Diagnostic criteria for detection of postinfarction ischemia by quantitative analysis of stepwise dobutamine radionuclide ventriculography.

BACKGROUND: Application of the traditional diagnostic criteria validated for exercise testing may be inappropriate when dobutamine infusion is associated with radionuclide ventriculography (RNV). The objective of this study was to establish appropriate diagnostic criteria for evaluation of dobutamine stress testing with RNV for the detection of early postinfarction ischemia. METHODS: RNV was performed at baseline and during dobutamine infusion in 10 control subjects and in 30 patients who were studied within 1 week after uncomplicated myocardial infarction. Several quantitative parameters including left ventricular ejection fraction (EF), regional EF, and absolute change in global and regional EF were tested. In addition, regional wall-motion changes were scored by visual analysis. The limit of normal response for each quantitative parameter was defined as the 95th percentile of the distribution observed in control subjects. On the basis of predischarge clinical evaluation and exercise stress testing, patients were grouped as having evidence of residual ischemia (group 1, 15 patients) or no evidence of ischemia (group 2, 15 patients). Sensitivity, specificity, and accuracy in the detection of postinfarction ischemia were calculated for each parameter, and empiric receiver-operating characteristic curves were generated. RESULTS: The limits of the normal response to dobutamine infusion in the control subjects were found to be a 7.5% and a 12.5% increase in global EF at low and high dose, respectively. Median baseline EF was significantly lower in patients than in control subjects (Mann-Whitney U test: P < .001). There were no differences in resting EF between group 1 and group 2 patients. However, median high-dose EF and increase in EF were significantly lower in group 1 as compared with group 2 patients (56% vs 72% and 5% vs 17%, respectively; Mann-Whitney U test: P < .0001 for both). A biphasic (up-and-down) response with initial increase at low dose followed by decrease or no change in EF at high dose was observed in 66% of group 1 patients and only in 13% of group 2 subjects. The best criterion for detection of postinfarction ischemia was a change of <12.5% in global EF at high-dose dobutamine infusion, with 80% predictive accuracy. The traditional criterion, a change of <5 % or a decrease in EF, yielded an accuracy of only 63%. The association of a change of <12.5% with a biphasic response increased accuracy to 87%, with a sensitivity of 93% and a specificity of 80%. Quantitative regional wall motion analysis showed greater diagnostic accuracy than visual wall motion scoring (83% vs 70%). CONCLUSIONS: Application of appropriate criteria and stepwise quantitative analysis of RNV during dobutamine infusion may increase accuracy in the detection of early postinfarction ischemia.

Current status of vesicoureteral reflux diagnosis.

In children, diagnostic imaging for vesicoureteral reflux (VUR) encompasses both radiologic and sonographic modalities. The former comprises voiding cystourethrography (VCUG), the most widespread method for examination for reflux, and radionuclide cystography (RNC). In RNC the radiation exposure is significantly less than in VCUG with continuous fluoroscopy, but the anatomic details depicted are much inferior. With the introduction of pulsed fluoroscopy, the radiation exposure of VCUG has been markedly curtailed. VCUG is the first imaging choice for the urethra. The sonographic diagnosis of VUR with the intravesical administration of an ultrasound (US) contrast agent-voiding urosonography (VUS)-is being used increasingly in the routine diagnostic imaging work-up of reflux. Various sonographic reflux examination methods had been tried in the past. The real breakthrough came with the availability of stabilized US contrast media. Further impetus came with the introduction of harmonic US imaging. The recent release of a second generation US contrast medium promises to make a further positive impact on VUS. The diagnostic accuracy of contrast-enhanced VUS has been found to be comparable to radiologic methods. VUS is primarily performed in follow-up studies and as the primary reflux examination modality in girls.

Inhibitory actions of motor cortex following unilateral brain lesions as studied by magnetic brain stimulation.

Noninvasive transcranial magnetic stimulation (TMS) of the motor cortex not only induces short-latency, motor-evoked potentials (MEP) in contralateral muscles, but also inhibitory phenomena. One type of inhibitory action appears directly after the MEP in contralateral muscles and can be visualized by blockade of tonic, voluntary electromyographic (EMG) activity (postexcitatory inhibition, PI). Evidence for a cortical origin of PI, especially in its later part, was derived from double cortical stimulation in previous studies and is further supported by examination of PI in patients with focal hemispheric unilateral brain lesions in the present study. Thirty patients with different sites of vascular or tumour lesions were studied by TMS. In 6 patients with circumscribed lesions of the primary sensorimotor cortex a significant shortening of PI to contralateral muscles was observed. In 7 patients with focal lesions of the thalamus or internal capsule, in 6 patients with lesions of the premotor cortex and in 5 patients with lesions restricted to the parietal or temporal lobe, a significant prolongation of PI to the contralateral muscles was detected. Six patients with transient ischemic attacks showed either prolongation or shortening of PI. We conclude that PI is predominantly generated in the primary motor cortex, correspondingly its damage causes shortening of PI. In contrast, damage to brain areas that project to the primary motor cortex is followed by prolongation of PI. This remote effect on the primary motor cortex may result from disinhibition of cortical interneurones.

