Orosomucoid and haptoglobin polymorphisms in two Spanish Pyrenean populations.

Serum samples of autochthonous individuals from the Garrotxa prepyrenean region, and the Pallars Sobirà valley were screened for HP (Haptoglobin) and ORM (Orosomucoid) types by polyacrylamide isoelectric focusing and electrophoresis. The allele frequencies estimated were HP*1 = 0.3902 and ORM1*S = 0.3697 for Pallars Sobirà, and HP*1 = 0.4161 and ORMS1*S = 0.3691 in the Garrotxa. While our results for the HP system are within the range of variation of the Iberian Peninsula, the ORM1 protein shows a remarkable degree of variation among these populations.

[Measurement of fecal hemoglobin-haptoglobin complex as a new diagnostic tool of lower gastrointestinal tract diseases].

An immunoradiometric assay (IRMA) of human hemoglobin-haptoglobin complex (Hb-Hp complex) was established for the purpose of clinical application as a new fecal occult blood test. This IRMA reacted with free human Hb and Hp as well as Hb-Hp complex and its measurable range was 3.125 through 800 ng/ml. Its cross-reaction with other animal Hb and serum was little enough to neglect. The immunoreactivity of Hb-Hp complex was more stable than that of free Hb in incubation with gastric juice and fecal extracts. Of 48 stools from the patients with colorectal cancer, 44 (91.7%) gave positive (above 10 ng/ml in fecal extract) test in Hb-Hp complex-IRMA, whereas 35 (72.9%) (p < 0.05) gave positive result (above 10 ng/ml in fecal extract) in Hb-IRMA. At proximal site of cancer, positive rate of Hb-Hp complex in feces was significantly high (p < 0.05) compared with Hb. Of 64 stools from the patients with colorectal adenomas, 56 (87.5%) gave positive test in Hb-Hp complex, whereas 46 (71.9%) (p < 0.05), 28 (43.8%) (p < 0.01) and 33 (51.5%) showed positive in Hp, Hb and Guaiac test, respectively. In every size and site of adenoma, fecal Hb-Hp complex showed significantly high (p < 0.01) positivity compared with Hb and Guaiac test. These results suggest that the measurement of Hb-Hp complex in fecal extracts may be a useful tool for the early detection of lower gastrointestinal diseases.

[Distribution of haptoglobin genotypes among persons subjected to chronic occupational irradiation in significant doses].

Haptoglobin (Hp) genotype distribution was studied on 508 individuals from 52 to 78 years of age exposed 35-40 years ago to occupational chronic irradiation at doses considerably exceeding the maximally allowable level. Essential and noncoinciding changes in Hp genotype distribution were found among individuals who were exposed to total doses of external chi-irradiation exceeding 500 cGy or with 239-Pu incorporation exceeding 7.4 kBq. Irradiation-induced changes differed from age-dependent shifts in genotype distribution. It was suggested that variation of mortality from malignancies among individuals with different Hp genotypes might be the cause of these changes.

[Haptoglobin phenotypes and biochemical parameters of blood serum].

Haptoglobin (Hp) phenotypes were determined in 646 subjects aged 25 to 76. Hp phenotype distribution in the examinees (13.6% for type 1-1; 49.4% for type 2-1; and 37.0% for type 2-2) was found as expected. Levels of eighteen metabolic, enzymatic, hormonal, and immunologic blood serum parameters were studied in subjects with various Hp phenotypes. Elevated levels of uric acid (by 12%, p < 0.01) and lactate dehydrogenase activity (by 7%, p < 0.05) in Hp 1-1 type were revealed as against other Hp types. A higher uric acid level in Hp 1-1 type was found in subjects of different age and both sexes, whereas LDH activity elevation was observed only in men aged over 55.

The distribution of haptoglobin types in various regions of Turkey.

The distribution of haptoglobin (Hp) types and the gene frequencies of Hp1 in various regions' of Turkey are discussed. The Hp1 gene frequency ranged between 20.4 percent for Turks living in the eastern part of Turkey and 32.6 percent in those who either are at present living in or have migrated from Western Thrace. Generally, the Hp1 gene frequency in the Turkish population is 26.5 percent, a figure similar to that recorded for Asians. Minor variations in the Hp1 gene frequencies in various parts of Turkey may be assumed to have been caused in certain regions by intermingling with the local populations.

