A one-year review of the Eye Injury Registry of Indiana.

In the first year of operation, the Eye Injury Registry of Indiana collected data on 171 severe eye injuries and categorized them according to the circumstances of the injury, severity of tissue disruption and visual function. Many preventable causes of injury were documented. By understanding the contributors of accidental injury, prevention strategies can be implemented. This collaborative project should continue to provide useful information specific to Indiana concerning the root causes of eye injury.

Canadian women's perspectives on ovarian cancer.

OBJECTIVE: To describe the perspectives of Canadian women living with ovarian cancer regarding their experiences with the disease. DESIGN: A cross-sectional survey of a convenience sample of Canadian women with ovarian cancer. SETTING: Survey questionnaires were sent to physicians in 26 cancer programs that treat women with ovarian cancer and to ovarian cancer self-help groups for subsequent distribution to women. PARTICIPANTS: Women diagnosed with ovarian cancer and able to read English or French. MAIN OUTCOME MEASURES: A variety of individual items in the survey related to information received, communication, physical and psychosocial symptoms, impact of illness and quality of life. RESULTS: A total of 315 women returned the survey. The average age of the respondents is 59 years. Each province and territory is represented in the sample. Over one-half of the women received a diagnosis of ovarian cancer within a month of seeking help for a concern and 85% had multiple treatment modalities. The majority of the women felt adequately informed (80%) and were satisfied with communication with their physicians (mean of 4.1 to 4.5 on a 5-point scale). A majority (62%) said that their lifestyle had changed as a result of their disease. Problems were experienced most frequently regarding side effects (58%), fear of recurrence (54%), sleeping difficulties (46%), bowel difficulties (44%), fear of dying (36%) and difficulty concentrating (32%). Many who experienced problems reported receiving inadequate help for them (16% to 49%). Quality of life was reported as significantly lower following the diagnosis and treatment of ovarian cancer (p = 0.0001). CONCLUSION: This study provides an important foundation for further investigation. There is a pressing need for research regarding the early identification of ovarian cancer and issues of support and coping. Care for women with ovarian cancer requires the expertise of a range of disciplines and community-based agencies working collaboratively as a team.

Serum uric acid and its correlates in Chinese adult populations, urban and rural, of Beijing. The PRC-USA Collaborative Study in Cardiovascular and Cardiopulmonary Epidemiology.

BACKGROUND: Reports on serum uric acid (SUA) levels in Chinese populations are sparse, but there is evidence that hyperuricaemia and gout are not uncommon. This paper characterizes SUA levels, their correlates, and their relationship to blood pressure (BP) and prevalent high blood pressure (HBP) for urban and rural adult population samples in north China. METHODS: In 1987-1988, a cross-sectional study was carried out, using standardized methods, on men and women aged 40.58 in a Beijing area urban steel mill (N = 2013) and on rural farms (1507). Main outcome measures were SUA, systolic and diastolic blood pressure (SBP, DBP), and prevalent HBP (SBP > or = 140 or DBP > or = 90 mmHg or receiving an antihypertensive drug). RESULTS: Mean SUA levels for men were 5.75 mg/dl in urban and 5.58 mg/dl in rural settings; for women, 4.67 and 4.48 mg/dl. Mean values were higher with age in women, but not in men. Age-standardized prevalence rates of HBP were significantly higher in upper SUA strata (men > or = 7.00, women > or = 6.00 mg/dl) than lower SUA strata both with and without inclusion of those on antihypertensive drugs. Mean SUA levels were correspondingly higher in hypertensive than non-hypertensive people. In multiple regression analyses, body mass index and serum triglycerides were strongly associated with SUA. Also SBP, DBP, and HBP were generally associated with SUA for the whole population sample, with smaller coefficients after excluding those on antihypertensive drugs. However, in these multivariate analyses the strength of the association was low order. CONCLUSION: In addition to its strong association with body mass index, SUA is independently related to serum lipids, particularly triglycerides, and to serum glucose. While some of the univariate relation of SUA to BP is apparently due to the strong relation of body mass to both SUA and BP, a low order significant relation between SUA and BP remains with control for BMI.

The fine specificity of regulatory T cells. IV. Idiotypic complementarity and antigen-bridging interactions in the anti-lysozyme response.

The hen egg-white lysozyme (HEL)-specific B cells which are activated in response to HEL share a cross-reactive idiotype as well as submolecular specificity. Helper T cells of two different specificities are implicated in their optimal activation, one recognizing the idiotype and the other, an epitope on the HEL molecule opposite to the one recognized by B cells. Thus, efficient T-B collaboration involves both idiotypic complementary and antigen bridging.

