[Effect of enteral administration of sodium ribonucleate on the synthesis of amino acyl t RNA in the liver and skeletal muscles of rabbits in experimental hypercorticism].

Daily, for 14 days, rabbits of one group were injected with corticotropin, i.e. ACTH-zinc-phosphate (10 units/kg), whereas rabbits of another group were given (in addition) sodium ribonucleate (40 mg/kg) through a tube into the stomach. Formation of lysyl-tRNA, leucyl-tRNA, and alanyl-tRNA in the liver and the skeletal muscles proved to be significantly greater in the animals which received ACTH together with sodium ribonucleate, as compared to that in the animals given the hormone alone. Hyperglycemia, hepatomegaly, and emaciation were less pronounced in the animals given both the preparations.

Primary carcinoma of the liver in Malawi: a review of 24 cases.

A consecutive series of 24 cases of primary carcinoma of the liver in Malawi has been investigated. Histologically, all were hepatocellular carcinomas (HCC). All patients were African Bantus, the average age was 42.7, and the sex ratio was men 3.5:women 1. The duration of symptoms attributable to HCC was about 5 months previous to admission to hospital and was in no case preceded by clinically manifest cirrhosis. The clinical picture was rather uniform with pain in the region of the liver, emaciation and nodular hepatomegaly as the most important features. One of the patients had repeated attacks of hypoglycaemic coma. Sera from 11 out of 13 patients contained alpha-feto-protein. Hepatitis-associated antigen and antibody in the serum were found in 7 and 6 out of 16 and 14 cases respectively. Serum B12 and serum unsaturated B12 binding capacity were moderately raised in most patients. The prognosis was poor, the average time of survival was 4.8 weeks after admission. The cause of death was most frequently hepatic coma. HCC in the African Bantu shows some different features from the same disease in the Western Hemisphere: The incidence is much higher; the patients are younger. The neoplasm commonly develops in a clinically latent cirrhosis. The latter is not caused by alcohol, but is presumably a sequel of hepatitis. It is possible that aflatoxin is the carcinogenic factor, acting more readily in a cirrhotic than in a normal liver.

[Tactics in the treatment of postoperative intestinal fistula].

The experience with treatment of 331 patient with external intestinal fistulas is summarized. It is necessary to remove the high non-shaped "emaciating" fistulas at day 1-2 after their development. A rational method for the treatment of other non-shaped intestinal fistulas is their open management with active aspiration, or flowing drainage. Conservative therapy of the shaped tubular fistulas is effective only within first 4 mos, and later, the operative treatment is indicated to the patients.

[A case of miliary tuberculosis with cutaneous lesion].

Miliary tuberculosis associated with cutaneous lesion has been rarely reported. We report a case of miliary tuberculosis in whom the cutaneous lesion was confirmed as tuberculosis by skin biopsy and bacterial examination. A 46-year-old man was admitted because of cough, fever, sore throat and abnormal shadow on the chest X-ray. Physical examination revealed an emaciated man with two ulcerous lesions overlying yellowed crust on the chest wall and fine crackles on the left side of the lung. Laboratory workup revealed a white blood cell count of 10,000 with 15% lymphocytes and positive CRP. Chest X-ray film showed the infiltration with cavity formation in left upper lung field and nodular dissemination. His tuberculin reaction was negative on admission. Sputum, urine and secrete from cutaneous lesion were positive for the acid-fast bacilli and the culture grew Mycobacterium tuberculosis. Examination of the skin biopsy specimen and bone marrow aspiration showed Langhans giant cells around necrotic lesion; therefore the diagnosis of miliary tuberculosis was made. After an initiation of antituberculosis therapy with combined regimen composed of streptomycin (SM), isoniazid (INH), ethambutol (EB) and rifampicin (RFP), this patient improved significantly. Although the case report of miliary tuberculosis tends to increase recently, the report of cutaneous lesion is relatively rare in association with miliary tuberculosis. We discussed this subject with reference to the literatures.

Destructive ocular myiasis in a noncompromised host.

