[Massive localized lymphedema: a reactive lesion simulating liposarcoma (case report].

The authors present a case-history of massive localized lymphedema in a 54 year old female patient (height 167 cm, weight 113 kg). The history of the lymphedema lasted about 1 year. Its growth was not accompained with subjective complaints. It was diagnosed as a pendulous tumor of soft tissues in the thigh, 70 x 65 cm large. In preoperative diagnosis it was classified as a liposarcoma. The tumor lesion was removed and sent for bioptic examinations. Both histological and immunohistochemical biopsies denied benign or malignant nature of the soft tissue tumors and confirmed the diagnosis of a massive localized lymphedema.

[Myxoid liposarcoma of the mediastinum: a case report].

We describe a mediastinal myxoid liposarcoma in a 47-year-old man who developed recurrent lung infection, treated by surgery associated with radiotherapy. The report of this case and a review of literature bring the authors to outline the epidemiological, clinical and therapeutic profile of this rare tumor.

Well-differentiated giant "lipoma-like" liposarcoma of the posterior mediastinum: a case report.

While liposarcoma is the most common malignant mesenchymal neoplasm in adults, a mediastinal position is rare. We describe here the case of a 74-year-old female with an hourglass-shaped mass in the posterior mediastinum, which was probably present on a roentgenogram 4 years earlier. On a CT scan of the chest, the mass appeared non-homogeneous with a mainly adipose content. A preoperative ultrasonically guided tru-cut transthoracic biopsy revealed the presence of adipose tissue with mildly atypical cytological features. The mass was therefore completely excised by means of a right thoracotomy. The postoperative course was uneventful and the patient is alive and well and has had no recurrence in the 6 months since surgery. We review here the natural history, pathology, and prognosis of the disease, and discuss methods of diagnosis and management of such lesions.

Primary liposarcoma of the skull.

Only 14 cases of primary liposarcoma of bones have been reported in the available world literature. This is the first report of its origin from the calvaria. The tumor showed excellent response to deep x-ray therapy.

[Dedifferentiated liposarcoma of the omentum].

The authors present the case of a 56 years old patient, from the surgery department, with the diagnostic: "Tumoral abdominal mass". Ultrasound investigation and CT-scan suspicioned a malignant omental tumor. Intraoperator, we found a tumor with macroscopic malignant features (sarcomatous like), greow in omentum; the large border of the stomach being fixed but not invaded by it; the dorsal parietal peritoneum of the right lumba and the little omentum were involved, too. We removed all of the sarcomatous tumor, large omentum, small omentum and the gastric fixed segment. Also, the parietal dorsal peritoneum with metastasis, was surgically removed. The patient's postoperator evolution was very good. The histological report established the diagnostic: "dedifferentiated liposarcoma of the omentum". We considered the case to be interesting because of the difficulties in diagnostic and the rarity of it.

Distinction between well-differentiated liposarcoma and intramuscular lipoma by power Doppler ultrasonography.

BACKGROUND: This study evaluates the feasibility of ultrasonography in the distinction between well-differentiated liposarcoma (WDLS) and intramuscular lipoma (IL). MATERIALS AND METHODS: Three WDLSs and 9 ILs were included. Gray scale images were assessed for echogenicity, textural pattern and margins. Power Doppler ultrasonography was used to assess the number of detectable flow velocity signals in a 3 x 3 cm area. Furthermore, the ratio of the area occupied by colour flow signals relative to the selected area was determined. RESULTS: Gray scale images showed no differences between WDLSs and ILs. However, power Doppler showed more than 2 flow velocity signals in all WDLSs, whereas only 11% of the ILs had 2 signals. In all WDLSs, colour flow signals occupied more than 30% of the selected area. In contrast, ILs were characterized by a low color-dot ratio. Histologically, increased vascularity was found close to malignant cell invasions. CONCLUSION: Power Doppler ultrasonography is feasible to evaluate increased vascularity and thus differentiate WDLSs from ILs.

[Right atrioventricular metastasis of myxoid liposarcoma. Prolonged course after repeated surgery and chemotherapy].

The authors report a case of cardiac metastasis of a myxoid liposarcoma of the thigh in a 54 year old woman. The treatment associated repeated surgical ablation and chemotherapy. The drugs used induced some cardiotoxicity which limited their prescription but nevertheless, the patient survived 5 years. Previous reported cases do not describe survival lasting more than 2 years and associated chemotherapy would therefore seem to be a valuable adjuvant, increasing the life expectancy of such patients.

Spermatic cord liposarcoma: a report of four cases.

Spermatic cord liposarcoma is a rare pathology (1-4); currently about one hundred cases are documented. The therapy of choice is surgery, followed sometimes by radiotherapy. We herein describe our experience of 4 cases between 1995 and 2000, with median follow-up of 34 months (mean 48 months, range 28-95 months), in order to stress the role of orchifuniculectomy, even when mass-ablation first procedure may seem radical.

Treatment of myxoid liposarcoma by marginal or intralesional resection combined with radiotherapy.

