[Preoperative embolization for extensive facial hemangiomas].

An extensive hemangioma of the lower face is discussed as an example illustrating the use of therapeutic--particularly preoperative--arterial embolization in the treatment of extensive facial hemangiomas. In this procedure a thrombusinducing material is inserted into the lumen of a blood vessel to occlude this vessel, and thus to reduce blood flow to the lesion. This promising technique opens up new possibilities in the treatment of extensive hemangiomas of the maxillofacial region. It may be used either as a definite therapy or as pretreatment procedure enabling a safer and simpler surgical approach.

[Microsurgical treatment of trigeminal neuralgia].

129 cases of trigeminal neuralgia were treated microsurgically. The roots of trigeminal nerves were found to be affected by arteries in 96 cases, by veins in 23, and by both arteries and veins in 10. According to these relations, the disease was divided into 3 types: (1) contiguous type (49 cases, 37.9%), vessels 1 mm close to the nerves without any compression; (2) compressive type (42, 32.5%), vessels close to the nerves with compression; (3) adhesive type (38, 29.5%), vessels and nerves adhered to each other. A piece of dacron was used to wrap the nerve root and fixed by gelfoam and silver clip. Pain relieved immediately after operation in 123 cases with an effective rate of 98.4%.

Prevention of gastric complications in hepatic arterial chemoembolization. Balloon catheter occlusion technique.

Gastric complications may be a problem in transcatheter arterial chemoembolization, if the right gastric artery branches distally from the proper hepatic artery or its branch, or the accessory left gastric artery arises from the left hepatic artery. We occluded the proper hepatic artery with a balloon catheter to redirect blood flow in these gastric branches toward the liver. This procedure prevented chemoembolic agents from flowing into the gastric branches, and accordingly protected against gastric complications even when chemoembolization was performed by injecting the agents proximally to the gastric branches.

[Arterial embolization of an arteriovenous fistula with hemobilia after blind liver puncture].

Five days after percutaneous liver biopsy we observed in a 42-year-old man with alcoholic liver cirrhosis severe hemobilia requiring transfusions of packed red cells. By means of super-selective arterial embolization, using gelfoam, the bleeding source, an av-fistula, was successfully occluded. Iatrogenic hemobilia, although seen after percutaneous liver biopsy only in app. 0.005% of the cases, is today the most important cause of biliary bleeding, mainly as a complication (app. 3% of the cases) of the widespread use of interventional procedures of the biliary tree (e.g. PTCD). Therapeutically arterial embolization should be considered first if possible.

[Chemoembolization therapy with lipiodol, cisplatin and etoposide for hepatocellular carcinoma].

Chemoembolization using CDDP, VP-16 and lipiodol was carried out for 7 patients with hepatocellular carcinoma (HCC). CDDP/lipiodol, CDDP/VP-16, CDDP/lipiodol (lipiodol 2-10 ml, CDDP 1-2 mg/kg, VP-16 100 mg/body) and gelatine sponge were administered in that order through the catheter located in the proper, or right or left hepatic artery. Three patients underwent hepatic resection 38-50 days after this treatment. Complete necrosis of the tumor was recognized in the one case, although the portion of necrosis did not exceed 70% in large sized HCC as the diameter of more than 10 cm. In 4 unresectable cases the decreases in tumor size were observed by ultrasonography and computed tomography. The response was: 3 partial responses and 1 no change. One out of 4 cases could undergo hepatic resection 17 months after this treatment. Two patients are alive 20 months after this treatment, although one patient died of HCC after 25 months. Serious side effect was not observed.

Life-threatening post-nephrolithotomy hemorrhage resulting in cessation of kidney function, treated by transcatheter arterial embolization with subsequent restoration of excretory function.

A massive post-nephrolithotomy renal hemorrhage was successfully controlled by selective arterial embolization using Gelfoam particles. Prior to embolization, the kidney showed no excretory function due to complete obstruction of its collecting system by blood clots. Long-term observation revealed restoration of excretory function of the treated kidney and only minimal loss of its parenchyma.

[Embolization by femoral catheterization of tumors supplied by the external carotid artery. 40 cases].

Experiences from therapeutic embolization by way of superselective catheterization of the external carotid artery, in 40 cases of cranio-cerebral, maxillo-facial and cervical tumours are reported. Using Spongel, embolization offered definite advantages compared to simple arterial ligation: it is easily performed as a supplementary procedure to diagnostic superselective angiography; a meticulous technique will provide superselective arterial occlusion with obstruction of vessels down to the precapillary size (diameter 150 to 200 mu). Embolization is advocated as a preoperative measure in meningeomas (particularly at the skull base), naso-pharyngeal angiofibromas, intracranial and cervical chemodectomas, in which cases profound bleeding during surgery may be expected and as a palliative measure in cases where surgery and radiation therapy fail to control recurrence of a tumour.

[Patient at hemorrhagic risk. Prevention and local control].

The Authors make a short analysis of the main alterations of the coagulative system and of the anticoagulant medicines, used in patients with normal hemostasis, but suffering from cardiovascular disease. It is underlined the importance of a close collaboration between the odontologist and the hematologist or the general practitioner, to be able to make a correct pre-operating prophylaxis, if necessary. All the local procedures that the odontologist must prepare to prevent or at least stop the possible hemorrhagic troubles are finally analysed.

[The movement of antineoplastic solutions in transcatheter hepatic arterial embolization].

