CONCLUSION: Attitudes to mentally ill people are shaped on three distinct levels, the psychological, social and cultural, respectively. Demographic variables partly play the role of placeholders covering the underlying relationships.

Acceptance of third-party cardiac but not skin allografts induced by neonatal exposure to semi-allogeneic lymphohematopoietic cells.

Neonatal tolerance is exclusively donor-specific when assessed by skin allograft survival and in vitro alloreactivity assays. In contrast, we reported previously that acceptance of primarily vascularized cardiac allografts was not donor-specific in C3H/He (C3H, H-2(k)) mice treated as neonates with BALB/c-derived (BALB, H-2(d)) lymphohematopoietic cells, but included third-party C57BL/10 (B10, H-2(b)) allografts. The present study examined whether this unusual pattern is limited to heart grafts in this strain combination, and defined the relative importance of the donor cell H-2(d) haplotype for third-party cardiac allograft acceptance. C3H neonates were injected with (C3HxBALB)F1 bone marrow and spleen cells. Tolerance was assessed at age 8-10 weeks by transplantation of heart or skin allografts from several donor strains, and by in vitro assays of proliferation and cytotoxicity. Additionally, cells from H-2(d) and H-2(b)-expressing strains on BALB or non-BALB minor histocompatibility (miH) antigen backgrounds were tested as tolerizing inocula. Prolonged survival of cardiac grafts from all donor strains was observed in neonatally treated mice, whereas skin grafting and in vitro assays demonstrated donor-specific hyporesponsiveness. Both H-2(d) haplotype and non-H-2 miH background of graft donor and tolerizing cell donor were important to third-party cardiac allograft acceptance. These results suggest that the functional alteration in alloreactivity induced by neonatal alloantigen exposure depends partly on method of assessment.
