[Idiopathic chylopericardium. Apropos of a new case. Review of the literature].

The authors report a new case of primary chylopericardium in an asymptomatic 26 year old male. In view of the negativity of the complementary etiological investigations a surgical subxiphoid window was performed and this confirmed the diagnosis. After failure of isolated evacuatory drainage, the patient was cured by pericardectomy. The anatomo-clinical, biochemical and diagnostic features of this condition are described based on a review of the other cases of primary chylopericardium reported in the literature. Surgical pericardectomy seems to be the only effective long term treatment.

[Certain aspects of clinically mild, non-tropical chyluria].

Chyluria despite pseudo- banality may pose several diagnostic and therapeutical problems. Its etiopathogenesis is unclear, too. Prolonged loss of the lymph observed in patients with chyluria due to inherited anomalies of lymphatic system may lead to serious deficits and homeostasis disorders. Basing on these thesis, the authors present two patients with chyluria due to inherited anomaly of extraperitoneal lymphatic system and review available literature. Particular role in the clinical follow-up of these patients plays their immunologic status. The authors did note deficiency of cell-mediated immunity measured with skin tests and lymphocyte T blastic transformation test, and increasing IgG deficit, and particularly annoying IgA deficit in the patient with long-lasting, massive chyluria.

Lymphonodovenous anastomosis in the treatment of chyluria.

Thirty cases of chyluria were treated by means of lymphonodovenous anastomoses according to the principles of lymphovenous shunt. A conical tissue of lymph node close to the greater saphenous vein in the inguinal region was removed and the remaining tunnel-shaped node was anastomosed to the vein to drain the lymph into the venous system. Twenty-one cases were followed up for six months after the operation. Among them, 16 (76.2%) showed disappearance of chyluria, and 2 (9.5%) were improved, giving an effective rate of 85.7%. This operation avoids damage to both the afferent and efferent lymphatic vessels, and affords a large anastomotic stoma for free passage of the lymph into the vein.

[Chyloperitoneum causing intestinal obstruction].

The two cases reported here show intestinal obstruction may occur by coagulation of lymph around the loops. They permit us to better understand the physiopathology of blockage of the lymph vessels whether congenital as in the first case or traumatic as in the second case. Traumatic rupture of the cisterns chyli is exceptional. The histological lesions observed on clamped biopsies show clearly the pathology of protein-losing enteropathy.

[A case of retroperitoneal cavernous lymphangioma with the content of chylous fluid].

We report a case of retroperitoneal lymphangioma in a 26-year-old man whose chief complaint was lumbar discomfort. Ultrasonography revealed pararenal cyst on his left side. At the operation, we discovered a cystic tumor between peritoneum and Gerota's fascia. Pathological study indicated cavernous lymphangioma containing chylous fluid. There has been no recurrence for a year after the operation.

[Epidemiological survey on patients with advanced filariasis in Shandong Province with bancroftian filariasis basically eliminated].

This paper reports on the incidence of advanced filariasis after basic elimination of bancroftian filariasis since 1983 in Shandong Province. Investigation was carried out in a population of 166,776 between 1984 and 1988 in 252 villages of Teng Xian and other 5 counties/cities, the erstwhile highly endemic areas. A total of 1,038 filariasis patients were found with an average incidence of 0.6%. Among them, 383 were with elephantiasis, 357 with chyluria and 298 with hydrocele. 902 cases (86.9%) who suffered the disease before the elimination of filariasis and 136 cases (13.1%) after it, 125 (91.9%) being chyluria cases. The oldest of the 1,038 cases was 86 years of age and the youngest, 6 years of age. The course of duration as 8-74 years in elephantiasis cases, 82.8% of them (317 cases) had previously lymphangitis and/or lymphadenitis. In the past three years 16.4% (52 cases) suffered from prolonged or intermittent acute lymphangitis and/or lymphadenitis. The results of this survey indicated that, after the basic elimination of filariasis in Shandong Province together with thorough clearance of infection source, elephantiasis and hydrocele persisted while new cases of chyluria continued to develop. Therefore, in such areas more emphasis should be put on the treatment of clinical patients. New patients should be surveyed and old patients be treated actively so as to reach the goal of eradicating filariasis.