The prevalence and management of trauma in the public domain: an agency and clinician perspective.

This descriptive study surveyed the ways in which trauma survivor needs have been addressed within the public mental health system in one sector of Connecticut. Information was obtained about the prevalence of traumatic history among consumers, the existence of policy related to trauma, trauma screening and cross-referral mechanisms, and trauma-related supervision and education. The study maps current service system functioning related to the provision of trauma-specific services at both the agency and the clinician level. The study data indicate a need for service enhancement and increased service system collaboration and integration to permit access to a full range of trauma interventions. The data also indicate a need for increased clinician education and supervision specific to trauma both across agencies and among individual clinicians.

What explains community coalition effectiveness?: a review of the literature.

Community coalitions have become popular vehicles for promoting health. Which factors make coalitions effective, however, is unclear. The study's aim was to identify coalition-building factors related to indicators of coalition effectiveness through a review of the empirical literature. Published articles from 1980 to 2004 that empirically examined the relationships among coalition-building factors and indicators of coalition effectiveness were reviewed. Two indicators of coalition effectiveness were examined: coalition functioning and community-wide changes. A two-phase strategy was employed to identify articles by reviewing citations from previous literature reviews and then searching electronic reference databases. A total of 1168 non-mutually exclusive citations were identified, their abstracts reviewed, and 145 unique full articles were retrieved. The review yielded 26 studies that met the selection criteria. Collectively, these studies assessed 26 indicators of coalition effectiveness, with 19 indicators (73%) measuring coalition functioning, and only two indicators (7%) measuring changes in rates of community-wide health behaviors. The 26 studies identified 55 coalition-building factors that were associated with indicators of coalition effectiveness. Six coalition-building factors were found to be associated with indicators of effectiveness in five or more studies: formalization of rules/procedures, leadership style, member participation, membership diversity, agency collaboration, and group cohesion. However, caution is warranted when drawing conclusions about these associations due to the wide variations in indicators of coalition effectiveness and coalition-building factors examined across relatively few studies, discrepancies in how these variables were measured, and the studies' reliance on cross-sectional designs.

Contribution of trans-splicing, 5' -leader length, cap-poly(A) synergism, and initiation factors to nematode translation in an Ascaris suum embryo cell-free system.

Trans-splicing introduces a common 5' 22-nucleotide sequence with an N-2,2,7-trimethylguanosine cap (m (2,2,7)(3)GpppG or TMG-cap) to more than 70% of transcripts in the nematodes Caenorhabditis elegans and Ascaris suum. Using an Ascaris embryo cell-free translation system, we found that the TMG-cap and spliced leader sequence synergistically collaborate to promote efficient translation, whereas addition of either a TMG-cap or spliced leader sequence alone decreased reporter activity. We cloned an A. suum embryo eIF4E homolog and demonstrate that this recombinant protein can bind m(7)G- and TMG-capped mRNAs in cross-linking assays and that binding is enhanced by eIF4G. Both the cap structure and the spliced leader (SL) sequence affect levels of A. suum eIF4E cross-linking to mRNA. Furthermore, the differential binding of eIF4E to a TMG-cap and to trans-spliced and non-trans-spliced RNAs is commensurate with the translational activity of reporter RNAs observed in the cell-free extract. Together, these binding data and translation assays with competitor cap analogs suggest that A. suum eIF4E-3 activity may be sufficient to mediate translation of both trans-spliced and non-trans-spliced mRNAs. Bioinformatic analyses demonstrate the SL sequence tends to trans-splice close to the start codon in a diversity of nematodes. This evolutionary conservation is functionally reflected in the optimal SL to AUG distance for reporter mRNA translation in the cell-free system. Therefore, trans-splicing of the SL1 leader sequence may serve at least two functions in nematodes, generation of an optimal 5'-untranslated region length and a specific sequence context (SL1) for optimal translation of trimethylguanosine capped transcripts.

Determination of plasma dexamethasone in the mother and the newborn after administration of the hormone in a clinical trial.

