Peripheral nerve injury associated with fracture or fracture-dislocation of the pelvis in dogs and cats: 34 cases (1978-1982).

Peripheral nerve injury was associated with fracture or fracture-dislocation of the pelvis in 23 dogs and 11 cats. In most cases, peripheral nerve injury resulted from ilial fracture with craniomedial displacement of bone fragments, or from sacroiliac fracture-dislocation with cranial displacement of the ilium. Sciatic nerve injury was associated with fracture of the acetabulum in 2 dogs. A combination of sensory, voluntary motor, and reflex abnormalities were observed in the ipsilateral extremity of affected animals. Signs of severe pain were noticed in 4 of the dogs and in 1 cat. Eight of the animals died or were euthanatized for reasons not directly related to the severity of peripheral nerve injury. Of the remaining 26 animals, 21 (81%) had good or excellent limb function 16 weeks after peripheral nerve injury was sustained. Limb function was lost or self-mutilation occurred in 4 (15%) of the animals.

[The bone marrow in Hodgkin's disease following radiotherapy and splenectomy].

A study was made of the bone marrow status in unirradiated zones of 33 patients with stage I-II Hodgkin's disease in complete 9-12 year remission after therapeutic irradiation of the lymphatic collectors of the upper part of the trunk in combination with irradiation of the spleen (16 patients) or splenectomy (17 patients). The total count of myelokaryocytes, myelogram, a relative and absolute content of lymphoid cells, immature granulocytes and elements of the erythroid series were defined in the punctates of the upper portion of the ilium. T- and B-lymphocyte count, the number of granulocytomacrophage (CFU-C) and stromal (CFU-F) precursor cells were defined using morphocytochemical and immunological methods. At that time an increase in the relative and absolute content of C- and B-lymphocytes was noted. The T-cell count and the total number of myelokaryocytes, on the one hand, and the content of immature granulocytes and erythronormoblasts, on the other hand, showed correlation of various degree which was particularly noticeable in the group of unoperated patients. The total number of myelokaryocytes in 1 microliter of the bone marrow of the patients after splenectomy, on an average, significantly exceeded that in the group of patients with the irradiated spleen. These changes were considered to be a result of the rearrangement of T-differentiating lymphocytes with their raised accumulation in the bone marrow after irradiation of a considerable volume of the lymphoid tissue and spleen or after splenectomy.

Surgical treatment of giant cell tumors of the pelvis.

There are many problems concerning management of giant cell tumors in the pelvis: diagnosis, surgical approach, adequacy of treatment, reconstruction, and prognosis. Of 58 cases of giant cell tumor of bone from 1962 to 1985, five cases arose in the pelvis, two in the ilium, two in the ischium, and one in the pubis. Surgical procedures included en bloc excision in four cases and intralesional excision in one. Reconstruction included sacroiliac fusion in one, iliofemoral fusion in one, and pelvic prosthetic replacement in another. Recurrence and metastasis did not occur in any of the cases. All patients were able to walk without crutches. A good surgical approach isolates major vessels and nerves, and permits excision to prevent recurrence and control bleeding. Reconstructive surgery may be required for treatment of a pelvic giant cell tumor.

Osteopenia of the pelvis associated with Mseleni joint disease. Radiological aspects.

Mseleni joint disease, an endemic form of osteoarthritis (OA), is associated with osteopenia (OP). In a detailed investigation of the disorder, the degree of OP was assessed in 593 radiographs of the pelvis which were taken during surveys of four populations: rural blacks from Mseleni, rural blacks from Manguzi (a similar population to Mseleni), a black control group of rural Tswanas and a white control group from Johannesburg. Only females were included because the number of males in the rural populations was too small. The radiographic trabecular pattern was assessed at four sites: sacrum, ilium, pubis and ischium. Four grades were used: O = normal bone, 1 = minimal OP, 2 = definite OP, 3 = severe OP. The sum of the grades at the four sites made up the OP score for the individual. The OP scores increased with age in all groups, the age-specific mean OP scores did not differ in the non-OA subgroups of all four populations, and subgroups with OA had significantly higher age-specific mean OP scores than those without OA.

A multivariate time-series descriptive case study of chiropractic care in the treatment of cervical pain.

