Data mining: data analysis on a grand scale?

Modern data mining has evolved largely as a result of efforts by computer scientists to address the needs of 'data owners' in extracting useful information from massive observational data sets. Because of this historical context, data mining to date has largely focused on computational and algorithmic issues rather than the more traditional statistical aspects of data analysis. This paper provides a brief review of the origins of data mining as well as discussing some of the primary themes in current research in data mining, including scalable algorithms for massive data sets, discovering novel patterns in data, and analysis of text, web, and related multimedia data sets.

Computerized health assessment and education for adolescent HIV and STD prevention in health care settings and schools.

This report summarizes a number of computer applications from a decade of experience with computerized adolescent health assessments and automated health education in a large health maintenance organization, military adolescent clinics, detention facilities, health fairs, and public schools. Feedback from over 5,000 adolescents is given for patient responses to and acceptance of computerized health assessments and preferences for automated health education modalities, including multimedia. Implications of the computerized approach to assessment of adolescent health risk behaviors and the provision of automated health education and HIV/STD prevention are considered in terms of efficiency, reliability, and access to health screening and educational services.

Information issues in telemedicine systems.

This paper examines the use of the Integrated Services Digital Network (ISDN) for telemedicine from the computer scientist's point of view. As increased bandwidth becomes available at reasonable cost, it becomes possible to employ graphical user interfaces in telemedicine systems. This means that human-computer interaction (HCI) techniques can be applied to the interface design. This paper describes some of these as applied to telemedicine by other authors. HCI techniques can be considered to be part of the development process of telemedicine systems. An integrated, multimedia-based telemedicine system which emulated the concept of a hospital information system would have many benefits, such as a consistent user interface with appropriate clinical metaphors, integrated patient records with facilities to store graphical information, and remote access. Telemedicine has much to offer in terms of providing higher standards of patient care through improved response times by clinicians who have the required information immediately available. The patient also benefits from convenience factors, such as being able to consult locally a remote specialist.

Teleworking in connection with technical aids for disabled persons.

An experimental teleworking arrangement was established between Milan and Rome in order to evaluate the use of multimedia technology and remote working for improving the service provided by an information and advice centre for the disabled. This activity requires a highly interdisciplinary approach and both clinical and technical expertise. The hypothesis was that offering the lone expert in Rome the possibility of teleconsulting with colleagues in Milan (where the whole range of expertise was available) would provide a comprehensive service for disabled clients asking for advice on technical aids in Rome. The results of an experiment demonstrated the feasibility of such teleworking and allowed a teleconsulting method to be defined that fits the specific needs of the technical-aid information service and improves quality.

WIRM: an open source toolkit for building biomedical web applications.

This article describes an innovative software toolkit that allows the creation of web applications that facilitate the acquisition, integration, and dissemination of multimedia biomedical data over the web, thereby reducing the cost of knowledge sharing. There is a lack of high-level web application development tools suitable for use by researchers, clinicians, and educators who are not skilled programmers. Our Web Interfacing Repository Manager (WIRM) is a software toolkit that reduces the complexity of building custom biomedical web applications. WIRM's visual modeling tools enable domain experts to describe the structure of their knowledge, from which WIRM automatically generates full-featured, customizable content management systems.

Parents as partners in obtaining the medication history.

OBJECTIVE: Patient-centered information management may overcome barriers that impede high-quality, safe care in the emergency department (ED). The utility of parents' report of medication data via a multimedia, touch screen interface, the asthma kiosk, was investigated. Our specific aims were (1) to estimate the validity of parents' electronically entered medication history for asthma and (2) to compare the parents' kiosk entries regarding medications to the documentation of ED physicians and nurses. METHODS: We enrolled a cohort of parents to use the asthma kiosk and tested the validity of this communication channel for medication data specific to pediatric asthma. Parents' data provided via the kiosk during the ED encounter and the documentation of ED nurses and physicians were compared with a telephone-based interview with the parent after discharge that reviewed all asthma-specific medications physically present in the home. Treating clinicians in the ED were blinded to the parents' kiosk entries. RESULTS: Sixty-six parents were enrolled and 49 of 66 (74.2%) completed the gold standard interview. When analyzed at the level of individual medications, the validity of parental report was 81% for medication name, 79% for route of delivery, 66% for the form of the medication, and 60% for dose. Parents' report improved on the validity of documentation by physicians across all medication details save for medication name. Parents' report was more valid than nursing documentation at triage for all medication details. CONCLUSION: Parents can provide an independent source of medication data that improves on current documentation for key variables that impact quality and safety in emergency asthma care.