A case of destructive ocular myiasis resulting in complete loss of the globe in two days time is documented. To the best of our knowledge this is the first report of such a severe involvement in a healthy and non-compromised host. Mechanical removal and good local hygiene helped heal the wound. The larvae were isolated to be that of Chrysomyia bezziana (screwworm fly). This is possibly the first report of destructive ocular myiasis caused by Chrysomyia bezziana from the Indian subcontinent and the second in world literature. Infestation of human eyes with larvae of flies (myiasis) has been reported. Serious consequences of destructive myiasis are seen in emaciated and diseased patients. Only one report of total destruction of the globe by maggots of Chrysomyia bezziana exists in the literature. As in previous communications, the patient in this report had no predisposing factors both systemic and local. We here in document a case of orbital myiasis leading to rapid destruction of the globe within two days in a healthy and a non-compromised patient.

Toxicological and biological studies on Japanese quails fed graded levels of furazolidone.

Furazolidone (FZ) was administered to 42-day-old female Japanese quails as a feed additive at doses of 0, 200, 400, 600 and 800 ppm for a period of 28 days. Dose-dependent effects were observed. High levels of FZ (600 and 800 ppm) significantly altered growth, decreased feed consumption, caused marked atrophy of the ovaries and oviducts leading to cessation of egg laying, and resulted in higher mortality. Hepatotoxicity was evidenced by an increase in serum aspartate aminotransferase (AST), alanine aminotransferase (ALT) and alkaline phosphatase and a decrease in serum total protein, in addition to degenerative changes of the hepatocytes in FZ-treated birds. A rise in serum urea was also observed. Symptoms leading to death included a loss of appetite causing emaciation followed by nervous disturbances (compulsive movements and circling). No signs of cardiomyopathy were observed. Japanese quails did not tolerate FZ at a concentration (400 ppm) recommended for the prevention of salmonellosis in poultry.

[A case report of very huge dermatofibrosarcoma protuberans].

A 63-year-old male with dermatofibrosarcoma protuberans of the right hip is presented. The patient noticed a small nontender mass 40 years ago and then the tumor enlarged gradually. The patient was very emaciated and needed to receive blood transfusions. The massive multinodular tumor was pedunculated and on it's surface, there were areas of hemorrhage and secondary infection. The operative specimen weighted 2270 g and measured up to 25 by 20 by 10 cm. To our knowledge, this case represents the largest primary tumor of a dermatofibrosarcoma protuberans in review of English literature.

Diagnostic features of ovine progressive pneumonia.

Ovine progressive pneumonia, a chronic, insidious disease of adult sheep, has a relentless course leading to dyspnea, emaciation, and death. Clinical observations and serologic tests are adequate for making a tentative diagnosis. The agar gel immunodiffusion test seems to be the best serologic procedure for indicating infection with the virus but cannot be used to predict morphologic changes or clinical disease, inasmuch as many clinically unaffected animals carry the virus. A definitive diagnosis is based on finding lesions and isolating virus. Affected lungs are large and heavy as a result of interstitial accumulation of lymphoid cells and fibromuscular tissue. Frequently, interstitial lesions are accompanied by bronchopneumonia from secondary bacterial infection. The causal virus can be isolated from infected lungs by cocultivation with primary ovine or bovine cells.

Premature bovine parturition induced by ponderosa pine: effects of pine needles, bark and branch tips.

Three components of Ponderosa pine trees were administered via gavage to pregnant cows to determine their ability to induce premature parturition. When tips of branches, bark or needles were administered (2.7 kg/day) starting at 250 d of pregnancy, early parturition was induced in an average of 3.4, 5.0 and 9.1 d, respectively, compared to 31.8 d for water gavaged controls (P less than 0.01). Less pine material was required to induce parturition with tips and bark (9.4 and 11.2 kg/cow, respectively) than with needles (25.0 kg/cow P less than 0.05). Cows fed pine material retained fetal membranes longer after parturition; 13.0 d for needles, 10.5 d for bark and greater than 3.0 d for tips versus less than 0.5 d for controls (P less than 0.01). However, 2 of the cows fed tips died within 48 hours after parturition with fetal membranes intact and the other 2 cows fed tips became depressed, pyrexic and emaciated. They survived after extended veterinary care with antibiotic and fluid therapy. Administration of pine materials modified serum progesterone concentration profiles; there was significant elevation in progesterone 1-3 days after pine material was administered. Subsequently, pine material induced a premature decline in progesterone levels prior to parturition, which occurred more rapidly in cows fed bark and tips suggesting a dose response relationship. Pine material also modified serum cortisol concentration profiles; the main difference was a prolonged elevation in cows receiving tips. Branch tips and bark from Ponderosa pine are more potent in inducing parturition than needles.

