Long-term outcome of tooth repulsion in horses. A retrospective study of 61 cases.

The records of 61 horses undergoing tooth repulsion for treatment of alveolar periostitis were reviewed. Seventeen of 36 horses (47%) in which maxillary teeth were removed had serious postoperative complications, such as infection of a second tooth, bone sequestration, chronic sinusitis, draining tracts, retained dental packing, feed impaction of the alveolus or sinus, suture-line dehiscence, or skin-flap sloughs. Eight horses required at least one additional surgical procedure. Eight of 25 horses (32%) in which mandibular teeth were removed had serious postoperative complications, and four horses required an additional surgical procedure. Hospitalization lasted 2 to 61 days (median, 22 days) for maxillary teeth and 3 to 35 days (median, 8 days) for mandibular teeth. Long-term follow-up (at least 5 months) was possible in 47 horses. Twenty-four of 30 horses (80%) with maxillary tooth repulsion healed without further problems; six horses had persistent nasal discharge. Fourteen of 17 horses (82%) with mandibular tooth repulsion healed with no further problems or with only minor complications; three horses had a chronic draining tract.

True and nonspecific alveolitis sicca dolorosa related to operative removal of mandibular third molars.

Alveolitis sicca dolorosa (ASD) following removal of 145 mandibular third molars from 109 healthy university students was studied. The operations were all performed under local anaesthesia by the same oral surgeon under similar conditions, using similar postoperative procedures. True ASD was considered to have occurred if a patient returned to the surgeon seeking relief of the typical symptoms of ASD before the scheduled check-up date. Non-specific ASD was considered to have occurred in a patient who recorded an increase in pain intensity on a Visual Analogue Scale (VAS) from the third or fourth postoperative day onwards but did not return to the surgeon. True ASD was diagnosed in 5% of cases and non-specific ASD in 15% of cases.

Maxillary single-tooth replacement utilizing a novel ceramic restorative system: results to 30 months.

This study reports on the first longitudinal results of an alumina (70%)-zirconia (30%) ceramic restorative system for implant-supported, single-tooth replacement in the anterior maxillary jaw. Eighteen patients (9 men, 9 women, mean age = 42.4 years) were treated with 22 implants placed into 19 extraction sockets and 3 residual ridges. Eight implants were immediately loaded with nonoccluding provisional prostheses. All implants were definitively loaded with fully occluding ceramic restorations after osseointegration and soft tissue maturation. Patients were monitored from 7 to 30 months (mean = 18.1 months) after loading. All implants osseointegrated and were successfully restored. One case of abutment screw loosening occurred because of patient parafunction (bruxing), and another patient reported chewing pain attributed to malocclusion. Both problems were successfully resolved without further incidence. Within the context of this study, single-tooth replacement in the anterior maxillary jaw was successfully achieved with alumina-zirconia ceramic single-tooth restorations with up to 30 months of clinical function.

Does metronidazole prevent alveolitis sicca dolorosa? A double-blind, placebo-controlled clinical study.

The effect of a single preoperative dose of metronidazole in the prevention of alveolitis sicca dolorosa (ASD) after surgical removal of one impacted, non-infected mandibular third molar was investigated. A patient sample of 270 were given either 1000 mg of metronidazole or placebo at least 30 min before surgery. The preoperative recordings included gender, age, tooth to be removed, experience of surgeon, time of test medication, and duration of surgery. No difference was found between the metronidazole and placebo groups in the occurrence of ASD. The duration of surgery and the experience of the operating surgeons had no effect on the occurrence of ASD. The present study failed to demonstrate any preventive effect of a single dose of metronidazole on the development of ASD.

Clinically significant oroantral communications--a study of incidence and site.

The incidence of clinically significant oroantral communications (OACs) after simple tooth extraction during the period 1980-9, at the Faculty of Dentistry, Mahidol University, Bangkok, Thailand, was studied retrospectively. The total number of OACs was 87. The tooth most frequently involved was the upper first molar, followed by the second and third molars. The difference, however, was not statistically significant (P > 0.05). The sockets of the palatal roots of the first and the second molars were most frequently involved. There was no statistically significant difference between the incidence of OACs on the right and the left sides (P > 0.05).

The relationship between the indications for the surgical removal of impacted third molars and the incidence of alveolar osteitis.

Six hundred forty-two impacted third molars were surgically removed in 412 patients. Before surgery, each patient was assessed clinically and radiographically, and the reason for the removal of each tooth was specifically recorded. One hundred eighty-two of the impacted teeth were removed for prophylactic reasons and 460 for therapeutic reasons. As much as possible, standardization of the operating procedure and environment, and of the preoperative and postoperative regimens was observed. After surgery, each case was followed to determine the absence or presence of signs and symptoms of alveolar osteitis. It was found that several factors seem to contribute to the development of alveolar osteitis; however, the most significant related finding was that the reason for the extraction, that is, whether the extraction was undertaken for therapeutic or prophylactic reasons.

A retrospective analysis of peri-implant tissue responses at immediate load/provisionalized microthreaded implants.

PURPOSE: The aim of this retrospective study was to examine the peri-implant tissue status at immediately provisionalized anterior maxillary implants 12 to 30 months following tooth replacement. MATERIALS AND METHODS: This is a retrospective study of 43 microthreaded, TiO2 grit-blasted implants placed in healed ridges and immediate extraction sockets to restore maxillary anterior and premolar teeth in 28 patients. The cortical bone position relative to the implant reference point was evaluated at implant placement and 6 to 30 months following restoration. Radiographs were assessed using 7x magnification. The distance from the reference point to the cortical bone was measured to +/- 0.1 mm. The relationship of the peri-implant mucosa to the incisal edge of the definitive prosthesis was recorded. RESULTS: Four implants in 3 individuals failed during the first 6 weeks following placement and provisional loading. Cortical bone adaptation from the time of implant placement up to 30 months following restoration ranged from 0.0 mm to 1.5 mm (average, 0.33 +/- 0.40 mm mesially and 0.28 +/- 0.37 mm distally). The mean radiographic measurements from the interproximal crestal bone to the contact point were 4.53 +/- -0.91 mm (mesial) and 4.06 +/- 0.98. Maintenance and growth of papilla was observed in this group of immediate provisionalized single-tooth implants. Definitive abutment or abutment screw loosening was not observed. DISCUSSION: The linear clinical and radiographic measures of peri-implant tissue responses suggest that proper implant placement is followed by supracrestal biological width formation along the abutment and preservation of toothlike tissue contours. This may influence buccal peri-implant tissue dimensions. CONCLUSIONS: Generalized maintenance of crestal bone and the increased soft tissue dimension with maintenance of peri-implant papilla were identified as expected outcomes for immediate loading/provisionalization of microthreaded, TiO2 grit-blasted implants. Control of peri-implant tissues can be achieved to provide predictable and esthetic treatment for anterior tooth replacement using dental implants.
