Mosaic pattern of lung attenuation on CT scans: frequency among patients with pulmonary artery hypertension of different causes.

OBJECTIVE: The purpose of this study was to determine the frequency with which a mosaic pattern of lung attenuation is seen on chest CT scans in patients with various causes of pulmonary artery hypertension (PAH). MATERIALS AND METHODS: Chest CT scans of 64 patients with known PAH were reviewed to assess the patterns of lung attenuation. Patterns of lung attenuation were divided into three categories: class I, homogeneous lung parenchymal attenuation; class II, slightly heterogeneous lung attenuation that does not conform to the anatomic boundaries of the secondary pulmonary lobule; and class III (mosaic pattern), heterogeneous lung attenuation in geographic regions with well-defined borders corresponding to the anatomic units of single or multiple secondary pulmonary lobules. The patients medical histories were reviewed to determine the primary cause of PAH for each patient. RESULTS: Peak pulmonary artery pressure of the patients in our study averaged 74 mm Hg (range, 36-194 mm Hg). Twenty-one patients had PAH due to lung disease: 17 patients, due to cardiac disease; and 23 patients, due to vascular disease. Three other patients had PAH due to miscellaneous causes. Of the 23 patients with PAH due to vascular disease, 17 patients (74%) had a mosaic pattern of lung attenuation. Of the 21 patients with PAH due to lung disease, one patient (5%) had a mosaic pattern of lung attenuation. Among the 17 patients with PAH due to cardiac disease, two patients (12%) had a mosaic pattern of lung attenuation. A mosaic pattern of lung attenuation was seen significantly more often in patients with PAH due to vascular disease than in patients with PAH due to cardiac or lung disease. CONCLUSION: A mosaic pattern of lung attenuation can be seen on CT scans in patients with PAH due to vascular disease, cardiac disease, or lung disease. However, the mosaic pattern is seen significantly more often in patients with PAH due to vascular disease than in patients with PAH due to cardiac or lung disease.

Sarcoidosis with pulmonary fibrosis: CT patterns and correlation with pulmonary function.

OBJECTIVE: The purpose of our study was to identify CT patterns of pulmonary fibrosis in patients with sarcoidosis and to correlate these patterns with pulmonary function tests. MATERIALS AND METHODS: We conducted a retrospective review of CT scans of 80 patients with proven sarcoidosis and evidence of fibrotic changes on chest radiographs. RESULTS: Three main CT patterns were identified: bronchial distortion (n = 38, 47%), mainly central; honeycombing (n = 23, 29%), mainly peripheral; and linear (n = 19, 24%), mainly diffuse. In most cases, a pattern was clearly identified as shown by the good agreement between observers (kappa = 0.87). Nodules were significantly associated with the linear (87%) and distorted (71%) patterns, but not with the honeycomb pattern (35%). The honeycomb pattern was most often associated with restriction and decreased lung diffusing capacity for carbon monoxide. Patients with bronchial distortion had lower expiratory airflow rates. The linear pattern was generally associated with the least functional impairment. CONCLUSION: CT may be a useful tool for defining subgroups of patients with fibrotic pulmonary sarcoidosis. CT reveals three main patterns that may reflect different distributions of fibrotic lesions in the lung with different functional pulmonary impairments. The persistence of active pulmonary lesions suggested by the presence of nodular lesions was often associated with linear and distorted patterns.

Ideal amino acid pattern for 10-kilogram pigs.

