[Prostatic squamous cell cancer and its differential diagnostic contrast with squamous cell metaplasia].

Thirteen (2.2%) squamous-cell carcinomas were found among 600 autopsy cases of prostatic carcinoma. In the surgical material squamous-cell carcinoma was observed in 3 cases (1.7%). Clinical and morphological analysis of these 16 cases together with the published observations shows squamous-cell carcinoma of the prostate to have a grave clinical course and bad prognosis. One observation of the erroneous histologic diagnosis of carcinoma in case of squamous-cell duct and gland metaplasia of the prostate is reported. The differential-diagnostic differences between dyshormonal squamous-cell metaplasia and squamous-cell carcinoma of the prostate are described.

Immunohistochemical identification of lysozyme in pseudopyloric gland metaplasia in Crohn's disease.

Using an immunoperoxidase technique lysozyme (LZM) was identified in the ileum of patients with Crohn's disease in inflammatory cells of the lamina propria, in Paneth cells and in pseudopyloric gland metaplasia. These observations demonstrate a further analogy of pseudopyloric gland metaplasia with true pyloric glands, and may offer an additional explanation for the raised concentration of serum-LZM in patients with Crohn's disease.

[Osteogenic metaplasia or residual embryonic endometrial ossification?].

There are only about 90 cases described in the literature of the presence of an intra-uterine osseous structure--so the condition is not a common one. Often the aetiology is unknown. The purpose of this work is to describe the clinical picture and the anatomo-pathological characteristics on one form which leaves practically no doubt as to how the presence of bony structures in the uterus arise. We call this residual endometrial-ossification (O.R.E.) and note osteogenic or osseous metaplasia. We define O.R.E. as the presence of an osseous structure inside the uterus which does not arise from metaplasia of the cells of the endometrium.

Immature Paneth cells in intestinal metaplasia of gastric mucosa.

Various histochemical techniques were performed on paraffin sections of gastric mucosa with intestinal metaplasia to distinguish mature from immature Paneth cells (PC) and to correlate their relative incidence with the degree of differentiation of metaplastic epithelium. The results show a higher incidence of immature PC in the poorly differentiated type of intestinal metaplasia than in the well-differentiated type. The identification of immature PC may be proposed as a further criterion in the evaluation of gastric dysplasia.

Metaplasia. An overview.

Metaplastic changes are common lesions found at widespread locations, occurring in both reactive and neoplastic tissues. Metaplasia has the following characteristics: (1) it can arise not only as a proliferation and transformation of immature (stem) cells, but also as a direct or indirect transformation of mature cells; (2) through metaplasia, a given cell may exhibit epithelial, mesothelial, or mesenchymal characteristics, regardless of the tissue of origin; and (3) its importance lies in that metaplastic changes can often be symptomatic and may simulate tumors clinically, grossly, or histologically.

Adenocarcinoma of the breast with osseous metaplasia. Report of a case with needle aspiration cytology.

A 68-year-old black female developed a painless left breast mass that increased in size over a nine-month period before the patient sought medical attention; fine needle aspiration biopsy was performed as part of the initial workup. Only occasional malignant cells were present on the slides despite the use of a good aspiration technique on this large mass. The additional finding of rare osteoblasts and multinucleated cells, interpreted as osteoclasts, suggested osseous metaplasia within the lesion. The diagnosis of osseous metaplasia in an infiltrating ductal carcinoma of the breast was therefore suggested by careful cytologic screening of the material from a firm lesion that yielded smears with otherwise inexplicably low cellularity.

[A case report of mastopathy with cartilagenous and osseous metaplasia].

A case of mastopathy with cartilagenous and osseous metaplasia is reported. The patient is a 57-year-old Japanese woman complaining of a tumor measuring around 1.2 cm in diameter near the nipple of the left breast. The xeromammographic diagnosis was adenosis with calcification. Histological examination identified an intracystic tumor composed of papilloma and adenosis with cartilagenous and osseous metaplasia. This is the first Japanese case of its kind.

