Effect of composition on the physicochemical properties and active substance release from gelatin-alginate sponge.

The aim the study was physicochemically characterize and develop ability of the active substance (cefradine) from the implantable porous carriers. The drug delivery systems consisting of the gelatine and alginic acid sodium salt and glycerol (GL) or peanut oil (AO). Gelatin-alginate sponge was prepared by foamed components and next freeze-dried this foam. The composition of the sponges affected on the sorption ability and on the stability to proteolytic enzymes. Owing to porous structure obtained specific profile of active substance release.

[Experimental study on treatment of acute limb ischemia with vascular endothelial growth factor-121 gene transfer].

OBJECTIVE: To investigate the feasibility of intramuscular gene therapy for acute arterial ischemic diseases by use of plasmid pcDNA3-VEGF121 and to evaluate therapeutic efficiency of vascular endothelial growth factor (VEGF) by different routes of administration. METHODS: Fifty New Zealand White rabbits were randomly assigned to either gelation sponge carrying-pcDNA3-VEGF121 (n = 18), intramuscular injection-pcDNA3-VEGF121 (n = 18), or pcDNA3 (as control group, n = 14). After ligation of the external iliac artery and complete excision of the femoral artery, 500 micrograms of the plasmid pcDNA3-VEGF121 were transfected into the muscles of the ischemic limb by gelation sponge carrying or direct intramuscular-injection. Immediately after gene transfection, blood flow of the internal iliac artery were measured. VEGF121 gene expression was detected by RT-PCR after 2 days, 1 week, 2 weeks, 3 weeks and 4 weeks of transfection. After 30 days, blood flow of the internal iliac artery, angiographic score and histological vessels of ischemic hindlimbs were measured respectively. RESULTS: In the two VEGF-treated groups, VEGF121 mRNA expressed in the transfected ischemic muscles after 2 days and lasted 2 weeks. Immediately after gene transfection, blood flow of the internal iliac artery had no significant difference between three groups. After 30 days, blood flow of the internal iliac artery, angiographic score and capillary density were significantly greater in both VEGF-treated groups than in control group. Complexity of vascular branching and vessel density of gelation sponge-VEGF treated limbs were significantly greater when compared with the intramuscular-injection limbs. CONCLUSION: These findings suggest the feasibility of employing gene therapy of pcDNA3-VEGF121 could augment collateral development and tissue perfusion in an animal model of hindlimb ischemia, and gelation sponge carrying VEGF gene may respect a potential therapy methods.

Preoperative chemolipiodolization of the whole liver for hepatocellular carcinoma.

BACKGROUND: Preoperative transarterial chemoembolization is not routinely recommended before hepatectomy for resectable hepatocellular carcinoma. This study evaluated the effect of preoperative whole-liver chemolipiodolization. PATIENTS AND METHODS: A retrospective comparison of background characteristics, operative results and long-term outcome was performed between 36 patients with chemolipiodolization confined to the tumor (selective group) and 23 patients with chemolipiodolization also involving the noncancerous liver (whole-liver group). RESULTS: There were no serious side-effects in either group and the operative outcome did not differ between the two groups. Tumor diameter was significantly greater in the selective group, but other pathological characteristics were comparable. The 5-year disease-free and overall survival rates for the selective and whole-liver groups were 11.9% and 33.0% (p=0.0191) and 44.9% and 73.2% (p=0.0121), respectively. CONCLUSION: These results indicate that preoperative whole-liver chemolipiodolization reduces postoperative recurrence and prolongs survival in patients undergoing resection of hepatocellular carcinoma.

[Different dressings in surgery].

The presented review of literature gives data on the structure, mechanisms of action and properties of different types of the present-day dressing materials and means for treatment of wounds: sorption, protective, activated with medicines and atraumatic ones. It is shown that medical dressings keep the priority significance in treatment of wounds of different etiology which is due to the effectiveness, availability and simplicity in using them in different conditions.

A review of dry socket: a double-blind study on the effectiveness of clindamycin in reducing the incidence of dry socket.

This double blind clinical study was undertaken to evaluate the effectiveness of locally applied clindamycin in Gelfoam in reducing the incidence of dry socket formation after third molar surgery. A total of 1,021 third molar extractions were performed, and 520 mandibular third molar extractions of varied surgical difficulty were evaluated. The results indicated that this technique was effective. This study also demonstrates that the incidence of dry socket after third molar surgery is significantly higher in the mandible than the maxilla, in smokers than in non-smokers, and in females currently on oral contraceptives.

[Radiofrequency ablation combined with transcatheter arterial chemoembolization for hepatocellular carcinoma--CT examination during the follow-up period].

