Educational level of spouses and risk of mortality: the WHO Kaunas-Rotterdam Intervention Study (KRIS).

BACKGROUND: A 9.5-year follow-up of the Kaunas-Rotterdam Intervention Study (KRIS) provided the opportunity of investigating whether the educational level of wives was associated with their husbands' risk of mortality and coronary heart disease (CHD), independently of the educational level of the husbands themselves. METHODS: The data represent populations consisting of 2452 Lithuanian men and 3365 Dutch men. These men were extensively screened for cardiovascular risk factors in the period 1972-1974. During the follow-up 303 Kaunasians and 350 Rotterdammers died. We used the Cox proportional hazards and the logistic regression models to analyse the data. RESULTS: We found that men whose spouses had little education apparently had an increased risk of all-cause mortality, even when their own educational level was taken into account. The relative risks (RR) were 1.57 in Kaunas and 2.15 in Rotterdam. The results for fatal and non-fatal myocardial infarctions were compatible with this finding, especially in Rotterdam. The prevalence of smoking was higher among men whose wives had primary schooling only. Nonetheless, adjusting the effect of the wife's educational level on her husband's mortality risk for all coronary risk factors in the husband only partially explained the association. CONCLUSIONS: The spouse's educational level appears to have independent effects on a man's risk of mortality in both eastern and western European communities. The results strongly suggest that including characteristics of the socioeconomic status of an individual's spouse would improve studies of socioeconomic differences in health.

Thyroid dosimetry in the western trace of the Chernobyl accident plume.

According to World Health Organization guidelines (WHO/SDE/PHE/99.6), the reference level for consideration in stable iodine prophylaxis is based on the inhalation exposure pathway. In the western trace of the Chernobyl accident, the measurement of airborne (131)I fractions (aerosol-associated, gaseous reactive and gaseous organic) indicates that airborne gaseous reactive and, especially, organic (131)I fractions were the major contributors to thyroid exposure due to inhalation. The contribution of inhaled short-lived radioiodines was negligible. To attain more precise thyroid exposure evaluation, (131)I dose factors were determined as a function of age and prevalence of stable iodine deficiency. The results demonstrate that children with a stable iodine deficiency experienced at least two times higher thyroid doses than did children with a dietary iodine sufficiency. The results of these investigations demonstrate that in thyroid dosimetry it is important to know the stable iodine status as well as to have a standardised method for airborne radioiodine measurements, especially for consideration of stable iodine prophylaxis based on the inhalation exposure pathway.

Environmental releases of radioactivity and the incidence of thyroid disease at the Ignalina Nuclear Power Plant.

The Ignalina Nuclear Power Plant consists of two Russian-made RBMK-1500 reactors. The plant uses Lake Druksiai as a natural reservoir for cooling water. Within the framework of the revised radiation dose limitation system, site-specific routine release conversion factors and maximum annual effective doses for the dominant radionuclides and pathways were evaluated for both atmospheric and aquatic releases. Using calculated release conversion factors, the locations of the highest predicted activity concentrations were determined for air and for the dilution zone of heated effluent water during the period 1984-1998. Committed effective doses for critical group members were less than 0.001 mSv for Ignalina Nuclear Power Plant airborne releases and less than 0.05 mSv for aquatic releases. These dose estimates are lower than the 1 mSv dose limit for the adjacent population. In the case of Ignalina Nuclear Power Plant, taking into account the uncertainties, a recommendation for the administrative dose constraint is 0.25 mSv y(-1). This dose level may scarcely affect human health. Interestingly, during screening for thyroid disorders, endocrinologists and pediatric-endocrinologists determined a dominance of abnormal thyroids (up to 60%) among school children in the vicinity of Ignalina NPP. The data on neonatal screening for congenital hypothyroidism and transient hyperthyrotropinemia, however, suggested a possibility that the majority of abnormal thyroid cases were related to stable iodine deficiency. Thus, the influence of Ignalina Nuclear Power Plant on thyroid disorders is highly conjectural and unlikely to be associated with the observed levels of childhood thyroid disease.