[Morphological and ultrastructural bases of the lymphatic vascular system].

The lymphatic vascular system plays a pivotal role in the control of the interstitial homeostasis. This review illustrates the morphological and ultrastructural features of precollecting, pre- and postnodal collecting and absorbing peripheral vessels. A special attention is devoted to the endothelium of the absorbing lymphatic, its cytological features, kinds of intercellular contact, contractile properties and occurrence of valves. The role of the different mechanisms involved in transendothelial transport, namely membrane diffusion, vesicular route, open junctions and intraendothelial channels, are considered. The morphological reality of "intercellular openings" and "intraendothelial channels" is also discussed. The latter are "dynamic units" which carry on a remarkable task in the drainage, toward the lymphatic lumen, of macromolecules (chylomicra, glycoproteins, etc.) and cellular elements (lymphocytes and polymorphonuclear leukocytes). On the other hand, macrophages follow a transcellular pathway. The factors which possibly underlie these two modes of transendothelial migration are also discussed.

[Lymphangioma of the mediastinum as a cause of chylopericardium].

Chylopericardium is a rare from of pericardial effusion. A case of a ten-year-old female patient with a chylopericardium incidentally detected during a mild respiratory infection through cardiac enlargement at the chest X ray is reported. The techniques used to achieve this diagnosis are described, a special emphasis being put on the CT-scan and lymphangiography, which have not been used in such cases so far. These techniques provided very useful data which were determinant for the success of the proposed therapy. This and other cases reported in the paper indicate that the most common cause of primary chylopericardium is a mediastinal lymphangioma.

[Preliminary study of therapeutical mechanism on chyluria by heat-cleaning and hemostasis drugs].

Ten cases of filarial chyluria were treated with Chinese medical herbs. The results showed that the average concentration of serum histamine of 142 ng/ml before treatment dropped to 93 ng/ml after treatment. The average lymph-vessel pressure of 9.93 cmH2O in dorsalis pedis dropped to 4.74 cmH2O after cure. The prevalent microcirculation obstacle of medium to serious grade also was improved or recovered. The average serum viscosity of 1,884 before treatment dropped to 1,725. These results indicated that the chronic lymphatic inflammation occurring in pelvis or behind peritoneum due to perennial stimulation of adult filariae, caused the high concentration of inflammatory medium tissues, the expansion of micro-vessels and the increase of lymph-vessel pressure. Therefore, the microcirculation obstacle was the principle pathogeny of filarial chyluria.

Remission of filarial chyluria after treatment of coexistent conditions.

Between April 1965 and March 1984, 659 patients (mean age 52.3 years) with chyluria were encountered (probably from filariasis) and 323 were followed for a mean period of 71.9 months (range 1 to 210 months). Of the 323 patients, chyluria was apparently exacerbated by another condition in 132 (40.8%), and often remitted with effective treatment of the coexistent condition. The findings suggest that chyluria is often accompanied by other physiologic or pathologic processes and treatment of these coexistent conditions should be the initial therapeutic approach.

[Chylous lymphangioma of the pancreas (case report)].

We presented a 47 year old lady in whom, during the investigation for unstable arterial hypertension and lumbar pain, ultrasonography and computed tomography showed a polycystic mass retroperitoneally in the level of the pancreas. During operation a chylous lymphangioma of the pancreas was found and was almost entirely excised. The remaining mass was opened and left to drain into abdominal cavity. The recovery was uneventful. Hystology confirmed chylous limphangioma. The patient stayed symptom-free with normal clinical, ultrasonographic and laboratory data for more than a year after surgery. Lumbar pain disappeared and blood pressure became normal.

Nontraumatic chylothorax: a review of diagnosis and treatment.

The incidence of nontraumatic chylothorax is sufficiently low that most primary care physicians are unclear about the criteria for this diagnosis and the proper treatment options. A ten-year review of patients with pleural effusion treated by the university medical service identified 12 individuals with chylothorax. These patients and a current review of the English language medical literature provide us an opportunity to review the clinical presentation of chylothorax. The differential diagnosis and treatment options are reviewed.