Two growth assays and one nitrogen balance experiment were conducted to develop an ideal amino acid pattern for 10-kg pigs. Crossbred pigs were fed chemically defined amino acid diets containing four indispensable amino acid (AA) patterns: 1) the Illinois final amino acid pattern (IFP), a recently developed AA profile for purified diets; 2) the Illinois ideal amino acid pattern (IIP), a modification of IFP; 3) the Wang and Fuller ideal amino acid pattern (WFIP); and 4) the 1988 National Research Council (NRC) amino acid requirement pattern for 10-kg pigs (NRCP). A mixture of dispensable AA consisting of glutamate, glycine, and proline that had been proven to be an efficient mixture of dispensable AA nitrogen was fed together with the indispensable AA patterns. Diets were made isonitrogenous and isoenergetic within experiments. In Exp. 1, pigs were given ad libitum access to experimental diets with AA levels set above the NRC AA requirements. Regardless of which AA pattern was fed, pigs had similar (P greater than .05) daily gains, daily feed intakes, and gain:feed ratios. In Exp. 2, all levels of indispensable and dispensable AA were reduced to 50% of levels present in Exp. 1. When pigs had ad libitum access to these diets, daily gains of pigs fed IIP were superior (P greater than .05) to those of pigs fed IFP or NRCP, but similar (P greater than .05) weight gains occurred in pigs fed IFP, WFIP, and NRCP. In Exp. 3, the efficiency of nitrogen utilization of the four indispensable AA patterns was evaluated by a nitrogen balance experiment in pigs equally fed the same experimental diets fed in Exp. 2. Pigs fed NRCP utilized nitrogen with an efficiency of 74%, which was less (P less than .001) than the efficiencies of 79 to 80% obtained in pigs fed IFP, IIP, and WFIP. Nitrogen retained (grams) per gram of nitrogen intake from indispensable AA was greater (P less than .01) for IIP than for either IFP or WFIP. The results of these experiments indicate that WFIP contains excesses of leucine, valine, phenylalanine plus tyrosine, methionine plus cystine, and threonine for pigs between 10 and 20 kg BW. Also, NRCP is probably first-limiting in leucine and also limiting in other AA, resulting in lower nitrogen utilization than IIP. The pattern of indispensable AA in IIP (grams of AA/100 g lysine) is as follows: lysine (100), methionine+cystine (60), threonine (65), tryptophan (18), phenylalanine+tyrosine (95), leucine (100), isoleucine (60), valine (68), arginine (42), and histidine (32).

Interference in identifying tactile patterns: response competition and temporal integration.

When two vibrotactile spatial patterns-one target and one nontarget pattern-are presented to the same location, subjects may have difficulty in identifying the target pattern. The difficulty may be in part the result of the target's and nontarget's being processed to the point of evoking competing responses. Subjects err by responding with the nontarget. Target identification may also be interfered with because the target and nontarget form a composite pattern through temporal integration, and subjects are unable to separate the features of the target and nontarget patterns. The present study addressed the issue of whether a response competition explanation of interference is consistent with the existence of temporal integration and the formation of composite patterns. A series of three experiments examined temporal integration and its relationship to response competition. The results indicate that both temporal integration and response competition can occur under similar stimulus conditions, and that both can interfere with the identification of target patterns. The results further indicate that even under conditions in which composite patterns are formed, the two patterns that constitute the composite are still identifiable. It is concluded that the interference in pattern identification attributed to response competition is not a result of temporal integration.

Atypical BCR and ABL D-FISH patterns in chronic myeloid leukemia and their possible role in therapy.

D-FISH uses DNA probes with fluorescence in situ hybridization to detect two fusion signals in cells with a t(9;22)(q34;q11.2) from patients with chronic myeloid leukemia (CML). Using D-FISH, 147 patients with CML were studied and considerable macro genetic variation was observed among their Ph-chromosomes. Typical D-FISH signal patterns were observed for 81% of patients, but three different atypical patterns were seen in 19% of patients. Atypical patterns among Ph-chromosomes were consistent with loss of the 3' portion of BCR that is usually translocated to chromosome 9, or loss of the 5' segment of ABL that usually remains on chromosome 9, or loss of both the 3' translocated BCR and 5' residual ABL hybridization sites. Atypical patterns were associated with all forms of Ph-chromosomes including t(9;22)(q34;q1.2), complex translocations and masked. The normal range for 500 interphase nuclei for patients with typical patterns is < 1%. By comparison, the normal range for patients with either of two atypical patterns was < or = 1.8% and for patients with the other atypical pattern was < or = 23%. Thus, special scoring criteria are needed to detect and quantify nuclei with atypical patterns using D-FISH. The proportion of patients that responded to therapy with interferon alpha-2b or interferon alpha-2b and ara-C for 36 patients with typical patterns was similar to 7 patients with atypical patterns.

