Fourteen children admitted to the Hospital de Pediatría of the C.M.N. of the I.M.S.S. with the diagnosis of acute infectious laryngotracheobronchitis were divided into two groups with similar conditions for their study. The benefit derived from the application of recemic epinephrine with intermittent positive pressure was confirmed 15 and 30 minutes later by means of a double blind study; the improvement was swift and spectacular, but temporary; therefore, such treatment must be given only in hospitals, since relapses may show up two to four hours later, making in dangerous to send the patients home or to apply to ambulatory cases. The use of racemic epinephrine is proposed for hospitals having equipment and personnel trained in its management and the convenience of further studies is set forth to determine the period of effectiveness of racemic epinephrine and the advantage of giving repeated doses.

In spite of regular vaccinations against equine influenza we are still observing a high rate of patients with respiratory problems in the equine practice. In our practice in Munich Riem, into which a horse clinic has been integrated, 4000 horses have been treated in 1985 alone. 874 of these showed some form of respiratory disease (= 22%). All of the 4000 horses have only been treated curatively, i.e. preventive forms of treatment such as vaccinations, deworming and examinations for insurance companies and prior to selling have not been taken into account. From 1983 until today in our clinic 256 horses with COPD had undergone clinical treatment. While out-patient horses had been treated for acute, subacute and chronic diseases of the respiratory system, in-patients were examined and treated for chronic respiratory disorders. Hospitalised horses had previously undergone treatment for varying periods of time. Apart from endemic or epidemic horse affections with signs of acute infections, the majority of patients treated belonged to a basically sound population. In the latter cases it is often just not possible to ascertain the individual causes of respiratory disease. Following documentation of the history of the present illness and after careful clinical examination including determination of breath frequency and type, lung auscultation and endoscopy (laryngoscopy, bronchoscopy), a symptomatic treatment of the horse is usually initiated. In cases of laryngitis or laryngotracheitis inhalations may be successful.(ABSTRACT TRUNCATED AT 250 WORDS)
A 10-year experience with 105 tracheal lesions is reported. Of these, 61 were inflammatory stenoses (27 post-tracheotomy, 26 post-intubation, 6 mixed and 2 post-irradiation), 30 neoplastic stenoses, 12 traumatic lesions and 2 degenerative lesions. The therapeutic approach should consider the type, site, extension and multifocality of the lesion. In inflammatory stenoses resection followed by end-to-end anastomosis was performed: resection with Montgomery T-tube in 16, and laryngeal release in 3 since the resected portion was extensive. In neoplastic forms it is necessary to utilize techniques (Montgomery or laser) which permit the respiratory lumen to remain open since surgery is not always possible. As for traumatic lesions in 4 cases resection followed by end-to-end anastomosis, in 5 a simple suture of a lesion and in 3 intubation, was performed. This experience showed that the treatment of choice for tracheal stenoses is resection with primary anastomosis. The Montgomery T-tube prosthesis and laser therapy were also effective.

The authors made an analysis of incidence of tracheobronchitis in patients with tracheostomy. It was established that tracheobronchitis followed 15 of 57 tracheostomies. The use of UV autologous blood irradiation in 17 patients with tracheostomy gave only one case of tracheobronchitis while in 40 operated patients without the procedure there were 14 cases of the complication. Thus, the authors believe that the administration of UV autologous blood irradiation in tracheostomy gives 6 times less incidence of tracheobronchitis in the patients, the method being of value in prevention of the complications.