Aroclor 1254-induced intestinal metaplasia and adenocarcinoma in the glandular stomach of F344 rats.

Ingestion of diets containing Aroclor 1254, a mixture of polychlorinated biphenyls, for 2 years led to a dose-related increase in the incidence of focal lesions in the glandular stomachs of male and female F344 rats. The incidence of stomach lesions was 6% in control specimens and in specimens from rats fed a diet containing 25, 50, or 100 ppm Aroclor 1254, the incidences of stomach lesions were 10, 17, and 35%, respectively. The majority of gastric lesions in treated rats were histologically identified as intestinal metaplasia characterized by an architecture resembling that of intestinal crypts and particularly by goblet cells, which stained with Alcian blue and periodic acid-Schiff reagent. Adenocarcinomas were found in six specimens. Most (88%) of the lesions were located in the pyloric region of the glandular stomach. No multiple lesions were observed among 47 control specimens examined; however, nine cases of multiple lesions were observed in 30 lesion-containing specimens from Aroclor 1254-treated rats. Although the exact relationship between gastric intestinal metaplasia and adenocarcinoma remains to be established, they commonly coexist and may share initiating mechanisms.

[A study of apocrine resembled metaplasia of endometrium in the patient with functional uterine bleeding and their hormone levels (author's transl)].

Relationship between endometrial findings and serum hormone concentrations in patients with functional uterine bleeding (FUB) was not clarified yet. One hundred patients affected by FUB were divided into four groups according to their endometrial patterns: 1) proliferative phase(P), 2) secretary phase(S), 3) atrophic secretary phase(A-S) and 4) cystic glandular hyperplasia(C). Serum hormone levels which included LH, FSH, prolactine(PRL), estradiol and progesterone were measured by radioimmunoassay. 79% of the patients we studied were between 21 to 45 years and 21% were under 20 years or over 46 years of age. None of the patients received hormone therapy prior to the study. P(45%) and S(30%) were frequently encountered in histological manifestation of FUB while C was found no more than 4%. In 21% of the patients revealed apocrine resembled metaplasia of endometrium(AP), the cells resembled to the epithelium of apocrine sweat glands. This metaplasia was found in every endometrial patterns, but was seen in A-S most frequently. Serum concentrations of hormones in patients classified as C were characteristically and inclined to be hypergonadotropic and hypoestrogenic. High PRL levels were found in group S. All hormone levels except for FSH were statistically lower in the group of AP than that in groups where no AP was found.

[Mechanisms of metaplasia of the ductal epithelium of organs of the pancreatico-biliary system].

It has been established in experiments on young rats that during induction of canalicular hypertension or introduction into the ducts of a little tube made of aluminium foil, the ductal epithelium undergoes metaplasia of the intestinal type in the absence of marked inflammatory lesions. In the process of metaplasia there appear loci of proliferation due to the involvement of precursor cells and prolongation of the phase of DNA synthesis. Such mechanisms are likely to underlie the development of precancerous conditions.

Correlation between precancerous bronchial metaplasia and cigarette consumption, and preliminary results of retinoid treatment.

Vitamin A and its derivatives, so-called retinoids, can prevent squamous metaplasia induced not only by vitamin A deficiency but also by carcinogenic hydrocarbons. An aromatic retinoid, such as ET1, has been shown to prevent chemically induced papillomas in mice and to amplify certain immunologic reactions. Heavy smokers, 106 volunteers, were submitted to fibrobronchoscopy with bronchial biopsies. An index of metaplasia (IM) was calculated on the basis of microscopical examination of a total of 9,633 sections of 1,010 biopsies. Despite the subjectivity of the estimates of cigarette consumption, this was significantly (P less than 0.02) and positively correlated to the IM. Eighty-five percent of the women had a low IM as compared to only 42% of the men (P less than 0.01), although there was no significant difference in the reported cigarette consumption. Fifty-two subjects had an IM greater than 15% and were given 25 mg ET1 orally daily for 6 months. The bronchoscopy was repeated in 30 patients following completion of the 6-month treatment. The IM was significantly (P less than 0.01) reduced after treatment.

