[The morphological changes in the skin of the prepuce in patients with xerotic balanoposthitis].

The authors analyze the findings of histologic and electron-microscopic examinations of skin biopsy specimens from 15 patients with xerotic balanoposthitis. The results evidence that xerotic balanoposthitis morphologically is characterized by atrophy of the epidermis, vacuolar dystrophy of the basal cells, changed structure of keratohyalin granules, presence of a vast edematous area under the epidermis, absence of elastic fibers, inflammatory cellular infiltrate under the edematous area. During initial manifestations of the disease and its progressive stage more manifest changes are detectable in the derma. Atrophy of the epidermis was not observed, a lesser edematous area was seen under the epidermis, and, in contrast to the typical pattern of sclerotic and atrophic lichen, a dense, more superficially located cellular infiltrate in the derma. Small-granular substance is detectable in the edematous area under the epidermis, as are numbers of microfibrils and this diffusely located collagen fibrils that indicate synthesis of fibrillar substances.

[Experience with combined maxillary and mandibular osteotomies].

Possibilities are discussed to achieve useful results for both alveolar ridges by combining standard procedures of orthopedic surgery used for single jaw operations. The 8 patients were not routine cases. The indications were based on several different reasons. 3 patients first got orthodontic treatment followed by surgery. In the other five cases orthodontics had been impossible for various reasons. All cases were treated with single stage procedures and did not relapse.

[Pathogenetic mechanisms of the development of "hot flushes" in patients with climacteric disorders of the sympathetic-adrenal type].

Examinations of 19 patients aged 49 to 55, suffering from climacteric disorders of sympathoadrenal nature, have revealed during 42 hot flushes an activation of the sympathoadrenal section of the autonomic nervous system (increased arterial pressure, heart and respiration rates, and skin galvanic reflex), as well as activation of peripheral vasodilatation, elevated skin temperature, and increased concentrations of LH, ACTH, thyrotropic hormone (TTH), cortisol, aldosterone, and testosterone. These data suggest that the trigger mechanism of a hot flush in the patients with the climacteric syndrome represents modified (due to changes in the external or internal conditions) activity of the LH releasing factor-secreting neuron structures and of the central temperature-sensitive neurons. Activation of the sympathoadrenal section of the autonomic nervous system and of the hypothalamo-hypophyseo-adrenocortical system is explained by their important role in the adaptive reactions of the body. Increase of the TTH level results from increased activity of the thyrotropin releasing factor, contributing to the central mechanisms of thermoregulation.

[Pathogenetic mechanisms of the development of the cephalgic form of premenstrual syndrome in patients of older age groups].

Measurements of the blood plasma gonadotropic hormones, sex steroids, and biogenic amines, and investigation of the patterns of vegetative reactions to functional tests during both menstrual cycle phases, carried out in 26 patients with the cephalgic form of the premenstrual syndrome, aged 39-49, and in 13 age-matched women without this syndrome, have shown the contribution of changes in progesterone, serotonin, and histamine levels to the pathogenesis of this condition. These studies have also demonstrated a reduction of the adaptive potentials of the body, i. e. no regular rise of the tone and functional activity of the autonomic nervous system sympathoadrenal section in the lutein phase of the cycle, as well as the development of inadequate reactions in some patients aged 44-49, in whom the disease runs the most severe and long course.

[Clinico-epidemiologic observations on the relation of the prevalence of risk factors and the incidence of essential arterial hypertension at a school collective. I].

In 466 pupils the blood pressure values and the risk factors for essential hypertension were studied. Numerous environmental risk factors were detected, which, depending on each individuals type of reactivity, can be decisive in inducing increase of blood pressure values and the occurrence of arterial hypertension disease at very young ages.

[Fungal involvement of the penis in patients with penile condylomata acuminata].

In a study on 129 patients with penile condylomata acuminata we recognised an incidence of 32% penile yeast affection. Candida was found in 54%, Torulopsis in 30% and Rhodotorula in 17% of all cases. The high rate of yeasts not belonging to the candida species is remarkable. There was no correlation between our test results regarding the cell-mediated immunity against candida albicans and the local yeast infection.