Radiofrequency ablation (RFA) combined with transcatheter arterial embolization (TAE) can increase the volume of coagulation necrosis to treat patients with hepatocellular carcinoma. Furthermore, in clinical practice, RFA combined with TAE using iodized oil and gelatin sponge often induced the sub-segmental or segmental necrosis toward the liver periphery of the ablated lesion. In this study, we compared the CT findings and histological characteristics of peripherally spreading necrosis induced by this combination therapy for 12 patients with hepatocellular carcinoma. In all cases, complete necrosis of ablated lesions and peripherally spreading necrotic areas were confirmed by CT examination. The histochemical (lactate-dehydrogenase, maleate-dehydrogenase, and NADPH-diaphorase) stained specimens, biopsies from ablated lesions and peripherally spreading necrotic areas, were absent suggesting a 100% cellular destruction. No incomplete local treatments after the therapy were obtained during the 4-26 months of follow-up periods. We conclude that RFA combined with TAE using iodized oil and gelatin sponge makes it possible to induce the segmental or sub-segmental necrosis including tumors.

Treatment of gingival recession with a platelet concentrate graft: a report of two cases.

The goal of this report is to present a novel treatment strategy for mucogingival defects. The use of platelet concentrate gel in a collagen sponge carrier, combined with a coronally positioned flap procedure, is presented in two cases. Complete root coverage was achieved in both cases. Optimal esthetic results, with excellent soft tissue contour and texture, were observed. The patients were satisfied with the appearance. This technique warrants further investigation.

Bronchoscopic closure of bronchopleural fistula using gelfoam.

Management of a persistent bronchopleural fistula (BPF) can be a therapeutic challenge. The etiological factors responsible for BPF include pulmonary tuberculosis, post-thoracic resection surgeries, trauma, malignancy, necrotising infections and rupture of lung abscess. The immediate management of BPF is drainage of the pleural cavity with insertion of an intercostal drainage tube. Patients with BPF may also require surgical intervention in the form of a wedge resection or lobectomy or muscle flap surgery. We report a case of a peripheral BPF secondary to a bacterial infection, which was successfully managed by the instillation of gelfoam via flexible bronchoscopy.

[Preoperative embolization for meningiomas using PVA particles].

Preoperative embolization for highly vascularized and large meningiomas is an indispensable technique for facilitating their surgical removal by decreasing blood loss during the operation. This is a report of 4 large and highly vascularized meningiomas in the skull base, on which embolization of feeders was performed preoperatively by PVA (Polyvinyl alcohol foam) particles (150-250 micron produced by INGENOR CO, Paris) and small strips of gelfoam (0.5 x 0.5 x 3-5mm). Under EEG monitoring, Isosorbide dinitrate was used for prevention and relief of vascular spasm. Lidocaine injection tests (Xylocaine 2%: 50mg mixed in equal volumes with Iopamiron 300) were performed for checking before embolization. In the intracranial portion, standard taper steerable guide wire was changed to seeker flexible soft-tip guide wire. In two cases, the meningioma was located in the medial part of the sphenoidal ridge. In the other two cases, one meningioma was in the lateral part of the sphenoidal ridge and the other was in the olfactory groove. In all 4 cases, we successfully performed embolization without complication. In one case, we had to perform embolization twice, because of revascularization detected by angiography 3 weeks after the first embolization. In this latter case, we had performed central embolization only, by using PVA particles, having left feeder without occlusion (peripheral embolization) using gelfoam. The result suggested that it was also necessary to perform peripheral embolization especially if the tumor is fed by large tortuous and irregular abnormal vessels. Peripheral embolization may prevent PVA particles from washing out and causing progressive thrombosis by PVA particles.(ABSTRACT TRUNCATED AT 250 WORDS)

Growth of intraperitoneally transplanted ascites hepatoma, AH39 cells, in the area of gelatin sponge inoculation into the abdominal wall of Donryu rats.

A piece of gelatin sponge was inoculated submesothelialy in the abdominal wall of female Donryu rats, and 1, 2, 4, 7, 14, or 21 days later, 1 x 10(5) cells of ascites hepatoma AH39 were intraperitoneally transplanted. Tumor cells were detected in the area of gelatin sponge inoculation in each group 2 to 5 days after the transplantation. Generally, the number of rats in which tumor cells were detected was larger in those transplanted with tumor cells within a short period after the gelatin sponge inoculation. Changes in the area of gelatin sponge inoculation and nontreated area in the rats which received intraperitoneal transplantation of the tumor cells on the 1st day after gelatin sponge inoculation were studied. Tumor cells were detected in the area of gelatin sponge inoculation as early as 2 days after the tumor transplantation, whereas they were detected on the 14th day in nontreated areas. Tumor detection was preceded by inflammatory reaction in both cases.
