[Intermittent pneumatic compression of leg and thigh: a new successful method for the prevention of postoperative thrombosis].

In a prospective study, 54 patients undergoing joint-replacement operations were randomized into a control and a treatment group. In the treatment group low pressure intermittent compression (Phlebo-Dynastat) to calves and thighs was started on the day of operation and continued until patients were fully mobilized. In 34 evaluable patients undergoing total hip arthroplasty the incidence of deep vein thrombosis (DVT) as determined by venography was 43% in the control group and 22% in the treatment group (p = 0.18). In 17 patients undergoing total knee replacement, DVT occurred postoperatively in 70% of the control group but in none of the treatment group (p = 0.002). The difference between the control and Phlebo-Dynastat group when comparing all patients was highly significant (p less than 0.005). Low pressure intermittent venous compression offers a welcome alternative method of thrombosis prophylaxis in patients with increased risk of bleeding.

Hibernoma, an uncommon tumor as a differential diagnosis of liposarcoma of the thigh.

Hibernomas are benign tumors derived from brown fat of hibernating animals and of human infants. The pathogenesis and malignant potential of hibernomas remain unclear. They occur generally in adults with a peak incidence in the third decade and with a predominance in women. However, the lesions may have an heterogeneous presentation that may mimic liposarcomas on MRI. Our report presents a case of hibernoma of the thigh whose the diagnosis initially suggested that it was a mixed liposarcoma with myxoid and well differentiated components.

[About the proposition of strength of animal-experimental results with hip endoprosthesis. A comparative analysis of the stress of the upper thigh of man and of four-leg (author's transl)].

Precise knowledge of the biomechanic of the hip of man and of animal is necessary to evaluate the procedures of adaption of the femur after endoprosthesis with animal experiment and the transfer into the endoprosthesis of the human medicine. After functional-morphological and biomechanical researches (Pauwels), the upper femoral end of the man is highly stressed by bending at direct level while lowly stressed at sagittal level. The present own biomechanical analysis of the hip of the dog reveal - under consideration of the gaits "pace", "trot" and "gallop" - that the coxal end of the femur of the four-leg compared with that of man is highly stressed by bending at sagittal level as well as at direct level. The observations are herewith clearly explained that intense reconstructions of the bone at artificial human thighs are found mainly at lateral and medial circumference of the bed of the prosthesis, at animal experiment however at dorsal, ventral as well as medial circumference.

Resistance emergence among P. aenrginosa in a rat thigh-abscess model. Comparison of imipenem and meropenem treatment.

Resistance emergence to carbapenem antibiotics was studied in a rat-thigh abscess model. Abscesses were developed in three groups with a total of 15 P. aeruginosa strains (three rats per strain). Groups were assigned to imipenem or meropenem treatment while one was left antibiotic-free. Test strains were fully susceptible to these antibiotics and the "Mutant Preventing Concentrations" of imipenem and meropenem over these strains were comparable. Antibiotic serum levels, assessed by serum bioassay test, were similar among therats. After four days, rats (n=45) were sacrificed and carbapenem resistant mutants were selected on imipenem (4 mg/L) and meropenem (4 mg/L) supplemented agar plates. Resistant variants of three strains, from four abscesses, were detected; one in the meropenem group, two in the imipenem and one in the untreated group. The MICs of imipenem and meropenem for the mutants were increased fourfold times or even higher of their counterparts. Resistance emergence under antibiotic pressure in P. aeruginosa has been shown in various conditions. To our knowledge, however, resistance emergence in abscess and also the comparison of imipenem and meropenem in this regard has not been studied before.

Comparative evaluation of calf, thigh and arm circumferences in detecting low birth weight infants--Part II.

In an earlier study usefulness and validity of calf circumference in the identification of low birth weight (LBW) infants was reported. To evolve a simple indicator in identifying LBW in community, comparative evaluation of three simple measurements, i.e., circumferences of calf (CC), thigh (TC) and arm (AC) was done with respect to their sensitivity and specificity. Though all the three measurements showed a high degree of correlation with the birth weight, calf circumference tended to be most sensitive in identifying almost 95% of LBW infants. Having established the superiority of CC, a two color tape demarcating LBW zone from normal birth weight using 10 cm CC as cut off point was tested by two independent investigators and two ANMs. It was observed that only 5% of cases were misclassified either as LBW or normal weight by the tape with hardly any inter individual variation. Measurement of calf circumference being simple and easy even in the hands of paramedics, it would be used as indicator of LBW and neonatal mortality in the community.

A comparative study of the myosin light chain kinases from myoblast and muscle sources. Studies on the kinases from proliferative rat myoblasts in culture, rat thigh muscle, and rabbit skeletal muscle.

