Magnetic resonance imaging in acetabular residual dysplasia.

Eighteen hips in 15 children showed persistent roentgenographic evidence of congenital acetabular residual dysplasia at four- to eight-year follow-up evaluations. The mean follow-up period was 5.8 years. These hips were subsequently evaluated by magnetic resonance (MR) imaging. MR imaging studies revealed thick unossified chondroepiphyseal cartilage of the acetabular roof adjoining the ilium in all hips. The cartilage in these hips counterbalanced the steep acetabular slope seen on roentgenograms. Initially these hips were regarded as candidates for an acetabular reconstruction, but 11 hips displayed sufficient cartilaginous acetabular coverage on MR images. MR imaging promotes more accurate selection of patients for acetabular reconstruction and aids in the choice of the most appropriate type of acetabular reconstruction.

Anatomy of the pelvic joints--a review.

In adults, after the os ilium, os ischii and os pubis have joined together by ossification to form the os coxae, there is usually one joint between the hip bones ventrally (the pubic symphysis) and several more complex joints between the hip bones and os sacrum dorsally (sacroiliac, "axial sacroiliac" and accessory sacroiliac joints). These joints carry the weight of the upper part of the body, but they shall also enable pelvic distention during labour. Pathological conditions in these joints are common, and increased knowledge concerning their normal antomy is important for better understanding of these conditions.

Anatomical basis for transplantation of microsurgical anastomosed autologous and allogenic bone transplantants for reconstruction of the mandible in animal experiments (Göttingen-Minipig).

The possibilities of extraction and transplantation of bone with microsurgical vascular anastomosis for reconstruction of mandibular defects were to be examined in animal experiments. On the basis of the arterial vascularization of the ossa coxae of the Göttingen minipig which has been demonstrated, the cranioventral region of the os ilium is especially suitable as transplant. The separate vascularization of this bone area allows the extraction of a bone block with a sufficiently long afferent vascular stump. The experimental operations performed in the meantime display an excellent rate of healing of the microsurgically anastomosed bone transplants in the mandible.

Reconstruction of severely resorbed edentulous maxillae using osseointegrated fixtures in immediate autogenous bone grafts.

A surgical technique for rehabilitation of severely resorbed edentulous maxillae using fixed prostheses or overdentures supported by osseointegrated fixtures in immediate autogenous corticocancellous bone grafts from the ilium is described. The results of the first 23 consecutively treated patients are reviewed. The mean observation time was 4.2 years (range 1 to 10 years). A total of 124 fixtures was originally placed into the grafts, supplemented with 16 fixtures inserted later into seven of the jaws. Throughout their observation period, 17 of the patients had continuously stable prostheses. The remaining five had overdentures, and one patient had resorted to a conventional complete denture. After 4 years, 12 of 16 patients had continuously stable prostheses. Corresponding values at 5 years were 7 of 8 patients. Calculated from the date of abutment connection, 82.1% and 81.6% of the original fixtures were clinically stable and radiographically osseointegrated after 4 and 5 years in function, respectively. From the date of fixture placement, the corresponding figures were 75.3% and 73.8%, respectively. The mean marginal bone loss after the first year of prosthesis function was 1.49 mm. The annual marginal bone loss thereafter was about 0.1 mm. The results indicate that this technique is worthwhile for patients with extreme maxillary atrophy and who cannot wear conventional complete dentures.

Musculoskeletal deformities following treatment of Wilms' tumour.

Wilms' tumour is one of the most common neoplasms of infancy and childhood. Current treatment regimens result in a cure rate of about 80% for localized tumours but may also cause musculoskeletal deformities. Assessment of 21 patients previously treated for Wilms' tumour showed that all had flank atrophy on the treated side. Radiologic abnormalities included asymmetry of vertebral bodies, vertebral end-plate irregularities, scoliosis, kyphosis, platyspondyly and hypoplasia of the ilium. Although the vertebral changes following radiotherapy for Wilms' tumour are present from an early age and the potential is great for an increase in spinal deformity with growth, few spinal curves progress past 20 degree. Since one cannot predict which curves will progress, all such patients need careful orthopedic follow-up until skeletal maturity is achieved.