Species-specific changes in regulatory elements of mouse haptoglobin genes.

Although expression of the haptoglobin (HP) as an acute phase reactant is evolutionarily conserved among mammals, there are differences among species with regard to the hormones required for stimulation. Using primary hepatocyte cultures, we show that in Mus caroli, as in rat, IL-1 and IL-6 are stimulatory, whereas in M. domesticus, as in humans, IL-1 response is diminished. In vivo, an acute inflammatory process increases hepatic HP expression in both mouse species up to 30-fold but minimally affects the low level HP expression in the lung. To define the species-specific differences in regulation, we isolated the hormone-responsive elements of the HP gene from the Mus species, M. domesticus, M. caroli, and M. saxicola. Functional studies in transfected hepatoma cells revealed an exceptionally strong dexamethasone response for all three murine HP gene elements. The IL-6 response was less prominent than in rat or human. A modest response to IL-1 was observed in M. caroli and M. saxicola. A mouse-specific insertion of a polypurine sequence led to a binding site for the PEA3 transcription factor in the HP gene promoter of M. domesticus and M. saxicola, but not M. caroli. The specific regulatory effects of glucocorticoid receptor, C/EBP beta, and Ets proteins were documented by co-transfection.

[Isoprotein composition of alpha 1-antitrypsin, haptoglobin, transferrin, and lipid peroxidation in the blood in inflammatory lung diseases in children].

Relationship was studied between the polymorphism of alpha 1-antitrypsin (Pi), haptoglobin (Hp) and transferrin and the development of chronic unspecific impairment of children lungs. Combination of protein isoforms PiM3M3 and Hp 1-1 was found to increase the risk of the lungs unspecific impairments in persons with lingering pneumonia and caused more severe development of the disease. Relatively low activity of the isoform PiM3M3, as compared with other forms of the PiMM system, and more intensive and nonadequate rate of lipid peroxidation in patients containing HP 1-1 appear to be responsible for aggravation of these impairments.

[Estradiol-binding properties of a sheep haptoglobin complex with hemoglobin. II. Preliminary data on quantitative characteristics of interaction].

The estradiol-binding properties of the sheep haptoglobin-hemoglobin complex (Hp-Hb) have been studied. The time and temperature dependencies of the steroid binding have been established the tightly bound 17 beta-estradiol, which is not detached from the complex during precipitation or extraction, has been shown to form a part of the total amount of the steroid capable of bending to Hp-Hb. Disturbances in the protein-protein interactions between Hp and Hb lead to the dissociation of estradiol as well as to the loss by the protein of its estradiol-binding activity. The values of relative competitive activity for several steroids suggest that some structural elements of the 17 beta-estradiol molecule play a role in estradiol-protein interactions. It is assumed that the Hp-Hb complex has two or more 17 beta-estradiol binding sites.

[Results of the treatment of patients with recurrence of pulmonary tuberculosis with different types of haptoglobin].

Phenotyping of haptoglobin Hp has been performed in 74 patients with recurrent tuberculosis. In patients with recurrences homozygous variant of phenotype Hp 2-2 occurs more frequently, while heterozygous variant Hp 2-1 less frequently. In carriers of phenotype Hp 2-2 recurrent tuberculosis is found more frequently, runs more seriously, heals in the presence of destruction less actively. These reasons underlie high incidence of chronic forms. Subjects with Hp 2-2 (55.3%) need special attention in conduction of recurrence prophylaxis. In carriers of phenotype Hp 2-1 pulmonary tuberculosis relapses less frequently, runs less seriously, resists to treatment rarely.

Polymorphisms of the haptoglobin peptide chains in Pyrenean populations.

