A randomized multicenter study comparing the efficacy and bleeding pattern of a single-rod (Implanon) and a six-capsule (Norplant) hormonal contraceptive implant.

To compare the contraceptive efficacy, tolerability, and bleeding patterns, 200 healthy female volunteers received, in an open, comparative, randomized, multicenter study in China, either a single-rod (Implanon) or a six-capsule (Norplant) contraceptive implant for 2 years with an optional extension of up to 4 years. Women were exposed to Implanon for 341.6 woman-years and Norplant for 329.1 woman-years. There were no pregnancies during the study. Per 90-day reference period, the median number of bleeding/spotting days with Implanon decreased from 33.5 in the first period to 19.0-21.5 days in the last year. Similarly, with Norplant, the median number of bleeding/spotting days decreased from 34.5 to 18.0-23.0 days, respectively. The number of bleeding/spotting episodes during year 1 was 2.0 per 90-day reference period with Implanon and 3.0 per period with Norplant (p < 0.05 for periods 1-4). For the remaining 90-day periods, there was no statistical difference between the two groups. In general, there was less frequent bleeding with Implanon compared with Norplant, whereas the incidences of amenorrhea and infrequent bleeding were higher with Implanon than with Norplant. The mean overall incidence of prolonged bleeding fell markedly during the study, from 66.0% in reference period 1 to 27.3% in period 16 with Implanon and from 69.0% to 21.7% with Norplant, respectively. The most common adverse events were related to disturbed bleeding patterns, which were also the major reasons for discontinuation (Implanon n = 8; Norplant n = 14). Normal menses returned in almost all subjects within 3 months after removal of the implants. Implanon was inserted in a mean time of 0.61 min and Norplant in 3.90 min (p < 0.001). Similarly, the mean time required to remove the implant was significantly shorter for Implanon than for Norplant (2.18 min vs 11.25 min, p < 0.001). The maximum time required for removal of the implant was 10 min for the Implanon group and 60 min for the Norplant group. In both groups, blood pressure and hemoglobin were not affected, whereas body weight tended to increase. It can be concluded that both contraceptive systems demonstrated excellent contraceptive efficacy and were well tolerated. Compared with Norplant, there was less frequent bleeding with Implanon, whereas the incidence of infrequent bleeding and amenorrhea was higher. Implanon was significantly quicker to insert and to remove than was the multiple capsule system.

RETRACTED: The effects of Implanon on lipid metabolism in comparison with Norplant.

This open, randomized study was intended to assess the effects of the new single-rod contraceptive implant (Implanon) containing etonogestrel on lipid metabolism in Indonesian women, in comparison with the six-rod implant Norplant, containing levonorgestrel. The effects of both products were compared with a control group using an intrauterine device (IUD) over a 3-year period. A total of 135 healthy volunteers of childbearing potential, aged 22 to 41 years, were enrolled in Jakarta, Indonesia. Ninety volunteers were randomized to use Implanon (n = 45) or Norplant (n = 45), and a nonrandomized group of 45 Multiload Cu 250 IUD users served as a control. Serum concentrations of total cholesterol, triglycerides, HDL cholesterol, LDL cholesterol, apolipoprotein AI, apolipoprotein AII, and apolipoprotein B were measured. The ratios of HDL cholesterol to total cholesterol, of HDL to LDL cholesterol, and of apolipoprotein AI to apolipoprotein B were determined. Lipid and lipoprotein determinations were done at screening and after 3, 6, 12, 18, 24 and 36 months. Contraceptive efficacy and insertion and removal times were also recorded. Mean changes from baseline in the lipid and apolipoprotein parameters, although occasionally statistically significant, were small in all groups (less than 1 standard deviation of the mean concentration at baseline) and clinically not significant. Statistically significant differences between the Implanon and Norplant groups were only occasionally observed. In both implant groups, total mean cholesterol, LDL cholesterol, and apolipoprotein AI concentrations tended to decrease during the study, but statistically significant changes from baseline were only occasionally observed, suggesting that drug-related factors are not involved. The mean ratios of HDL/total cholesterol and the HDL/LDL cholesterol showed very little change over time in both implant groups; slight and statistically nonsignificant increases were noted at most time points. Similar changes were seen in the group of IUD users. It can be concluded that changes from baseline in the lipid and apolipoprotein parameters tested were generally small and did not differ between Implanon and Norplant.

Comparison of potency of human carcinogens: vinyl chloride, chloromethylmethyl ether and bis(chloromethyl)ether.

The alpha-chloroether carcinogen chloromethylmethyl ether (CME) and its impurity bis(chloromethyl)ether (BCME) are direct-acting alkylating agents. Vinyl chloride (VC) is an indirect-acting carcinogen but its accepted carcinogenic intermediate, chloroethylene oxide, is also an alpha-chloroether. Both CME-BCME and VC have been in industrial use since about 1950. Hence, they were selected for comparison of potency as human carcinogens using numerous epidemiologic reports. There were 115 deaths due to angiosarcoma of the liver among several hundred thousand VC-exposed workers on the basis of reports from 10 countries during 1955 and 1984. Reports from five countries cited a total of 87 respiratory cancer deaths among only 3024 CME-BCME-exposed workers. If a recent court settlement in the United States is taken into account, the number of respiratory cancer deaths due to CME-BCME rises to 117. On the basis of these numbers of cancer deaths, and the levels and durations of exposure, it is concluded that VC is a weak human carcinogen compared to CME-BCME.