Isolation of Enterococcus faecalis in previously root-filled canals in a Lithuanian population.

The occurrence of Enterococcus faecalis in root canals of previously root filled teeth with apical periodontitis requiring retreatment was studied in Lithuanian patients. Twenty-five asymptomatic teeth were included in the study. Avoiding contamination microbiological samples were taken from the canals before and after preparation and irrigation with sodium hypochlorite and EDTA. Microbes were isolated from 20 of 25 teeth. E. faecalis was isolated from 14 of those 20 culture positive teeth, usually in pure culture or as a major component of the flora. Second samples taken after preparation revealed growth in 7 of the 20 teeth. Five of the seven cases were E. faecalis in pure culture. Isolation of E. faecalis was not related to the use of any particular root filling material in the original root filling. The results indicate that, rather than previous chemical treatment, it is the ecological conditions present in the incompletely filled root canal that are important for the presence of E. faecalis in these teeth.

Management of Lithuanian children's acute diarrhoea with Gastrolit solution and dioctahedral smectite.

OBJECTIVE: Acute gastroenteritis represents a major cause of morbidity and mortality worldwide among children, and rehydration treatment has been one of the cornerstones in the management strategy. The natural clay dioctahedral smectite (Smecta) increases intestinal barrier function and is effective against infectious diarrhoea in children. The purpose of this work was to compare the efficacy and tolerance of Lithuanian children's diarrhoea treatment with dioctahedral smectite combined with hypotonic oral rehydration solution (ORS)--Gastrolit--versus Gastrolit alone to establish the influence of Smecta on serum electrolyte balance in young children with diarrhoea and mild or moderate dehydration. METHODS: Smecta combined with ORS (study group) and ORS alone (control group) were evaluated in a multicentre, open, randomized trial in 54 children aged 6-48 months hospitalized for acute diarrhoea (mostly rotavirus aetiology) and signs of mild and moderate dehydration. The main outcomes examined were duration of diarrhoea, fever, number of vomiting episodes, and serum electrolyte balance before and after treatment. RESULTS: The mean duration of diarrhoea was significantly shorter in the study group (42.3 +/- 24.7 h) than in the control group (61.8 +/- 33.9 h). No side effects of Smecta were observed. The changes of sodium, potassium, chloride and calcium concentrations after treatment were minimal and in the normal range. CONCLUSIONS: Smecta significantly reduced the duration of diarrhoea, was safe and well tolerated, and had no impact on the adsorption of electrolytes. Smecta could be used together with ORS in children suffering from acute gastroenteritis (without uncontrollable vomiting) with mild and moderate dehydration.

Development of structural cardiac adaptation in basketball players.

BACKGROUND: As specific features of the basketball player's myocardial structure and function are still rather poorly investigated, we aimed at comparing left ventricular (LV) echocardiographic indices in players of different age groups. METHODS: Male basketball players aged 8-13 years (n=31), 14-17 years (n=31) and 18-28 years (n=31) were examined using standard echocardiography. End-diastolic LV posterior wall thickness and internal diameter, as well as interventricular septum thickness, were measured by M-mode. Relative wall thickness was calculated by dividing the sum of the thicknesses of the interventricular septum and LV posterior wall by LV internal diameter. LV mass and ejection fraction were also calculated. The morphological LV parameters were corrected for body surface area. The peak early (E) and peak late (A) transmitral flow velocities were measured using pulsed Doppler, and the ratio (E/A) was calculated. RESULTS: Significant differences in the absolute internal LV diameter among the age groups disappeared after allometric scaling. However, relative wall thickness as well as body size indexed LV posterior wall thickness and LV mass were significantly greater in the adolescents and adults than in the children. Players of different age groups did not differ in the E/A ratio, while ejection fraction was elevated in adolescents. CONCLUSIONS: Regular basketball training results in moderate cardiac hypertrophy in adolescents and adult athletes due to thickening of myocardial walls.

Factors influencing the caries decline in Lithuanian adolescents--trends in the period 1993-2001.