This study has analyzed the haptoglobin genotype frequencies in over 900 samples from populations living on the Northern slopes of the Pyrenees. The results emphasize the importance of systematically determining the frequencies of the Hp1S, Hp1F, Hp2SS, Hp2FF, Hp2FS alleles. Hp1S was the predominant allele, the HpIS/Hp1 distribution varying between .5 and .69 as in most European populations. Hp2 alleles were observed in low frequency with differences in geographically distinct samples. Pyrenean populations in the western zones (Basques and Baronnies) were found to contain the Hp2FF allele while those in eastern regions, the Hp2SS allele. We have speculated that Hp2SS is as old as the Hp2FF, arising from southern Mediterranean areas, and suggest that Pyrenean groups have different origins.

Serum haptoglobin concentration in growing swine after intranasal challenge with Bordetella bronchiseptica and toxigenic Pasteurella multocida type D.

The acute phase reaction, in association with progressive atrophic rhinitis (AR), was monitored for 3 wk using serum haptoglobin (HPT) quantification in thirty-six, 15 kg swine after intranasal challenge with varying doses of Pasteurella multocida type D (toxigenic strain) and Bordetella bronchiseptica. The challenge doses were administered alone or in combination with pigs divided into 9 isolated treatment groups. Increasing doses of B. bronchiseptica were associated with lower serum HPT (P < 0.05), whereas increasing doses of P. multocida tended to increase serum HPT (0.05 < P < 0.10). Significant and positive correlation of mean HPT and AR score was found in these pigs; increased AR scores were associated with elevated mean HPT concentration (r = 0.41, P < 0.01). A significant interaction between P. multocida and B. bronchiseptica dose indicated that increasing the dose of B. bronchiseptica, for a fixed P. multocida dose, was associated with less AR (P < 0.05). The AR scores were greater in pigs given P. multocida, than B. bronchiseptica alone. These results indicate that a complex interaction between Pasteurella multocida and Bordetella bronchiseptica causes progressive atrophic rhinitis and alters serum HPT concentration in swine.

A comparison of serum haptoglobin levels between acute exacerbation and clinical remission in asthma.

BACKGROUND: Bronchial asthma is characterized by airway inflammation, which underlies the phenomenon of bronchial hyperresponsiveness. The concentration of serum haptoglobin (Hp), one of the acute phase reactant proteins, has been reported to correlate with bronchial hyperresponsiveness. The extent to which bronchoconstriction or airway inflammation contributes to airflow obstruction of exacerbation is presumed to determine the responsiveness to the initial bronchodilator therapy. OBJECTIVE: To see whether the Hp levels vary with the disease status of asthma, and also to test whether the Hp level at an acute exacerbation (AE) is correlated with the degree of response to initial bronchodilator therapy. METHODS: We measured serum Hp levels in 50 children with asthma at the times of an AE and a clinical remission(CR), and analysed the data according to the response to the initial bronchodilator therapy at AE. RESULTS: The serum concentration of Hp at AE (228.5 +/- 80.8 mg/dl, mean +/- SD) was significantly (P < 0.01) higher than that at CR (152.3 +/- 49.8 mg/dl) in the total study population. The difference of Hp levels between AE and CR was more marked (101.7 +/- 82.2 mg/dl) in the subjects (n = 19) who responded poorly (post-bronchodilator FEV1 < 75% predicted) to the initial bronchodilator therapy at AE than that (61.0 +/- 56.5 mg/dl) of those (n = 31) who responded well (post-bronchodilator FEV1 > or = 75% predicted). The Hp level at AE correlated with the degree of response to initial bronchodilator therapy (r = (-)0.36, P < 0.05), whereas it had no relationship with the severity of exacerbation (r = 0.04, P = 0.79). CONCLUSION: Our results showed that Hp levels may be increased at the time of exacerbation in a given asthma patient. The finding that the elevation of Hp level at AE is more marked in the cases with poor response to initial bronchodilator therapy at AE suggests that the increased Hp level at AE in asthma might reflect the degree of airway inflammation.

Postmortem diagnosis of diabetic metabolic derangement: elevated alpha 1-antitrypsin and haptoglobin glycosylation levels as an index of antemortem hyperglycemia.