A new ratiometric fluorescence probe as strong sensor of surface charge of lipid vesicles and micelles.

We report on the ability of a new fluorescent probe, 4-(2-pyren-1-yl-vinyl) pyridine, 1, to respond to micelles and phospholipid vesicles of different surface charge. The probe gets incorporated into micellar and membranous assemblies and shows a large red-shift in the fluorescence emission maxima especially when the surface charge of the organized media is anionic. The effect on the photo-excitation of the probe is very clear and pronounced as it can be easily visualized. The sample color upon photo-excitation changes from blue to orange/red once the probe experiences negatively charged vesicular or micellar surfaces. These results make the probe molecule useful as a reporter for sensing electrostatic environment in biological membranes and related organized assemblies.

Insulin-like growth factors I and II are unable to form and maintain their native disulfides under in vivo redox conditions.

Insulin-like growth factor (IGF) I does not quantitatively form its three native disulfide bonds in the presence of 10 mM reduced and 1 mM oxidized glutathione in vitro [Hober, S. et al. (1992) Biochemistry 31, 1749-1756]. In this paper, we show (i) that both IGF-I and IGF-II are unable to form and maintain their native disulfide bonds at redox conditions that are similar to the situation in the secretory vesicles in vivo and (ii) that the presence of protein disulfide isomerase does not overcome this problem. The results indicate that the previously described thermodynamic disulfide exchange folding problem of IGF-I in vitro is also present in vivo. Speculatively, we suggest that the thermodynamic disulfide exchange properties of IGF-I and II are biologically significant for inactivation of the unbound growth factors by disulfide exchange reactions to generate variants destined for rapid clearance.

Use of the novel combined contraceptive vaginal ring NuvaRing for ovulation inhibition.

OBJECTIVE: To assess the effects of the combined contraceptive vaginal ring NuvaRing on ovarian function. DESIGN: Randomized, open-label, crossover study. SETTING: Clinical pharmacology unit. PARTICIPANT(S): Sixteen healthy female volunteers. INTERVENTION(S): Group 1: one cycle of combined oral contraceptive containing desogestrel (150 microg) and ethinyl estradiol (30 microg) (desogestrel/EE COC), followed by a NuvaRing treatment period. Group 2: NuvaRing treatment period followed by a cycle of desogestrel/EE COC. MAIN OUTCOME MEASURE(S): Follicular diameter, serum hormone concentrations (follicle-stimulating hormone, 17beta estradiol, luteinizing hormone, and progesterone), and endometrial thickness. RESULT(S): NuvaRing use for the recommended period of 3 weeks resulted in complete inhibition of ovulation, as assessed by vaginal ultrasound (follicular diameter) and by serum luteinizing hormone and progesterone concentrations. Inhibition of ovulation was maintained for an additional 2 weeks of NuvaRing use. Ovarian suppression between the groups was comparable. Furthermore, ovarian suppression after 3 weeks of NuvaRing use was comparable to that on day 21 of DGS/EE COC intake. NuvaRing was well tolerated. CONCLUSION(S): NuvaRing completely inhibited ovulation throughout the normal 3-week period and the extended period of use. Ovarian suppression was comparable to that with desogestrel/EE COC.

Ovarian function during the use of a single contraceptive implant: Implanon compared with Norplant.

OBJECTIVE: To study the mechanism of action of Implanon, a single-rod contraceptive implant containing etonogestrel, in healthy women during 3 years. DESIGN: Prospective, randomized comparison with Norplant. The study was conducted for 2 years in Sweden but was extended to 3 years in Finland. SETTING: Two outpatient clinics. PATIENT(S): Thirty-two healthy women who were between 18 and 40 years of age with normal ovulatory cycles. Seven women receiving Implanon and three receiving Norplant participated in the third year. INTERVENTION(S): On or between days 1 and 5 of a spontaneous menstrual cycle, the subjects received either the etonogestrel-containing implant (Implanon) or the levonorgestrel-containing implant (Norplant). MAIN OUTCOME MEASURE(S): Ultrasonography was performed and/or progesterone concentrations were determined to confirm ovulation in a control cycle. Follicular development, endometrial thickness, and serum concentrations of 17beta-estradiol and progesterone were assessed twice per week during 4-week periods at regular intervals and after implant removal for 6 weeks to monitor return of ovulation. Times required to remove the respective implants were evaluated, as were possible complications. RESULT(S): Seven women who received Implanon and three who received Norplant completed 3 years of study. There were no pregnancies. Ovulation was observed for the first time with Norplant after 18 months. The first ovulation with Implanon was observed after 30 months. Mean endometrial thickness was <4 mm during treatment with Implanon from month 12 onward. The mean (+/-SD) time to remove Implanon was 5.9 +/- 3.4 minutes. The mean (+/-SD) time to remove Norplant was 17.9 +/- 9.9 minutes. Ovulation resumed promptly after the use of either implant. CONCLUSION(S): Results from this study provide convincing evidence of 3-year contraceptive efficacy with Implanon, mainly by ovulation inhibition.

