Moisture permeability of the total surface bearing prosthetic socket with a silicone liner: is it superior to the patella-tendon bearing prosthetic socket?

The purpose of this study was to examine the moisture permeability properties of materials used for total surface bearing (TSB) socket with a silicone liner, a combination of Silicone Suction Socket or Icelandic Roll-On Silicone Socket (ICEROSS) and an acrylic plastic sheet (Degaplast), patella-tendon bearing (PTB) socket, a combination of Pe-Lite and Degaplast, and wooden socket made of poplar. Moisture permeability of the socket materials was measured as the diminution of water in a container after 12 hours in a climatic chamber. Eight containers with their open, top side were uncovered (no material) or sealed with one of the socket materials; the experiment was repeated four times. One-way analysis of variance followed by Bonferroni's test was applied to examine the differences in moisture permeability. Moisture permeability levels were as follows: no material, 85.9 +/- 1.3 g; poplar, 4.3 +/- 0.4 g; Silicone Suction Socket, 1.1 +/- 0.2 g; ICEROSS, 1.0 +/- 0.2 g; Pe-Lite, 0.8 +/- 0.1 g; 3S + Degaplast, 0.8 +/- 0.1 g; ICEROSS + Degaplast, 0.8 +/- 0.2 g; and Pe-Lite + Degaplast, 0.8 +/- 0.1 g. There were significant differences between the uncovered container and the others, and between poplar and the others (P < 0.05). We concluded that the TSB socket with a silicone liner is not superior to the PTB socket with regard to moisture permeability, and that it is necessary to develop a new prosthetic socket that allows heat release and drainage of sweat.

An orthopaedic scoring system for nail-patella syndrome and application to a kindred with variable expressivity and glaucoma.

To help determine whether there is a genetic basis to the substantial variability observed in nail-patella syndrome (NPS), we devised a scoring system that quantifies the severity of the orthopaedic characteristics in NPS. Use of this system to score affected members in three generations of a single kindred revealed wide variability of severity of orthopaedic findings both within and between generations. Genetic testing in this family supported, but did not prove, a previously reported theory that the severity of the NPS in the offspring is modulated by the allele contributed by the unaffected parent. Evaluation of nonorthopaedic characteristics revealed the presence of glaucoma and the absence of kidney disease in this family. It is important that patients with NPS be evaluated for renal disease and glaucoma.

[Patella dislocation. Therapeutic principles and prevention of late complications].

In this review we have compared the different kinds of treatment of acute dislocation of the patella regarding the effects and the complications. The literature describes a variety of treatments and no standard treatment exists. Most studies are not randomised prospective studies and the follow-up period is often short. We conclude that there is a need for randomised, prospective studies with a large number of patients and with a long follow-up period in order to find the best kind of treatment. Treatment modalities for acute as well as recurrent dislocation are described in the general section of the review. In order to achieve the best results and decrease the number of complications it is necessary to have a clear diagnosis of APD and knowledge of predisposing factors.

[Aneurysmal bone cyst of the patella. A case report and literature review].

We report a case of an aneurysmal cyst localized in the patella of a 37-year-old man. The lesion was secondary to a chondroblastoma at six years follow-up after initial curettage and bone graft. It were no recurrence. Treatment of aneurysmal cysts depends on the degree of articular involvement. We made a detailed study of 11 cases of this rare localization of aneurysmal cysts reported in the literature.

Age trends in human chiasma frequencies and recombination fractions. II. Method for analyzing recombination fractions and applications to the ABO:nail-patella linkage.

A new method is presented for studying the relationship between human recombination fractions and parental age at the time of conception. Assuming the sex specific recombination fraction to be a linear function of age, a feasible computer algorithm is described whereby the likelihood of multigenerational families can be calculated. Using this method and the likelihood ratio test, it is found that for the ABO:nail-patella linkage age (P= .17)is more significant than sex (p= .23) in its effect on the recombination fraction. The age effect, if it is real, appears to be limited to males: the paternal recombination fraction decreases by .0062(+/- .0036) per year.

Osteoblastoma of the patella: case report.

A 34-year-old man came to our clinic because of left knee pain and mild swelling. Local tenderness over the peripatellar area was observed during physical examination. The range of motion of the knee was full. The radiographic presentation of the patella revealed an osteolytic lesion with a thin sclerotic rim without evidence of extra-articular involvement. The computed tomography revealed an upper pole intraosseous lesion. Intralesional curettage and allogeneic bone grafting were carried out. The pathologic examination demonstrated primitive osteoblasts, with osteoid features and many giant cells. A diagnosis of the osteoblastoma was made. Following surgery, the patient regained full range of motion and was pain-free. Complete healing of the lesion without evidence of recurrence was noted in follow-up radiographs 2 years postoperatively.

Chondroblastoma of the patella associated with an aneurysmal bone cyst.