This paper presents a multivariate quantitative description of the temporal course of 13 clinical variables measured before, during and after a multimodal chiropractic treatment package. Clinical measures included total and peak daily cervical pain, unleveling of the pelvis, shoulders and cranium; bilateral grip strength; and cervical ranges of motion. Intervention techniques included heel wedges; cervical, abdominal, and foot exercises; and spinal adjustments (cervical spine and ilium). Data are presented in tabular and graphic formats, and limitations in validity and reliability are reviewed. Interpretive limitations included confounding by multiple and sequential interventions and by lack of experimental control comparisons. However, the study suggests the plausible influence of biomechanical and adjustive care upon mediators and clinical outcomes. Strengths inherent in multivariate time-series research are noted.

Compression pain in a diver with intraosseous pneumatocysts.

A 30-yr-old diver experienced pain in the area of the sacroiliac joint during the descent phase of air diving to less than 10 ATA. Computed tomography of the pelvis demonstrated two gas-filled cysts within the ilium. The mechanism by which this lesion causes pain is discussed and reports of gas within bone are reviewed.

[Nature of stromal elements of the human bone marrow capable of releasing humoral stimulators of precursor cells of granulomonocytopoiesis].

Whole bone marrow fragments of trepanobiopsies of the upper flaring portion of the ilium were cultivated over 1 to 8 weeks in a liquid phase of the agar drop-liquid medium system. Fifty-two hematological patients and controls were examined in order to study the character of regeneration of different stromal cells and their role in the production of stimulants of precursor cells of granulomonocytopoiesis. It was shown that during cultivation of bone marrow fragments precursors of the stromal cells form a monolayer at the dish bottom, forming a capsule around the fragments and proliferating inside the medullary lacunae. Endosteum is the main source of the regeneration of the stromal elements. The studies did not reveal any relationship between the colony-stimulating activity of the trepanobiopsies and the area occupied in them by sinuses, fibrous and osseous tissue. A positive correlation was established between the CSA and the number of fatty cells. The magnitude of the area of the stromal cell monolayer did not influence colony formation. Based on the data obtained the authors suggest that fatty acids are the main producers of the CSA within the system under consideration.

Costal bone changes similar to hypertrophic osteopathy associated with pulmonary and abdominal mesothelioma in a dog.

An 8-year-old male Irish Setter was admitted because of nonweight bearing lameness of the left forelimb. Radiography failed to reveal any bony lesions of the forelimb; however, extensive periosteal new bone formation over most rib surfaces and an atypical cardiac silhouette suggestive of a mediastinal mass with pleural effusion were observed. New bone formation was seen on the ilium and a filling defect in the urethra, distal to the prostate, was apparent with contrast urethrocystography. Necropsy revealed scirrhous mesothelioma of the thorax and abdomen, multifocal periosteal exostoses, paraprostatic cyst, and benign prostatic cystic hyperplasia. Tumor cells were associated with the periosteal exostotic costal bone. The specific cause of the lameness was not determined.

[Some remarks concerning the tissue-air-ratio (TAR) in the case of bilateral total body irradiation (author's transl)].

The TAR's resulting from the bilateral 60Co-irradiation of an homogeneous whole body phantom and of a quadratic phantom are measured in their midlines and at their surfaces of incidence. They are compared with measurements on a patient. The dose contributions of scattered radiation and its effective energy are determined by means of a simple water-phantom. Investigations concerning the influence of the "os ilium" on the midline dose showed that besides the former mentioned other bone structures too in the pelvic area are responsible for the low rectal dose in patients. Rectal doses therefore should not considered representative for the midline dose. The variation of the midline dose depending on the source distance is discussed.

[Arterial vascularization of the os coxae].

Study of the arterial vascularization of the coxal bone by microangiographic methods. Nutritive and periosteal blood-vessels come from Iliaca interna and Iliaca externa arteries. The ilium is vascularized by the Ilio-lumbalis artery which is a nutritive blood vessel. A eriosteal vascularization also exists. Ischium and pubis have a periosteal vascularization. Blood vessels are oriented toward the sus-acetabulum area where many anastomoses exist.

[Isthmic spondylolisthesis with spontaneous development in adults. Apropos of 8 cases].

Eight cases of progressive deterioration in spondylolisthesis in adults have been observed. In 7 cases, the displacement was between the 4th and 5th lumbar vertebra and only once between the 5th lumbar vertebra and the sacrum. The mean age of the patients was 37 years. Several factors leading to this deterioration are considered - disc degeneration, antero-posterior imbalance of the spine and inadequate fixation of the 4th lumbar vertebra to the ilium.