[Studies on suppurative lesion and bone distribution of peroral antimicrobial agents (LAPC and TFLX) to experimental infected rabbits].

The models of infection in the rabbits mandible using Streptococcus milleri and Bacteroides fragilis by the method of Satoh-Heimdahl were prepared. A penicillic peroral antimicrobial agent, lenamipicillin (LAPC), and pyridone carboxylic acid peroral antimicrobial agent, tosufloxacin tosilate (TFLX), were administered to infected and healthy groups. Drug concentrations in the serum, pus, mandible, maxilla, humerus, sternum, costa, ilium, femur, tibia, liver, and kidney were determined by a biological method. The data were submitted to pharmacodynamic analysis and compared. The following results were obtained. The concentration of LAPC in the serum and various bone tissues was 1.2-6.4 times higher in the infected group than in the healthy group. The concentration of TFLX in the serum and bone tissues was 1.1-3.7 times higher in the infected group than in the healthy group. Moreover, concentrations in the liver 1.1-fold and 2.1-fold higher in the infectious group than in the healthy group in the LAPC and TFLX administration groups, respectively, and 1.1-fold and 1.3-fold higher in the kidney in the LAPC and TFLX groups, respectively. Both LAPC and TFLX diffused to the pus more satisfactorily than to the serum. The Cmax ratios of pus to infected serum were 1.48 and 1.81 for LAPC and TFLX, respectively, and the AUC ratios of pus to infected serum were 1.31 and 1.62 in LAPC and TFLX, respectively. These results indicated that both LAPC and TFLX are distributed satisfactorily to the local foci in the mandible, and that their systemic diffusion exerted immediate clinical effects.

[Studies on bone and suppurative lesion distribution of antimicrobial suppositories (Ampicillin and Ceftizoxime) to experimental infected rabbits].

The investigation has been made regarding transfer of the antimicrobial agents into the osseous tissues in rabbit and into the mandibula in experimental infected rabbit model by making intrarectal administration of ampicillin (ABPC) and ceftizoxime (CZX). The preparation of the infected model were made according to the procedure of Satoh and Heimdahl. Pharmacokinetic analysis was made by measuring concentration of these antimicrobial suppositories transferred according to the biological measuring method in serum, mandibula, maxilla, ilium, tibia, sternum, liver and pus. The results were as follows: 1) In the infected group given ABPC, the concentration of the drug in mandibula and maxilla were approx. 21.5-52.9% of those in serum. 2) In the normal healthy group given ABPC, their concentration in tissues were approx. 13.9-23.7% of those in serum. 3) In the infected group given CZX, their concentration in pus and osseous tissues were approx. 12.7-61.8%. 4) In the normal healthy group given CZX, their concentration in osseous tissues were approx. 12.4-22.9% of those in serum. 5) In a comparison made between the infected and normal healthy group, transfer of the drugs into the infected group were observed in higher rates in serum and all osseous tissues. 6) The serum concentration of the antimicrobial suppositories (ABPC and CZX) by intrarectal administration showed transfer in far higher rate than those by oral administration and exhibited transfer of concentration at its peak equal to that by intramuscular administration.

[Clinical and radiologic status of the hip joint in adolescents and children with late detection of anomalies of the hip joint].

The study analyzed 35 questionnaires (70 hips) with an overdue discovered anomaly of the hip joint. 32 cases (91.5%) were girls, 3 cases (8.5%) boys, while the relation of girls to boys was 10.6:1. The average age at the moment of discovery of the defect was 3.9 months and in the period of control 12-13 years. All the questionnaires were conservatively treated but it was not observed that the method of treatment had any influence on the final result. According to analysis of clinical and radiological parameters the authors accepted the thought that these hips should be followed up until the end of their growth, and when an indication for operative treatment has been set it should take care about the development of the acetabulum as well as the shape of the proximal edge of the femur and the orientation of the ilium bone.

[Dental implants and bone grafts].