A RIA method is described for the measurement of dexamethasone in maternal, cord, and neonate plasma in a collaborative multicenter clinical trial to evaluate the efficacy of antenatal steroid therapy in the prevention of respiratory distress syndrome. The antiserum raised against dexamethasone-3-carboxymethyloxime-BSA conjugate was highly specific in that the endogenous steroids and 11-dehydrodexamethasone, a metabolite of dexamethasone, had a cross-reaction of less than 2.0%. Both pregnancy and cord plasma had to be purified by either gel filtration and/or paper chromatography. The overall recoveries of dexamethasone were 75.3 +/- 6.7% and 48.9 +/-6.7% for maternal and cord plasma samples, respectively. The intra- and interassay coefficients of variance for maternal plasma were 8.4% ad 9.1%, respectively, and for cord plasma were 11.9% and 10.9%, respectively. There was a good correlation between the dexamethasone values obtained by RIA when compared with those obtained by performance liquid chromatography in some cord plasma specimens. The recoveries of added dexamethasone and its metabolite by high performance liquid chromatography were also found to be good. The average maternal plasma dexamethasone level was 37 ng/ml 2 h after im injections of 5 mg dexamethasone phosphate every 12 h. The half-life of dexamethasone measured after the discontinuation of the drug was 216 min. Dexamethasone appeared too be cleared very rapidly from the circulation of the fetus and neonate. Because 11-dehydrodexamethasone was not present in significant amounts in the neonate, it was concluded that other factors in addition to the conversion of dexamethasone to its 11 dehydro metabolite were responsible for the rapid clearance of dexamethasone from fetal and neonatal circulation.

Comparative histopathologic evaluation of animal studies: murine studies with L-tryptophan and constitutents.

Following a 1994 collaborative research workshop on animal studies of the eosinophilia-myalgia syndrome (EMS), the Research Advisory Council (RAC) concluded that it would be instructive to have histopathologic slides from the different investigators and laboratories evaluated simultaneously using the same pathologist, pathologic criteria, and scoring system for cross-study comparisons of effects. The author evaluated the skin, underlying fascia and supporting tissues, and muscles, stained with hematoxylin and eosin, from 20 separate studies, representing 4 mouse strains, several protocols, and tissues from over 250 mice. The author concluded that, based on the material available, the findings are inconsistent, nonreproducible, and do not support a conclusion that L-tryptophan, case associated L-tryptophan, 1,1'-ethylidenebis(tryptophan), 3-phenylamino alanine, or a combination thereof, under the conditions of the studies, induce lesions of skin or muscle in the C57 Black, Balb/c, SJL, or B10.s strains of mice.

Results from the inaugural year of the Collaborative Islet Transplant Registry.

The National Institute of Diabetes and Digestive and Kidney Diseases (NIDDK) established the Collaborative Islet Transplant Registry (CITR) in September 2001. In its inaugural year, CITR represents the efforts of 12 collaborating North American islet transplant centers reporting 86 islet transplant recipients' data (1999-2003) and 173 processed pancreata leading to 158 infusion procedures. Recipient median age was 42.2 years, duration of diabetes was 30 years, and over 66% of the recipients were female. Twenty-eight patients received a total of 1 islet infusion, 44 patients received 2 islet infusions, and 14 patients received 3 islet infusions. The median age of the deceased donor was 44 years and body mass index was 28.2. Median time from cross clamp to pancreas recovery was 27 minutes, while duration of cold ischemia was 7 hours. Over 77% of the processing facilities used a density gradient for islet purification. For recipients of only 1 infusion, approximately 8665 total islet equivalents/kg were infused; recipients of 2 infusions received 14,102 islet equivalents/kg, and recipients of 3 infusions received 22,922 islet equivalents/kg. At 6 months after the last infusion, 61.1% of the recipients were reported to be insulin independent; at 12 months, 57.9% were reported to be insulin independent. There have been no deaths reported to CITR; 45 serious adverse events were reported. Through its collaboration with the islet transplant community and its interaction with professional societies and federal agencies, CITR is positioned to provide current and comprehensive information on clinically significant outcome measures in islet transplantation.

[Assessment of occupational exposure to PAH in coke-oven workers of Taranto steel plant through biological monitoring].

OBJECTIVE: This study aims at evaluating the occupational exposure to polycyclic aromatic hydrocarbons (PAH) through biological monitoring and at investigating the influence of selected genetic polymorphisms on the levels of internal dose biomarker. DESIGN: A cross-sectional study has been conducted on the basis of biological measurements in order to evaluate the levels of PAH exposure. SETTING: PAH exposure assessment has been carried out in a coke-oven of one of the largest European steel plants (ILVA, Taranto), in the context of the program of medical surveillance carried in collaboration with the ILVA Occupational Health Service. PARTICIPANTS: 355 coke-oven workers exposed to PAH were enrolled from the 3 batteries of the coke oven plant. Maintenance workers and industrial cleaners were included. MAIN OUTCOME MEASURES: The measurement of urinary levels of 1-hydroxypyrene (1-OHP) as biomarker of internal dose has been carried out, as well as the evaluation of metabolic polymorphisms of the cytochrome P450 1A1 (CYP1A1), glutathione S-transferases mu (GSTM1) and theta (GSTT1) as biomarkers of susceptibility. RESULTS: The median value of urinary 1-OHP was 1,05 microMol/Molcreat. The difference between the groups was statistically significant, with the highest value observed in the maintenance group (median 1,71, range 0,06-14,69 microMol/Molcreat). It is remarkable that 25% of the workers exceeded the proposed benchmark guideline value of2,3 microMol/Molcreat. No statistical difference was found in relation to smoking habits. Workers with GSTT1 null genotype had higher 1-OHP levels than those with wild type (p = 0,06). CONCLUSIONS: The levels of biomarker of internal dose reflect a critical exposure to PAH in the coke-oven plant. Further observations are needed to better investigate the role of GSTT1 on the 1-OHP levels.

