Stressors of living with HIV/AIDS: patients' perspectives.

With the prospect of increasing numbers of patients with human immunodeficiency virus (HIV) and acquired immunodeficiency syndrome (AIDS), healthcare professionals (professionals) need to be better informed about patients' perspectives in order to provide efficacious, compassionate, and efficient care to patients. In Taiwan, the biomedical research and healthcare professionals' perspectives have dominated research related to HIV/AIDS. The purpose of the study was to investigate the stressors experienced by patients which have, thus far, been under-explored. This study used a phenomenological approach to gain insight into the patients' experience. In-depth, open-ended interviews were used to collect data. Interviews were tape recorded and transcribed verbatim to maintain data integrity and to reduce perceptual bias. The open coding procedure was used to analyze the interview transcripts. Study participants were 14 male patients with an average age of 35.71 year-old, who had known their positive HIV status for an average of 24.9 months. Patients experienced multiple stressors because of having the life threatening illness of HIV/AIDS. Their stressors were identified as follows: being confronted with the diagnosis, fear of disclosure, unpredictability of their physical condition, hypervigilance toward physical symptoms, emotional upheaval, and constrained life planning. The findings of this study form the basis for professionals to design services particularly addressed to patients' needs, for example, informed testing, and pre- and post-counseling. Further efforts could focus on developing systemic intervention programs to help patients better cope with the challenges of living with HIV/AIDS.

Privacy and security compliance in the E-healthcare marketplace.

Complying with security and privacy regulations proposed by HHS in response to the Health Insurance Portability and Accountability Act (HIPAA) will require healthcare managers to address both internal and external business interactions and initiatives. The proposed regulations mandate certain procedures regarding administration, physical safeguards, technical security for data integrity and confidentiality, and technical security against unauthorized access. In particular, the proposed regulations require organizations to contractually ensure that vendors adhere to the regulations. Healthcare organizations also must implement training procedures for staff members who have contact with protected health information and designate a privacy officer to guard against improper disclosure of such information. Documented policies for organizational decision making are vital to an organization's efforts to implement procedures for compliance with the regulations.

A genome-wide map showing common regions of loss of heterozygosity/allelic imbalance in breast cancer.

We report here a new framework map that integrates data from 143 studies on the loss of heterozygosity/allelic imbalance in breast cancer. Full details of these loss of heterozygosity maps can be found at the web site (http://www.nottingham.ac.uk/pdzmgh/loh/) that accompanies this report. By combining results from these data, we have also been able to highlight and identify minimum commonly deleted regions on each chromosome. In addition to finding commonly deleted regions at both the BRCA1 and BRCA2 loci, which confirmed the power of the technique, 24 other regions were identified on 16 different chromosomes.

Using Kleisli to Bring Out Features in BLASTP Results.

BLASTP gives a good overall indication of what function a protein might have. However, analysis of BLASTP reports to discover various domain features in the protein is still tedious. We address this problem by using the modern data integration system, Kleisli, to bring out annotated features of BLASTP results. We further strengthen our solution by incorporating additional information from SEG, ClustalW, hmmPfam, etc. It is also noteworthy that the codes of our implementation is sufficiently short to be presented in its entirety.

Development and implementation of a decision support system for carotid artery stenosis: the Carotid Ultrasound Report Enhancement (CURE).

The management of carotid artery stenosis is an art evolving into a science, increasingly informed by clinical trials of medical management versus carotid endarterectomy (CEA). Ideas about optimal management depend on the confluence of patient-specific variables, surgical expertise, and the state of medical knowledge. In this complex and progressing setting, an up-to-date decision support system could help physicians apply the latest evidence to patient care. Carotid ultrasonography (US) studies provide an excellent opportunity to aid in the therapy of carotid stenosis. We developed a Carotid US Report Enhancement (CURE) to augment carotid US reports with treatment-specific prognostic information and patient-specific portions of the American Heart Association's 1998 guideline for the management of carotid artery stenosis. In the process of designing and implementing the CURE software, we encountered and eventually solved a variety of problems. The first problem was that US test was not always precise enough to distinguish between a moderate and mild carotid stenosis. Likewise, the standard US reports did not elucidate several technical problems that decreased the reliability of the US result. Third, although 17 of 18 physicians agreed to receive the CURE reports, they requested non-incriminating wording. Fourth, vascular surgeons supervising the US laboratories were reluctant to support the CURE report if they thought it would be construed as prompting self-referral. Finally, information about some comorbid conditions (e.g. a history of atrial fibrillation) could not be obtained reliably from the patients. The result of responding to these problems is a decision support program that is increasingly robust, able to detect many of its own limitations, and capable of integrating data from multiple sources. A randomized controlled trial now in progress will evaluate the clinical impact of the CURE program.