Branching patterns of right pulmonary artery in cardiovascular anomalies.

The branch of the right pulmonary artery (RPA) to the upper lobe of the right lung (RUL), the truncus anterior of the RPA, and the pars anterior of the left pulmonary artery, which supplies the left upper lung lobe (LUL), were demonstrated by both dissection of postmortem specimens and angiography for 20 infants and children, by angiography only for 57, and by specimen dissection only for 59 (total 136). In posteroanterior angiograms, the RUL artery branches from the RPA near the right lateral border of the vertebral column, while the LUL artery or arteries arise more laterally, near the left midclavicular plane. This pattern is reversed in situs inversus (eight patients studied). Thirty-nine patients in other categories of congenital cardiovascular disease showed an abnormal RUL or LUL arterial pattern, including pulmonary isomerism, right lung type (RUL artery pattern present bilaterally, 12 patients); pulmonary isomerism, left lung type (RUL artery pattern absent bilaterally, 11 patients); scimitar syndrome (RUL artery pattern normal, 1 patient; absent bilaterally, 4 patients); and left pulmonary artery sling (RUL artery normal, one; hypoplastic, one; absent, two patients). Five patients with tetralogy of Fallot (TOF) with right aortic arch (RAA) and 1 of 15 with RAA not TOF or situs inversus showed a relatively large RUL artery arising more laterally than usual. Three of six patients with double outlet right ventricle had the LUL artery larger than usual plus two accessory RUL arteries, and one patient with crossed pulmonary arteries showed a similar pattern. Two patients with single ventricle had an RUL artery of normal pattern although the RUL bronchus was absent, and one patient with single ventricle and situs inversus had a comparable pattern in the left lung. The ease of demonstration of the right and left upper lobe branches of the pulmonary artery by dissection or angiocardiography warrants greater attention to their patterns in patients with congenital cardiovascular disease. Dissociation of upper lobe bronchial and vascular patterns is unusual and may also be of diagnostic value.

The effect of hand position and pattern motion on temporal order judgments.

Subjects made temporal order judgments (TOJs) of tactile stimuli presented to the fingerpads. The subjects judged which one of two locations had been stimulated first. The tactile stimuli were patterns that simulated movement across the fingerpads. Although irrelevant to the task, the direction of movement of the patterns biased the TOJs. If the pattern at one location moved in the direction of the second location, the subjects tended to judge the first location as leading the second location. If the pattern moved in the opposite direction, that location was judged as trailing. In a series of experiments, the effect of the spatial position of the hands and fingers on TOJs and the perception of the direction of pattern movement were examined. Changing the position of the hands so that the patterns no longer moved directly toward each other reduced or eliminated the effect of motion on TOJs. In a variation of Aristotle's illusion, the moving patterns were presented to crossed and uncrossed fingers. The results indicated that, contrary to Aristotle's illusion, the subjects processed the moving patterns relative to an environmental framework, rather than to the local direction of motion on the fingerpads. Presenting the patterns to crossed hands produced results similar to those obtained with crossed fingers: The subjects processed the patterns according to an environmental framework.

The effect of location on the discrimination of spatial vibrotactile patterns.