The study aimed to relate a number of factors to the numbers of decayed, missing and filled teeth (DMFT) in 12- and 15-yr-olds examined and interviewed in 1993 and 2001. A total of 1250 children in 1993 and 935 in 2001 were clinically examined and completed a detailed questionnaire on dental health-related topics. Factors were related to the DMFT count applying multiple regression analysis. From 1993 to 2001, the mean DMFT decreased approximately 30% in both age cohorts. Despite an observed improvement in dental health in Lithuanian children, the number of Lithuanian children reporting frequent sugar consumption, brushing teeth irregularly and visiting the dentist only in case of emergency was still high compared with Western countries. The DMFT values were related to the fluoride content in the drinking water, oral hygiene, residency and year of examination.

Genetic history: I. The Schneersons of Lubavich.

Genetic history in pedigree form is a useful tool in historical studies. This is illustrated by the Schneerson family dynasty, which has led the Lubavich sect of ultra-orthodox Hasidic Jews since its inception in the 18th century. The current leader is the seventh successive Schneerson to head the Lubavichers.

The molecular characterization of Gaucher disease in South Africa.

DNA from 29 southern African Gaucher disease patients was analyzed for five common Gaucher disease mutations: 1226G, 1448C, 84GG, IVS2 + 1 and 1504T. The origins of the patients were as follows: 14 Ashkenazi Jews; 6 Gentile Caucasoids; 8 Negroids; and one of mixed ancestry. The 1226G allele accounted for 80% of disease alleles in the Jewish patients, 50% of alleles in the Gentile Caucasoid patients and was absent from the Negroid patients. The 1448C allele was present in both the Jewish (1 of 24 alleles) and Negroid patients (3 of 16 alleles). Single-strand conformation polymorphism analysis was successfully used to detect mutation 1226G. This system also revealed the presence of mutation 1297T in a Jewish patient and a novel point mutation, 1277T, in an Afrikaans-speaking Caucasoid patient. Screening of 360 unrelated, healthy Ashkenazi Jewish volunteers to estimate the frequency of disease alleles in the local population led to the detection of 17 carriers: 16 possessed the 1226G allele (frequency = 0.0222), and one the 1297T allele (frequency = 0.0014). Using these results, together with the fact that only 92% of "Gaucher alleles" would be detected in this study, we estimate the disease carrier frequency in the Ashkenazim of South Africa to be 0.05, or approximately 1:20. A reliable carrier screening programme can now be offered to the local Jewish community. The majority of the disease alleles in the two Gentile groups remain uncharacterized.

Association of a melanocortin 4 receptor (MC4R) polymorphism with performance traits in Lithuanian White pigs.

The melanocortin 4 receptor is expressed in virtually all brain regions of mammals and plays an important role in energy homeostasis. Polymorphisms in this gene may thus be related to growth and obesity. In pigs, a non-synonymous polymorphic site was described (Asp298Asn) and demonstrated to affect cAMP production and to alter adenylyl cyclase signalling. Association studies revealed significant linkage of this mutation with production trait in pigs. In this study, 207 Lithuanian White pigs were genotyped at the MC4R locus and analysed on relationships between genotype and breeding values for several performance traits. The observed allele and genotype frequencies did not deviate significantly from Hardy-Weinberg equilibrium (wildtype allele 0.59; mutant allele 0.41) and are comparable with those described in other Large White populations. The mutant Asn298 allele of the MC4R gene was significantly associated with increased test daily gain, higher lean meat percentage and lower backfat thickness. There was a trend towards an improved feed conversion ratio (p = 0.065) in animals with the mutant allele whereas no significant effect was found on lifetime daily gain. These results indicate that the MC4R polymorphism should be integrated in selection programmes in the Lithuanian White to improve carcass composition.

Characteristics of patients with erosive and nonerosive GERD in high-Helicobacter-pylori prevalence region.