Chondroblastoma is a rare, benign tumor of bone, accounting for about 1% of all bone tumor cases. It tends to affect the epiphyseal ends of long bones, most often in males during the first and second decades of life. It has well-characterized radiographic and histologic features but despite its histologically benign appearance a few cases of metastases have been reported. Local recurrences after curettage and bone grafting occur in 11% to 25% of cases. The features of a patellar chondroblastoma are the same as for other locations. In reviewing the literature we found an unusually high male-to-female ratio. It is interesting that the usual treatment of the patellar chondroblastoma has been patellectomy, whereas curettage and bone grafting has predominated in the other locations. We present a computer tomography and magnetic resonance imaging study of a case of chondroblastoma of the patella associated with an aneurysmal bone cyst. To our knowledge, it is the seventh case reported and the second with computer tomography and magnetic resonance imaging studies. We also review and discuss in detail all the cases of patellar chondroblastoma that we found in the literature.

[A case of painful defect of unknown origin of the pediatric patella].

An uncertain history of potential trauma versus possible developmental variants and disorders in childhood may cause differential diagnostic problems in pediatric radiology. We report the case of a 12-year old boy with an unclear unilateral defect of the patella and long-persisting complaints in his knee. The plain radiological and MRI morphology of this lesion are described and possible differential diagnoses are discussed.

Locked lateral patellar dislocation with impaction fracture of patella.

Irreducible lateral patellar dislocation is rare. It has previously been described with rotation around a vertical axis. We describe a case of irreducible lateral patellar dislocation in a 66-year-old lady with an impaction fracture of medial facet of patella locked on the lateral femoral condyle. Closed reduction was unsuccessful and the patient was treated by total knee replacement. Such a case has not been described previously in the literature.

Osteochondrosis of the superior pole of the patella: two cases with histologic correlation.

Two cases of osteochondrosis of the superior pole of the patella are reported with histologic findings. Both patients were young girls; one had mild cerebral palsy. Sixteen cases of this disorder have been documented but without histologic study. The histologic features of these two cases showed osteonecrosis with reparative changes. These findings support that this entity is similar to other osteochondroses of the quadriceps mechanism: Osgood-Schlatter disease and Sinding-Larsen-Johansson disease.

A familial syndrome of aniridia and absence of the patella.

This is the first report of a syndrome of aniridia and aplasia of the patella. The origin of the defect arises spontaneously in the grandmother of the proband and must be assumed to be a de novo mutation. There is no associated chromosomal abnormality or overt biochemical or other somatic defect. This is an extremely rare mutation exhibiting a dominant autosomal form of inheritance. The gene for aniridia is a dominant gene whose penetrance here is 100%. The combination of aniridia and bone malformations in general is very rare. This linked defect probably represents one of the most rare of human genetic abnormalities. The penetrance of the combined abnormality seems to follow that of the more well-known aniridia gene.

[External hypertension syndrome of the patella. Its significance in the recognition of arthrosis].

Some chondromalacia are caused by an hyperpressure on the lateral articular facet of the patella. The authors describe the clinical signs and the radiological findings in this condition. It is due to a retraction of the lateral patellar ligament. They have operated on 133 cases and reviewed 115 knees. After a simple section of the lateral patellar ligament, results were satisfactory in 80 percent of the cases. Biopsies of the malacic cartilage were performed and pathological changes were described.

Biologic resurfacing of the patella with quadriceps tendon flap: a rabbit study.

In this experimental study, 10 rabbits underwent arthrotomy with exposure of the patellar bone (group A), 10 rabbits underwent arthrotomy of the right knee without touching the patella (group B), and 10 rabbits comprised the control group (group C), which was included only for evaluation of functional scoring activities. A 2-mm thick flap of quadriceps tendon reflected downward to cover the patellar bone denuded of cartilage, which resulted in formation of hyaline cartilage in 87% of cases in group A and 89% in group B, incomplete differentiation of the mesenchymal tissue in 7% in group A and 9% in group B, and fibrous tissue or bone formation in 6% in group A and 2% in group B. In group A, 22% of formed cartilage had a regular surface, 22% had superficial horizontal lamination, 44% had fissuring, and 12% showed severe disruption. In group B, these values were 56%, 33%, 11%, and 0%, respectively. In group A, normal cellularity was noted in 33% of cases, slight hypocellularity in 67%, and no moderate hypocellularity. In group B, these values were 56%, 33%, and 11%, respectively. This procedure should be considered in young patients with loss of articular cartilage for whom other procedures or patellectomy are unsuitable solutions.

Recurrent dislocation of the patella. Relation of treatment to osteoarthritis.

Eighty-one knees surgically treated and twenty-six conservatively treated for recurrent dislocation of the patella in seventy-eight patients were studied. The average follow-up on the surgically treated knees was eight years and on the conservatively treated knees, sixteen years. In the non-surgical group dislocations tended to become less frequent with advancing age and there was very little evidence of osteoarthritis. After the sixty-nine tibial tubercle transfers there was a 20 per cent recurrence rate; further procedures were often needed, and the incidence of late osteoarthritis was disturbingly high. After the twelve soft-tissue corrections without movement of the tibial tubercle, dislocation recurred in three, but late osteoarthritis was not seen.

[Operative treatment of recurrent dislocation of the patella by Brückner (author's transl)].

It is reported on the experiences of 43 patients, which surgery was performed on because of recurrent dislocation of the patella. In all these cases we applied the restraining operation by Brückner. The advantage of this method is the earlier training without a cast. The operative method and the postoperative training are explained. Until now recidives were not discovered.