The present study examined whether the locations of patterns on the skin affected the ability to process information about their shapes. In Experiment 1, pairs of spatial vibrotactile patterns, using the array from the Optacon, were presented sequentially to subjects' left index fingerpads. The location of each pattern in a pair was varied randomly among four locations on the skin. The subjects responded "same" or "different" on the basis of the shapes of patterns, regardless of their locations. Discrimination accuracy was highest and response time fastest when patterns occupied identical locations (ILs), and performance suffered with increasing distance between patterns. In Experiment 2, pairs were presented to corresponding points or to noncorresponding points on separate fingerpads. When patterns occupied corresponding points on separate fingers, accuracy was lower than when patterns occupied ILs on a single finger, but higher than when patterns occupied noncorresponding points on separate fingers. The results suggested that discriminability declined partly because patterns did not occupy ILs, and partly because separate locations had different densities of innervation.

Vertical shift of the medial rectus muscles in the treatment of A-pattern esotropia: analysis of outcome.

A retrospective study was performed on 18 consecutive patients with A-pattern esotropia and no apparent oblique muscle dysfunction, mechanical restriction, paresis, or previous muscle surgery. All patients underwent graded bilateral medial rectus recession for their esotropia with simultaneous vertical upshift to treat the A-pattern. The quantitative relationship between amount of upshift, amount of A-pattern correction, preoperative A-pattern, and preoperative esotropia was examined. We found that the amount of A-pattern correction was closely correlated with the size of the A-pattern preoperatively (r = 0.83), independent of amount of upshift. While the change in A-pattern did correlate with the amount of the upshift (r = 0.60), it was not a significant independent predictor of the surgical response. The amount of recession had little influence on the effectiveness of the procedure in correcting the vertical incomitance, and the transposition did not seem to affect the correction of the basic esotropia, adversely. We conclude that medial rectus recession with vertical upshift of the muscle insertions is an effective procedure for correcting the vertical incomitance in A-pattern esotropia, and that the amount of A-pattern correction achieved is determined primarily by the size of the preoperative A-pattern and not the amount of upshift.

Lifestyle Combination Patterns as Key Modifiable Factors for Type 2 Diabetes Mellitus Risk among Middle-Aged Korean Men.

BACKGRUOUND: The increasing prevalence of type 2 diabetes mellitus (T2DM) worldwide highlights the need to understand risk factors and effective prevention strategies. Although individual lifestyle factors associated with diabetes risk have been identified, research on their collective interactions is limited. This study aimed to identify lifestyle combination patterns in middle-aged Korean men and evaluate their impact on T2DM risk. METHODS: A total of 2,332 middle-aged men without T2DM at baseline (2001-2002) from the Korean Genome and Epidemiology Study (KoGES) cohort were included. T2DM incidence was tracked through the 8th follow-up survey (2017-2018). Lifestyle combination patterns were identified using factor analysis based on sociodemographic, lifestyle, and dietary data. Cox regression was used to assess T2DM incidence across patterns. RESULTS: Four lifestyle combination patterns were identified: 'Healthy Lifestyle,' 'Low Carb & High Protein,' 'High SES & Irregular Lifestyle,' and 'Bad Eating Habits.' The risk of developing T2DM varied across patterns. The 'Healthy Lifestyle' and 'Low Carb & High Protein' patterns showed a slight decrease in risk in T3, but the differences were not significant. The 'High SES & Irregular Lifestyle' pattern was associated with a higher T2DM risk in T3 than in T1 (hazard ratio [HR], 1.25; 95% confidence interval [CI], 1.05 to 1.55), but the association was attenuated after adjustment for family history. The 'Bad Eating Habits' pattern showed a 1.21-fold higher risk in T3 (HR, 1.21; 95% CI, 1.01 to 1.47). CONCLUSION: This study underscores the existence of distinct lifestyle combination patterns and their differential implications for T2DM risk. These findings support the need for tailored preventive strategies based on lifestyle patterns.

Evidence for divergence of restriction fragment length polymorphism patterns following in vivo replication of porcine reproductive and respiratory syndrome virus.