Preparing the wound bed--debridement, bacterial balance, and moisture balance.

Successful diagnosis and treatment of patients with chronic wounds involve holistic care and a team approach. The integration of the work of an interdisciplinary care team that includes doctors, nurses, and allied health professionals with the patient, family, significant others, and caregivers offers an optimal formula for achieving wound resolution. Such an approach challenges practitioners and everyone participating in wound care to integrate data and information that arise from a number of sources and mitigating factors. In this article, the authors define the changing paradigm that links treatment of the cause and focuses on three components of local wound care: debridement, wound-friendly moist interactive dressings, and bacterial balance. The authors demonstrate that the treatment of chronic wounds can be accomplished through a series of recommendations and rationales based on the literature and their experience. These recommendations lay the groundwork for thorough assessment and evaluation of the wound.

Advances in high-throughput mass spectrometry.

The evolution of high-throughput drug discovery is readily apparent as the pharmaceutical industry continues to stress the rapid progression of new chemical entities and biological agents through drug discovery and development pipelines. Mass spectrometry and high performance liquid chromatography-mass spectrometry have played an instrumental role in the support and advancement of all facets of high-throughput drug discovery. The introduction of new instrumentation has extended the breadth of mass spectrometric-based capabilities from the characterization of high-throughput organic synthesis products to early adsorption, distribution, metabolism and excretion profiling. Additionally, advances in the capacity and throughput of mass spectrometry systems have concurrently led to the introduction of data management tools to address automated data reduction, archival and mining, as well as analytical data integration to chemical and biological databases.

A database system for the analysis of biochemical pathways.

To provide support for the analysis of biochemical pathways a database system based on a model that represents the characteristics of the domain is needed. This domain has proven to be difficult to model by using conventional data modelling techniques. We are building an ontology for biochemical pathways, which acts as the basis for the generation of a database on the same domain, allowing the definition of complex queries and complex data representation. The ontology is used as a modelling and analysis tool which allows the expression of complex semantics based on a first-order logic representation language. The induction capabilities of the system can help the scientist in formulating and testing research hypotheses that are difficult to express with the standard relational database mechanisms. An ontology representing the shared formalisation of the knowledge in a scientific domain can also be used as data integration tool clarifying the mapping of concepts to the developers of different databases. In this paper we describe the general structure of our system, concentrating on the ontology-based database as the key component of the system.

Nutrient management for coastal zones: a case study of the nitrogen load to the Stockholm Archipelago.

This study investigates cost-effective solutions of decreasing the nutrient load to a coastal area, using a drainage basin approach. The study is applied to the Stockholm Archipelago, a coastal area of the Baltic Sea suffering from eutrophication caused by the load of nutrients entering the area. Nitrogen is the nutrient of concern in this study since it is the limiting nutrient of the Archipelago. The main sources of nitrogen are wastewater plants, agriculture, and atmospheric depositions. The final impact of a deposition depends on the buffering capabilities it is subject to on its trajectory from the source to the recipient. This is the reason for using a recipient oriented approach, in which the focus is to reduce the final impact of a deposition. The model integrates data over hydrology, land cover, land use, and economy in order to find the optimal allocation of measures. Results indicate that in order to achieve cost effectiveness, the major part of nitrogen load reduction to the Archipelago should be done at the wastewater plants and by constructing wetlands. The minimum annual cost of reaching a 50% reduction of the load to the Archipelago was estimated to around 191 million Swedish crowns (around $19 million).