The WEBD project: a research of new methodologies for a distant-learning 3D system prototype.

OBJECTIVES: To create and to spread a new interactive multimedia instrument, based upon virtual reality technologies, that allows both the running simulation of machines and equipment and the reproduction via Web of complex three-dimensional (3D) anatomical models such as the skull. METHODS: There were two main aspects of the project, one of design engineering and the other biomedical engineering, for the creation of "artificial" and anatomical objects. The former were made with 3D Studio Max R4 by Autodesk, San Rafael, CA, while the latter were created starting from real bones scanned with a CT system or a surface scanner and elaborated with different programs (3D Studio Max R4, Scenebuilder by Viewpoint, New York, NY and Spinfire by Actify, San Francisco, CA). The 3D models were to be integrated into web modules and had to respect file limits while preserving a sufficient definition. Two systems of evaluation were used, a questionnaire on a selected sample and an external evaluation by a different university. RESULTS: The Viewpoint format offers the best interactivity and size reduction (up to 96% from the original 3D model). The created modules included production of radiological images, rapid prototyping, and anatomy. The complete "3D Distant Learning Prototype" is available at www.webd.etsii.upm.es. CONCLUSIONS: The software currently available permits the construction of interactive modules. The verification on the selected sample and the evaluation by the University of Naples show that the structure is well organized and that the integration of the 3D models meets the requirements.

Mobile messaging: emergency image transfer.

This is the first reported integration of picture archiving and communications systems (PACS) with general packet radio services (GPRS)-compliant mobile telephones allowing digital image transmission using multimedia messaging service (MMS) technology for mobile telementoring (advice and teaching).

The image of dentistry in contemporary culture.

The multimedia portrayals of dentists and dentistry have expanded in scope. Prevention and esthetics have replaced drilling and extraction in the public perception of dental practice. According to the author, dentists themselves are no longer treated exclusively as buffoons or sadists. Instead, he writes, they are more apt to be seen as solid citizens, occasionally as romantic figures and even as complex, realistic human beings.

A low-cost, high-quality method for remote recording of presentations.

OBJECTIVE: The purpose of this article was to develop a low-cost method for high-quality remote capturing and recording of multimedia presentations. CONCLUSION: The method for making high-quality digital recordings of presentations by using the mix of freely available software presented in this article offers great potential for institutions on a budget as well as others seeking viable alternatives to current methods of presentation recording.

Using the Web to develop an EN conversion course.

This article describes the use of a web-based environment to develop a modularised enrolled nurse (EN) conversion programme. It details the development process and the reasons for choosing a web-based environment for the programme. In the age of multimedia developments, sharing the processes and the problems encountered will benefit those who access future programmes.

[Computer-aided occupational health education for post-graduates].

Nowadays, it's inevitable that occupational physicians enhance the activities in the field of occupational health through the use of the computer. Moreover, computer-aided self-education programs are one of the most useful tools for occupational physicians because they must learn a large volume of multidiscipline expertise for occupational health. Thus, we tried to apply a practical training program using the software of our own making in the post-graduate training course of fundamental occupational health. One is a self-education software for the study of electrocardiograms, and the other is a simple system to manage the serial data of a medical checkup and the reports of an inspection tour in the workplace. By this system one is also able to receive information such as a chest X-ray via the internet. Twenty-three students appraised our programs as for the contents (easily understandable, adequate volume); teacher's attitudes (enthusiastic approach to the training, enough time for questions, attention to the reaction of the class) and free comments. Because 4.3 points (at most 5.0) on an average was obtained, we are encouraged to make multimedia instructional materials on occupational health.