Fatal diabetic metabolic derangement is difficult to diagnose postmortem because of the paucity of characteristic morphologic findings. Hyperglycemia is an indicator of diabetic derangement. Conventional biochemical parameters for postmortem diagnosis of antemortem hyperglycemic states are not sufficiently resistant to antemortem and postmortem non-diabetic influences or are suited only for long and medium-term assessment of diabetes control. In the search for other, more reliable, indices of immediately antemortem blood glucose levels, we investigated the value of glycosylation levels of serum proteins with very brief biologic half-lives: a) In vitro studies were performed on the glycosylation course of the short-lived serum proteins alpha 1-antitrypsin (alpha 1-AT) and haptoglobin (HP). b) Glycosylation levels were measured after purification of alpha 1-AT and HP from sera of living and deceased non-diabetics and diabetics. c) The resistance of alpha 1-AT and HP glycosylation levels to autolysis was investigated. Our studies revealed the following: 1) alpha 1-AT and HP glycosylate considerably more rapidly than either albumin or hemoglobin. This rapid glycosylation, combined with the rapid turnover of both proteins; facilitates detection of short-term changes in glycemia. 2) alpha 1-AT and HP glycosylation levels are autolysis-stable and can be assessed even after advanced hemolysis. 3) alpha 1-AT and HP glycosylation levels appear to allow reliable ante- and postmortem discrimination between normoglycemic and hyperglycemic metabolic states. As a tool in the postmortem diagnosis of antemortem hyperglycemic states, alpha 1-AT and HP glycosylation levels combine the advantages of a short-term parameter with resistance to non-diabetic influences.

[On the use of concentrated haptoglobin in the treatment of a haemolytic transfusion accident of the ABO-system (author's transl)].

The clinical course and the treatment of a case of severe haemolytic ABO-incompatibility are described. In addition to the routine therapy 3 times 2000 units of haptoglobinconcentrate were given since it is known according to the literature to metabolize free haemoglobin. The clinical result seems to confirm the beneficial effect. The course remained free of complications and the hospital stay was not prolonged. Further clinical experience is needed to confirm the effectiveness of the treatment with haptoglobin in cases with severe haemolysis.

[Selection criteria for the establishment of reference values in tropical zones. Application to specific proteins: C-reactive protein, haptoglobin, transferrin in gabonese children].

About 220 children (110 boys and 110 girls) aged 18 months to 10 years, 65.9% have been selected from anamnesis, clinical, and biological criteria to produce reference values on specific proteins dependent inflammatory, anemia and hemolysis (C-reactive protein = CRP; Transferrin = TRF and Haptoglobin: HPT). Specimens have been analysed by Nephelometric immuno-chemical method. For the two groups, A1 [18 months-5 years] and A2 [5-10 years], the reference values of the whole study population are reported: CRP (A1 3.35 +/- 3 mg/l: A2 2.40 +/- 2.30 g/l, with a significant difference at Student Fisher "t" test p < 0.03); TRF (A1 4.05 +/- 1.5 g/l; A2 4.50 +/- 1.4 g/l; NS, p > 0.05) HPT (A1 2.55 +/- 2.0 g/l: A2 1.20 +/- 1.10 g/l; S(r) p < 10(-5)). Furthermore, for TRF, HPT we must consider the sex in the results meaning because of significant difference into boys and girls.

[The effects of administration of haptoglobin for hemolysis by extracorporeal circulation].

It is well known that hemolysis by extracorporeal circulation is major cause of renal failure after open heart surgery. The purpose of this study is to investigate the effects of administration of haptoglobin (Hp) during extracorporeal circulation. The patients were divided into two groups: Group I 10 patients underwent open heart surgery for extracorporeal circulation; Group II 10 patients underwent open heart surgery for extracorporeal circulation with the administration of 4,000 IU of Hp. The serum level of total Hp was elevated in group II during bypass, and reduced at 1st day in both groups more than pre-operative level. The serum level of total Hb was elevated in both groups during bypass, and reduced at 1st day within normal limits. Free serum Hb was found in group I at 30 min after start bypass and increased during bypass and urinary Hb was also found. However, in group II free Hb was not found during and after bypass. Urinary NAG and alpha 1 Mg levels of group I were significantly higher than those of group II. The administration of Hp during extracorporeal circulation prevents the increment of serum free Hb and is effective for protection of renal function.
