Multiple sclerosis among Orientals and Caucasians in Hawaii: a reappraisal.

A comparative retrospective study of Oriental and Caucasian patients with multiple sclerosis (MS) failed to reveal significant differences between the two groups regarding historical data, such as sex ratio, mode of onset, age at onset, duration of illness, and clinical course, suggesting that MS is essentially the same disease in both groups. However, certain clinical features were more frequent in Orientals: Severe bilateral optic nerve involvement; acute transverse myelopathy, especially of a recurrent nature; clear-cut sensory levels; and optic-spinal-brainstem and optic-spinal involvement. Environmental factors may also be responsible.

International medical education between Hawaii and Thailand over Internet2.

International medical education sessions have been successfully conducted by videoconferencing using Internet2. The sessions were between two tertiary care medical centres, in Honolulu and Bangkok. However, video quality was lower than for similar sessions using ISDN and audience satisfaction was less. The main reasons for the lower quality were network congestion and bandwidth allocation by the videoconferencing equipment. Software to ensure quality of service is available, but is not easy to implement. There were also network security problems and the costs were high. Our international videoconferences averaged 40-50 hours per year, an activity level at which connection costs were lower for ISDN than for Internet2. It appears that Internet2 videoconferencing for medical education is best reserved for academic institutions that have other high-bandwidth network requirements.

Physical activity before pregnancy and following childbirth in a multiethnic sample of healthy women in Hawaii.

PURPOSE: Investigate physical activity and related psychosocial factors reported by new mothers. METHODS: Women (N = 79) were surveyed to determine their physical activity patterns before and after childbirth. Barriers and facilitators of physical activity were also identified. FINDINGS: Most women (63%) were ethnic minorities, mean age was 31.8 +/- 5.5 years, and their infants' mean age was 8.2 +/- 3.8 months. Combining women's reported physical activity before their pregnancy and after childbirth resulted in four significantly different groups: (1) 21.5% were inactive before and after birth; (2) 22.7% were active before and after; (3) 12.6% were inactive before, but active after birth; and (4) 43.0% were active before but inactive or irregularly active postpartum (p < .0003). Barriers to physical activity included personal issues, including lack of support from a spouse, and parenting duties. Facilitators to being active included social support for exercise and availability of childcare. CONCLUSIONS: New mothers are at high risk for inactivity and reductions in previously established levels of physical activity. They also have specific barriers and facilitators to being physically active. Future research should develop effective methods for increasing physical activity in mothers of infants.

Structural birth defects associated with oral clefts in Hawaii, 1986 to 2001.

OBJECTIVE: To identify structural birth defects that occur in association with oral clefts. METHODS: Data were obtained from a birth defects registry and included all infants and fetuses with cleft palate without cleft lip or cleft lip with or without cleft palate delivered from 1986 to 2001. For 47 specific structural birth defects, rates among oral cleft cases were compared with the rates among all infants and fetuses with major birth defects, excluding those with oral clefts. RESULTS: Among cleft palate only cases, the rates were significantly higher than expected for encephalocele, microcephaly, and syndactyly. Among cases of cleft lip with or without cleft palate, the rates were significantly higher than expected for anophthalmia/microphthalmia, single ventricle, reduction deformity of upper limbs, and reduction deformity of lower limbs. When cases of cleft palate only and cleft lip with or without cleft palate were compared as to the rates for particular birth defects, the rates of the defects were either higher or lower than expected in both oral cleft categories for 38 (81%) of the defects. CONCLUSIONS: Certain birth defects were more frequently associated with oral clefts than might be expected. For the majority of defects, their patterns of association were similar between cleft palate without cleft lip and cleft lip with or without cleft palate.

Effects of aging and dilution on attraction and toxicity of GF-120 fruit fly bait spray for melon fly control in Hawaii.

Attractiveness and toxicity of GF-120 Fruit Fly Bait (Dow AgroScience Indianapolis, IN) to melon flies, Bactrocera cucurbitae Coquillett, were examined to assess the effects of concentration and aging. We tested dilutions of 20, 40, and 80 ppm (AI) (spinosad) against water controls. The 80 and 40 ppm treatments were significantly more attractive than the 20 ppm and control treatments. Attraction was compared between baits aged for 2 and 24 h, fresh bait and water controls. Age had significant effects on both attractiveness and toxicity of GF-120. Baits aged for 2 h were 11 times less attractive to female melon flies than fresh bait. Mortality rates were reduced by 50% when GF-120 was subjected to rain. Our results suggest the need for frequent applications of GF-120 to obtain maximum benefits, particularly in wet tropical climates.

Seasonal and yearly population dynamics of two exotic helminths, Camallanus coti (Nematoda) and Bothriocephalus acheilognathi (Cestoda), parasitizing exotic fishes in Waianu Stream, O'ahu, Hawaii.

Exotic poeciliid fishes introduced into Hawaiian freshwaters are responsible for the introduction of several exotic parasites, of which the most important are Camallanus cotti and Bothriocephalus acheilognathi in terms of potential disease threat to native stream fishes. This roundworm and tapeworm are the most prevalent and abundant freshwater fish helminths in Hawaiian streams. This study examined the seasonal and yearly population structure of C. cotti and B. acheilognathi to determine if the tropical Hawaiian environment characterized by low climatic variability permits continuous opportunities for parasite transmission regardless of time of year. Camallanus cotti displayed seasonal differences in prevalence and mean abundance, whereas B. acheilognathi did not. Camallanus cotti prevalence and mean abundance were higher in the Hawaiian summer (47.7%, 0.79) than in winter (25.8%, 0.36). A seasonal relationship of C. cotti levels is likely explained by extensive rains associated with the Hawaiian winter season, which may act to decrease parasite transmission by flushing infected poeciliid hosts, intermediate copepod hosts, and possibly free-living infective worm stages downstream. Bothriocephalus acheilognathi displayed low prevalence and mean abundance in both summer (4.0%, 0.06) and winter (6.5%, 0.07), and it may be difficult to detect seasonal changes due to these low levels. Camallanus cotti prevalence and mean abundance remained relatively constant from the summer of 1995 to the summer of 1999, indicating that levels of this roundworm are stable in Waianu Stream. Whereas B. acheilognathi prevalence and mean abundance were low during the summer of 1995 and the summer of 1997, a dramatic peak in prevalence and mean abundance was observed in the summer of 1998 (41.2%, 1.06), with levels decreasing sharply in the summer of 1999 (4.4%, 0.07). It appears that B. acheilognathi also is present in stable populations at low levels, even though levels rose sharply during a single year.