Technegas and Pertechnegas particle size distribution.

Technegas and Pertechnegas are radioactive aerosols produced in a commercial generator and used for lung scintigraphy. The aerosols are produced by first evaporating to dryness standard technetium-99m generator eluate (99mTcO4 in normal saline) in a graphite crucible (the simmer stage) and then heating this to 2500 degrees C (the "burn" stage). The aim of this work was to measure the particle size distributions of these agents and relate this to regional lung deposition. Factors that may vary during production of the aerosol in routine use were investigated to determine whether they influenced the particle size. Activity size distributions were measured using a serial wire-screen diffusion battery. The Technegas size distribution was approximately log-normal with an activity median diameter of 158 nm and a geometric standard deviation of 1.5. The results for Pertechnegas were similar. The median size increased with the number of simmers and with the time from generation. The increase in size with the number of simmers is thought to be due to the increased salt content in the crucible prior to the "burn". The predicted lung deposition is 37% in the alveolar region and 5% in the bronchial region. Significant changes in deposition are not predicted over the range of particle sizes measured.

The production and biological distribution of yttrium-90 labelled antibodies.

An antibody has been labelled with generator produced yttrium-90 (90Y) via a chelating group (DTPA) covalently attached to the protein. An average yield for protein labelling of 76% was obtained. The blood clearance of iodinated and 90Y labelled antibody is similar in rats and mice. However, in contrast to iodine, the yttrium label accumulates in the liver after release from the protein.

Determination of weighting functions for energy-weighted acquisition.

Energy-weighted acquisition (EWA) is an image filtering technique, with a different spatial filter (weighting function) for each energy. The imaging characteristics of EWA are governed by the weighting functions used during the acquisition of the image. The determination of weighting functions is more complicated than the determination of energy windows in conventional imaging because the number of degrees of freedom is much greater. A methodology by which weighting functions can be produced is described. The weighting function is determined by minimizing a generalized chi-square with variable contributions from coefficients quantifying key image characteristics, e.g., signal-to-noise ratio, spatial resolution, and scatter fraction. Varying the importance of these characteristics gives us a workable function-generation tool, able to address a variety of clinical needs. The resulting weighting functions exhibit good scatter reduction properties at various scatter depths, as demonstrated by measurements of line source response functions in a scattering medium at depths from 5 to 14 cm. Energy weighting can also be used to compensate for collimator penetration from high energy gamma rays. Weighting functions are tested in the laboratory using both planar and SPECT phantoms.

Generation of humic and fulvic acid from Callovo-Oxfordian clay under high alkaline conditions.

Low-carbon-containing clay from four different depths (447 to 516 m) of the Meuse Haute Marne (MHM) site is kept in contact with alkaline solution simulating conditions expected from cement dissolution in the near-field of a nuclear waste repository. Original organic material in the clay consists mainly of aliphatic hydrophobic compounds basically without oxygen-containing functional groups. After contact with 'solid young fluid' (mimicking cement dissolution, initial pH 13.22) for approximately one and a half years, high concentrations of hydrophilic organic matter are found (243-355 mg DOC/L). Characterization by solubility behavior, UV/Vis absorption, IR and fluorescence properties show that the dissolved hydrophilic organic matter has the characteristic features of humic and fulvic acids. Estimation of humic and fulvic acid content via UV/Vis spectroscopy results in 97.5 (+/-9.7)% of DOC being humic and fulvic acid. The results indicate that this could be an important source of complexing mobile organic matter influencing the mobility of radionuclides in a nuclear waste repository under consideration for this site. Investigations were conducted under oxic conditions representing the situation in the excavation disturbed zone (EDZ) of an underground facility. Sample amounts were very small and thus some characterization results are partly of preliminary character.

Interception, retention and translocation under greenhouse conditions of radiocaesium and radiostrontium from a simulated accidental source.