Transcatheter hepatic arterial embolization (TAE) with gelatin sponge particles soaked in anticancer agents has been widely employed in the treatment of Hepatocellular carcinoma (HCC). The mechanism of TAE has been explained by blocking of blood flow to the tumor, however the role of anticancer agents used with embolic materials has remained unclear. The purpose of this study is to prove the role of anticancer agents used in TAE. 1) In eighteen cases of HCC and 4 cases of metastatic liver cancer, TAE was performed with gelatin sponge, anticancer agents and contrast media. The livers were examined by CT 4, 24 and 48 hours after TAE. A selective retention of contrast media containing anticancer agent in the tumor area was observed in 14 of 14 cases examined 4 hr. in 15 of 17 cases examined 24 hr and in 6 of 6 cases examined 48 hr after TAE. In the cases of metastatic liver cancer, retention of contrast media, was also observed in 4 of 4 cases examined at 24 hr and 3 of 3 cases examined 48 hr after TAE. 2) TAE was done in 12 cases of HCC by using gelatin sponge soaked in anticancer agent mixed with 99M-Tc-pertechnetate. Livers were observed by a scintillation camera 4 hr and 24 hr after TAE. A selective retention of 99M-Tc-pertechnetate in the tumor was observed in 12 of 12 cases examined 4 hr and in 3 of 3 cases examined 24 hr after TAE. 3) In one cases of HCC, hepatectomy was performed 2 days after TAE.(ABSTRACT TRUNCATED AT 250 WORDS)

[Behavior of anti-cancer drug of transcatheter hepatic segmental arterial chemoembolization using lipiodol mixed with an anti-cancer drug followed by gelatin sponge assessed by Tc-99m pertechnetate].

The behavior of an anti-cancer drug consisting of transcatheter hepatic segmental arterial chemoembolization using Lipiodol mixed with an anti-cancer drug followed by gelatin sponge (segmental Lp-TAE: Seg-Lp-TAE) was assessed in comparison to that of the non-segmental Lp-TAE to whole liver (Lp-TAE) by means of Tc-99m pertechnetate (RI). Injection of Lipiodol mixed with Adriamycin and 740 mBq of RI followed by gelatin sponge was carried out from segmental or subsegmental hepatic artery in 12 cases of hepatocellular carcinoma and from proper or right hepatic artery in 11 cases (9 with hepatocellular carcinoma and 2 with metastatic liver tumor). The distribution and retention of RI were studied from the time activity curves of ROIs in tumor and non-tumor area, and scintigrams at 4 and 24 hours after injection. The integral value ratio of tumor to non-tumor was significantly higher in Seg-Lp-TAE, indicating that es the distribution of RI after Seg-Lp-TAE is selective in the segments where tumor exists. In Seg-Lp-TAE, the T1/2 was higher and the count ratio of tumor to non-tumor was significantly higher at 4 hours and even higher at 24 hours after injection. These results suggest that an anti-cancer drug reinforces the effect on tumor with little adverse effect on normal liver in Seg-Lp-TAE, which is more advantageous to the treatment of hepatocellular carcinoma.

[Chemoembolization combined with hepatic arterial induction of endogenous TNF and anticancer agents for hepatocellular carcinoma--a case report].

Antitumor effect of TNF has been demonstrated to be increased with some kinds of anticancer agents. We reported antitumor effect of hepatic endogenous TNF induced with gamma-IFN and OK-432 for hepatocellular carcinoma (HCC). To increase antitumor effect of transcatheter arterial embolization (TAE), hepatic arterial chemoembolization was performed with a mixture of gamma-IFN, OK-432 and gelatin sponge following a mixture of Doxorubicin and iodized oil (LPO) on the first time. Serum alpha-fetoprotein decreased from 18,903 ng/ml to 470 ng/ml but elevated three months after these procedures. Following the above procedure, hepatic arterial embolization with a mixture of gelatin sponge and Actinomycin D as an inhibitor of RNA was given the second time. Serum alpha-fetoprotein decreased under 5 ng/ml and computed tomography revealed decreased tumor size and low density area following this second procedure. Hepatic arterial chemoembolization with a mixture of hepatic induction of endogenous TNF and anticancer agents may well be beneficial for survival of patient with HCC.

[Evaluation of preoperative chemoembolization using ethiodized oil, cisplatin and gelatin sponge (sandwich therapy) for hepatocellular carcinoma].

The significance of preoperative chemoembolization using ethiodized oil, cisplatin and gelatin sponge (Sandwich therapy) for resectable hepatocellular carcinoma (HCC) was evaluated. One hundred and thirteen patients with solitary and less than 10 cm sized HCC who underwent radical hepatic resection were chosen for this study. Fifty-three patients received Sandwich therapy before surgery (Group A), and the remaining 60 patients under-went surgery without any preoperative treatments (Group B). Any background factors between two groups were not significantly different. The anticancer effects of this therapy were evaluated by histologic examination in 31 patients who had preoperative Sandwich therapy. In 22 of 31 patients (71%), the main nodules were completely necrotic. The ratios of patients with complete necrosis in daughter nodules were 7/12 (58%), in portal vein tumor emboli, 7/10 (70%), in intracapsular invasions, 11/21 (52%), in extracapsular invasions, 4/11 (36%). The 4-year disease-free survival rates in Group A and Group B were 56% and 27% respectively, and the rate of the former was significantly higher than that of the latter (p less than 0.05). The 4-year survival rates in Group A and Group B were 83% and 53% respectively. The rate of Group A was also significantly higher than that of Group B (p less than 0.01). We concluded that preoperative Sandwich therapy was very significant to obtain successful long-term disease-free survival and survival in regard to relatively early stage HCC.