An intelligent system for multivariate statistical process monitoring and diagnosis.

A knowledge-based system (KBS) was designed for automated system identification, process monitoring, and diagnosis of sensor faults. The real-time KBS consists of a supervisory system using G2 KBS development software linked with external statistical modules for system identification and sensor fault diagnosis. The various statistical techniques were prototyped in MATLAB, converted to ANSI C code, and linked with the G2 Standard Interface. The KBS automatically performs all operations of data collection, identification, monitoring, and sensor fault diagnosis with little or no input from the user. Navigation throughout the KBS is via menu buttons on each user-accessible screen. Selected process variables are displayed on charts showing the history of the variables over a period of time. Multivariate statistical tests and contribution plots are also shown graphically. The KBS was evaluated using simulation studies with a polymerization reactor through a nonlinear dynamic model. Both normal operation conditions as well as conditions of process disturbances were observed to evaluate the KBS performance. Specific user-defined disturbances were added to the simulation, and the KBS correctly diagnosed both process and sensor faults when present.

Agar derivatives for chromatography, electrophoresis and gel-bound enzymes. III. Rigid agarose gels cross-linked with divinyl sulphone (dvs).

Agarose cross-linked with divinyl sulphone (DVS) is a new matrix for chromatography and immobilized enzymes that has distinct advantages over common agarose gels. It has outstanding mechanical stability as compared with these gels, and the rigid gel beads form beds permitting very high flow-rates. In addition, DVS-agarose is superior to agarose gels with respect to chemical stability in acid and neutral media. In alkaline solutions above pH 8, there is a slow elimination of the sulphone-containing bridges, but without noticeable concomitant dissolution of the gels below pH 12 for moderately or highly cross-linked gels. The DVS-agarose is sufficiently thermostable to be heated in an autoclave.

Detection and quantitation of lactoferrin in bovine whey samples by reversed-phase high-performance liquid chromatography on polystyrene-divinylbenzene.

An existing RP-HPLC method for the measurement of the major bovine whey proteins in bovine whey samples and powders has been extended to include the analysis of the minor bovine whey protein lactoferrin. Lactoferrin could be detected and quantitated at levels down to 0.2 microg and linear calibration was observed between 0.2 and 30 microg. Reliable quantitation of lactoferrin in whey samples could be achieved provided the bovine serum albumin to lactoferrin ratio did not exceed 10:1. Quantitative data obtained by the RP-HPLC method compared favourably with data obtained by Mono S analytical chromatography.

Coated hydrophilic polystyrene-based packing materials.

A very hydrophilic high-performance liquid chromatographic base support was created from microparticulate, macroporous poly(styrene-divinylbenzene) beads. An organic monomer containing cross-linking functionalities was coated on the poly(styrene-divinylbenzene), followed by a catalyzed cross-linking reaction. The coatings formed contain only stable chemical bonds (e.g., C-C, C-O-C), and easily-derivatized hydroxyl moieties. This coated base support was evaluated for hydrophilicity, chemical stability, solvent compatibility, rigidity, and irreversible adsorption. Derivatives of the coated base support were made and applied in various modes of chromatography.

Effect of the content of divinylbenzene in ion-exchange resins on the chromatographic separation of alpha-cyclodextrin and glucose.

In the chromatographic separation of alpha-cyclodextrin and glucose, the parameters relating to the adsorption equilibria and rate processes of the solutes were evaluated for the sodium forms of cation-exchange resins with contents of divinylbenzene (DVB) of 2, 4, 6 and 8%. By using these parameters, the chromatograms of the solutes were then calculated. It is concluded that the resin with 6% DVB is most suitable for the chromatographic separation of the solutes on a preparative scale.

Gas chromatographic studies of vinyl chloride in air by catalytic hydrogenation to ethyl chloride.

In order to investigate the amount of vinyl chloride in air, the hydrogenation of vinyl chloride to ethyl chloride is used. This method of determination is preferable to the other methods described in the literature because: (a) it proves that only vinyl chloride goes through hydrogenation by its displacement to the ethyl chloride peak in the chromatogram; (b) the actual amount of vinyl chloride can be obtained in the case of superimposed or interfering peaks (e.g., various gases contained in air samples), either by calculating the ethyl chloride peak area or by the reduction of the peak area in the region of the same retention time as the vinyl chloride peak in the related chromatogram. In this study various kinds of catalysts for hydrogenation purposes at temperatures ranging from -20 to 250 degrees have been used. Among the catalysts used palladium gives the best results at low temperature.