Myosin light chain kinases have been isolated from rat thigh and rabbit skeletal muscle and cultured rat myoblasts. From these preparations, two types of kinases can be distinguished: calcium-dependent and calcium-independent. Both types of kinases can phosphorylate isolated P-light chains of myosin from several sources (skeletal muscle, cardiac muscle, and platelet). Data are shown which support the phosphorylation of the same site on the non-muscle P-light chains by both types of kinases. The rates of these reactins are, however, different for the two types of kinases. Kinetic analysis of the myoblast kinase shows differing affinities for various P-light chains (non-muscle greater than cardiac greater than skeletal). In the proliferative rat myoblast, phosphorylation of myosin is a prerequisite for actin activation of the myosin ATPase activity.

Changes in hemodynamic and post-flights orthostatic tolerance of cosmonauts under application of the preventive device--thigh cuffs bracelets in short-term flights.

SPECIFIC AIMS: to evaluate the influence of the use thigh cuffs "Bracelet" on the hemodynamic adaptation to microgravity during short-term (up to a month) space flights, in-flight tolerance to LBNP-tests and post-flight orthostatic tolerance. 6 cosmonauts applied and 7 others did not apply the occlusive cuffs when on flight. The "Bracelet" device notably relieved the cosmonauts from the subjective discomfort following by the blood redistribution at initial period of exposure to microgravity. It was established that "Bracelet" lessened shifts in central and peripheral hemodynamics typical for exposure to microgravity, venous stasis in the cervical-cephalic region in particular. There were no differences between the hemodynamic reaction on LBNP-test in cosmonauts who applied and not applied "Bracelet" during short-term flights. The objective data are received, that the application of the device during short-term space flight does not make negative effects on post-flight orthostatic tolerance.

[Covering of ischial pressure sores using a fasciocutaneous flap from the posterior surface of thigh (modified Griffith method) after mattressing with the biceps femoris. Apropos of 11 cases].

The ischial region is the commonest site for pressure sores in paraplegics capable of sitting. The choice of treatment must be guided by two imperatives: the need for a thick tissue cover (mattressing) and perfect trophicity, the risk of recurrence which always remains possible. The association of a modified Griffith fasciocutaneous flap from the posterior surface of the thigh and a hamstring muscle flap (preferably biceps femoris) providing mattressing of the ischial region, appears to satisfy these imperatives. The other repair procedures are discussed in order to ensure the best management of the paraplegic patient's cutaneous capital, due to the continual risk of a new pressure sore or a recurrence of the ischial pressure sore.

Vaginal reconstruction using perineal-thigh flaps with subcutaneous pedicle.

A technique of vaginal reconstruction using bilateral, perineal-thigh flaps with subcutaneous pedicle is described. In this procedure, the flaps were raised bilaterally and introduced into an artificial space between the urinary bladder and rectum. The blood supply for the flaps flows from the perineal artery through anastomotic branches to the external pudendal artery. The authors used the technique on four patients, and all the flaps survived entirely. There was no complication. According to a more than two-year follow-up survey, the reconstructed vaginas are expansible and contract little. No stent is needed. There is good sensitivity in the wall of the artificial vagina because sensory nerves run through the flaps.

Comparative dose-effect relations at several dosing intervals for beta-lactam, aminoglycoside and quinolone antibiotics against gram-negative bacilli in murine thigh-infection and pneumonitis models.

Relatively few animal studies have investigated the influence of dosing regimens on the efficacy of antibiotics possessing different pharmacodynamic characteristics. We evaluated the impact of dosing interval on the relative efficacy and potency of beta-lactams, aminoglycosides, and ciprofloxacin against Pseudomonas aeruginosa. Escherichia coli, and Klebsiella pneumoniae in murine pneumonitis and thigh-infection models. We used a sigmoid dose-response model to determine the maximal bactericidal effect at 24 hours (Emax) and the total dose required to achieve 50% of Emax (P50) at several dosing intervals. P50S (a measure of drug potency, in mg/kg/day) for beta-lactams increased 14- to 73-fold with longer intervals in both models. Dosing interval had little impact on P50S for aminoglycosides or ciprofloxacin. Emax varied among drugs but displayed no dependence on dosing interval. This method of analysis allows comparison of efficacy and dependence of potency on dosing regimen among different classes of antibiotics.

Acute anterior thigh compartment syndrome complicating quadriceps hematoma. Two case reports and review of the literature.

Acute compartment syndrome of the thigh has been reported infrequently. To date, only eight cases from isolated blunt trauma without fracture have been reported. Two additional cases caused by intramuscular hematomas following blunt, low-energy trauma, which were treated successfully with emergency fasciotomies, are presented. The morbidity from this syndrome varies from mild, with quadriceps weakness, fatigue, and myositis ossificans, to severe, with limb-threatening vascular compromise. Morbidity can be avoided if a high level of suspicion is maintained, compartment pressures are measured, fasciotomies are performed, and hematomas are drained. Postoperatively, patients can expect a dramatic decrease in pain and a quick return of quadriceps function with aggressive physical rehabilitation.

[Clinical application and improvement of an anterolateral thigh flap].

