Follicle puncture with oocyte harvest or ovarian inspection for an early corpus luteum was carried out in 13 patients undergoing laparoscopy during routine infertility workup. Laparoscopies were carried out 28 to 38.5 hours after the onset of the luteinizing hormone surge so that there would be a concentration of data around the expected time of ovulation. A rapid luteinizing hormone assay was used. All the corpora lutea were found 38 hours or more after the onset of the luteinizing hormone surge. Between 28 and 38 hours after the onset of the surge, intact follicles containing oocytes in various stages of maturation were found.
Accurate prediction of the time of ovulation is important for infertile women considering in vitro fertilization. Preovulatory follicular growth was measured by using ultrasound every 2 to 24 hours and laparoscopic aspiration of the follicular fluid was performed in sixty six patients with clomiphene-HCG cycle. The mature oocyte was examined in aspirated fluid. In ten fertile women with spontaneous cycles, preovulatory follicles were measured by ultrasound and the largest follicular diameter was 18.7 +/- 1.9mm (mean +/- S.D.), of which the growth rate was 1.39mm/day. Follicular diameter was well correlated with the aspirated volume (r = 0.847; p less than 0.001), the largest diameter of the dominant follicle being 28.3 +/- 1.5mm and the second follicle 23.3 +/- 6.1mm. The second follicle grew rapidly after HCG injection and the mature oocyte was collected in spite of rupture of the dominant follicle. The size of the dominant follicle reaching a maximum diameter of 25mm remained almost unchanged for 24 hours before ovulation. The recovery rate of mature oocyte by ultrasound in monitoring of follicles was 87.5%. Ultrasound is a useful technique to use in monitoring follicular growth and ovulation.
A service programme of in-vitro fertilization ( IVF ) and embryo transfer (ET) has been established in a small unit in Western Australia after a successful pilot study which was undertaken in 1981. The key features of the programme include simplified monitoring of the follicular phase of stimulated cycles; oocyte retrieval and ET are both undertaken during routine daytime work schedules. The results of the first eight months of the programme are presented, during which 13 further pregnancies were generated and 10 healthy infants were delivered (seven boys and three girls). In the last session in which 49 patients took part a mean of 2.3 mature preovulatory oocytes were collected by means of a double-lumen aspiration/flushing needle from 98% of 48 patients who reached the laparoscopy stage. Ninety-four per cent of 48 patients proceeded to embryo transfer by means of a double-catheter technique; the pregnancy rate was 20.8% per laparoscopy or 22.7% per embryo transfer.

In vitro fertilization was attempted during a spontaneous cycle in 17 out of 97 patients. Preovulatory urinary estrogen and LH maximas were assayed in order to time celioscopy 24 hours after the onset of the triggering urinary LH maxima. In 8 patients, comparison with plasmatic LH shows that the timing of spontaneous ovulation can be precisely predicted from urine assays. However, when the triggering LH release is weak, LH must be above or equal to 25 IU/g of creatinine with a creatininuria above or equal to 0.4 g/l in two urine samples at a three-hour interval.
In vitro fertilization of human oocytes is successful only when several techniques are perfectly mastered. Accurate prediction of imminent ovulation by rapid radioimmunoassay of luteinizing hormone in the plasma, suitable hormonal treatment or ultrasonography, or a combination of these, leads to the recovery of mature oocytes. Factors such as suction strength and bore size of the aspiration needle may interfere with the recovery of follicular oocytes during endoscopy. Capacitation of the spermatozoa, the most critical part of the whole process, requires the presence of serum or serum albumin. Fertilization and embryonic development in vitro occur in well defined experimental conditions. However, in spite of all the precautions currently taken, the rate of success, in terms of pregnancies continued to term, is still much lower than that observed under natural conditions. Much better results will likely be obtained in the near future. The literature suggests that in vitro fertilization and embryo transfer do not have any harmful physical effects on the offspring. Moreover, the laws of biology suggest that in vitro fertilization of human oocytes does not raise any ethical problem with regard to the potential offspring. However, it is extremely difficult to identify ethical problems related to the influence of the technique of in vitro fertilization on the evolution of man and human society.

Seven hundred twenty-five women of proven fertility recorded the presence of cervical mucus at the vulva in 7514 menstrual cycles. The mean cycle length of the 6472 "normal" cycles was 28.5 days (standard deviation +/- 3.18). The peak day of mucus discharge was the last day of slippery, raw-egg-white-like mucus and occurred on average on day 15 (+/- 2.6). The fertile period was defined as any day on which mucus was reported before the peak day until 3 days after the peak. Its mean length was 9.6 (+/- 2.6) days. The probability of pregnancy was maximal on the peak day and declined on the days before and after the peak.
The possibility of a correlation between biological (E2) and morphological (U.S.) parameters, in view of obtaining a correct prevision of follicular maturation and ovulation is discussed. In this study normal patients with a tubal sterility were treated with clomiphene citrate and clomiphene plus H.M.G. The statistical analysis was performed by means of the linear correlation (r) and the relative test of significance (Student T.). The data obtained in this study show that notwithstanding that the rising plasma concentration of E2, which precedes ovulation, is the first and more constant marker of follicular maturation, because of its wide range of variability its usefulness in prediction of ovulation is nearly limited to spontaneous cycles. In fact in stimulated cycles the rising plasma-E2 concentration is rather an independent variable regarding the growth of single follicles (F.D.).

Prediction of ovulation was established by correlation of clinical parameters, follicular development by ultrasound, and estradiol, progesterone, and luteinizing hormone (LH) determination in 71 menstrual cycles. Laparoscopic follicular aspiration was accomplished in 41 of those cycles. A 28-hour interval from the ascending limb of the LH seems to be the "ideal time" for retrieval of a preovulatory oocyte. The variability in the amount of LH to which the follicle is exposed during the LH surge seems to indicate that there is a relatively low specific value necessary for ovulation. Ovulation occurs approximately 10 +/- 5 hours from the LH peak. Progesterone occurs in relation to the LH surge and is helpful for the retrospective analysis of the menstrual cycle.