Induction of intestinal metaplasia in the glandular stomach of rats by X-irradiation prior to oral administration of N-methyl-N'-nitro-N-nitrosoguanidine.

CD/CRJ rats were subjected to localized X-irradiation of the stomach and given N-methyl-N'-nitro-N-nitrosoguanidine (MNNG) in the drinking water. Rats given MNNG alone and non-treated rats were used as controls. Upon sacrifice at 15 months after the initial MNNG administration, intestinal metaplasia was observed; the histology was of complete type and the incidence was 100% in rats treated with X-rays and MNNG, whereas in rats treated with MNNG alone the intestinal metaplasia was of incomplete type and its incidence was 80%. However, the incidence of gastric cancer in rats treated with MNNG alone was 25%.

Ultrastructural observations on preneoplastic epidermoid metaplasia in the lungs of rats.

Epidermoid metaplasia, of a type that consistently progresses to epidermoid carcinoma, was induced in rats at the interface of pulmonary tissue and intrapulmonary deposits of beeswax that contained either benzo(alpha)pyrene or the heptane-soluble fraction of cigarette smoke condensate. The preneoplastic epithelium was searched for morphological harbingers of neoplasia and compared with the type of reactive bronchial metaplasia, rarely associated with cancer, that results from chronic murine pneumonitis. The carcinogen-laden beeswax caused both proliferation and migration of epithelial cells from bronchi adjacent to the wax deposits. These initial proliferating cells were characterized by a lack of differentiation and were similar to the basal cells of bronchial epithelium. The migrating epithelium rapidly established itself at the interface of wax and viable lung tissue as a highly differentiated, stratified squamous epithelium. This epithelium was not qualitatively different from normal squamous epithelium. However, quantitative differences were evident from both normal epithelium and the metaplastic epithelium resulting from nonspecific injury. In the preneoplastic epithelium, the first indications of aggressive behavior occurred in foci of dysplastic basal cells that sent cytoplasmic intrusions through defects in the basement membrane. Characteristically, these dyplastic basal cells had numerous infoldings of the nuclear membrane and prominent nucleolar alterations. All strata of preneoplastic cells contained excessively complex interdigitations of the plasma membrane and unusually large numbers of desmosomes and tonofilaments. In contrast, the nonspecific metaplastic epithelium consisted almost entirely of multilayered uniform cells that closely resembled immature but otherwise normal basal cells of respiratory epithelium.

Oncocytic metaplasia of bile duct epithelium in hepatic GVHD.

A 15-year-old female underwent matched unrelated BMT for chemotherapy resistant ALL. She died 3 months later from septicaemia complicating grade IV GVHD. Light and electron microscopic examination of the post-mortem liver confirmed GVHD and showed oncocytic metaplasia of interlobular bile duct epithelium. We report that oncocytic metaplasia of bile duct epithelium is part of the pathological spectrum of hepatic GVHD.

Gastric cell phenotypes and intestinal metaplasia in Polynesian and non-Polynesian residents of New Zealand.

Previous studies have indicated that the frequency of different cell phenotypes as well as of the mucosal extension of intestinal metaplasia (IM) in the gastric mucosa varies in different geographic regions (Scandinavia, Japan, Mexico, Hawaii, and mainland U.S.A.). These variations appear to be influenced by environmental factors but not by the race of the individuals. In the present work, 241 gastrectomy specimens from the two main racial groups in New Zealand, indigenous Pacific Island Polynesians (mainly Maoris) and non-Polynesian settlers (mainly of European ancestry), were examined. The occurrence of histologic changes known to be influenced by the environment such as intramucosal gastric cysts, intestinal metaplasia (IM), ciliated metaplastic cells, and cells with large vacuoles were recorded. The mucosal extension of IM was classified into low IM (having one to five areas with IM in at least one of the sections) and high IM (having one area with IM in all microscopic fields in at least one of the sections or occupying entire fields in one or more sections). Polynesians with adenocarcinomas and peptic ulcers were younger than non-Polynesians. The gastric mucosa in younger Polynesians harboring an intestinal type carcinoma or a peptic ulcer differed from that of non-Polynesians; Polynesians more often had intramucosal cysts, high IM scores, or ciliated metaplastic cells at a younger age than non-Polynesians. These results suggest that the gastric mucosa of Polynesians may be exposed to environmental factors (ulcerogenic or carcinogenic) at an earlier age than the gastric mucosa of non-Polynesians in New Zealand.

