[Computed morpho-densitometry and its possibilities in experimental and clinical studies].

The paper deals with the automatic morphodensitometric analysis of various biomedical objects (such as interphase chromatin in the cellular nuclei of chondroblasts and lymphocytes or human peripheral blood, opacities in the lens of chick embryos. A complex algorhythm of computational image processing was created for submolecular restructuring of interphase chromatin cell nuclear examinations. It includes specific methods of staining and unifies different quantitative analytical methods: photometrical, geometrical, gradiental, informational, and statistical. The blood cells of the children who had received low-dose radiation were detected by employing a new method of TV image computational processing of DNA-stained lymphocytes. The approach demonstrated by this study provides a sensitive and meaningful mean for diagnosing human diseases and can be useful in the clinical setting.

Bone densitometry and histomorphometry in patients with hairy cell leukemia.

Tumor necrosis factor alpha, the cytokine that participates in the autocrine growth control of hairy cell leukemia has strong bone resorptive properties. This prompted us to look for bone involvement in HCL. Bone mineral density (BMD) was not decreased in 14 HCL patients who did not have radiographic evidence of bone destruction. Osteopenia was found in only two HCL patients with skeletal complications of the disease and bone pain preceded diagnosis in both cases. Lymphomatous infiltration of the vertebral bodies T 12/L1 was confirmed histologically in the female patient while bilateral necrosis of the femoral head preceded the diagnosis of HCL in the male patient. Histomorphometry of bone biopsy samples was performed in another 12, previously untreated male HCL patients. Trabecular bone volume was found to be reduced in 11, greatly reduced in 5 of them, while osteosclerosis was found in only one patient. The increase of trabecular bone pattern factor, a new parameter for simple quantification of trabecular interconnection indicated a poorly connected trabecular lattice in eight of these 12 patients.

[An epidemiological study on osteoporosis using ultrasound bone densitometry].

Ultrasonic bone density measurements were performed in a population to study the relationship of bone density to age, body size, fracture, hip and back pain, and their diagnostic value for osteoporosis. In additions other factors related with osteoporosis diagnosed using ultrasonic measurements values were examined. The study subjects were thirty-eight men and sixty-three women who participated in community health examinations in 1993 in Akabane, Aichi prefecture. Indices evaluated for ultrasonic bone density measurement were speed of sound (SOS), broadband ultrasound attenuation (BUA) and stiffness index (SI). 1) In men, means for SOS and SI values declined with age from the forties to the seventies, but BUA was not related to age. In premenopausal women, SOS, BUA and SI were not related to age, but an age related decline was observed in postmenopausal women. 2) BUA was positively related to body weight in men and women, while the relationship with SOS and SI was not as strong. 3) Two postmenopausal women experienced fractures due to osteoporosis, and their SI values were less than 59%. About fifty percent of the subjects had hip and back pain but no clear association of SI to prevalence rate was observed. 4) Osteoporosis was diagnosed using SI values. Cases of osteoporosis were relatively older, had more years since menopause, were lower in height, body weight and thickness of skinfolds, but had higher serum total cholesterol levels others. 5) Further prospective epidemiological studies should be performed to evaluate the validity of this diagnostic criterion using SI values, and to clarify the relationship of lifestyles to ultrasonic bone density measurement and osteoporosis.

[Ultrasound densitometry and the index of Jhamaria in the diagnosis of senile osteoporosis].

Over 90% of the calcaneum is made of trabecular bone, whose turnover is high and which is therefore very sensitive to bone metabolism changes. Thus, we evaluated the mineral content of the calcaneum with the semiquantitative Jhamaria index adapted by Aggarwal et al. (1986) and ultrasound measurements. With the latter method, broadband ultrasound attenuation (BUA) values were calculated, which indicate ultrasound signal breadth reduction when passing through the bone. The two techniques were used to examine 99 women, whose age ranged 65-79 years, referred for dorsolumbar pain and suspected senile osteoporosis. This condition was diagnosed with dorsolumbar spine radiographs and considering as osteoporotic only the patients with one or more vertebral collapses. Thus, any other cause of osteopenia was excluded with bone metabolism serourinary indices. The two groups of patients thus obtained (group A: 42 osteoporotic women with vertebral collapse and group B: 57 osteoporotic women with no vertebral collapse) exhibited significantly different Jhamaria index and BUA values (group A: 2.5 +/- 0.9, B: 3.7 +/- 0.96, A: 46.3 +/- 13.6, B: 60.2 +/- 15.6) (p < 0.001). The diagnostic accuracy of calcaneal BUA and Jhamaria indices, in the two groups, was 85.7% and 81%, respectively. The analysis of the mean values showed no significant correlation between BUA and Jhamaria index in the group of patients with vertebral collapse. On the contrary, the correlation of the two indices in the group of patients with no vertebral collapse exhibited slight statistical significance (r = 0.38). To conclude, on the basis of our results the calcaneum is confirmed as a reliable peripheral index of vertebral BMC and ultrasound measurements are suggested as a simple noninvasive method which can be used to diagnose senile osteoporosis.