The reconstruction of an edentulous maxilla with bone graft is not a challenge. It is considered when the remaining bone cannot provide a firm anchorage for implants. The grafts are harvested from the ilium, the chin or the cranial vault. According to the case, the implants are placed either simultaneously or 6 months later after control with C.T. Scan. They are uncovered after another 6 month period, as in the usual procedure, and osseointegration is checked. The percentage of success on 24 operated cases seems at least equal to the regular cases without bone graft. Longer follow-up is needed for reliable statistics.

[Clinical study of soaked treatment of chronic osteomyelitis with herbal medicine].

This paper represented 500 patients with chronic osteomyelitis who were treated with Chinese medicinal herbs, Gan-ling Immersion Formula, in which focuses were soaked in combination with other therapies from 1986-1989. Healing rate was 83.2%. Effective rate was 98.0%. This therapeutic method was more effective for treating chronic osteomyelitis of the hand and the foot with shorter course. It was also good for treating refractory skin ulcer and scar, ilium osteomyelitis and sclerosing osteomyelitis. This method was immediately effective for acute attack of chronic osteomyelitis. Fistula in half of the patients could be closed within two therapeutic course. Most of the non-union and delayed-union of the bones could be unionized as chronic osteomyelitis was healed. Most dead bones could remove spontaneously, some could be absorbable, and seldom should be removed by operating on. These herbs are easy to get. The method was so simple and so safe that it could provide a new way of treating chronic osteomyelitis. Experiments showed Gan-ling Immersion Formula possessed relative strong anti-inflammation and external antibiotic effects.

Pelvic osteomyelitis in children.

Six children (7 to 16 years of age) with pelvic osteomyelitis are described. Sites of involvement included the pubis in three patients, the ilium in two patients, and the ischium in one patient. All were right-sided. Each patient presented with a history of fever and an abnormal gait. In four, the point tenderness indicated the site of bony involvement. All patients had pain on abduction but free passive range of motion of the hip. Soft tissue swelling was present on admission pelvic roentgenograms in five patients. Intravenous pyelogram revealed deviation of the bladder toward the midline in each of four patients studied. Roentgenographic changes typical of osteomyelitis developed in four patients ten days to ten weeks after onset of symptoms. In four patients in whom an organism was identified, Staphylococcus aureus was isolated from blood and/or bone. All isolates were methicillin-sensitive and two were penicillin-sensitive. Purulent material was drained from three of the five patients who underwent surgical exploration of the pelvis. All patients received parenteral antistaphylococcal therapy for 3 to 5 1/2 weeks (mean, 4 weeks). Oral antibiotics were given to five patients for an additional 3 to 14 weeks. All patients recovered completely.

Triple osteotomy of the pelvis. A review of 51 cases.

From 1980 to 1987, 43 patients (51 hips) were treated by triple pelvic (ischium, ilium, and pubis) osteotomy for moderate to severe symptomatic acetabular dysplasia. They were followed postoperatively for an average of 48 months (range, 18 to 84 months). In 41 patients (43 hips; 96%) there was dramatic relief of pain. The mean center-edge angle improved from 13.4 degrees to 36.1 degrees. The coverage of the femoral head was improved by an average of 25%. The authors conclude that triple osteotomy of the pelvis gives consistently good and predictable results with regard to pain relief and adequate femoral head coverage.

Endometriosis of the obturator nerve. A case report.

A 33-year-old woman, para 1001, had pain in the inner right thigh and proximal right leg weakness. At surgery a locally invasive, fibrotic lesion was found encircling the right ureter, right internal iliac vein and obturator nerve. It extended to the periosteum of the ilium. The lesion was dissected successfully without damage to vital structures, and endometriosis was confirmed histologically.

[Location, classification of histological subtype and rate of lung metastasis in radioactive phosphorus induced rat osteosarcoma: special reference to relation of dosage].