The behaviour of radioactive aerosols released from a severely damaged nuclear reactor and deposited on cereals was simulated under controlled conditions. 137Cs- and 90Sr-labelled aerosols were generated by volatilisation at high temperature of an artificially spiked pellet of depleted UO2. After cooling and maturation the aerosols were allowed to deposit on spring wheat (Triticum aestivum L. var. Arbon) cultures grown on lysimeters under greenhouse conditions. At the time of contamination the wheat plants were at different stages of development, from early vegetative growth (two leaves) until nearly mature (end of flowering). The estimated interception coefficient (micro) amounted to 13.1 m2 kg(-1); such a high value may be explained by the experimental conditions that created an over-saturated atmosphere during the contamination process and wet leaf surfaces. The first simulated rain, applied 6 days after the contamination, removed four times more 137Cs (54%+/-12 of the intercepted radionuclides) than 90Sr (15%+/-20) from the aerial parts. At harvest approximately 2% of the Sr and less than 1% of the Cs initially intercepted by the aerial parts is recovered for plants contaminated during the early development stages. A significantly higher proportion of the intercepted activity is still present for plants contaminated in the late development stages. The translocation to grains (TLF) increases when deposit occurs closer to the mature stage of the plant. The initial decrease of TLF values that we observed for strontium contamination in the earliest development stages is most probably due to the contribution of root uptake. Ploughing and re-sowing after the first rain, applied as a countermeasure reduced the 137Cs content in leaves and stems at harvest approximately 3 times but had no effect on the 90Sr content in vegetative organs. It reduced the 137Cs-contamination level in edible parts (grain) by a factor of 2 compared to the unploughed control, but doubled the 90Sr content.

The use of Kr-81m in ventilation imaging.

Radioactive gases are very useful in the study of pulmonary disorders because they allow an assessment of regional lung function. The most widely used radioactive gases are Xe-133 and, more recently, Kr-81m. This paper is a review of the physical characteristics of Kr-81m, its production from the parent Rb-81, the different methods to produce Rb-81, the different types of generators which can be used in hospitals, and the clinical applications of pulmonary ventilation imaging. The paper suggests that Kr-81m offers several advantages over previous methods that used Xe-133 due to its improved spatial resolution, its capacity to provide superimposable ventilation and perfusion images in multiple projections, and its total lung dose which is ten times lower than that of Xe-133.

Quantitative assessment of overall inflammatory bowel disease activity using labelled leucocytes: a direct comparison between indium-111 and technetium-99m HMPAO methods.

The ideal imaging method in inflammatory bowel disease would reliably detect inflammation, identify the correct intestinal location, and assess the severity of the disease. The aim of this study was to compare scintigraphic methods of quantifying overall disease activity using both indium-111 (111In) and technetium-99M (99mTc) HMPAO labelled leucocyte scans. The four day faecal excretion of 111In was measured after 111In scintigraphy in 24 patients known to have inflammatory bowel disease. The same patients also underwent 99mTc HMPAO scanning. The scans were performed 10 days or less apart with no changes in treatment between scans. Bowel activity on the 99mTc HMPAO scans was assessed using a computer based method (scan score) and a visual grading method in a further 54 99mTc HMPAO. The results showed a close correlation between inflammatory activity defined by faecal 111In excretion and the scan score generated from the computer analysis of the 99mTc HMPAO image (Spearman rank correlation: rs = 0.78; p < 0.001). Accurate information to localise inflammatory activity could be obtained by simple visual assessment of both types of scan images, although image quality was superior with 99mTc HMPAO. Qualification of disease activity from 99mTc HMPAO images by visual grading was associated with a large variability, only 69% of scans had similar scores when graded by three observers. Computer generated image analysis was more reproducible. In conclusion, in inflammatory bowel disease 99mTc HMPAO scintigraphy and faecal 111In excretion correlated well. Either method can quantify and localise the inflammation. As 99mTc HMPAO scanning provides a quicker result, with a lower radiation dose, and avoids faecal collection, it may be the preferred method.

Reduction in experimental infarct size by recombinant human superoxide dismutase: insights into the pathophysiology of reperfusion injury.