Local recurrence-free survival rate for myxoid liposarcoma (MLS) is related to negative surgical margins. The goal of surgery in the treatment of MLS is to obtain tumor-free surgical margins. When a tumor is adjacent to critical structures, wider resection, which can result in amputation, would be selected for local control. While recognizing that marginal or intralesional resection is associated with high risk of recurrence, circumstances occur that lead surgeons to conduct conservative surgery. To determine whether marginal or intralesional resection combined with radiotherapy can achieve local control, 10 cases (12 tumors) were reviewed involving MLS treated with marginal (8 tumors) or intralesional resection (4 tumors) followed by postoperative radiotherapy (50-70 Gy, average 59.2 Gy). No recurrences of MLS were observed locally at the mean follow-up of 58.1 months. The development of severe radiation-related complications was not detected. This medium follow-up study suggested that this method can achieve a high rate of local control.

[A clinical study of primary lipoma and liposarcoma of the orbit].

3 cases of primary lipoma and 2 cases of liposarcoma originating from the orbit were confirmed histopathologically. The clinical features, pathologic findings, ultrasonography, computed tomography, and nuclear magnetic resonance imaging of the tumours and the differential diagnoses were discussed.

[Gastric liposarcoma: report of a case].

The authors present a case of a well differentiated gastric liposarcoma. The tumor was situated in the submucosa of the antrum, had a diameter of 9 cm, and showed well defined borders. A subtotal gastrectomy with BI reconstruction was performed. No abdominal metastases were observed. The patient is in good condition, without recurrence of the tumor 19 months after its removal.

[A case of renal capsular liposarcoma].

A 70-year-old man was admitted to our hospital with microscopic hematuria. Abdominal echography, computed tomography and magnetic resonance imaging revealed renal capsular tumor, and radical nephrectomy was performed. The tumor was diagnosed histopathologically as liposarcoma (well differentiated type). This tumor did not invade the renal parenchyma but extended from the upper pole to the lower pole on the surface of the renal capsule. He has been alive without disease for 14 months after surgery.

Primitive round cell liposarcoma of the omentum: diagnostic value of ultrastructural study.

A 25-year-old woman presented with abdominal distension first observed 1 month earlier. She had a rapidly growing omental tumor that was eventually diagnosed as round cell liposarcoma by ultrastructural examination. This case illustrates the importance of ultrastructural study and the limitations of immunohistochemistry in the diagnosis of such tumors, particularly when they grow in unusual locations.

[Changes in the mechanism of action of insulin on fatty tissue accumulations in lipomas, Madelung's lipomatosis and liposarcoma].

In this paper, the preliminary results regarding the alterations of the biological action mechanism of insulin in several types of pathological accumulations of fat, i.e. lipomas, Madelung's Lipomatosis and liposarcoma, are presented. The results indicate significant alterations both at the insulin receptor and post-receptor levels, with reduced biological activity of this hormone, which could have and inductive role in the pathogenesis of such entities.

Activation of metalloproteinases-2 and -9 by interleukin-1alpha in S100A4-positive liposarcoma cell line: correlation with cell invasiveness.

BACKGROUND: The S100A4 gene may affect the invasive properties of tumor cells through modulation of metalloproteinases (MMPs) and their inhibitors (TIMPs). MATERIALS AND METHODS: In the human liposarcoma cell line, SW872, we analyzed the expression of S100A4 protein by immunohistochemistry and flow cytometry. The production of MMP2, MMP9, TIMP1 and TIMP2 was assessed by gelatin zymography and enzyme-linked immunoabsorbent assay before and after interleukin-1alpha (IL-1alpha) and interleukin-6 (IL-6) stimulation; cell invasiveness was measured by Matrigel invasion assay. RESULTS: S100A4-positive SW872 cells responded to IL-1alpha with induction of immunoreactive MMP2 and TIMP1 and with activation of both MMPs, the latter significantly associated with an increase of cell invasiveness. Treatment with IL-6 induced less significant variations resulting in a more stable invasive behavior. CONCLUSION: These data show that S100A4-positive SW872 respond to interleukins by influencing the behavior of factors involved in extracellular matrix degradation and emphasize the predominant role of MMP activity status on the positive regulation of cell migration mechanisms.

[Liposarcoma originating in the neck and the mediastinum after removal of mediastinal lipoma].

A 47-year-old man, who had undergone removal of an upper mediastinal lipoma 9 years previously, was referred to our hospital for swelling of the right neck and wheezing. The chest X-ray showed enlargement of the upper mediastinum. Contrast-enhanced chest computed tomography and magnetic resonance imaging demonstrated a tumor with fatty density extended from the upper mediastinim to the right side of the neck. Two-stage tumor resection was carried out. On gross pathological examination the tumor was soft and fatty. Microscopic examination revealed well differentiated liposarcoma. Postoperative radiation therapy (50 Gy) was carried out. The patient remains free of disease 3 years after radiation therapy.