OBJECTIVE: To determine stability of the restriction fragment length polymorphism (RFLP) pattern of a porcine reproductive and respiratory syndrome vaccine virus and patterns of other viral strains as they replicate in pigs. SAMPLE POPULATION: Field samples of porcine reproductive and respiratory syndrome virus (PRRSV) and samples from 2 weaned pigs, 2 nursery-age pigs, and 5 gilts experimentally infected with PRRSV. PROCEDURE: PRRSV was isolated from field samples, experimentally infected pigs, or pigs that were in contact with experimentally infected pigs. For each virus, RNA was isolated from infected cells, and RFLP patterns were determined. RESULTS: 61% of field samples had 2-5-2 RFLP patterns characteristic of the vaccine virus, 32% had field virus RFLP patterns, and 7% had intermediate RFLP patterns that indicated a virus with a close relationship to the vaccine virus. Viruses isolated from experimentally infected pigs had no change in RFLP patterns after up to 13 weeks of in vivo replication and transmission to contact pigs. CONCLUSIONS AND CLINICAL RELEVANCE: RFLP patterns distinguish the vaccine and field strains of PRRSV; however, as the vaccine virus spreads among a swine population, the RFLP pattern can change to a related intermediate pattern. A glycine at residue 151 of open reading frame 5 is another marker for the vaccine virus; this glycine is rapidly lost and eventually replaced with arginine as the vaccine virus replicates in pigs.

[Structure and spatial pattern dynamics of dominant populations in a natural forest in north Guangdong Province].

Five 1000-m2 plots were set up in a forest dominated by Schima superba, Castanopsis fargesii and Pinus massoniana in Luoba Nature Reserve, Shixing County, north Guangdong Province. Field data were obtained by tally with contiguous grid quadrat method. The analysis of population structure and spatial distribution patterns of the dominant species in the forest shows that S. superba and C. fargesii had a large reserve of seedlings, with up to 1538 and 9763 seedlings per hm2, respectively, showing a pyramid shape of population structure. Both populations had a high mortality for seedlings of classes I and II in the process of development to saplings. The population structure of P. massoniana was an inverse pyramid type, with no seedlings and only 4 saplings per hm2, but 44 large trees per hm2. The spatial distribution patterns of P. massoniana and C. fargesii in coniferous broadleaved mixed community chiefly dominated by broadleaved trees were uniform, and in those chiefly dominated by P. massoniana were aggregated, but there were different causes of pattern dynamics for P. massoniana and C. fargesii. S. superba in both communities showed an aggregated distribution pattern. The aggregation intensity was different, with K values from 0.4280 to 0.9750. Regarding the distribution patterns in different development stages, S. superba was aggregated in different stages, while C. fargesii varied with uniform distribution pattern for the large trees and aggregated distribution pattern for the saplings and median trees. As for P. massoniana, its median trees showed an aggregated distribution pattern, while both the saplings and large trees were uniform. The variations in spatial distribution patterns of populations in different development stages were closely related to the biological and ecological characteristics of the species, but different block size had an impact on the outcome of spatial distribution patterns.

Echocardiographic patterns and prognosis in heart failure.