To determine the importance of reperfusion injury and the ability of the free-radical scavenger recombinant human superoxide dismutase (h-SOD) to prevent it, open-chest dogs underwent 90 min of proximal circumflex coronary artery occlusion, and only at the moment of reperfusion received either h-SOD (400,000 IU bolus into the left atrium followed by a 300,000 IU iv infusion over 1 hr) or saline. After 48 hr the surviving animals were killed and measurements were made of the risk region (by postmortem angiography) and infarct size (by gross pathology). All measurements were made by investigators blinded to treatment given, and the code was broken only at the end of the study. Hemodynamic variables and collateral flow during ischemia were similar in the two groups. Infarct size in control animals (n = 8) averaged 22.4 +/- 3.1% of the left ventricle and 52.2 +/- 7.1% of the risk region, compared with 13.3 +/- 0.8% of the left ventricle and 33.6 +/- 2.1% of the risk region in h-SOD-treated dogs (n = 8) (p less than .05). Infarcts in treated animals were not only smaller, but also exhibited a distinctive "patchiness," suggesting protection along vascular distributions. Furthermore, analysis of the relationship between infarct size and collateral flow measured during ischemia in the two groups indicated that protection by h-SOD was greatest in animals with the lowest collateral flows. This study supports the concept that reperfusion of ischemic myocardium results in a separate component of cell damage, presumably linked to the generation of oxygen free radicals on reflow. Since the h-SOD preventable reperfusion component of injury was most pronounced in hearts with the most severe ischemia, scavenging of oxygen radicals at the time of reflow may offer a novel and particularly promising therapeutic approach for the protection of ischemic myocardium.

Evaluation of the alveolar-capillary membrane permeability using 99mTc-HMPAO aerosols in severe diffuse interstitial fibrosis.

Local information on permeability of the alveolar-capillary barrier (PACB) can be ascertained by parametric images, after inhalation of radioarosols and computer processing. Our aim is to compare the results of 99mTc-HMPAO aerosols on PACB studies with those of 99mTc-DTPA aerosols, a standard technique. We compared the two techniques in separate samples: normal controls and patients with severe lung interstitial pathologies. Perfusion studies using 99mTc-MAA have also been performed in all patients. The aerosols were produced using ultrasound and lowered surface tension solution of 99mTc-HMPAO and 99mTc-DTPA. The time-activity curves (TACs) for every pixel on the lung area were used to calculate the half-disappearance times (T1/2). Parametric images were then generated with those times. The comparison of the results obtained with 99mTc-HMPAO and 99mTc-DTPA aerosols suggests that the first ones are more specific for local alterations of the lung epithelial transport in the pathologies studied. This method distinguishes between permeability deficiency due to local perfusion decrease and ACB deterioration.

Acid-citrate-dextrose compared with heparin in the preparation of in vivo/in vitro technetium-99m red blood cells.

Red blood cells labeled in vivo/in vitro with Tc-99m (Tc-99m RBC) were prepared in a series of 21 patients and two normal volunteers. In each subject both heparin and acid-citrate-dextrose (ACD) solutions were used to label tandem blood samples. The immediate preinjection binding efficiency (BE) was then determined. In each of the 23 studies, the ACD preparation yielded superior BE. The average BE was 93.47% (+/- 3.78) with ACD and 87.23% (+/- 4.29) with heparin. With the ACD method the effect of carrier Tc-99 may be as great as a 24% reduction in BE observed when initial eluates from long-ingrowth-time generators were used. Improved image quality with minimal renal and urinary-bladder activity results with ACD labeling. It is concluded that the use of ACD results in superior RBC labeling with less nontarget activity relative to heparin and is preferred over heparin for preparing in vivo/in vitro Tc-99m RBC.

Potential pancreatic imaging agents. Tellurium-123m labeled DL-alpha-amino-gamma-(phenyltelluro)butyric acid.

This report describes the first successful preparation of a 123mTe-labeled alpha-amino acid as a potential pancreatic imaging agent. Tellurium-123m labeled DL-alpha-amino-gamma-(phenyltelluro)butyric acid was prepared by basic hydrolysis of the radiolabeled 5-[beta-(phenyltelluro)ethyl]hydantoin. The hydantoin was prepared by the reacitron of 123mTe-labeled phenyltellurol, generated by sodium borohydride reduction of diphenyl ditelluride, with 5-(beta-bromoethyl)hydantoin. Tissue distribution studies in rats with the 123mTe-labeled amino acid for periods varying from 30 min to 24 h demonstrated only marginal pancreatic accumulation of radioactivity. The significant result of these studies is that a general synthetic method has been developed for the preparation of 123mTe-labeled amino acids.

Comparison between radioactive aerosol, technegas and krypton for ventilation imaging in healthy calves.