Radioactive phosphorus (P-32) was administered intraperitoneally to Wistar female rats at 2-week intervals. P-32 was administered at a total dose of 3 mCi (a single dose of 200 microCi, 15 times) to Group 1, 2.4 mCi (1 microCi/g body weight, 15 times) to Group 2, and 1.4 mCi (1 microCi/g, 10 times) to Group 3. Osteosarcoma was induced in 29 out of 80 rats (36%) in Group 1, 28 out of 40 (70%) in Group 2, and 31 out of 40 (78%) in Group 3. Bone tumor was predominant in the trunk (spine, ilium etc.) in Group 1 (86%), and in the extremities (femur, tibia etc.) in Group 2 (64%) and Group 3 (71%). Histological findings revealed neoplastic osteoid formation in all lesions. Osteoblastic type developed more in Group 1 than in Group 3, and fibroblastic type developed more in Group 3 than in Group 1. The rate of lung metastasis was significantly higher in Group 3 (94%) than in Group 1 (21%, p less than 0.01) or Group 2 (72%, p less than 0.05). This experimental method, especially that used for Group 3, appears to be useful for studying the basis of human osteosarcoma.

Osteointegration of bone graft in porous-coated total hip arthroplasty.

Since 1983, 19 patients have had bone grafting of acetabular defects in association with a porous-coated acetabular prosthesis. The defects were defined by anatomic location to assess the rates of incorporation at different sites. The average time to incorporation was 12 months, judged by trabecular continuity. Superolateral grafts showed a greater degree of rarefaction than medial grafts. Nonprogressive migration of the graft was seen in only two cases before full incorporation of the graft. Developing lucency was seen up to 2 mm at the prosthesis-graft interface but not at the graft-ilium level. Roentgenograms suggested that integration of porous acetabular prostheses into bone may improve results of reconstructive surgery of the acetabulum.

Lymph nodes in hare (Lepus europaeus Pallas 1778). III. Lymphocentrum iliosacrale.

The majority of nodes of iliosacral lymphocenter are attached to the wall of a. iliaca communis dextra et sinistra from its origin to the rise of a. circumflexa ilium profunda. The total number of nodes of the lymphocenter ranges from 5 to 11, without any marked dextro-sinistral asymmetry. The size of nodes, as expressed by their length, showed a wide range from 1.5 to 20 mm and in more than one half of nodes it was 5 to 10 mm. The mass of individual nodes ranged from 1.7 to 98.9 mg and in one half of them was up to 20 mg. The total mass of all nodes of lymphocenter fluctuated individually from 131 to 340 mg with a slight asymmetry in favour of the right-hand side. Regarding a considerable positional variability of nodes the authors do not hold a further particularization of iliosacral lymphocenter for reliable and they consider the nodes under study to be a uniform topical group.

[Case report of long-term treatment of a cleft lip and palate patient].

Cleft lip and palate patients have narrow maxillary arches and mesio-distal disharmony between the jaws caused by cicatricial contracture from cleft lip and palate surgeries. In such cases, early orthodontic treatment is required including improvement of the mesio-distal disharmony of the jaws. Following orthodontic treatment, prosthodontic treatment is required to improve the masticatory function and esthetic appearance in the cleft area. Thus, improving the occlusion of cleft lip and palate patients takes a long time. This case report concerns such a longterm handling of over 17 years from the initial treatment at 5 years 3 months till the final prosthodontic treatment for an under-developed maxilla with partial closure of the fistula to which ilium was later grafted following orthodontic treatment. Considerations are discussed about timing of the initiation of orthodontic treatment, closure of the oronasal fistula and the final prosthodontic restoration.

Treatment of dislocation of the hip, detected in early childhood, based on magnetic resonance imaging.

In seven children in whom congenital dislocation of the hip was diagnosed after they began to walk, magnetic resonance imaging provided important information for treatment. In five children, it revealed adhesion of the capsule to the ilium, an inverted limbus, and a displaced transverse ligament; these findings indicated a need for open reduction. In the other two children, magnetic resonance imaging demonstrated that the capsule was redundant and appeared to have been stripped from the posterosuperior part of the acetabular rim; this allowed closed reduction. Repeat magnetic-resonance imaging in these two hips suggested that the capsule gradually shrank after several months. The findings in these two patients suggested a long-standing Grade-I congenital dislocation of the hip, as described by Dunn and by Ogden. Children who have this type of congenital dislocation can be treated with closed reduction, provided there is prolonged immobilization of the hip.