[Hereditary osteo-onychodysplasia. Histomorphometric bone study. Apropos of a case].

The authors report on a patient with hereditary osteo-onychodysplasia (HOOD). Osteoporosis was demonstrated upon histomorphologic bone examination. HOOD is a rare inherited disorder. Typical features include dysplasia of the nails, patellae, elbows and ilium. Nephropathy occurs in 48% of patients. Numerous renal histologic studies have led to the concept that the nail-patella syndrome is an inherited metabolic disorder affecting the connective tissue. Our findings from histomorphologic bone assessment bring further evidence in support of this hypothesis.

[Results of surgically treated scaphoid pseudarthrosis using the Matti-Russe method].

For the treatment of scaphoid pseudarthrosis which is recognized sufficiently soon after an accident and where are no arthritic changes, the best method is that described by Matti-Russe; in our series we found healing in 95%. By grafting an autologous cortico-cancellous bone from the ilium we had no failures when we diagnosed and operated on the pseudarthrosis in the first five years after injury. We, therefore, now operate also on fractures with delayed union. The resection of the radial styloid process in combination with the Matti-Russe method has shown no advantages, and we have abandoned this modification.

[Method of preparing durable supravital skin grafts].

A method for in vitro perfusion of human full-thickness skin grafts is described for the first time. A skin flap from the inguinal region, supported by A. circumflexa ilium superficialis and A. epigastrica superficialis, is incubated at 37 degrees C in physiological plasma exchange solution under an atmosphere of 95% O2 and 5% CO2. The main blood vessels of the skin graft are cannulated and perfused (s. table 1). The pressure of the perfusion medium is 4.5 kPa - 5.5 kPa and the flow rate 90 ml/h - 120 ml/h. The epidermis of the flap is incubated with H-3-thymidine. Our technique provides maintainance of cell vitality over at least 48 hours. The thymidine incorporation is not impaired. Our system enables us to study the action of different substrates on surviving human skin under constant conditions.

Posterior intervertebral body fusion. A 25-year study.

The benefits derived from intervertebral body fusion using a posterior surgical approach were studied in 500 patients. The cases were reviewed during a 25-year period, with an average follow-up period of more than five years. Only eight patients did not return for adequate postoperative evaluation. Seventy-five percent of patients underwent primary excision of the disc with fusion, 11% had spondylolisthesis defects, and in 19% a second operation was performed after prior excision of the disc had failed. Discography provided the most reliable diagnostic procedure after physical examination. Disc degeneration and protrusion occurred 1 1/2 times more often in the fourth than in the fifth disc. Fusion occurred in 90% of patients. Eighty-two percent of patients rated the end result as excellent or good. Two blocks of autogenous bone from the ilium were used in all cases to produce the fusion. The surgical procedure and the complications that occurred are described.

Foot deformities secondary to gluteal injection in infancy.

We report six cases of foot deformities in children due to sciatic nerve dysfunction that appear to have been caused by gluteal intramuscular injection. Of four patients who developed cavovarus foot deformities, two also exhibited a component of ankle equinus. A fifth patient had a calcaneocavus foot deformity. In the sixth patient foot palsy resolved completely 1 year after injury, and no deformities developed. Identification of the muscle imbalance, as well as appropriate correction of the deformity before the implementation of muscle balancing procedures, led to the attainment of a satisfactory plantigrade gait in the first five patients with deformities. Bases on our experience with these six cases and reports in the literature, we recommend that the superolateral gluteal area between the crest of the ilium and the greater trochanter be properly defined as the preferred site for intramuscular injection.

Iliacus abscess.

Iliacus muscle abscess originating from the sacroiliac joint was diagnosed prior to operation in a 13-year-old boy with the assistance of technetium-99m bone imaging and computerized tomography. Preoperative identification of the iliacus abscess enabled the surgeon to select an anterior (apophyseal splitting) retroperitoneal approach, which permitted drainage of both the ilium and the iliacus muscle.

Computed tomography. Its use in space-occupying lesions of the musculoskeletal system.

The value of computed tomography in the diagnosis and management of lesions of the musculoskeletal system is being determined. Illustrative cases including a lipoma of the thigh, osteochondroma of the hip, simple cyst of the ilium, recurrent liposarcoma of the lumbar spine, desmoid tumor in the gluteal region, and postoperative interspace infection of the lumbar spine are presented to indicate the potential value of this procedure in the diagnosis and planning of treatment of musculoskeletal lesions.