Since 1984, the authors performed 15 anterolateral thigh cutaneous and myocutaneous flaps with vascular anastomosis and 1 island flap. Among them, 1 failed and 15 survived completely with satisfactory results. The sizes of the flaps used ranged from 6 x 7 cm to 11 x 20 cm. The flap seemed to have a constant vascular anatomy with a long pedicle and a large diameter, so that dissection of the flap can be accomplished easily. The quality of the flap was good and suitable for the repair of soft tissue defect of the hand and foot. In this paper, improvement of operative technique and two representative cases are presented.

[Waist-hip and waist-thigh index in obesity especially in relation to weight reduction].

Waist-hip index (WHI) and waist-thigh index (WTI) were determined in 18 obese males and 24 females, respectively, before and after weight reduction, in comparison to this in 12 males and 10 females in normal weight. Under weight reduction the two indices in males were significantly reduced in correlation to the relative weight. In females analogous changes could not be proved. Furthermore, the WHI was significantly lower in persons with normal weight than in obese ones (males and females), the WTI only in females. - In obese males, however, not in males with normal weight the WHI processes a prediction value for the atherogenic risk, which is at least adequate to the value of up to now wellknown parameters (relative weight, body mass index etc.).

Deep onchocercomata close to the thigh bones of a Liberian patient.

During nodulectomies on a 53 year old Liberian woman, multiple onchocercomata were detected on the femur distal to the greater trochanter on both sides of the body. The nodules were attached to the periosteum. This location along the shaft of the thigh bone provides a further hint as to where clinically undetectable deep onchocercomata can be located.

Evaluation of ciprofloxacin alone and in combination with other antibiotics in a murine model of thigh muscle infection.

The therapeutic efficacies of ciprofloxacin and some comparative drugs were evaluated in a model of thigh muscle infection in neutropenic mice. Ciprofloxacin was found to be highly effective against all gram-negative and gram-positive bacteria tested. Combinations of ciprofloxacin and either gentamicin or beta-lactam antibiotics exhibited additive or slightly synergistic effects against the Enterobacteriaceae and gram-positive isolates. Azlocillin was found to increase significantly the efficacy of ciprofloxacin against Pseudomonas aeruginosa. Antagonistic interactions with vancomycin, which impaired the bactericidal activity of ciprofloxacin against Streptococcus faecalis, were observed only in vitro but not in vivo, whereas erythromycin also slightly reduced the therapeutic efficacy of ciprofloxacin in vivo.

Isolation and properties of beta-D-galactoside-specific lectin from chick embryo thigh muscle.

A beta-galactoside-specific lectin, capable of agglutinating trypsinized rabbit erythrocytes, was isolated from 13-day-old embryonic chick thigh muscle and purified 1000-fold by affinity chromatography on asialofetuin/Sepharose and Sephadex G-100. A quantitative hemagglutinin assay based on the disappearance of single erythrocytes in a Coulter electronic particle counter was devised to measure lectin activity at different steps of purification. The molecular weight of the lectin was determined by gel filtration to be approximately 31,000, whereas polyacrylamide gel electrophoresis in sodium dodecyl sulfate gave a value of approximately 15,000, suggesting that the lectin is a dimer. The lectin is unstable below pH 5, and it requires the presence of dithiothreitol for the retention of maximal activity. The major portion of this lectin is membrane-bound; only 50% of the activity present in the muscle homogenate could be isolated in soluble form by extraction of muscle acetone powder with a buffer of high ionic strength. In view of the lack of a calcium requirement for its activity, the role of this lectin in myoblast fusion, a calcium-dependent phenomenon, is not clear.

[A case of hepatocellular carcinoma with left thigh-bone metastasis].

A 57-year-old man with hepatocellular carcinoma with left thigh-bone metastasis is reported. He was admitted to the hospital with left leg pain. A diagnosis of bone metastasis of unknown origin was based on X-ray films. Physical examination revealed hepatomegaly. Liver function tests showed a slight increase of alkaline phosphatase, GOT and GPT. In serological tests, the levels of alpha-fetoprotein and carcinoembryonic antigen were high. HBs antigen was negative. From the above data, hepatocellular carcinoma was diagnosed.

The thigh-lacer: ambulatory non-operative treatment of femoral shaft fractures.

Thirty patients with fractures of the femoral shaft were treated by closed reduction, tibial-pin traction for three to six weeks, and early ambulation with crutches in a plastic thigh-lacer. Extensive quadriceps-strengthening exercises were performed. When the patient achieved active extension of the knee against gravity, weight-bearing as tolerated was encouraged. All of the fractures united. Follow-up ranged from eight to forty months. Malunion (angulation greater than 10 degrees) occurred in three patients and there was some loss of knee motion (less than 90 degrees of flexion) in one patient. Infection, refracture, non-union, and shortening of greater than two centimeters were not seen. This method of treatment yielded results comparable to those obtained with cast-bracing, without the high incidence of loss of knee motion, angulation, and prolonged cast treatment associated with that form of treatment.