Telematics and smart cards in integrated health information system.

Telematics and information technology are the base on which it will be possible to build an integrated health information system to support population and improve their quality of life. This system should be based on record linkage of all data based on the interactions of the patients with the health structures, such as general practitioners, specialists, health institutes and hospitals, pharmacies, etc. The record linkage can provide the connection and integration of various records, thanks to the use of telematic technology (either urban or geographical local networks, such as the Internet) and electronic data cards. Particular emphasis should be placed on the introduction of smart cards, such as portable health cards, which will contain a standardized data set and will be sufficient to access different databases found in various health services. The inter-operability of the social-health records (including multimedia types) and the smart cards (which are one of the most important prerequisites for the homogenization and wide diffusion of these cards at an European level) should be strongly taken into consideration. In this framework a project is going to be developed aiming towards the integration of various data bases distributed territorially, from the reading of the software and the updating of the smart cards to the complete management of the patients' evaluation records, to the quality of the services offered and to the health planning. The applications developed will support epidemiological investigation software and data analysis. The inter-connection of all the databases of the various structures involved will take place through a coordination center, the most important system of which we will call "record linkage" or "integrated database". Smart cards will be distributed to a sample group of possible users and the necessary smart card management tools will be installed in all the structures involved. All the final users (the patients) in the whole network of services involved will be monitored for the duration of the project. The system users will also include general practitioners, social workers, physicians, health operators, pharmacists, laboratory workers and administrative personnel of the municipality and of the health structures concerned.

NIGHTINGALE MM system--the work in progress.

A multimedia computer based system for the self-learning of Nursing Informatics has been designed as a part of NIGHTINGALE project. A video thread has been designed as a central structure and "a real life anchor" for the abstract concepts related to the Electronic Patient Record. The hypermedia structure of the application has been maintained through the mapping of illustrated IT entries to specific points in the video sequences.

VR educational environments for learning in Europe.

Currently education is either class tutoring based or self-learning based using books, audio tapes, video tapes, CD-ROMs. None of the above media has been proven the best and the unique tool for education. An example presented is the creation of a virtual world in which children can learn by interacting with foreign children without leaving their environment. This approach is being implemented under a Joint Multimedia taskforce project. (VIRLAN).

Clinical applications of an ATM/Ethernet network in departments of neuroradiology and radiotherapy.

An integrated system for the multimedia management of images and clinical information has been developed at the Isituto Nazionale Neurologico C. Besta in Milan. The Institute physicians have the daily need of consulting images coming from various modalities. The high volume of archived material and the need of retrieving and displaying new and past images and clinical information has motivated the development of a Picture Archiving and Communication System (PACS) for the automatic management of images and clinical data, related not only to the Radiology Department, but also to the Radiotherapy Department for 3D virtual simulation, to remote teleconsulting, and in the following to all the wards, ambulatories and labs.

Virtual medical trainer. Patient assessment and trauma care simulator.

The Virtual Medical Trainer (VMET) combines multimedia sound and graphics with physiological engines, medical-procedures databases, and 3-D patients to produce an interactive environment that can mimic the cognitive pre-hospital assessment and care demands of a real emergency. VMET uses a reconfigurable component software and training framework that allows a uniform user interface, ease of increasing training complexity, and expansion of the software components. VMET provides an opportunity to experience a range of trauma scenarios prior to the challenge of an actual trauma situation.

Proposals and solutions for autonomy of the disabled and elderly.

The aim of this paper is to present DEFIE (Disabled and Elderly people Flexible Integrated Environment) a project of the CEC to develop a multimedia integrated system that allows the elderly and motor and/or sensory impaired to command and control domestic and working environments with a high degree of self-sufficiency and safety. The goal of this study has been to implement a precompetitive prototype of a modular system capable of satisfying the needs of a heterogeneous group of disabled and elderly users.