The use of lung scintigraphy in calves necessitates the validation of a ventilation (V) imaging agent compatible with clinical applications. This study aimed at defining the value of an inhaled radioactive aerosol (99mTc-DTPA) and a 'pseudogas' (Technegas) in the assessment of regional V in healthy conscious calves by comparing 99mTc-DTPA and Technegas deposition (D) images to V(V) images obtained from the steady-state inhalation of the short half-life krypton 81 (81mKr) gas. Images were compared by analysis of radioactivity distribution in computer-generated regions of interest within the right lung and D to V ratio images were generated in order to highlight areas of mismatching between 99mTc-DTPA or Technegas and 81mKr distributions. Results of this analysis showed that the 99mTc-DTPA aerosol droplets were unable to reach the lung parenchyma because of significant particle impaction in the major conducting airways. Better definition of the ventilated lung was obtained when using Technegas because of minimal deposition in conducting airways. Furthermore, the Technegas and 81mKr distribution patterns were highly equivalent.

Electrical charge characteristics of long-lived radioactive dust.

Respirable long-lived radioactive dust (LLRD), i.e., dust containing long-lived radionuclides such as 238U, 232Th, 226Ra, 224Ra and 228Th, interacts with unipolar and bipolar atmospheres through diffusion charging, electrical charge neutralization, and electrical self-charging mechanisms. Because of these interactions, and depending on the type of dust as well as its method of production, LLRD is found in electrically charged and neutral states. Electrical charge is important because it influences the deposition of particles in the human respiratory system. Particle size, electrical charge, and radioactive particle size distributions were measured in an area of an underground U mine where U ore crushing and transportation operations were conducted. In addition, concurrent measurements of 222Rn progeny and 220Rn progeny were made. A variety of instrumentation was used, such as particle counters. The electrical charge associated with dust generated in the primary crushing operation was substantially higher (3e- at 1 microns and approximately 500e- at 3 microns) than for the conveyor belt (2e- at 1 microns and approximately 50e- at 8 microns). In both cases the charge distribution was significantly higher than that predicted by Boltzmann's distribution.

A comparison between visual and quantitative analysis in a prospective evaluation of labelled 111In leucocyte imaging in vascular infection.

In a continuing evaluation of 111In-oxine labelled leucocyte imaging in vascular surgery, we have studied 16 patients with a clinical diagnosis of possible vascular graft infection. We have evaluated both visual and semi-quantitative analysis of the images obtained and have interpreted these in the light of the subsequent clinical outcome. Full length or multifocal uptake was seen in six patients, all of whom eventually required graft excision with two limbs surviving, and one death. These patients had a significantly higher uptake ratio (median = 3.26) than those with either localized (median = 1.12; p = 0.0027) or negative images (median = 0.72; p = 0.0003). Of four patients showing localized uptake only, one required amputation for continuing sepsis. Six patients had negative images, and had normal DSA and CT scans. Uptake ratios could not distinguish between those with localized images and those with negative images. Computer generated vertical profiles aided separation of patients with presumed localized and negative images. Semi-quantitative analysis has proved to be a reliable method which should allow a more direct comparison of the efficacy of various investigative techniques and of the results of therapy, independent of intra-observer subjective bias.

Granulocyte margination in bone marrow: comparison with margination in the spleen and liver.

The kinetics of radiolabelled granulocytes in the reticuloendothelial system were studied in order to evaluate granulocyte margination in bone marrow. A total of 34 patients took part in a two-part study. In the first part, bone marrow uptake of indium-111-labelled granulocytes was retrospectively analysed in early (3-h) and late (24-h) images in 26 patients, 13 with bronchiectasis and 13 with enclosed abdominal abscesses. The ratios between early and late counts from the bone marrow, spleen, liver and inflammatory lesion were used to quantify granulocyte margination in bone marrow, postulating that if the lesion to bone marrow ratio at 24 h exceeds the value at 3 h, then the "excess" bone marrow counts on the early images would represent margination. In the second part, this suggestion was prospectively tested using Rutland-Patlak graphical and deconvolution analysis of dynamic data, acquired in 8 patients undergoing routine scanning with technetium-99m HMPAO-labelled granulocytes. In the first part of the study, it appeared that the bone marrow is a regional site of granulocyte margination, like the spleen, with at least one-half of the 3-h marrow signal arising from marginated granulocytes, compared with about two-thirds from the spleen. In the second part, it was found that the gradient of the Patlak plot, based on spleen and marrow, continuously decreased, consistent with bi-directional movement of cells between these organs and the blood. Granulocyte pooling in the marrow was confirmed with deconvolution analysis, which generated biphasic retention functions for marrow and spleen. These curves were also consistent with two-way granulocyte exchange, and gave mean cell transit times in both organs of about 12 min and probabilities of extraction on each pass of 5-10%. We conclude that granulocytes marginate in bone marrow to an extent similar to that in the spleen.