Lymphocytic leukemia and lymphosarcoma in a rabbit.

Lymphocytic leukemia and lymphosarcoma were diagnosed in a rabbit with lethargy, emaciation, and pallor. The diagnosis was made on the bases of results of hematologic analysis, cytologic evaluation of a bone marrow specimen, and histologic examination. The lymphosarcoma was identified to be of T-cell origin. Leukemia is rarely diagnosed in rabbits, although lymphosarcoma is fairly common in this species.

[Corynebacterium suis infection in swine. 1. Clinical diagnosis with special consideration of urine studies and cystoscopy].

Experimental and spontaneous infections with Corynebacterium suis in sows were investigated. In early stages animals show no clinical disorders or only for a short time. However, there are already marked changes in urinary samples (hematuria, proteinuria, leukocyturia, gross alterations). Using an endoscope mucosal irritations can be seen mainly on the floor of the bladders. In chronic cases alterations in urine are more pronounced. If a pyelonephritis is present in addition to the cystitis, general signs of illness are evident including anorexia, emaciation, anemia, subnormal body temperature and abortions. Bladders demonstrate an erosive and ulcerative, hemorrhagic cystitis on the whole mucosal surface. Uremia appears only in late stages of the disease.

Concurrent maedi-visna virus infection and pulmonary adenomatosis in a commercial breeding flock in East Anglia.

The seroprevalence of maedi-visna virus infection in thin potential cull ewes aged over two years in a flock in East Anglia increased from 3.7 per cent in August 1985 to 39.0 per cent in September 1987 and 93.3 per cent in May 1989. This increase coincided with the first appearance of sheep pulmonary adenomatosis in the flock. Four emaciated ewes which were dyspnoeic were necropsied between 1987 and 1989. Maedi and pulmonary adenomatosis were confirmed histologically in one of these ewes and pulmonary adenomatosis was confirmed in the other three.

[Role of cryptosporidiosis in diarrhea among hospitalized adults in Bamako].

Etiologies of infectious diarrhoeas in hospitalised adults have been studied during one year; research of cryptosporidium and isospora oocysts is being made with Henriksen-Pohlenz and Kato methods. Diarrhoea is associated with a positive HIV serology in 40% cases. Cryptosporidium spp is found in 38% of cases. In 91% cases of cryptosporidiosis HIV serology is positive. Cryptosporidiosis is the main cause of AIDS diarrhoeas in Mali. 3 cases of isosporiasis are associated with cryptosporidiosis in AIDS patients. Emaciation and dehydration are the main signs of severity. Diarrhoea's profusion, its chronicity and inefficiency of the treatments explain the heavy death rate of cryptosporidiosis among seropositive patients, which reaches 40% during the first two weeks of hospitalisation.

Salmonellosis in an adult dairy cow.

An outbreak of diarrhoea occurred in a Jersey herd after the introduction of new stock. One of the cows was examined and treated unsuccessfully. Clinical findings included depression, fever, dehydration, congestion, signs of colic and a severe diarrhoea. The post mortem examination revealed emaciation, pseudomembranous enteritis, mesenteric lymphadenopathy and focal disseminated hepatic necrosis. Salmonella typhimurium was isolated from the faeces, mesenteric lymph nodes and liver.

[A case report of IgD myeloma with acute renal failure and deposition of lambda light chain along the tubular basement membrane].