The present situation regarding lymphatic filariasis in Cabo de Santo Agostinho, Pernambuco, Northeast Brazil.

A cross-sectional population-based survey on the occurrence of lymphatic filariasis was carried out in the municipality of Cabo de Santo Agostinho, Pernambuco, Northeast Brazil. 7,650 individuals of both sexes were examined (from 1,416 households) of whom six tested positive for microfilaria according to the thick blood diagnostic test. The age of the individuals examined varied from 0 to 98 (averaging 26.6 years), whilst the age of the microfilaria-positive individuals varied from 11 to 29, averaging 22.5 years. Five of the six positive cases were male. These cases were residents of the following areas: Pista Preta (one case); Ponte dos Carvalhos (four cases); and Pontezinha (one case). This last case from Pontezinha was autochthonous. Of the individuals examined, 109 (1.4%) cited complaints relating to filariasis. These results suggest that filariasis is being transmitted in the municipality of Cabo de Santo Agostinho, a finding that establishes the need to carry out disease control activities, and to collaborate with the planning of the national programme for the elimination of filariasis.

Description of three data sets: Collaborative Study on the Genetics of Asthma (CSGA), the German Affected-Sib-Pair Study, and the Hutterites of South Dakota.

Three different data sets with clinical data and markers from genome-wide screens were submitted for analysis at Genetic Analysis Workshop 12. In each study, participants were carefully characterized for asthma and related phenotypes. Testing for bronchial hyper-responsiveness using methacholine and standardized protocols was performed. Total serum IgE levels were measured using standardized techniques. In addition, similar questionnaire data on symptoms and relevant environmental exposures were obtained. Relevant clinical data and genotypes for the polymorphic markers used for each genome-wide screen were submitted. The data set from the United States Collaborative Study on the Genetics of Asthma represents a heterogeneous population consisting of both Caucasian and African American families ascertained through two siblings with clinical asthma from multiple centers. Likewise, the families from the German Asthma Genetics Group were also ascertained through two siblings with asthma at multiple centers. In a contrast to these data sets, Dr. Carole Ober and her collaborators submitted data from the inbred Hutterite population in South Dakota.

[Clinical quality management in rheumatoid arthritis].

Clinical quality management in rheumatoid arthritis (RA) aims at reducing inflammatory activity and pain in the short term and damage and disability in the long term. In the "Swiss Clinical Quality Management in rheumatoid arthritis" (SCQM) project, which started in 1997, a measurement-improvement system with feedback reports allows the rheumatologists to follow their RA patients with the aim of improving the quality of outcome. Inflammatory activity is measured with the Disease Activity Score (DAS28) and the Rheumatoid Arthritis Disease Activity Index questionnaire (RADAI), damage with an X-ray score and disability with the Stanford Health Assessment Questionnaire (HAQ). The feedback is used by the individual rheumatologist to optimize the therapy of his/her RA patients. Beside the aim of improving the quality of treatment, the SCQM projects wants to build a Swiss cohort of RA patients, to improve the collaboration of rheumatologists in the clinic and in practice and to establish standards of treatment in RA. In this paper we describe the SCQM project in detail, we show two cases illustrating the usefulness of the SCQM in the management of individual RA patients, and we present the cross sectional analysis of the first 1223 RA patients included in the project.

TGF-beta1-mediated activations of c-Src and Rac1 modulate levels of cyclins and p27(Kip1) CDK inhibitor in hepatoma cells replated on fibronectin.