[Endothelial metaplasia of the alveolar capillaries in experimental silicosis of rats].

We studied, by immunohistochemical and ultrastructural observations, the basic mechanism of interstitial fibrosis and endothelial metaplasia of alveolar capillaries in the lungs of Sprague-Dawley rats that received a single intratracheal infusion of silica particles. Immediately after silica administration, alveolar capillaries collapsed and their endothelial cells became necrotic over wide ranges. Alveolar myofibroblasts migrated into luminal silicaladen granulomas through defects in the alveolar linings. They proliferated and produced collagenous matrices that were incorporated into adjacent walls by coverage of regenerating epithelial cells. By day 4, endothelial cells of peribronchiolar small vessels had become positive for proliferating cell nuclear antigen. These endothelial cells apparently migrated along alveolar capillary tubes to replace injured endothelial cells through the processes of interdigitation of their cytoplasms. After one month, these alveolar endothelial cells, which were originally negative for factor VIII-related antigen (F-VIII) had become positive, especially in those segments in fibrotic scars. These endothelial cells developed numerous Weibel-Palade bodies containing F-VIII. Some endothelial cells formed specific ultrastructural fenestrations, which are unique to capillary endothelial cells that originated from bronchial arteries. The mechanism of endothelial metaplasia might consist of the following processes in endothelial cells that originated from broncho-pulmonary anastomoses: proliferation, migration, replacement, and differentiation of endothelial cells of bronchial arteries in the chronic fibrotic stages.

[Intestinal metaplasia of the gastric mucosa and its relation with gastric carcinoma].

Epidemical and morphological studies have show that there is an association between intestinal metaplasia (IM) and gastric carcinoma, and it has been considered by many to be a possible marker of premalignant change. IM is also a common finding in benign disease of the stomach, however, and recent studies have therefore been direct at identifying variants of intestinal metaplasia which may have a more specific association with gastric carcinoma. Also, it has been demonstrated that exist another factors that contribute (as the Helicobacter Pylori), which determine some events like the transformation of chronic gastritis to gastric carcinoma, preceded by the presence of metaplastic changes in gastric mucosa.

Effect of S-carboxymethylcysteine lysine salt on mucociliary clearance in rabbits with secretory cell metaplasia.

A single intratracheal instillation of porcine pancreatic elastase (PPE, 100 U/Kg) induces in rabbits bronchial secretory cell metaplasia as well as emphysematous changes. The mucus hypersecretion and the marked reduction of ciliated cells matched by a high percentage of atypical cilia are responsible for the delayed mucociliary clearance in this model. S-Carboxymethylcysteine lysine salt (SCMC-LYS, 0.35 g/Kg b.w.), given per os daily for 10 days starting 2 days before elastase administration, significantly ameliorated the mucociliary clearance. The pharmacological treatment did not modify the degree of secretory cell metaplasia and the percentage of atypical cilia, or prevent the alveolar wall destruction. At TEM examination, the morphological aspects of secretion occurring in bronchial tree of PPE-treated animals were rarely visible in the PPE + SCMC-LYS treated group. The beneficial effect of SCMC-LYS on mucociliary clearance may be ascribed to an antisecretagogue effect of this drug through elastase inhibition and to a reduction of mucus viscosity.