BACKGROUND: There are many variables with prognostic value in patients with heart failure (HF). Those related to left ventricular function are among the most important. Recently, the evaluation of the patterns of ventricular filling by pulsed Doppler echocardiography has been studied as a variable with prognostic value. OBJECTIVES: To evaluate the prognostic value of echocardiography variables (diastolic and systolic) in patients with HF. These variables were analysed in respect to hospital admission for cardiovascular reasons or death. MATERIAL AND METHODS: We evaluated 157 consecutive patients with HF and included 110 patients who were in sinus rhythm. The mean age was 68.2 +/- 0.9 years. HF was ischemic in 52.7%. Patients underwent echocardiography examination within the week of reference. The patients were grouped according to left ventricular (LV) systolic dysfunction (LV ejection fraction < 40%). We also classified patients in two groups according to the presence of a restrictive pattern in diastolic transmitral flow profile. Finally, we classified all patients in four groups according to their systolic function and diastolic pattern: Group I--systolic dysfunction and restrictive ventricular filling pattern. Group II--systolic dysfunction without restrictive ventricular filling pattern. Group III--without systolic dysfunction with restrictive ventricular filling pattern. Group IV--without systolic dysfunction without restrictive ventricular filling pattern. The events were death or hospital admission. The mean follow up time was 625 +/- 55 days. We did a statistical analysis and for all tests a p value < 0.05 was considered statistically significant. RESULTS: We found impaired LV systolic function (systolic HF) in 73.6% and restrictive ventricular filling pattern in 45.5%. During the follow-up 41.8% died or were admitted to hospital. Patients with systolic HF had lower admission free survival rate. Patients with restrictive ventricular filling pattern had lower admission free survival rate than those without. Group I had lower admission free survival rate than Group II and Group IV. Group IV had a higher admission free survival than all other Groups. CONCLUSIONS: These results support and expand previous observations that diastolic function variables, such as the pattern of ventricular filling (namely the restrictive) have independent prognostic value in patients with HF.

Mentally disordered offenders. Patterns in the relationship between mental disorder and crime.

Five patterns among mentally disordered offenders are distinguished by the relationship between mental disorder, on the one hand, and criminality, on the other. Pattern 1 offenders are those for whom crime is a response to psychotic symptoms, most often delusions or hallucinations. Pattern 2 offenders commit crimes motivated by compulsive desires, such as sex offenses by paraphiles and offenses regarded as evidence of disorders of impulse control. Pattern 3 offenders are those with personality disorder for whom the crime is merely one example of a maladaptive pattern of voluntary and knowing behavior. Pattern 4 offenders have coincidental mental illness that is unrelated to the crime. Pattern 5 offenders are those who become mentally disordered or feign mental disorder as a result of their crimes, such as those who dissociate upon seeing what they have done, those who become depressed in prison, those who become psychotic on death row, and those who malinger mental illness. Although these categories do not determine whether offenders are responsible for their behavior, some unknown proportion of Pattern 1 offenders do meet legal criteria for insanity, depending on the facts of each case and the applicable legal standards. It is arguable whether or not Pattern 2 offenders ever meet legal criteria of insanity. Offenders evidencing only Patterns 3, 4, or 5 are not candidates for an insanity defense.

Patterns of alcohol abuse from the viewpoint of multiple substance abuse.

1. As a result of examining the alcoholism of 172 subjects, from the view point of multiple abuse in a long-term process, we were able to classify alcoholism into three patterns: Pattern I is the alcohol alone type, Pattern II is the alcoholism with legal drugs, such as hypnotics and tranquilizers and Pattern III is the alcoholism with illicit drugs, such as organic solvents and methamphetamine. 2. We ascertained the alcohol alone abuse type (Pattern I) existed. 3. Pattern II resembles Pattern I in living status, delinquency, experience of criminal acts and characters except that the abusers in Pattern II were younger than those in Pattern I. 4. The abusers in Pattern III showed a trend to incline to multiple abuse. They had antisocial characters and family problems and the level of their social life was disrupted from their youth in many cases.

A study of longitudinal patterns of substance abuse with special reference to multiple use problems.

1. Sequential patterns of abuse were analyzed in 222 subjects of substance abuse who had admitted mental hospitals. They were classified into four patterns and seven sub-patterns. 2. Alcohol alone type (Pattern I) was different from other patterns of abuse in their initiating ages, family status and the levels of social life. A marked tendency to multiple abuse was found in Pattern II and III including organic solvents and/or methamphetamine. 3. However, even in Pattern II and III, alcohol abuse was found in 18% of the subjects. This suggests alcohol is an important hidden substance in multiple drug abusers. 4. Many substance abusers without alcohol had also alcoholics in their family members. Especially those who initiated their abuse in early ages are supposed to have constitutional and family factors which are common to alcohol and drugs. 5. Patterns II and III of organic solvents and methamphetamine abuse, the existence of peers and their strong influence upon users were of prime importance. Initiation of abuse from an early age and disruption of social life were marked in those patterns. 6. Many alcoholics started their abuse in earlier ages than expected. The study shows that there were two groups; the group whose social lives had been disrupted at early ages, and the one whose level of social life had been maintained. 7. Physical illnesses were mostly found in the patterns which include alcohol abuse.

