Determination of radiology staffing needs by regression analysis.

In the past, analysis of staffing needs has caused friction between radiology departments and hospital administration. To overcome this difficulty, the department of radiology at the University of New Mexico developed a regression-based analysis which ties departmental staffing needs to hospital statistics. Patient days and number of facility outpatient visits are used to project procedure statistics and staffing needs.

Teaching and evaluating ultrasound skill attainment: competency-based resident ultrasound training for AIUM accreditation.

Modern obstetrics and gynecology practice requires the frequent use of ultrasound and ultrasound training as a required component of obstetrics and gynecology residencies. Although programs do offer training in obstetric ultrasound imaging, education in gynecologic imaging is either absent or limited. This article describes a comprehensive ultrasound curriculum for obstetrics and gynecology residents that has been developed and implemented at the University of New Mexico. The curriculum is competency based and qualifies the graduating resident to seek American Institute of Ultrasound in Medicine laboratory accreditation.

A theory-based motivational approach for reducing alcohol/drug problems in college.

The Campuswide Alcohol and Drug Abuse Prevention Program (CADAPP) was implemented and evaluated over a 1.5-year span at the University of New Mexico (UNM). Drawing on self-regulation theory as a basis for understanding motivation for change, the program was designed to increase risk perceptions and thereby reduce the use of alcohol and other drugs among university students. The program was evaluated from 1988 to 1989 through repeated anonymous random sample surveys of all enrolled students on the UNM campus and on a similar control campus not implementing new prevention efforts during the same period. As predicted, relative to the control campus, students on the CADAPP campus after the program showed significantly higher perceived risks from substance use and significantly reduced levels of alcohol and marijuana use. These findings provide encouraging evidence for this theory-based approach to primary and secondary prevention.

L-tryptophan-related eosinophilia-myalgia syndrome: a case report.

Eosinophilia-myalgia syndrome (EMS) is characterized by intense eosinophilia and, very often, debilitating generalized myalgia in the absence of infectious or neoplastic causation. The Centers for Disease Control have established that the latter two features, along with an eosinophil count greater than 1000 cells per cu mm, are criteria for the syndrome. EMS has been reported in epidemic proportions over the last several months. Early data strongly suggested that in at least a small percentage of patients the syndrome leads to death. Epidemiological work in New Mexico, Minnesota, and Oregon has linked EMS most impressively to L-tryptophan-containing products (LTCPS).

Health care in the borderland.

For over 30 years, Hidalgo County, a geographically isolated and financially stressed community in the southwestern corner of New Mexico, has struggled to develop a stable primary health care service. The retirement of the county's general practitioner in the 1970s was followed by several decades of misses, near-misses, and out-and-out failures, when the community found it difficult to attract and impossible to keep a physician. In order to organize and fund a stable medical clinic, the community had to adapt to the realities of a new era in medicine. Primary care physicians in rural communities need access to medical information and to specialists, help in coping with the economic pressures of medical care, and support that will enable them to develop a sustainable lifestyle. Hidalgo County now has a modern health care delivery system. The experiences that led to the creation of the present clinic provide insight into the problems for the delivery of primary health care in remote areas and suggest solutions that may be relevant to other communities across rural America.

Stable isotopic carbon composition of apples and their subfractions--juice, seeds, sugars, and nonvolatile acids.

The 13C:12C ratios of 8 authentic apple juice samples and their subfractions were determined by mass spectrometry. Apples from Argentina, Mexico, New Zealand, and the United States were processed into juice; pulp was collected from the milled fruit and seeds were collected from the press-cake. Sugars, nonvolatile acids, and phenolics were isolated from the juice by treatment with ion-exchange resins and polyvinylpyrrolidone (PVPP). The mean value for all juice samples was -24.2% which is close to the values reported by other investigators. Juice from apples grown in Argentina, Mexico, and New Zealand did not differ from U.S. samples. The isotopic composition of the subfractions ranged from -22.0 to -31.0%. The values for the pulp were essentially the same as for juice. The sugar fraction was slightly less negative than the juice; the nonvolatile acid and phenolic fractions were more negative. The levels of nonvolatile acids and phenolics in apple juice are low, however, so these compounds contribute little to overall delta 13C values in juice.

Plant reproductive allocation predicts herbivore dynamics across spatial and temporal scales.