A case of IgD lambda type multiple myeloma that developed acute renal failure was presented. Histopathologic studies revealed apparent myeloma cast nephropathy with typical giant cells and light chain deposition along tubular basement membrane and inter-peritubular interstitium. Glomerular deposits of light chain were absent by electron and immunofluorescent microgram. The presented case showed a very uncommon histopathologic findings of unusual deposits of light chain, to tubulo-interstitium except for glomeruli, in addition to vertically rare IgD myeloma. A sixty-year old female who was suffered from acute renal failure and treated by hemodialysis for one month was admitted to our institute. Physical examinations revealed ill-apparent and emaciated uremic patient on admission. Laboratory and radiological studies demonstrated IgD lambda type monoclonal protein and numerous punched out appearance of skull bone. The diagnosis of IgD lambda myeloma was confirmed with aspiration bone marrow test. Anti-myeloma therapy of melphalan and prednisolone have started, however, fatal arrhythmia suddenly suffered the patient and resuscitation attempts failed to get any success one week after admission. Renal necropsy was performed and specimens were studied by light, immunofluorescent and electron microscope.(ABSTRACT TRUNCATED AT 250 WORDS)

[Tumor of the larynx in a child with Recklinghausen's disease].

The rare case of Recklinghausen disease in a 2.5 years old boy was presented. He was admitted to ENT Clinic of Pediatric Institute of Medical Academy in Poznań because of dyspnea and emaciation. He had coffee-with-milk coloured maculae on his skin, especially of hands and trunk. Laryngoscopy revealed the laryngeal tumor, which was excised by use of Kleinsasser equipment. Histopathologic answer was neurofibroma. The follow up proved good health without recurrencies.

Huntington's chorea: implications for nursing in India.

Huntington's Chorea is a rare hereditary disease characterized by chronic progressive chorea and dementia, the exact cause of which is still under study, although many hypotheses are present. The patient has abnormality in gait, choreic movements of the limbs, face, slow mental deterioration and ill sustained depression. At later stages the patient becomes emaciated and succumbs. It is yet untreatable and diagnosed by CT Scans, bio-chemical analysis, family history and classic clinic picture. This disease poses a challenge for effective nursing care. There are studies conducted about the nursing aspects--effects of individualized care, effect of musical therapy. It has been shown that diversional therapies, short walks and giving the patient freedom in activities of daily living would help. The prevention at the present day seems to be voluntary sterilization of those at risk after proper counselling.

[Acute autonomic and sensory neuropathy associated with galactorrhea-amenorrhea syndrome and intractable anorexia].

Acute autonomic and sensory neuropathy (AASN), one subtype of acute pandysautonomia, in which dorsal root ganglia and autonomic ganglia are involved is uncommon. Little is so far known on central nervous system involvement in AASN. In the present paper we described a rare case of AASN associated with the central nervous system manifestations such as galactorrhea-amenorrhea syndrome and intractable anorexia. A 30-year-old woman rapidly developed burning pain and numbness in her arms and legs as well as orthostatic syncope. She had severe anorexia and no no menstruation from onset. On physical examination, she was emaciated. There was marked orthostatic hypotension with tachycardia. Skin was dry. Moderate galactorrhea was detected. Neurological examination showed prominent paresthesia and dullness of superficial sensation, predominantly to pinprick and thermal stimuli, segmentally over the neck, occipital scalp, and extremities. Deep sensation was intact. She had no weakness or ataxia. Deep tendon reflexes were almost normal. NCV and SEP were normal, while EEG was abnormal. Sural nerve biopsy demonstrated axonal degeneration with the loss of myelinated, predominantly in small-caliber fibers, and unmyelinated fibers. The levels of HVA and MHPG in CSF were decreased. The autonomic nervous function tests revealed postganglionic dysfunction. alpha-adrenergic system was predominantly impaired, while beta-adrenergic system was relatively preserved. The endocrinological studies demonstrated mild or moderate elevation of PRL basal value and hyper-response of PRL and LH for TRH and LH-RH loading test, which suggested disorder of the hypothalamo-hypophysial system. Cranial MRI showed moderate dilatation of the 3rd ventricle.(ABSTRACT TRUNCATED AT 250 WORDS)