[Relationship between urban green-land landscape patterns and air pollution in the central district of Yichang city].

In this paper, four types of landscape structures and their green-land landscape patterns, including the landscapes of dominant green-land patch pattern, even green-land patch pattern, dominant wooded corridor pattern and building or concrete covering pattern (control) in the central district of Yichang city in Hubei Province, were analyzed respectively on the basis of landscape ecological theory. The atmospheric noise and the contents of SO2, NOx and total suspension particle (TSP) of the landscapes were monitored respectively by comparative method. The results showed that the landscape of building or concrete covering pattern (control) was mostly composed of woodless corridors and building or concrete covering patches with the tiptop green-land fragmentation index (18.125 3 ind x hm(-2)) and only had 1.00% green-land coverage, which had a relatively higher atmospheric noise and the highest TSP content in the landscape. The landscape of dominant green-land patch pattern had the highest green-land coverage (up to 43.59%) dominated by great green-land patches and the least fragmentation index (0.453 9 ind x hm(-2)), in which, the atmospheric noise weakened by 28.12% and the TSP content reduced significantly by 86.42%, comparing to the control. The landscape of even green-land patch pattern had the relatively lower green-land coverage (11.34%) and fragmentation index (2.751 1 ind x hm(-2)), which was mainly composed of the middle or small green-land patches and wooded corridors with a regular distribution. In the landscape, the TSP content reduced obviously by 46.62% of the control, while the effect of dust retention was only 53.95% of that in the landscape of dominant green-land patch pattern. In the landscape of dominant wooded corridor pattern, which was a traffic center and turned into a main pollution resource in the city, there were a relatively higher green-land fragmentation index (6.870 0 ind x hm(-2)) and the highest wooded corridor density (0.844 3 hm x hm(-2)) with only 6.13% green-land coverage, and the atmospheric noise and the TSP and NOx content increased by 21.47%, 5.08% and 9.06%, respectively, comparing to control. It was obvious that the greater the average area of the green-land patch and the lower the fragmentation index of green-land patches, the more effective the green-land on purifying air pollution.

Recurring patterns of inflammatory sinonasal disease demonstrated on screening sinus CT.

PURPOSE: In order to define specific features on screening sinus CT (SSCT) that will aid the endoscopic surgeon in his approach to patients with inflammatory sinonasal disease, we sought to answer four questions: 1) what recurring patterns of inflammatory sinonasal disease are evident on SSCT; 2) what is the relative frequency of these recurring patterns; 3) how do these CT patterns correlate with the known sinus mucociliary drainage routes; and 4) what are the characteristic radiologic features of each pattern? METHODS: We reviewed the clinical and radiologic records of 500 consecutive patients who underwent SSCT as a prelude to possible functional endoscopic sinus surgery. RESULTS: Five recurring radiologic patterns of sinonasal inflammatory disease were identified: 1) infundibular (129/500 or 26%), 2) ostiomeatal unit (126/500 or 25%) 3) sphenoethmoidal recess (32/500 or 6%), 4) sinonasal polyposis (49/500 or 10%), and 5) sporadic (unclassifiable) (121/500 or 24%) patterns. Normal SSCT was seen in 133/500 patients (27%). CONCLUSION: Identification of specific patterns of sinonasal disease permits grouping of patients into nonsurgical (normal CT), routine (infundibular, ostiomeatal unit, and most sporadic patterns) and complex (sinonasal polyposis and sphenoethmoidal recess patterns) surgical groups. Assignment of patients to radiologic patterns allows a tailored surgical approach.