It is still not known whether there are differences between erosive and nonerosive GERD. The aim of the present study is to evaluate the prevalence of Helicobacter pylori (HP) infection, and other differences between erosive and nonerosive gastroesophageal reflux disease (NERD) patients. One-hundred and four consecutive GERD patients (mean age: 41.6 +/- 12.3 years) were interviewed, endoscoped and tested for HP. Erosive GERD was defined according to the Los Angeles classification. Patients who had no erosions in the esophagus but complained of heartburn or/and acid regurgitation at least twice a week and for whom these symptoms had a negative impact on daily activities were considered to be NERD patients. Erosive GERD was identified in 53 (51%) patients (mean age: 41.0 +/- 12.7 years) and NERD in 51 (49.0%) patients (mean age: 42.2 +/- 11.9 years). HP infection was found in 32 (60.4%) erosive GERD patients, and 41 (80.4%) NERD patients, P < 0.05. Multivariate analysis revealed that there were two statistically significant prediction factors for NERD: female sex with odds ratio (OR) of 6.34 (95% CI: 2.41-16.64; P = 0.0002) and HP infection with odds ratio (OR) of 3.28 (95% CI: 1.26-8.58; P = 0.015). The presence of HP and female sex are found to be statistically significant predictors of NERD.

The Strategy and Attribution Questionnaire: Psychometric properties of a Lithuanian translation in an adolescent sample.

The present study provides a preliminary validation of the Lithuanian version of the Strategy and Attribution Questionnaire (SAQ, Nurmi, et al., 1995), a self-reported measure of social and cognitive behavioral strategies. An attempt to validate SAQ subscales was made by correlating them with the Youth Self-Report (Achenbach, 1991) and school grades. This was done with a sample of 14- to 17-year-old adolescents (N= 734). The results showed that although internal consistency reliabilities were good, they were lower when compared with the original version of SAQ. The SAQ subscales were moderately associated with various validity criteria, such as anxiety/depression, withdrawal, social problems and somatic complaints and school grades. Results also showed moderate correlations between similar strategic components across achievement and affiliative situations. The results provide preliminary evidence for concurrent validity of the Lithuanian version of SAQ.

Adolescents' social strategies: patterns and correlates.

An unselected sample of adolescents (n= 504) was given a test measuring their social strategies in affiliative context, and emotional and behavioral problems. Four groups of adolescents were identified according to the certain patterns of the social strategies they deployed: Active optimistic group (n= 107), Defensive functional group (n= 159), Avoiders group (n= 89), and Learned helplessness group (n= 140). The degree of stability of a cluster solution was studied when the method for cluster analysis was changed, showing that the four-cluster solution captures the essential relationships between the variables. The membership of the strategy groups was associated in theoretically meaningful ways to the anxiety/depression, withdrawal, and social problems.

Patterns of dental caries and treatment experience in elderly Lithuanians.

OBJECTIVE: The aim of the study was to evaluate patterns of caries experience in a representative sample of Lithuanians, aged 65-74. METHODS: This cross-sectional study included 301 participants (response rate 54%). Information was obtained from a clinical examination (caries experience, stimulated salivary flow rates and oral hygiene levels) and a questionnaire. The questionnaire comprised questions about oral, general, physical, mental and social health and about background, knowledge, attitudes and lifestyle. The study had a multidimensional approach to negative consequences of disease and positive aspects of health. For bivariate testing, t-test, ANOVA and Spearman's correlation were used. Factor analysis was combined with linear multiple regression for a multivariate study of caries experience patterns. RESULTS: Elderly Lithuanians were found to have lower levels of edentulousness (range 11-15%) than elderly people in other European countries. The mean number of missing teeth was also lower than in any of the neighbouring countries. A comparison of dentate and edentulous groups did not show any major differences. Those who reported that they had general disease had higher levels of oral health maintenance. In dentate elderly, caries experience differed according to place of residence, fluoride content in the drinking water, socioeconomic status, gender, lifestyle, and many other factors. The multivariate approach explained 52%, 61% and 55% of the variation in the number of filled or sound teeth (FS-T), decayed (DT) and missing teeth (MT) respectively. CONCLUSION: Levels of oral health maintenance and caries experience show substantial variation among elderly Lithuanians, according to many health-related characteristics. These elderly people require appropriate oral care, just as much as people in other population sub-groups.