Life-history theory suggests that iteroparous plants should be flexible in their allocation of resources toward growth and reproduction. Such plasticity could have consequences for herbivores that prefer or specialize on vegetative versus reproductive structures. To test this prediction, we studied the response of the cactus bug (Narnia pallidicornis) to meristem allocation by tree cholla cactus (Opuntia imbricata). We evaluated the explanatory power of demographic models that incorporated variation in cactus relative reproductive effort (RRE; the proportion of meristems allocated toward reproduction). Field data provided strong support for a single model that defined herbivore fecundity as a time-varying, increasing function of host RRE. High-RRE plants were predicted to support larger insect populations, and this effect was strongest late in the season. Independent field data provided strong support for these qualitative predictions and suggested that plant allocation effects extend across temporal and spatial scales. Specifically, late-season insect abundance was positively associated with interannual changes in cactus RRE over 3 years. Spatial variation in insect abundance was correlated with variation in RRE among five cactus populations across New Mexico. We conclude that plant allocation can be a critical component of resource quality for insect herbivores and, thus, an important mechanism underlying variation in herbivore abundance across time and space.

Strategies for state statutory language changes granting fully independent nurse practitioner practice.

For nurse practitioners (NPs) to be considered as a critical component of health care reform, they must change their state statutes to permit independent practice. This can be accomplished by plenary prescriptive authority including controlled substances. This article provides a summary of NP scope of practice and prescriptive privileges to date, together with a discussion of the strong statutory language used by Alaska and New Mexico with regard to independent practice. Comparisons of model independent scope and prescriptive authority statutory language are discussed along with prevalent opposition arguments, appropriate practitioner titles, and an enumeration of strategies for effective lobbying.

Filling the gaps. Systems offer two models of care management: community outreach and inpatient care.

The growing number of vulnerable people, such as the elderly and people with long-term disabilities, calls for healthcare providers to offer more programs to ensure a continuum of care. Client-focused care management programs offer access to such a continuum. Care managers understand services and reimbursement and can pull it all together for the client. The Sisters of Charity Health Care Systems, Cincinnati, have two models of care management. St. Joseph Coordinated Care provides extensive outreach to a large, culturally diverse New Mexico community, serving urban and rural clients. Penrose-St. Francis Healthcare System offers inpatient medical care management in Colorado. Coordinated Care at St. Joseph Healthcare System in Albuquerque is comprehensive, covering a wide spectrum of client needs--medical, social, and psychological. The program's central goal is to help individuals remain safely at home. Persistence and devotion to the client are the hallmarks of effective care management and the foundation of the new geriatric care management program at Penrose-St. Francis Healthcare System in Colorado Springs, CO. The program's goals are to improve inpatient geriatric care, smooth the patient's transition to alternative care settings, and ensure efficient and effective resource use during the patient's hospital stay.

Managing organizational mergers.

Reimbursement, competition, and other significant trends in the health field have stimulated an increase in mergers among health care organizations. Mergers inevitably produce significant ramifications for clinicians. Nurses have discovered that mergers produce a greater need for efficient patient care, but not necessarily the mechanism or guidance for achieving this goal. Nursing administrators are responsible for minimizing the negative impact of a merger on nurses while simultaneously promoting merger goals. Despite this increase in mergers and the subsequent challenge to clinicians, little information is available about how to manage mergers. This article reviews the impact of the merger between the Veterans Administration Medical Center and the Kirtland Air Force Base Hospital in Albuquerque, New Mexico. A nursing perspective is used to understand what problems arise and how nursing administrators can effectively manage these problems.

Tuberculosis surveillance in a state prison system.

After four inmates at two New Mexico prisons converted their tuberculosis skin tests, a mass screening program was carried out at all the State's adult correctional facilities (2,240 inmates). Previously unknown converters were found with a disproportionally high per cent of converters (6.8 per cent and 6.3 per cent) and reactors (14.4 per cent and 12.2 per cent) at the first two prisons. No index cases were found. State policy has been revised to include two-stage skin testing of new inmates, annual testing thereafter, and screening of all new correctional staff.

Woman to Woman: Coming Together for Positive Change--using empowerment and popular education to prevent HIV in women.

HIV risk is the product of social, cultural, economic, and interpersonal forces that create sex-role definitions and expectations that can lead to gender inequalities in health. Woman to Woman: Coming Together for Positive Change is an HIV/AIDS prevention intervention that takes into account that choices and actions may be constrained by poverty, gender roles, and cultural norms. This project educates and empowers, promotes women's perspectives, reaches women "where they are," and encourages women to speak of their experience. The following study presents the evaluation of the effectiveness of an empowerment based participatory HIV prevention program over a 3-year period, implemented in both rural and urban settings in New Mexico, targeting over 300 women at-risk of HIV/AIDS. In addition, the practicalities of conducting participatory process and outcome evaluation with a small nonprofit with minimal budget will be discussed.