The HIV/AIDS Prevention Research Synthesis Project: implications for federal HIV prevention policy.

Scientific research can strongly influence programs and policies at the local, state, and federal level. In this article we review implications for federal public health policy from the HIV/AIDS Prevention Research Synthesis Project, a project that integrates data from individual intervention studies to estimate the influence of HIV risk reduction interventions on social, behavioral, and community changes. We note several important policy changes that have occurred in HIV research and program development as a result of this project. We also describe the project's role in guiding future funding decisions.

Inter-state migration and national integration.

"In this paper an attempt has been made to assess the pattern of social mobilization, and also to identify the heterogeneity profile of the different states of the Indian union." Interstate migration flows are analyzed, and open or hospitable states are compared with insular ones in terms of net migration hospitality ratios. Findings indicate that geographical and cultural proximity of states encourages migration and thus integration. Data are from the 1961 and 1971 national censuses.

Ontologies for molecular biology and bioinformatics.

About five years ago, ontology was almost unknown in bioinformatics, even more so in molecular biology. Nowadays, many bioinformatics articles mention it in connection with text mining, data integration or as a metaphysical cure for problems in standardisation of nomenclature and other applications. This article attempts to give an account of what concept ontologies in the domain of biology and bioinformatics are; what they are not; how they can be constructed; how they can be used; and some fallacies and pitfalls creators and users should be aware of.

Adventures in environmental data reporting: high tech, low tech, and everything in between or Wisconsin DNR's reporting systems move toward the future.

Electronic data transmittal and data warehouses seem like obvious solutions for streamlining reporting systems and managing large bodies of data; however, regulatory agencies like Wisconsin Department of Natural Resources (DNR) face significant barriers in implementation. In addition to the development costs to the Agency, regulators may be limited by the capabilities of the regulated community and the perceived burden for small businesses and communities. Electronic systems can be implemented incrementally if supported by state regulations and processes for insuring data integrity.

[Electromyography activities of the head, neck and upper trunk muscles with mandibular movement in normal adults].

OBJECTIVE: To investigate the activities of head, neck and upper trunk muscles with mandibular movement in normal adults. METHODS: The integral data of surface electromyography (EMG) had been recorded to analyze the activities of anterior temporal (Ta), posterior temporal (Tp), sternocleidomastoid (SCM) and trapezius (TRAP) muscles with mandibular movement in ten normal adults. RESULTS: The former mentioned muscles acted constantly when the mandible was in the rest position. The activities of Ta, Tp and SCM muscles increased with protrusion of mandible, mouth opening, tapping, maximum clenching, and chewing movements. The amplitudes of the increased activity of these muscles were correlated significantly with each other. When the activities of Ta and Tp muscles increased to 56 times and 25 times respectively, the activity of SCM muscle increased to 5 - 6 times. The activity of trapezius muscle increased with protrusion of mandible and opening movement, but tended to stop with the maximum clenching and chewing movement. The correlations of left and right sides of the same muscles had been found. CONCLUSIONS: The muscles of head, neck and upper trunk can act as contributors to keep the mandibular positions and take part in the mandibular movements.

High throughput electrophysiology using a fully automated, multiplexed recording system.

The drug discovery process centers around finding and optimizing novel compounds active at therapeutic targets. This process involves direct and indirect measures of how compounds affect the behavior of the target in question. The sheer number of compounds that must be tested poses problems for classes of ion channel targets for which direct functional measurements (e.g., traditional patch-clamping) are too cumbersome and indirect measurements (e.g., Ca(2+)-sensitive dyes) lack sufficient sensitivity or require unacceptable compromises. We present an optimized process for obtaining large numbers of direct electrophysiological measurements (two-electrode voltage-clamp) from Xenopus oocytes using a combination of automated oocyte handling, efficient and flexible liquid delivery, parallel operation, and powerful integrated data analysis. These improvements have had a marked impact, increasing the contribution electrophysiology makes in optimizing lead compound series and the discovery of new ones. The design of the system is detailed along with examples of data generated in support of lead optimization and discovery.