Integrin-mediated cell adhesion transduces signals to regulate actin cytoskeleton and cell proliferation. While understanding how integrin signals cross-talk with the TGF-beta1 pathways, we observed lamellipodia formation and cyclin regulation in Hep3B cells, following TGF-beta1 treatment. To answer if integrin signaling via actin organization might regulate cell cycle progression after TGF-beta1 treatment, we analyzed cross-talk between the two receptor-mediated pathways in hepatoma cells on specific ECMs. We found that basal and TGF-beta1-mediated activation of c-Src and Rac1, expression of cyclins E and A, and suppression of p27Kip1 were significant in cells replated on fibronectin, but not in cells on collagen I, indicating a different integrin-mediated cellular response to TGF-beta1 treatment. Levels of tyrosine phosphorylation and actin-enriched lamellipodia on fibronectin were also more prominent than in cells on collagen I. Studies using pharmacological inhibitors or transient transfections revealed that the preferential TGF-beta1 effects in cells on fibronectin required c-Src family kinase activity. These observations suggest that a specific cross-talk between TGF-beta1 and fibronectin-binding integrin signal pathways leads to the activation of c-Src/Rac1/actin-organization, leading to changes in cell cycle regulator levels in hepatoma cells. Therefore, this study represents another mechanism to regulate cell cycle regulators when integrin signaling is collaborative with TGF-beta1 pathways.

Blood pressure in young blacks and whites: relevance of obesity and lifestyle factors in determining differences. The CARDIA Study. Coronary Artery Risk Development in Young Adults.

BACKGROUND: Middle-aged black men and women have higher blood pressure, on average, than whites. However, this pattern is inconsistent in children and adolescents. This study explores how differences in lifestyle factors in young adulthood may influence blood pressure patterns in the two races. METHODS AND RESULTS: The Coronary Artery Risk Development in Young Adults (CARDIA) study is an ongoing collaborative investigation of lifestyle and the evolution of cardiovascular disease risk factors in a random sample of young adults ages 18 to 30 years at baseline (1985 to 1986). Data from four examinations over 7 years were analyzed with the use of a method that simultaneously examined cross-sectional and longitudinal relationships of lifestyle factors and blood pressure. This study included 1154 black women, 853 black men, 1126 white women, and 1013 white men. Blacks had higher systolic blood pressure and diastolic blood pressure than whites at every examination. Racial differences were much greater in women than in men and increased over time. Within each sex-race group, average diastolic blood pressure over four examinations was positively associated with baseline age, body mass index, and alcohol intake and negatively associated with physical activity, cigarette use, and intake of potassium and protein. Longitudinal change in diastolic blood pressure was positively associated with changes in body mass index and alcohol intake. After adjustment for obesity and other lifestyle factors, black-white diastolic blood pressure differences were reduced substantially: 21% to 75% for men and 49% to 129% for women. Results for systolic blood pressure were similar. CONCLUSIONS: Differences in obesity and other lifestyle factors in young adults largely explain the higher baseline blood pressure and greater increase over time of blacks relative to whites.

Occupational Health Research in Developing Countries: The Experience in Ecuador.

There has been a growing volume of research regarding occupational hazards in developing countries during the past decade. Results of this work are increasingly appearing in established journals and forming part of the international database on the health effects of working conditions. However, despite many similarities to research conducted in economically advanced countries, research performed in developing countries are often quite different in their motivations, methodologies, and outcomes. To investigate these differences, the recent occupational health research experience in Ecuador was examined. Based on 15 reports that could be identified since 1980 as having sufficient information to analyze, the authors identified certain persistent themes: the preponderance of cross-sectional study designs; the limited availability of quantitative exposure measures, and the utilization of nonstandard clinical measures of outcome. Each of these limitations could be related to obvious conditions under which investigators were working, and each underscores the potential value of strategic alliances between local investigators and collaborators from developed countries. The review also documented the high importance in Ecuador to studies of "established" relationships between well-known toxicants and health. While such studies could be viewed as unoriginal or uninteresting by outsiders, they may form the basis of the domestic or regional research agenda, which cannot be overlooked or misunderstood if international collaborations are not to be exploitative.

Working together? Organizational and market determinants of collaboration between public health and medical care providers.

OBJECTIVES: This study examines organizational characteristics and market conditions likely to influence collaborative relationships between public health agencies and community medical care providers. METHODS: Public health directors in 60 US counties were surveyed by telephone concerning their relationships with area community hospitals (n = 263) and community health centers (n = 85). Multivariate models were used to estimate the effects of organizational and market characteristics on collaboration. RESULTS: Collaboration was reported among 55% of the hospitals and 64% of the health centers. Certain forms of collaboration were more likely in markets characterized by higher HMO penetration and lower HMO competition. CONCLUSIONS: Targeted efforts to facilitate collaboration may be required in settings where institutional and market incentives are lacking.