[Penile inflammation in ganders].

Since 1987 penis-inflammation and -prolapse were observed in north-german breeder-geese. Up to 28% of the ganders showed local symptoms. In the females no clinical signs of cloaca-inflammation were seen. The general condition of the birds was good and there was no increased mortality. Egg production and fertility were not influenced. Bacteriological examinations of the altered penis-tissues revealed in different frequency microorganisms belonging to the genera Mycoplasma, Candida, Bacteroides, Clostridium, Streptococcus, Micrococcus, Staphylococcus, Corynebacterium, Escherichia, Proteus and Pseudomonas. Within 70 days only one third of the diseased ganders recovered completely, the others still showed penis-necrosis or -deformation. It is recommended to examine all ganders prior to each sexual season and to eliminate affected birds.

[Diagnosis and therapy of ulcus molle today. Case report and review of the literature].

Chancroid, an ulcerous disease of the genitalia caused by Haemophilus ducreyi, occurs rarely but regularly in Germany. Exact diagnosis is based on the clinical features and a direct smear, and in particular on cultivation of the organism, showing its unique macromorphological characteristics. The sensitivity of cultivation has increased due to the development of selective media for primary isolation. Resistance problems during the last decade meant that a change to new therapeutic strategies was unavoidable. The work presented here includes a case report and a review of the recent literature, it illustrates modern methods of diagnosis and treatment of Haemophilus ducreyi infections 100 years after the first description of the organism.

De-Nol in the treatment of reflux esophagitis.

Twenty-eight patients with histologically proven reflux esophagitis were randomly allocated to receive either De-Nol (tripotassium dicitrate bismuthate) or placebo for three weeks. Significant decreases in symptoms frequency and severity were noted in De-Nol group comparing with placebo group (p less than 0.01). Objective improvement in esophageal endoscopic appearance was obtained in 78.5% of the patients treated with De-Nol and only in 14.2% of those receiving placebo. De-Nol appears to be an effective drug in the treatment of reflux esophagitis, but this should be confirmed by further studies.

[Streptococcal perianal disease and balanoposthitis].

Two cases are reported in which Lancefield group beta-haemolytic streptococci were isolated from culture- from the perineal skin. The physical examination showed perianal irritation and excoriation in both cases and seropurulent anal and balano-preputial discharge in the first only. To the best of our knowledge, this is the first case in which the streptococcal perianal disease is associated to a balanoposthitis and at the same time increments the clinical spectrum of the illness.

[Morphologic variants of genital ulcer caused by Haemophilus ducreyi].

The typical clinical picture, common variants and frequent complications of chancroid are presented in cases proven by culture. In general, the conventional view of the clinical spectrum of chancroid is substantiated. Certain types such as "chancre mou volant," however, could not be found in the present material. Their existence or association with Haemophilus ducreyi remains speculative.

[Is there a thrombophlebitis or subcutaneous lymphangitis of the penis?].

Twelve patients suffering from subcutaneous dorsal induration of the penis were investigated. The diagnosis of a lymphangitis or thrombophlebitis depends on clinical features. It is of nonvenereal origin, and the treatment, if necessary, consists in local or systemic antiinflammatory or antibiotic therapy.

[Bacteria detected in the duodenal aspirate of patients with biliary tract disorders].

In 614 patients with pains in the right hypochondrium the cyto-bacterioscopy and bile culture were performed. In 575 patients bile samples were obtained by sterile catheterization and in 39 patients by biliary tract puncture during surgical intervention. It resulted that for a reliable cyto-bacterioscopic examination and its agreement with bile culture results it is necessary to investigate only the samples with pH greater than or equal to 5 and during the first two hours after their collection. The isolated bacterial species can be considered to be etiologic agents of the biliary infection depending on the following parameters: significant quantitative threshold of minimum 10(5) colony forming units per milliliter, presence of inflammatory reaction and activity of the gastric acid barrier.
